Pulmonary Pneumatocele in a Pneumonia Patient

Pulmonary pneumatoceles are air-filled thin-walled spaces within the lung, and are rare in adult cases of pneumonia. We report the case of a 74-year-old male who was admitted with a cough and sputum production. He had been treated with oral dexamethasone since a brain tumorectomy six months prior. Contrast-enhanced computed tomography (CT) of the chest revealed a large pneumatocele in the right middle lobe and peripheral pneumonic consolidation. Bronchoalveolar lavage (BAL) was performed; cultures identified extended-spectrum β-lactamase (ESBL) producing Proteus mirabilis. A four-week course of intravenous ertapenem was administered, and the pneumatocele with pneumonia resolved on follow-up chest CT. To the best of our knowledge, this is the first reported case of pulmonary pneumatocele caused by ESBL-producing P. mirabilis associated with pneumonia.

Background/Aims: The risk of recurrence of colorectal adenoma among obese individuals without metabolic abnormalities or in those with metabolically healthy obesity is largely unexplored. Therefore, we longitudinally investigated the risk of adenoma occurrence in individuals undergoing surveillance colonoscopy according to metabolic status and obesity. Methods: This retrospective cohort study included 16,872 Korean adults who underwent their first screening colonoscopy between 2003 and 2012 and who then underwent follow-up colonoscopy until 2017. Participants were categorized into a metabolically healthy nonobese group (reference group), a metabolically healthy obese group, a metabolically abnormal nonobese group, and a metabolically abnormal obese group. Hazard ratios (HRs) for adenoma recurrence compared to the reference group were calculated in each group. Results: During a median follow-up duration of 47.3 months (interquartile range, 35.6 to 58.9 months), 3,673 (21.8%) and 292 (1.73%) participants developed adenoma and advanced adenoma, respectively. When age, sex, smoking, alcohol consumption, family history of colorectal cancer, and baseline adenoma risk were adjusted, the risk of adenoma recurrence was increased in metabolically healthy obese individuals (HR, 1.33; 95% confidence interval [CI], 1.12 to 1.57) and metabolically abnormal obese individuals (HR, 1.18; 95% CI, 1.08 to 1.30) but not in metabolically abnormal nonobese individuals (HR, 1.03; 95% CI, 0.94 to 1.13). Conclusions: In this study, metabolically healthy obese individuals and metabolically abnormal obese individuals exhibited increased risks of occurrence of colorectal adenoma diagnosed by surveillance colonoscopy. This finding implies that obesity itself, even without metabolic abnormalities, is associated with an increased risk of adenoma recurrence. (Gut Liver 2021;15:383- 390)
Joint Clock and Frequency Synchronization for OFDM-Based Cellular Systems

<P>In cellular systems, a basestation and mobile stations need to be synchronized before data exchange. Since the basestation clock reference is more accurate, a mobile station typically derives its clock reference from the basestation. But the carrier frequency offset due to Doppler shift may have harmful effects on the local clock derivation. This letter proposes a joint clock and frequency synchronization technique between a basestation and a mobile station, which is effective even with Doppler shift. We derive the joint estimation algorithm by analyzing the phase and the amplitude distortion caused by the sampling frequency offset and the carrier frequency offset. Simulation results showing the effectiveness of the proposed algorithm will also be presented.</P>
삶의 만족과 기부행동의 관계에서 기부단체인식의 매개효과와 소득의 조절효과

본 연구는 삶의 만족과 기부행동의 관계에서 기부단체인식의 매개효과와 소득의 조절효과를 검증하기 위하여 수행되었다. 본 연구는 아름다운재단의 2018 기빙코리아 일반국민조사 데이터를 이용하였다. 연구대상은 설문에 응답한 2,011명이다. 본 연구의 결과는 다음과 같다. 첫째, 삶의 만족과 기부행동의 관계에서 기부단체인식은 부분매개효과가 나타났다. 둘째, 삶의 만족과 기부행동의 관계에서 개인소득과 가구소득의 조절효과가 나타났다. 본 연구의 결과를 토대로 기부행동을 높이기 위한 교육, 마케팅, 홍보전략에 대하여 논의하였다. 후속연구로 각 변인의 하위요인을 고려한 연구가 필요함을 제언하였다. This study was conducted to verify the mediating effect of donation organization recognition and the moderating effect of income in the relationship between life satisfaction and donation behavior. It was used the panel data of the 2018 Giving Korea General Public Survey of the Beautiful Foundation. The subjects were 2011 who responded to the questionnaire. The results are as follows. First, in the relationship between life satisfaction and donation behavior, the recognition of donor organizations showed a partial mediating effect. Second, there was a moderating effect of personal income and household income in the relationship between life satisfaction and donation behavior. Based on the results, educational program, marketing, and promotion strategies to increase donation behavior were discussed. As a follow-up study, it was suggested that a study considering the sub-factors of each variable is necessary.

