The Epidemiological and General Characteristics of Asymptomatic COVID-19 Confirmed Patients

Purpose One of the main characteristics of COVID-19 is that it is transmitted SARS-CoV-2 even during asymptomatic period. Asymptomatic features of COVID-19 are the biggest stumbling block to eradication, we want to identify these characteristics and help preventive strategies. Method From January 2020 to August, 2021, a confirmed cases who were asymptomatic at the time of diagnosis were selected among residents of Seoul and identified the epidemiological and general characteristics. Result Among the total confirmed cases in Seoul, 27.2% have no symptom, and the rate of no symptom has been on the decline. It tends to be more men than women, and average age is 44.4 years. By age, people in 50s were the most at 18.2%, but recently, the ratio has been increasing in 20s. According to the cumulative infection path status of them, it was followed by contact with confirmed cases(45.5%), investigation of infection path(16.1%), work(5.9%), medical(5.6%) and religious institutions(4.7%). Since four months ago, the proportion of confirmed cases(57.7%) and investigating infection routes(22.0%) have been increasing, while medical(2.2%) and religious institutions(1.3%) have been decreasing. The mean Ct value(23.9) of asymptomatic confirmed cases was higher than the overall Ct value(19.5) of cases within the same period. From May 2021 to recently, the percentage of variant cases was 5.8%, lower than the total percentage of confirmed cases within the same period, 9.7%. And the rate of asymptomatic infection among breakthroughs was 30.8% and the total rate of asymptomatic infection within the same period was 27.2%. Conclusion The percentage of asymptomatic cases of COVID-19 in Seoul is on the decline, but it is still putting a strain on prevention by increasing the residual amount of infection in the community. In particular, as the proportion of the 20s with high activity is increasing, quarantine measures are required for early diagnosis, including the expansion of preemptive inspections.

Purpose This study conducted a retrospective study on 222 homeless patients hospitalized with TB using the communitybased homeless shelter named MISOKKUMTEO and 496 people who returned to the community after discharge, and discussed the role of MISOKKUMTEO by analyzing the cost-effectiveness and the survival rate. Method From January 2012 to December 2019, the data set was established using the data of MISOKKUMTEO for the homeless and the medical records of Seobuk. Result First, the total cost of medical treatment was high for patients who used MISOKKUMTEO. The higher the age of the outbreak, the higher the total cost of medical treatment, and the lower the total cost of medical treatment when married than single. Theoretical consideration showed the same Results as the family DOT saved treatment costs. The higher the BMI, the higher the total cost of care. Second, the total cost of care was found to have been higher for patients using MISOKKUMTEO, but the survival rate was higher for patients using there. When the total cost of care was 17, the cumulative survival rate was about 66%. Third, patients who enter the MISOKKUMTEO and leave the hospital are about 0.17 times more likely to die of TB than patients who did not use the there. In other words, the risk of death was reduced by 83%. Fourth, it was confirmed that patients who used Misoccumtae had a longer survival period than patients who did not use Misoccumtae, and that the total cost of medical treatment was less. In addition, it can be seen that for patients using MISOKKUMTEO, the cost of care is reduced as their lifespan increases. Conclusion The TB shelter admission is an alternative to patients who do not use facility because it costs less for patients who pass through there and the survival rate increases.
Effect of Chatbot Based Smart TB Management Service for Vulnerable TB Patients

Purpose Successful TB treatment is to take strong TB medicine thoroughly. However, vulnerable TB patients are at high risk of discontinuation of treatment. In Post-Corona New Normal era, the key to health care is digital transformation and patient empowerment. So we want experimentally to improve treatment compliance by applying medication management service that combines analogue TB medicine with digital technology. Method The chatbot TB management service using smartphone KakaoTalk provides various services such as medication notification, TB information contents, and one-on-one chat counseling. These service consists of a manager's page for medical staff and a Kakao Talk channel for tuberculosis patients. The manager can check the medication status every day through the registered patient's channel connection. And when the patients accesses the Dr. Colokman channel, they are provided with medication alarm services and TB-related information. The subjects are patients who are taking TB drugs with active TB without drug resistance, which includes drug-resistant TB, from September 2019 to August 2020. The subjects are vulnerable group diagnosed with active TB and was hospitalized from September to December 2019 and is being treated by OPD. Result There were 18 final chatbot enrollers and 20 controls. The ratio of male and female in chatbot enrollers was 13:5. By August 2020, the success rate of treatment was 76.9%, with 10 successful treatments, 2 stopped, 5 under treatment and 1 failed. In the control group, the ratio of male and female patients was 14:6. the success rate of treatment of them was 60%. Conclusion Voluntary medication management (named self-DOT) in community after discharge through chatbot service based on patient-to-doctor trust has been shown to improve treatment compliance by more than 15% by enhancing the empowerment of vulnerable groups. Therefore, it was helpful to utilize digital technology in addition to education so that they could take continuous TB medicine.
An exploratory analysis of tuberculosis-related research in North Korean Medical Journal

