      • Cathepsin D가 타액선종양에 미치는 영향에 관한 면역조직화학적 연구

        The purpose of the present study was to evaluate the localization of cathepsin D, and then further to prove the correlation with other clinical parameters, and to survey carcinogenesis and the role of various type of the tumor cells in the salivary gland tumors. 5 cases of normal minor salivary gland as controls, 14 cases of pleomorphic adenoma, 7 cases of mucoepidermoid carcinoma, 4 cases of adenoid cystic carcinoma, 1 case of acinic cell carcinoma and 4 cases of adenocarcinoma NOS from files of Dept. of Oral Pathology, School of Dentistry, Kyung Hee University were reexamined. The tissue sections obtained from paraffin-embedded blocks were stained with Hematoxylin-eosin, and immunostained with rabbit anti-human cathepsin D antibody using labeled streptavidin-biotin complex peroxidase method. The results were as follows : 1. The expression of cathepsin D revealed more frequently in malignant salivary gland tumors than that in benign salivary gland tumors, and particularly marked in mucoepidermoid carcinoma. 2. 11 cases of 14 cases of pleomorphic adenoma were positive immunoreactivity cathepsin D, and chiefly localized in ductal cells and tumor cell proper. 3. 7 cases of mucoepidermoid carcinoma were positive immunoreactivity to cathepsin D. Because this protease localized generally in the intermediate cells and the lining cells of pseudocysts, it suggests that these cells could have different biologic behavior compared with mucous cells, epidermoid cells and clear cells. 4. 3 cases of 4 cases of adenoid cystic carcinoma were positive immunoreactivity to cathepsin D. As the cathepsin D localized in peripheral cells of tumor parenchymes in a case of the recurred lesion, it proposes that tumor cells could more progressively infiltrate into adjacent stroma. 5. In one case of acinic cell carcinoma, cathepsin D showed positive immunoreactivity weakly in the clear cells. 6. 3 cases of 4 cases of adenocarcinoma NOS were positive immunoreactivity to cathepsin D. And tumor cells revealing abnormal mitotic features and pleomorphism were more markedly immunoreactive response.

        The purpose of the present study was to evaluate the expression of c-erbB-2 oncoprotein and localization of cathepsin D, and then further to prove the correlation with other clinical parameters, and to survey carcinogenesis and the role of various type of the tumor cells in the salivary gland tumors. A total of 30 salivary gland tumors(pleomorphic adenoma, n=14 ; mucoepidermoid carcinoma, n=7 ; adenoid cystic carcinoma, n=4 acinic cell carcinoma, n=1 ; adenocarcinoma, n=4) and 5 cases of normal minor salivary- gland as controls were retrieved from files of Dept. of Oral Pathology, School of Dentistry, KyungHee University. The tissue sections obtained from paraffin-embedded blocks were stained with Hematoxylin-eosin, and imrnunostained with rabbit anti-human c-erbB-2 oncoprotein antibody and rabbit anti-human cathepsin D antibody using labeled streptavidin-biotin complex peroxidase method. The results were as follows ; 1. 14 cases of pleomorphic adenoma were stained negatively to c-erbB-2 oncoprotein. But 11 cases of them were positive immunoreactivity to cathepsin D, and chiefly localized in ductal cells and tumor cell proper. 2. 3 cases and 7 cases of mucoepidermoid carcinoma were positive immunoreactivity to c-erbB-2 oncoprotein and cathepsin D, respectively. And tumor cells revealing abnormal mitotic features and pleomorphism were more markedly immunoreactive response. 6. After all, c-erbB-2 oncoprotein and cathepsin D partly seem to play an important role in the initiation and progression of salivary gland tumors or in the malformation of glandular cells. The case of only cytoplasmic immunoreactivity without membranous immunoreaction to c-erbB-2 oncoprotein was showed mostly in the ductal cells of normal salivary gland, benign salivary gland tumor and malignant salivary gland tumor. For that reason it suggests that cytoplasmic immunoreactivity could have no connection with progression and maintenance of salivary gland tumors.

        임신과 연관된 유방삽입물 주의 피막 구형 구축의 변화에 대한 임상적 연구

        It has been reported that the frequency of the development of peripheral capsuler contracture after breast augmentation surgery using silicone-gel implants amounts between 25 to 50%. Various treatments to cure or prevent the capsular contracture are mow being studied and introduced, including submuscular insertion of breast implants, steroid instillation, oral intake of Vit-E, postoperative breast massage, regional injection of antibiotics, and a saline-filled implantation without gel-bleed. This paper reports, along with the literal study, on the improvement of the softening of capsular contracture condition observed from 12 patients who have undergone pregnancy and delivery after breast augmentation surgery operated at the Plastic surgery departments of Ewha University Medical Center and Seoul National University Medical Center during 1985-1995. Conceivably, the softening of the capsular contracture is attributable to the hormonal change, immunologic and inflammatory alteration, and the masking effect resulting from mammary alveolar hypertrophy. It is believed that a study should be continued on the relationship between the pregnancy and the softening of capsular contracture condition.