<P><B>Abstract</B></P> <P>Rosuvastatin is a statin used to treat metabolic syndrome conditions, such as hyperlipidemia. It is relatively safe; however, fatal rhabdomyolysis or skeletal myopathy can sometimes occur. Therefore, to investigate the overall effects of rosuvastatin, including lipid lowering and adverse effects, metabolic profiling was performed using metabolomics and lipidomics after rosuvastatin administration. Specifically, the metabolic profiles between healthy subjects and patients with hyperlipidemia were compared and the metabolic changes related to the mechanism of the drug effect were proposed. Healthy volunteers (n = 32) and hyperlipidemic patients (n = 14) were orally administered rosuvastatin (20 mg) once a day for 3–8 weeks, and plasma and urine were collected. Metabolomics and lipidomics were performed using UHPLC-LTQ/Orbitrap/MS/MS for non-targeted analysis and UHPLC-TQ-MS/MS for targeted analysis. Using non-targeted analysis, we successfully profiled and identified 73 and 87 metabolites in healthy subjects and hyperlipidemia subjects, respectively. Through targeted analysis, we have also quantified 188 metabolites, including amino acids, biogenic amines, glycerophospholipids, and sphingolipids. The levels of L-carnitine, diacylglycerol, and acylcarnitines significantly decreased after rosuvastatin administration regardless of the group. The overall levels of fatty acids (FA) and lysophosphatidylcholines (LysoPC) increased, while phosphatidylcholines (PC) decreased only in the patient group. β-Oxidation decreased overall, while the production of polyunsaturated FA increased only in the hyperlipidemic patients. Using metabolic profiling, we have evaluated the alterations in the biochemical pathways, which may aid in a more detailed understanding of the effect of rosuvastatin. Patient-specific metabolomic and lipidomic profiles may serve as valuable markers for the understanding of the adverse effects associated with statin treatment.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Rosuvastatin decreases cholesterol, L-carnitine, acylcarnitine levels, and Lp-PLA<SUB>2</SUB> activity. </LI> <LI> PC levels decreased and FA and LysoPC levels increased significantly in the patient group with rosuvastatin. </LI> <LI> Polyunsaturated fatty acids such as arachidonic acid and linoleic acid in the patient group suggests impaired β-oxidation. </LI> <LI> Significantly increased glutamate may act as an alternative substrate to complement the energy impairment. </LI> <LI> Patient group specific upregulation of myristate and palmitate may lead to mitochondrial dysfunction. </LI> </UL> </P>

This paper presents a fault-tolerant control algorithm for the in-wheel motor drive electric vehicle. The proposed fault-tolerant control algorithm is developed based on parity equation and current estimation method. The nonlinear permanent magnet synchronous motor (PMSM) model and a PMSM control algorithm are developed to simulate the fault-tolerant control algorithm. The simulation results the validity of the proposed fault tolerant control algorithm.

This paper deals with formal characteristics of Hagia Sophia in Istanbul, designed by Anthemius of Tralles and Isidorus of Miletus. Influenced by the Platonic Academy in Athens, the two architects were highly skilled in mathematics and geometry. Accordingly, it is believed that a strong geometrical tradition characterizes the layout of the church. In this article, the geometric design of Hagia Sophia in relation to geometric methods of the classical periods such as ‘Root rectangle’, ‘Sacred Cut’, and ‘Vesica Piscis’ is interpreted. It is well-known fact that these techniques were known and used to the ancient Greek. It concludes that such geometric methods have come to be regarded as fundamental for the design of Hagia Sophia.