Purpose: Recent rapid changes in relations with North Korea have heightened interest in inter-Korean health and medicine exchanges on humanitarian ground. According to WHO reports, North Korea is one of the highest rates of TB in the the world. In this study, we explored the trend of research on tuberculosis by analyzing the research related to tuberculosis in the North Korean medical journal. Method: The North’s medical journals (Joseon Medical, Medical, Internal Medicine, Preventive Medicine and Basic Medicine) were five kinds, all of which were continuous. The publication year scope of the analysis was based on 2000 or later, and the research items on tuberculosis were divided into five categories. That is, after diagnosis, treatment, treatment, R&D, prevention and policy. Result: Tuberculosis was the most common disease published in the North’s medical journal, and the number of tuberculosis-related studies was on the rise recently. What should be noted in the journal was the DOT strategy, drug-resistant tuberculosis and drug susceptibility test, tuberculosis and immunological associations, and R&D including genetic variation. And we got the name of a health care doctor who did a lot of research. Also, the estimate in the North’s medical journal was a fragmental analysis of the North’s tuberculosis problem, and under the current system in the North, it is not enough to analyze the problem of the whole country’s tuberculosis outbreak. Conclusion: Tuberculosis is a major health problem in North Korea, but it is not enough to analyze its exact conditions. In the end, the accurate understanding of the current state of tuberculosis in North Korea can be confirmed through testimony or experience from North Korean refugees or academic exchanges with North Korean doctors who have done a lot of research.

We demonstrated the improved device performance offlexible perovskite light-emitting diodes (PeLEDs)on indium-tin oxide coated poly(ethylene terephthalate) (ITO/PET) substrates by using modified poly(3,4-ethylene dioxythiophene):poly(styrene sulfonate) (PEDOT:PSS) as a hole transport layer (HTL). Solvent modification and dopant modification of the PEDOT:PSS solution were carried out by addingisopropyl alcohol (IPA) and poly(sodium 4-styrenesulfonate) (PSS-Na), respectively, to the pristinesolution. Devices with the modified HTLs, PEDOT:PSS + IPA and PEDOT:PSS + PSS-Na, exhibited asignificant enhancement in both luminance and efficiency. The work function of HTL was improved toreduce the energy barrier against the perovskite layer and facilitate hole transport. The perovskitefilmsdeposited on the modified PEDOT:PSS layers showed a uniformly covered surface morphology withoutany defects, resulting in an increased PL intensity. In particular, the perovskitefilm coated on the dopantmodified HTL showed a better crystallinity and reduced luminescence quenching. The PeLED device withthe PEDOT:PSS + PSS-Na HTL exhibited a maximum luminance of 3900 cd/m2, a maximum currentefficiency of 25.1 cd/A, and a maximum external quantum efficiency of 5.9%.
Protein bindings to the regulatory region of the mouse protamine 2 promoter

To understand the regulation of transcription of the mouse protamine 2 (mP2) gene, the interactions between proteins and regulatory region of the mP2 promoter have been studied. The promoter DNA, -170 to +8, was analysed by footprinting assays to investigate elements for regulatory protein bindings. Two regions were observed as binding sites for the testis proteins. The sequence of site 1, -64 to -48, contained cAMP responsive element (CRE), and site 2, -87 to -67, contained CAAT-like element, Phosphorylation of proteins increased the binding affinities for these two sites and also transcriptional activity of mP2 promoter.
