        참가사리 분획물의 암 예방효과

        본 연구에서는 해조류 중 홍조류에 속하는 참가사리를 추출, 분획하여 항발암효과를 측정하였다. 참가사리 분획물을 4종의 암세포주 HT29, HepG2, MCF-7 및 B16-F10에 처리하였을 때 암세포 증식억제실험을 한 결과 대장암세포주인 HT29의 경우 GTMM층과 GTMB층에서 농도의존적인 효과가 나타났으며 첨가농도 150 ㎍/mL에서 각각 93.64%와 74.14%의 수치를 보였다. 간암세포주인 HepG2의 경우 150 ㎍/mL 첨가 시 GTMM층과 GTMB층이 각각 95.97%, 81.78%의 높은 증식억제효과를 보였으며 유방암세포주인 MCF-7에서는 GTMM층이 120 ㎍/mL과 150 ㎍/mL 첨가농도에서 각각 88.50%와 91.82%의 높은 암세포증식 억제효과를 나타냈다. 피부암세포주인 B16-F10은 다른 세포주에 비해 미약하나 역시 GTMM층이 최종첨가농도에서 86.20%의 억제를 나타내었다. 이와 같이 4종의 암세포주의 증식억제는 전반적으로 GTMM층과 GTMB층에서 높은 효과가 나타남을 알 수 있다. 한편 Western blot analysis를 통해 간암세포주인 HepG2에서 GTMM층의 처리에 따른 pro-apoptosis Bcl-2 family의 발현증가를 볼 수 있었으며, PARP 분절과 연관된 caspase-3, 7의 활성화를 확인할 수 있었다. 이처럼 GTMM층은 apoptosis의 작용에 의한 세포사멸을 유도하며, 이러한 apoptosis의 기전으로는 최소한 caspase의 활성화에 따른 절단 PARP 단백질의 증가, Bad, Bax 등의 apoptosis 관여인자가 작용하고 있음을 확인할 수 있었다. HepG2를 이용한 quinone reductase 유도활성여부를 측정한 결과 GTMM층이 45 ㎍/mL 및 60 μg/mL의 시료첨가농도에서 대조군에 비해 각각 2.34, 2.86배의 QR유도활성을 나타내었으며, GTMB층의 경우 최종첨가농도인 60 ㎍/mL에서 2.04 배의 효소활성을 나타내었다. In this study, we investigated anticarcinogenic effects of extracts from Gloiopeltis tenax (GT). GT was extracted with methanol (GTM), which was then further fractionated into four fractions by using solvent fractionation method, affording methanol (GTMM), hexane (GTMH), butanol (GTMB) and aqueous (GTMA) soluble fractions. We determined the cytotoxic effects of these fractions on cancer cells by MTT assay. Among various fractions of GT, the GTMM showed the strongest cytotoxic effect at concentration of 150 ㎍/mL, displaying 95.97% on HepG2 cell lines and 93.64% on HT?29 cell lines, respectively. And, the anti?proliferative effect of GT was accompanied by a marked in increase of levels of Bad, Bax, Bok and Bak protein and activation of caspase?3, caspase?7 and PARP protein. Also, we observed quinone reductase (QR) induced effects in all fraction layers of GT on HepG2 cells. The QR induced effects of the GTMM and GTMB on HepG2 cells at concentration of 60 ㎍/mL showing inductive indexes of 2.86 and 2.04 compared to the control value of 1.0.

      • Ergonomic-friendly total laparoscopic hysterectomy versus conventional abdominal hysterectomy: surgical outcomes

        Objective: Ergonomic constraints of surgeons have been a pivotal issue in minimally invasive surgery. The repetitive motion injuries can occur when surgeons are improperly positioned or uncomfortable while performing surgery. We sought to compare surgical outcomes between ergonomic-friendly total laparoscopic hysterectomy (EF-TLH) that aims to ease the physical workload of laparoscopists and conventional abdominal hysterectomy (TAH). Methods: A retrospective review of 1970 patients who underwent ergonomic-friendly total laparoscopic hysterectomy (n=909) or conventional abdominal hysterectomy (n=1061) between July 2013 to February 2019 was done. The ergonomically friendly setting of laparoscopic surgery begins with adjustment of operator seating position with minimal musculoskeletal pressure and flexibility of port site locations that are suitable for each operation. Results: Both groups shared similar patients’ characteristics including age, BMI, parity, pre-operative hemoglobin and previous abdominal surgery. EF-TLH group showed shorter operating time (P<0.001), less estimated blood loss (P<0.001), less post-operative hemoglobin drop (P=0.009), and fewer cases of transfusion during the operation (P<0.001). In the TAH group, the weight of the uterus was heavier, which is assumed to inevitably have more open cases as the uterus is larger. Conclusion: EF-TLH can be practical substitution for TAH with less operating time and blood loss when a hysterectomy is considered to be done laparoscopically while supporting optimal position of laparoscopists during surgery.

