감귤류 종자 추출물의 N-nitrosodimethylamine 생성억제에 관한 연구

The effect of inhibition on N-nitrosodimethylamine (NDMA) formation by methanol extracts from 6 kinds of citrus seed (Citrus sunki, Citrus natsudaidai, Citrus suleata, Citrus tangerina, Citrus grandis and Citrus obovoidea) were investigated. The contents of moisture, crude protein, crude fat, carbohydrate, ash, flavonoid and totat phenol in citrus seeds were 4~6, l1~15, 32~46, 22~45, 2~4%, 12~24mg% and 53~133㎎%, respectively. The solid contents and yield of citrus seed extracts were 0.8~1.0 and 0.7~1.1%, respectively. Nitrite-scavenging activity by methanol extracts from citrus seeds increased with increasing extract dosage. Furthermore, the nitrite scavenging activity was pH dependent being the highest at pH 1.2 (42.7~96.9%) and the lowest at pH 6.0 (19.9~62.6%). Scavenging effects of nitrite by reaction time showed high effects under 3hr reaction time. The inhibition effect of NDMA formation by the citrus seed was strengthened as the amount of extract increased. The inhibition rate of methanol extracts from citrus seeds on NDMA formation showed 1.2~39.8%, 21.3~60.1% and 47.4~94.0%, according to add 1, 3 and 5㎖, respectively. Therefore, the inhibition effect of NDMA formation by the citrus seed was strengthened as the concentration of extract increased.

Background: Predicting outcome after head trauma is of great interest for clinicians, especially in the early stage. It may provide a basis for therapeutic strategies and may be helpful to select different approaches. But, reliable outcome prediction from head trauma is still unresolved. The purpose of this study is to determine which clinical parameters can be used effectively after an event of head trauma. Methods: In our study we studied head trauma patients who came to the Emergency department of Korea University Medical School Anam Hospital from the 1st of June 1998 to 31th of January 2000, looked into medical records, and studied the records in a retrospective manner. The head trauma patients were divied into two group, a) favourable outcome-related and b) unfavourable outcome-related. The two groups were then subdivided according to their sex, age, the mechanism of their injury, the time they came in, alcohol drinking, loss of consciousness, vomiting, pupil change, previous medical illness, associated injuries and were seperately compared and analyzed. And the factors affecting Glasgow Outcome Scale(GOS) were analyzed in the parameters of Injury Severity Score(ISS), Revised Trauma Score(RTS), Glasgow Coma Scale(GCS), motor score of GCS(mGCS), Marshall Computed Tomographic Classification(MCTC). Statistically, t-test and Mann-Whistney rank sum test using Jandel-sigma were used in the study and then were assessed in Chi-square analysis methods. The statistical significance was determined at a level of p less than 0.05. Results: The total of the head trauma patients, added up to 129 people, 99:males and 30:females. Among this sum, 99 patients showed a favourable outcome and 30 patients showed an unfavourable outcome at discharge. The causes of injury included: automobile accident in 42 cases; fallss in 26; assaults in 6; and accidental-slipping injury etc in 55 cases. 12 patients had bilateral unreactive pupils on admission, 2 had unilateral reactive pupils, and 113 had bilateral reactive pupils. Compared to the favourable outcome-related head trauma group, the unfavourable outcome-relared head trauma group had more history of loss of consciousness, unreactive pupils and tended to have previous medical illness. There were no difference in alcohol related, vomiting and associated injuries between two groups(favourable outcome-related head trauma group: unfavourable outcome-related head trauma group). Substantial difference were observed in GCS, mGCS, ISS, RTS, MCTC between two groups. Patients with unfavourable outcome had a significantly higher ISS, lower RTS, lower mGCS. After MCTC, patients with mass lesion had unfavourable outcome than patients with diffuse injury. Conclusion: Age, history of loss of consciousness, previous medical illness and unreactive pupil change are predictives of outcomes of head trauma patients with respect to Glasgow outcome scale. The lower GCS, lower RTS, lower mGCS, higher ISS, mass lesion based on CT scan provide poorer prognostic outcome in patients with head trauma. Use of prognostic factors thorough complete history taking and physical examination would provide useful prognostic information and facilitate improved therapeutic decision-malting in head-injuried patients.
제 2급 와동에서 구치부 복합레진의 미세변연누출에 관한 연구