        Adverse pregnancy outcomes with assisted reproductive technology in non-obese women with polycystic ovary syndrome: a case-control study

        Objective: To investigate adverse pregnancy outcomes in non-obese women with polycystic ovary syndrome (PCOS) compared with obese-PCOS and control groups. Methods: Women with PCOS who underwent assisted reproductive technology (ART) from August, 2003 to December, 2007, were considered. A total of 336 women with PCOS were included in the study group and 1,003 infertile women who had tubal factor as an indication for ART were collected as controls. They were divided into four groups: a non-obese PCOS group, obese-PCOS group, non-obese tubal factor group, and obese tubal factor group, with obesity defined by a body mass index over 25 kg/$m^2$, and reviewed focusing on the basal characteristics, ART outcomes, and adverse pregnancy outcomes. Results: There was no difference among the groups' the clinical pregnancy rate or live birth rate. Regarding adverse pregnancy outcomes, the miscarriage rate, multiple pregnancy rate, and prevalence of preterm delivery and pregnancy induced hypertension were not different among the four groups. The incidence of small for gestational age infant was higher in the PCOS groups than the tubal factor groups ($p$ <0.02). On the other hand, the morbidity of gestational diabetes mellitus (GDM) was not high in the non-obese PCOS group but was in the obese groups. And in the obese PCOS group, the newborns were heavier than in the other groups ($p$ <0.02). Conclusion: Non-obese PCOS presents many differences compared with obese PCOS, not only in the IVF-parameters but also in the morbidity of adverse pregnancy outcomes, especially in GDM and fetal macrosomia.

        Complete genome sequencing of <i>Shigella</i> sp. PAMC 28760: Identification of CAZyme genes and analysis of their potential role in glycogen metabolism for cold survival adaptation

        <P><B>Abstract</B></P> <P> <I>Shigella</I> sp. PAMC 28760 (isolated from <I>Himantormia</I> sp. lichen in Antarctica) is a gram-negative, non-sporulating bacterium that has cellulolytic and amylolytic characteristics as well as glycogen metabolic pathways. In this study, we isolated <I>S.</I> sp. PAMC 28760 from Antarctic lichen, and present the complete genome sequence with annotations describing its unique features. The genome sequence has 58.85% GC content, 4,278 coding DNA sequences, 85 tRNAs, and 22 rRNA operons. 16S rRNA gene sequence analyses revealed strain PAMC 28760 as a potentially new species of genus <I>Shigella</I>, showing various differences from pathogenic bacteria reported previously. dbCAN2 analyses revealed 91 genes related to carbohydrate-metabolizing enzymes. <I>S.</I> sp. PAMC 28760 likely degrades polysaccharide starch to obtain glucose for energy conservation. This study provides a foundation for understanding <I>Shigella</I> survival adaptation mechanisms under extremely cold Antarctic conditions.</P> <P><B>Highlights</B></P> <P> <UL> <LI> <I>Shigella</I> sp.         Effects and safety of COVID-19 vaccination on assisted reproductive technology and pregnancy: A comprehensive review and joint statements of the KSRM, the KSRI, and the KOSAR

        Humanity is in the midst of the coronavirus disease 2019 (COVID-19) pandemic, and vaccines-including mRNA vaccines-have been developed at an unprecedented speed. It is necessary to develop guidelines for vaccination for people undergoing treatment with assisted reproductive technology (ART) and for pregnancy-related situations based on the extant laboratory and clinical data. COVID-19 vaccines do not appear to adversely affect gametes, embryos, or implantation; therefore, active vaccination is recommended for women or men who are preparing for ART. The use of intravenous immunoglobulin G (IVIG) for the treatment of immune-related infertility is unlikely to impact the effectiveness of the vaccines, so COVID-19 vaccines can be administered around ART cycles in which IVIG is scheduled. Pregnant women have been proven to be at risk of severe maternal and neonatal complications from COVID-19. It does not appear that COVID-19 vaccines harm pregnant women or fetuses; instead, they have been observed to deliver antibodies against severe acute respiratory syndrome coronavirus 2 (SARSCoV-2) to the fetus. Accordingly, it is recommended that pregnant women receive COVID-19 vaccination. There is no rationale for adverse effects, or clinical cases of adverse reactions, in mothers or neonates after COVID-19 vaccination in lactating women. Instead, antibodies to SARS-CoV-2 can be delivered through breast milk. Therefore, breastfeeding mothers should consider vaccination. In summary, active administration of COVID-19 vaccines will help ensure the safe implementation of ART, pregnancy, and breastfeeding.

        Enhancement of Optic Nerve in Leukemic Patients: Leukemic Infiltration of Optic Nerve versus Optic Neuritis

        Purpose: To identify magnetic resonance imaging (MRI) findings of leukemic infiltration of optic nerve and optic neuritis in leukemic patients with emphasis of clinical findings as reference standard to differentiate them. Materials and Methods: MRI and clinical findings of 7 patients diagnosed as leukemic infiltration of optic nerve (n = 5) and optic neuritis (n = 2) in our institution between July 2006 and August 2015were reviewed retrospectively. In particular, MR imaging findings involved perineural enhancement and thickening of optic nerve and its degree, signal intensity, laterality (unilateral/bilateral), intraconal fat infiltration and its degree, and associated central nervous system abnormalities. Results: Of 5 cases of leukemic infiltration of optic nerve, 4 cases showed positive cerebrospinal fluid (CSF) study for leukemia relapse and 1 case was positive on bone marrow (BM) biopsy only. Moreover, of 5 leukemic infiltration of optic nerve, 2 cases showed the most specific MR findings for leukemic central nervous system involvement including 1 prominent leptomeningeal enhancement and 1 chloroma. However, other MR imaging findings of the patients with leukemic infiltration or optic neuritis such as thickening and perineural enhancement of optic nerves are overlapped. Conclusion: Enhancement and thickening of optic nerve were overlapped MR findings in leukemic infiltration of optic nerve and optic neuritis. Our findings suggest that enhancing optic nerve thickening with associated central nervous system MR abnormality favors the diagnosis of leukemic infiltration of optic nerve, especially in patients with history of acute lymphoblastic leukemia. However, CSF and BM study were required for differentiation between leukemic infiltration of optic nerve and optic neuritis.