      • Da Vinci SP single-port robot-assisted radical trachelectomy and sentinel lymph node mapping

        Objective: Demonstrate a new instrument, technique or procedure Target: Adanced practicioners Methods: 31-year-old single woman who was diagnosed with poorly differentiated invasive squamous cell carcinoma on cervical biopsy was scheduled for robot-assisted trachelectomy for fertility-sparing. She showed no sign of gross mass or distant metastases on MRI and PET-CT imaging and clinical staging was IB1. In order to minimize complication and still retain radicality, the sentinel lymph node mapping with indocyanine green was planned. About 2.5cm long incision was made on umbilicus and we used GelPOINT Mini platform to insert a trocar for Da Vinci SP system as well as 12mm assistant trocar. During the operation, a fenestrated bipolar forcep, cautery spatula, ProGrasp forcep, and Mega Needle driver were used on Da Vinci SP system. In order to refer to sentinel node imaging, a monitor connected to PINPOINT endoscopic fluorescence imaging camera was placed next to robot console. After inserting uterine manipulator (RUMI), indocyanine green (ICG: 1.25mg/mL) was injected 1mL each in superficial and deep uterine cervix at 3 and 9 Oclock direction. A 5mm PINPOINT endoscopic scope was applied through assistant port and allowed real-time visualization of sentinel lymph node. Bilateral pelvic sentinel lymph nodes were identified on PINPOINT monitor and dissection was done with Da Vinci SP system. Uterine artery braches were dissected and only cervical and vaginal braches of uterine arteries were ligated. After skeletonizing both ureters, parametrium was excised and colpotomy was performed. The pathologic part of cervix was cut out and frozen biopsy on the resection margin was sent to pathology. With 1-0 Proline, cervical cerclage was done on the isthmic portion. Then the uterus and vaginal cuff was sutured with Quill 1-0 by single continuous manner. Results: Fertility-sparing trachelectomy with sentinel lymph node mapping was successfully executed using Da Vinci SP system and PINPOINT as assisting imaging device. The patient was discharged two days after the surgery without complication and showed no sign of recur after 6 months. Conclusion: Assisting imaging with near-infrared fluorescence camera with Da Vinci SP system can be efficient substitution for previous Da Vinci robot systems with multi-port arms while limiting the scar only to umbilicus and minimizing surgical complication by sentinel lymph node dissection.

      • Ergonomic-friendly total laparoscopic hysterectomy versus conventional laparoscopic hysterectomy for preinvasive cervical disease

        Objective: To demonstrate ergonomic-friendly surgical approach that aims to ease the physical workload of laparoscopists for total laparoscopic hysterectomy and compare surgical outcomes between ergonomic-friendly total laparoscopic hysterectomy (EF-TLH) and conventional total laparoscopic hysterectomy for preinvasive uterine disease. Methods: As the field of minimally invasive surgery grows, laparoscopists are exposed to risk of developing musculoskeletal disease due to increase in physical workload. The repetitive motion injuries occur when surgeons remain static position or improperly positioned while performing surgery. We sought to minimize physical burden to laparoscopists while performing total laparoscopic hysterectomy which is one of the most common gynecological surgery. Also, a retrospective review of 277 patients with pre-invasive uterine disease who underwent ergonomic-friendly total laparoscopic hysterectomy (n=150) or conventional total laparoscopic hysterectomy (n=81) between July 2013 to May 2019 (Table 1) was done. Preinvasive uterine disease included all stages of cervical intraepithelial neoplasia as well as pathologically-proven carcinoma in situ and patients with underlying benign uterine condition such as uterine myoma or adenomyosis were excluded. Results: In terms of surgical outcome of two groups, operating time was significantly shorter in EF-TLH group and estimated blood loss was much less in EF-TLH group. Post-op hemoglobin drop and transfusion during operation showed no significant difference between two groups. Conclusion: EF-TLH is efficient substitution for conventional TLH under similar circumstances with less operating time and blood loss when a hysterectomy is considered to be done while supporting optimal position of laparoscopists during surgery.

        A New Approach to Sport Public Diplomacy through National Role Conceptions in Governance Networks: A Case Study of the 2018 PyeongChang Winter Olympics

        Given the lack of empirical research on the links between public diplomacy (PD) and sporting events, this study aimed to conceptualize, as an analytical framework, a model of National Role Conceptions (NRC) for Sport Public Diplomacy (Sport PD) in network governance. Considering the PyeongChang Winter Olympics as a case study, this paper reveals four distinct articulations of Sport PD: (1) convertibility of soft power sources to governmental behavior; (2) a cohesive national identity with cultural prestige and national appeal; (3) mechanisms of global sport governance; (4) national roles, not necessarily confined to national identity, undertaken according to diplomatic purposes, as shown in the specific case of South Korea. These purposes included Motivator for global peace, Signifier of pan-Koreanism, and Rule-Changer of geopolitical power structure, according to their respective points in time (before, during, and after the Olympics). In addition, this paper reveals the limitations of Sport PD and concludes with some recommendations on inter-Korean relations.
