Objectives : Patient's behavior features are important factors which influences the clinical judgement including diagnosis. However, most psychiatrists build up a picture of patients' behavior from an amalgamation of their own brief observations and nurses' reports, which often lack in the objectiveness. Several behavioral scales have been developed to alleviate this difficulty, but the poor efficiency and reliability of these scales have made them less useful. The recently developed Ward Daily Behavior Scale is an objective tool for evaluating all the daily noteworthy behaviors of patients, and is easily applicable to wide ranges of diagnoses and ages. This study tried to prove the reliability and validity of the Ward Daily Behavior Scale-Korean version. Methods : The 112 patients, 63 males and 49 females, at a chronic psychiatric inpatient ward were selected as subjects. Experienced and unexperienced nurses rated patients' behaviors independently with the Ward Daily Behavior Scale-Korean version, after observing behaviors of subjects for 8 hours during day duty time. And then we tested the inter-rater reliability, internal consistency, and concurrent validity of this scale. Results : The Ward Daily Behavior Scale-Korean version proved to be both reliable and valid for measuring of behaviors of psychiatric inpatients. Conclusions : The Ward Daily Behavior Scale-Korean version will be a valuable tool to observe and quantify patients' behavior in psychiatric wards.

<P>Bucillamine is used for the treatment of rheumatoid arthritis. This study investigated the protective effects of bucillamine against cisplatin-induced damage in auditory cells, the organ of Corti from postnatal rats (P2) and adult Balb/C mice. Cisplatin increases the catalytic activity of caspase-3 and caspase-8 proteases and the production of free radicals, which were significantly suppressed by pretreatment with bucillamine. Bucillamine induces the intranuclear translocation of Nrf2 and thereby increases the expression of γ-glutamylcysteine synthetase (γ-GCS) and glutathione synthetase (GSS), which further induces intracellular antioxidant glutathione (GSH), heme oxygenase 1 (HO-1) and superoxide dismutase 2 (SOD2). However, knockdown studies of HO-1 and SOD2 suggest that the protective effect of bucillamine against cisplatin is independent of the enzymatic activity of HO-1 and SOD. Furthermore, pretreatment with bucillamine protects sensory hair cells on organ of Corti explants from cisplatin-induced cytotoxicity concomitantly with inhibition of caspase-3 activation. The auditory-brainstem-evoked response of cisplatin-injected mice shows marked increases in hearing threshold shifts, which was markedly suppressed by pretreatment with bucillamine <I>in vivo</I>. Taken together, bucillamine protects sensory hair cells from cisplatin through a scavenging effect on itself, as well as the induction of intracellular GSH.</P>

<P>In the present study, we investigated the cytoprotective efficacy of morin, a natural flavonoid, against oxidative stress and elucidated the underlying mechanisms in C2C12 myoblasts. Our results indicated that morin treatment prior to hydrogen peroxide (H2O2) exposure significantly increased cell viability and prevented the generation of reactive oxygen species. H2O2-induced comet-like DNA formation and gamma H2AX phosphorylation were also markedly suppressed by morin with a parallel inhibition of apoptosis in C2C12 myoblasts, suggesting that morin prevented H2O2-induced cellular DNA damage. Furthermore, morin markedly enhanced the expression of heme oxygenase-1 (HO-1) associated with the induction and phosphorylation of nuclear factor-erythroid 2-related factor 2 (Nrf2) and the inhibition of Kelch-like ECH-associated protein 1 (Keapl) expression. Notably, these events were eliminated by transient transfection with Nrf2-specific small interfering RNA. Additional experiments demonstrated that the activation of the Nrf2/HO-1 pathway by morin was mediated by the extracellular signal-regulated kinase (ERK) signaling cascade. This phenomenon was confirmed with suppressed Nrf2 phosphorylation and consequently diminished HO-1 expression in cells treated with a pharmacological inhibitor of ERK. Collectively, these results demonstrated that morin augments the cellular antioxidant defense capacity through the activation of Nrf2/HO-1 signaling, which involves the activation of the ERK pathway, thereby protecting C2C12 myoblasts from H2O2,-induced oxidative cytotoxicity.</P>