The use of flowable composite resins as liners in class Ⅱ packable composite restoration has been suggested by some manufactures. However, the contributions of this technique are unproven. The purpose of this study was to compare the gingival microleakage in class Ⅱ packable composite restorations with or without the use of flowable composite resins as liners. Slot cavities were prepared on both proximals of 80 extracted human molars and randomly assigned to 8 groups of 20 each. The gingival margins were located at 1mm above CEJ in 80 cavities (group1-4) and 1mm below CEJ in 80 cavities (group5-8). The prepared teeth were mounted in the customized tray with adjacent teeth to simulate clinical conditions and metallic matrix band (Sectional matrix) and wooden wedges were applied. After acid etching and application of Single Bond, each group was restored with the following materials using incremental placement technique: Group 1,5 (Filtek P60), group 2, 3, 4 and group 6, 7, 8 (AeliteFlo, TetricFlow, Revolution/Filtek P60). All specimens were thermocycled 500 times between 5℃ and 55℃ with 1 mimute dwell time, immersed 2% methylene blue dye for 24 hours and then rinsed with tab water. The specimens were embedded in clear resin and sectioned longitudinally through the center of restoration with a low speed diamond saw. Dye penetration at gingival margin was viewed at 20 margnification and analyzed on a scale of 0 to 4. Kruscal-Wallis One way analysis and Mann-Whitney Rank sum test were used to analyze the results. The results of this study were as follows. 1. The leakage values seen at the enamel margin were significantly lower than those seen at the dentin margin(P<0.05). 2. On the enamel margin, packable composite resins with flowable liners showed lower leakage than those without flowable liners, but there were no significant differences among the four groups(P>0.05). 3. On the dentin margin, four groups demonstrated moderate to severe leakage, and there were no significant differences in leakage values(P>0.05).
간접골성 고정원을 이용한 상악 구치부 원심이동 장치 종류에 따른 치아 이동 양상 평가

본 연구의 목적은 pendulum 장치, 미니임플란트를 동반한 pendulum장치(펜듈럼), 오픈코일 스프링 및 미니임플란트를 동반한 오픈코일 스프링 이용 시 각각의 치아이동 양상을 3차원적으로 분석하는 데 있었다. 상악 좌측 치조골 및 치아 모형을 제작하고, Calorific machine을 이용하여 모형상에서 대구치를 3 mm 원심이동시켰다. 실험은 5회씩반복 실시하였다. 모델을 전산화 단층 촬영한 후 V-Works를 이용하여 3차원 모델을 제작하였다. Rapidform상에서 3차원적으로 이동방향과 이동량을 계측하였고, 각각의 장치에 관한 통계적 유의성을 검정하였다. 교정용 미니임플란트를 간접 골성 고정원으로 이용하여 오픈코일 스프링으로 구치부를 원심으로 이동시켰을 때가 치체이동에 가장 가까운 이동양상을 보였고, 고정원 소실도 적게 나타났다 (p < 0.05). 오픈코일 스프링, 펜듈럼 장치 모두 미니임플란트를 부가적으로 이용했을 때 고정원 소실량이 적었다 (p < 0.05). 미니임플란트를 이용하지 않은 경우에는 두 장치의 고정원 소실량이 비슷하였다. 미니임플란트의 이용 여부와 관계없이 펜듈럼 장치로 상악 구치 원심 이동 시 오픈코일 스프링에 비해 제1대구치가 조절성 경사이동 양상으로 이동되었고 (p < 0.05), 제2대구치는 비조절성 경사이동 양상을 보였으며, 치관의 협측경사이동이 일어났다 (p < 0.05). 이와 같은 결과를 근거로 간접 골성 고정원을 이용한 오픈코일 스프링이 상악 구치의 원심 치체이동에 가장 효과적인 장치였으며, 펜듈럼 장치를 이용한 구치부 원심이동 시에는 추가적인 조절이 필요하다고 할 수 있다. Objective: The purpose of this study was to compare the three dimensional changes of tooth movement using four different types of maxillary molar distalization appliances; pendulum appliance (PD), mini-implant supported pendulum appliance (MPD), stainless steel open coil spring (SP) and mini-implant supported stainless steel open coil spring (MSP). Methods: These experiments were performed using the Calorific machine which can simulate dynamic tooth movement. Computed tomography (CT) images of the experimental model were taken before and after tooth movement in 1 mm thicknesses and reconstructed into a three dimensional model using V-works 4.0. These reconstructed images were superimposed using Rapidform 2004 and the direction and amount of tooth movement were measured. Results: The mean reciprocal anchor loss ratio at the first premolar was 17 - 19% for the PD and SP groups. The appliances using mini-implants (MPD or MSP) resulted in less anchorage loss (7 - 8%). On application of a pendulum appliance or MPD, distalization was obtained by tipping rather than by bodily movement. Furthermore, the maxillary second molar tipped distally and bucally. But on application of MSP, distalization was achieved almost by bodily movement. Conclusions: Regarding tooth movement patterns during molar distalization, stainless steel open coil spring with indirect skeletal anchorage was relatively superior to other methods.

