Strangulated Obturator Hernia 및 Internal Hernia 1예의 경험

An obturator hernia is one of the rarest forms of hernia. Since first successful operation by Henre Obre in 1851, almost 600 cases have been reported in the Foreign literature. Because of a hidden hernia and nonspecific manifestation, the preoperative detection is very difficult. Recently, we experienced one case of strangulated obturator hernia in a 86 years female patient who was not diagnosed preoperatively and combined with internal hernia and acalculous cholecystitis. So, we report it with review of literature.

Acute aortic occlusion is rare but poses a high mortality and morbidity. Prompt diagnosis and treatment is essential. Typical presentation is rest pain with bilateral absent femoral pulse. When the non-typical symptoms predominate such as paraplegia, acute abdomen or sudden-onset hypertension, diagnosis may be difficult. We experienced a case of acute aortic thrombosis who suddenly developed paraplegia and rapidly deteriorated within several hours. When the paraplegia developed, he was under treatment with heparin and urokinase, was thrombocytopenic, and the femoral arteries were pulsatile. Spinal cord compression due to bleeding complication had to be ruled out. Duplex sonography and lumbar CT scan were not diagnostic. Aortic thrombosis was diagnosed by abdominal CT scan. This case illustrates the need for high suspicion of acute aortic occlusion presenting with paraplegia especially in patients with associated atherosclerotic disease.
일란성 쌍태아의 치루 : A Congenital Etiology 증례보고 및 원인적 고찰

Fistula in Ano in the infant and children has certain characteristics that seem to seperate them from their adult counterpart. These are a preponderance in the first year of life, almost exclusive male predominance, simple superficial character and straight course of the tracts. These different findings lead to the need for etiological explanation. We had an experience of anal fistula in a pair of monozygotic twin who shows unique findings. Base on these findings, we presume that some environmental effect in utero causes the formation of the abnormal glands which are vulnerable for gross infection after birth, and these abnormal anal glands are inclined to distribute in the anterolateral portion of anal canal. These findings support the congenital etiology of fistula in ano in the infants.