We report a case of tsutsugamushi disease in a 26 year-old pregnant woman who was treated with azithromycin. Her gestation period was 27 weeks and she admitted with fever, rash, and eschar on the right shoulder. Currently recommended medications for the treatment of scrub typhus are doxycycline or chloramphenicol. But, these drugs are class D drugs according to the U.S. Food and Drug Administration (FDA) Fetal Risk Summary, so they couldn't be used to treat pregnant women. Recently, a few case reports suggested that azithromycin, a relatively new macrolide antibiotic, was effective and safe for the treatment of scrub typhus in pregnant women. And, there is no evidence that azithromycin causes harmful effects to the developing fetus or to children. On the basis of current in vivo test that confirms the effectiveness of azithromycin, it may be the drug of choice for the treatment of scrub typhus in pregnant women and children. <P>Samul-Tang (Si-Wu-Tang, SMT), composed of four medicinal herbs, is a well-known herbal formula treating hematological disorder or gynecologic disease. However, vascular protective effects of SMT and its molecular mechanisms on the vascular endothelium, known as the central spot of vascular inflammatory process, are not reported. The aim of this study was to investigate vascular protective effects of SMT water extract in human umbilical vein endothelial cells (HUVECs). Water extract of SMT was prepared and identified by HPLC-PDA analysis. Expression of cell adhesion molecules (CAMs) and heme oxygenase-1 (HO-1) and translocation of nuclear factor-kappa B (NF-<I>κ</I>B) and nuclear factor-erythroid 2-related factor 2 (Nrf2) were determined by western blot. Nuclear localization of NF-<I>κ</I>B and Nrf2 was visualized by immunofluorescence and DNA binding activity of NF-<I>κ</I>B was measured. ROS production, HL-60 monocyte adhesion, and intracellular nitric oxide (NO) were also measured using a fluorescent indicator. SMT suppressed NF-<I>κ</I>B translocation and activation as well as expression of CAMs, monocyte adhesion, and ROS production induced by TNF-<I>α</I> in HUVECs. SMT treated HUVECs showed upregulation of HO-1 and NO which are responsible for vascular protective action. Our study suggests that SMT, a traditionally used herbal formula, protects the vascular endothelium from inflammation and might be used as a promising vascular protective drug.</P>

low bone mineral density (BMD). The aim of this study was to identify the relationship with metabolic syndrome by BMD of femur in postmenopausal women. Methods: We evaluated 120 postmenopausal women who were visiteda health promotion center in a university hospital from January, 2008 to December, 2009. Data on their current medical diseases and medications were collected through a physician's history taking. Also, basic physical examinations and laboratory tests were performed on these subjects. Results: In adjusted analysis including age and other factors, only waist circumference had a close correlation with BMD of femur (P< 0.05). The BMD of femur had no correlation with other risk factors of metabolic syndrome. When stratified by BMD of femur, corrected BMD of femur revealed no significant correlation with the presence of metabolic syndrome. Conclusion: The BMD of femur among postmenopausal women with metabolic syndrome shows a positive tendency in abdominal obesity

The metachromatic behavior leading to the formation of aggregation between organic dyestuffs and anionic surfactants was investigated spectroscopically by varying the concentration with the addition sufactants. The appearance of a new band(meta-band) is explained as a high aggregation of organic dye molecular at a range 300 through 400 S/D value in dye-surfactant complex system. The deminishing of a meta-band at the above 700 of S/D value due to attribute to redistribution and the formation of micelle by the drim.
Doxylamine Succinate 과다복용 환자에서 발생한 횡문근융해증 1예

Doxylamine succinate is an antihistamine, classified as hypnotics and sedatives by the Ministry of Health and Welfare, and it can be obtained easily as an over-the-counter medicine. After doxylamine overdose, the anticholinergic effects of this drug can also lead to nausea, vomiting, and ileus as well as urinary retention, and patients often have dry mouth, absence of sweating, flushing, fever, and dilated pupils. Although Rhabdomyolysis seems unusually common with doxylamine poisoning, it can lead to acute renal failure and serious complications. A careful observation is needed, because disseminated intravascular coagulation, metabolic acidosis, acute cardiomyopathy, respiratory distress, compartment syndrome and peripheral neuropathy can occur in doxylamine intoxication. We report a case of rhabdomyolysis after doxylamine overdose.

이 연구는 경찰관의 가치의식을 통하여 우리나라 경찰조직의 윤리적 분위기와 경찰비리의 사회화를 실증적으로 분석하고, 경찰행정의 윤리성 제고를 위한 실천적 함의를 모색한다. 분석 결과, 우리나라 경찰관들은 경찰비리의 윤리적?법적 차원을 잘 이해하고 있으나 인권의식, 동료의 비행, 절도 행위에 대한 윤리적 민감성이 부족하고, 경찰 내 불신의 문화가 보편화되어 있으며, 경찰비리가 조직 내 사회화 과정을 통해 발전할 수 있음을 확인하였다. 이 연구는 경찰조직의 윤리성 제고를 위한 실천적 과제로서 간부후보생을 대상으로 한 리더십 윤리교육의 강화와 인사행정 전반에 걸친 윤리성 제고 노력, 그리고 조직의 윤리적 분위기를 개선하기 위한 문화사업의 전개를 촉구한다.