Renal transplantation is the treatment of choice in management of patients with end stage renal disease. We have experienced the 2 cases of renal transplantation with complications successfully managed by medical treatment. One case of erythrocytosis and another case of severe, recurrent acute graft rejection were observed respectively and recovered by medical management. In all cases, triple immunosuppressive agents(prednisolone, azathioprine and cyclosporine) was administerd. In the first case, 54 days later after transplantation, the hematocrit of the patient has risen to 57%. With 2 trials of phlebotomy, hematocrit became to 48% 141 days later and persisted below the level of 48% after that time. In the second case, during 2 weeks after operation, there were 2 episodes of severe, acute rejections. With aggressive steroid pulse therapy, these rejection episodes were managed without renal dysfunction. We report the 2 cases of renal transplantation with complications successfully managed by medical treatment.

Background: This study's objective was to determine the prognostic value of the base deficit measured in the emergency department(ED) and to determine whether base deficit can provide information not provided by advanced injury scoring system. Methods: This study was a retrospective analysis of data collected for two years. Thirty-two severe trauma patients who were admitted to the Emergency Department of Korea University Hospital were included in this study. The patients were divided into two groups: the normal base deficit group(-3 mmol/L to 3 mmol/L) and the elevated base deficit group(>3 mmol/L). The base-deficit value, age, sex, head injury, organ failure rate, and survival were considered, and the also Revised Trauma Score(RTS), Acute Physiology And Chronic Health Evaluation(APACHE II), and Injury Severity Score(ISS) were measured. Result: The elevated base-deficit group showed a lower survival rate and a higher organ failure rate compared to the normal base deficit group. Logistic Regression showed a strong association between base deficit and mortality rate. Among the previous injury scoring Systems(RTS, APACHE II, ISS), base deficit had the strongest correlation with RTS. By using base deficit value and the RTS together, we obtained a higher positive predictive value than that obtained by using base deficit or RTS alone. Conclusion: The admission value of the base deficit in the ED is a useful tool in predicting the outcome in severe trauma patients, and it can be an adjunct to previous injury scoring systems. As an advanced injury scoring System is developed in the future, the base deficit may have some significant role.
후두 편평세포암종에서 Galectin-1 및 -7의 발현 의의

Background and Objectives: The role of different Galectins in the pathogenesis of different types of malignancy is being profoundly investigated recently. In this study, the author investigated the level of Galectin-1 and -7 in the layngeal squamous cell carcinoma tissue to define its relationship to the tumor progression. Materials and Methods: Paraffin-embedded tissue specimens from 77 patients, who were diagnosed as laryngeal squamous cell carcinoma from 1993 to 2007, were immunohistochemically stained for Galectin-1 and -7. Results: Galectin-1 expression was positively correlated with tumor stage and nodal involvement. Galectin-7 expression was increased in better differentiated tumors. Conclusion: Expression of Galectin-1 and -7 can be used as a valuable indicator in laryngeal squamous cell carcinoma under certain limitations.

Background and Objectives : COX-2, the inducible form of cyclooxygenase(COX), is upregulated in transformed cells and in malignant cells, which have important roles in promotion of colorectal carcinogenesis, invasiveness and angiogenesis. Vascular endothelial growth factor(VEGF) may act as endothelial cell mitogen in various cancer tissues. It will increase tumor growth and angiogenesis. Aims of this study were to asses COX-2/VEGF expression and it's clinical correlation in sinonasal tumors and to investigate the effects on angiogenesis also. Materials and Method : The study samples were obtained from surgical removal of 27 cases of inverted papilloma(IPs), 5 cases of IP with malignant transformation(IP-MT), 16 cases of Squamous cell carcinoma(SCC) in sinonasal cavity COX-2 and VEGF expressions were assessed by immunohistochemical staining. Synchronism of COX-2/VEGF expression in one tumor cell was demonstrated with double immunofluorescence technique. Results : The expression rates of COX-2 were 37% in IP, 80% in IP-MT and 100% in SCC. The positive rates of VEGF were 29 6% in IP, 100% in IP-MT and SCC. COX-2 and VEGF expressions were increased as tumor progressed, and there is a positive correlation between COX-2 &EGF expressions. All clinical features which were compared are not associated with COX-2 &EGF expressions except malignant change in IP(p=0 028). Synchronism of COX-2/VEGF expressions was noted in all positive immunostaining samples. Conclusion : These findings suggest the role of COX-2 pathway could be involved in sinonasal tumor angiogenesis, which is modulated by VEGF COX-2 may act as autocrine factor in VEGF expression. There is a potential role for selective COX-2 inhibitors in the treatment of these lesions.
