      • 국소 폐병변에서의 역동적 조용증강 CT의 유용성

        The blood supply and metabolism of malignant nodules are qualitatively and quantitatively different from those of most benign nodules. We evaluated the efficacy of dynamic CT for differentiation of benign malignant lesions. Thirty-six patients with malignant nodule(n=20), tuberculoma(n=7), and pneumonic consolidation(n=9) were evaluated by thin section dynamic CT. CT numbers of lesion and aorta were measured before enhancement, and 1 minute, and 3 minutes after enhancement. Malignant lesions(maximum, 31.1±10.7 HU) enhanced significantly more than tuberculoma(maximum, 13.5±12.9 HU), but mass/aorta ratios were not so significantly different between malignant lesions(maximum, 0.61±0.27), tuberculoma(maximum, 0.36±0.15), and pneumonic consolidation(maximum,0.49±0.14). Dynamic CT is very useful for differentiation of malignant lesions from tuberculoma.

        A total of 40 tintinnids were identified and described taxonomically based on samples collected in the southern Korean waters, the East Sea (the Japan Sea) and the East China Sea from July 1998 to June 2000. We also discussed the characteristics of ecological distribution of tintinnids.

        It is Important to know the normal size, shape, and signal intensity of anterior cruciate ligament in the evaluation of knee injury Many studies have reported findings of tear or rupture as well as normal anatomy of these structure, especially with MR images. To our knowledge, there are few studies about normal structures of anterior cruciate ligament in MR images. The purpose of this study is to compare normal finding with tear of anterior cruciate ligament on MRI. Twenty-three cases with rupture of anterior cruciate ligament were evaluated on MRI, measuring the length, thickness, width of anterior cruciate ligament, and angles of anterior and posterior cruciate ligaments. Mean length, thickness, and width of anterior cruciate ligament in patients with rupture were 37.7±4.8mm, 9.0±1.2mm, 8.0±0.8mm, and mean angles of anterior cruciate ligament, and posterior cruciate ligament were 48.1±9.6˚, 118.8±13.1˚. Curvature ratio of posterior cruciate ligament was 0.25±0.09. The results showed significant differences between normal persons and patients with complete or partial rupture, and the mean angle and curvature ratio of posterior cruciate ligament which are known to related with indirect sign in tear of anterior cruciate ligament showed significant differences between ours and that of western countries. These results will be useful in the evaluation of patient wish rupture of anterior cruciate ligament, especially on MRI.

      • Ni(111) 고체표면에서 CO₂화학흡착에 관한 분자궤도론적연구

        Ni(111)표면위의 CO₂흡착자리에 따른 흡착세기는 di-ο⑴, 1-fold, 3-fold,2-fold, π/μ,μ, 그리고 π-site순이다. di-ο⑴ 흡착자리에서는 <OCO가 148。인 찌그러진 CO₂분자구조를 가진다. 1-fold, 2-fold 및 3-fold의 경우는 CO₂분자중 산소의 한 끝이 고체표면과 결합하는 수직형태의 화학흡착이 일어남을 알았다. CO₂분자가 찌그럼짐은 2πu-obital과 섞임이 일어나기 때문이다. We calculated seven coordinated mode of binding site on Ni(111) surface; di-ο π,μ,π/μ, 1-fold, 2-fold, and 3-fold. Calculated binding energies are 2.31eV, 0.81eV, 1.50eV, 1.62eV, 2.27eV, 1.91eV, and 2.19eV, respectively. Then we have found that the binding energy is strongest at the di-ο(1) site. The adsorbed molecular CO2 of the di-ο(1) site on the Ni(111) surface is strong chemisorption and chemisorbed CO2 bends because of metal d-mixing with 2πu CO2 orbital.

        노인 낮병원의 심리사회적 치료 프로그램이 노인의 신경인지기능과 우울 증상에 미치는 효과

        노인에게 있어서 신경인지기능의 저하와 우울 증상은 가장 흔한 장애이며 신체적, 정신적인 면에 많은 영향을 미치고 있다. 또한 이러한 장애중 비임상군에서 심리사회적 치료개입의 효과가 보고되고 있다. 이러한 측면에서 지역사회에서 노인에게 제공되는 심리사회적 치료개입의 중요성이 높아지고 있다. 이전의 연구에서는 대상이 특정 집단이거나, 비교군이 없거나, 치료효과에 대한 추적 관찰이 이루어지지 않는 등의 제한점이 있었으며, 현재 국내의 노인 낮병원은 소수에 불과하고 이러한 낮병원의 치료효과에 대한 연구는 거의 없는 실정이다. 따라서 본 연구는 지역사회 정신보건센터에서 운영하는 노인 낮병원에 참가하는 노인들을 대상으로 낮병원의 심리사회적 프로그램이 노인들의 신경인지기능과 우울 증상에 미치는 효과와 이에 대한 유지 효과의 유무를 알아보고자 하였다. 경기도 광주군 정신보건센터의 노인 낮병원에 참여한 노인을 치료군으로, 치료를 거부하거나 치료 중 탈락한 노인을 비교군으로 하여 10주간의 낮병원 치료전후와 치료종결 후 10주에 신경인지기능은 한국형 간이 정신상태검사(이하 MMSE-K)로, 우울 증상은 단축형노인우울척도(이하 SGDS)로 평가하여 두 군을 비교하였고 치료군의 치료 직후와 치료종결 후 10주를 비교하였다. 연구결과 노인 낮병원의 심리사회적 치료가 시간에 따른 SGDS점수 변화에 통계학적으로 유의한 영향을 미쳤으며, MMSE-K점수 변화에는 통계학적으로 유의한 영향을 미치지 않았다. 또한 10주 추시가 가능했던 치료군 중 약 43%가 10주 후 SGDS가 증가하여 호전되었던 우울증상의 악화를 보였다. 본 연구 결과 노인 낮병원의 심리사회적 치료 프로그램 개입은 노인에게 있어서 우울 증상을 호전시키며 신경인지 기능에는 직접적인 영향을 미치지 않는 것을 알 수 있었다. 또한 상당수에서 심리사회적 치료 개입으로 인한 우울 증상의 호전이 유지되지 않음을 알 수 있었다. 이는 노인우울증상의 치료에 낮병원의 심리사회적 치료 프로그램이 효과적일 수 있음과 치료 후 이를 유지하기 위한 부가적인 치료개입의 필요성을 시사한다. Cognitive impairment and depression are the most common symptoms affecting the elderly on physical and mental states. Studies have shown that these symptoms in mild cases are improved by psychosocial intervention, emphasizing the importance of therapeutic intervention provided in the community. The purpose of this study was to investigate whether the psychosocial program at a geriatric day care service has any therapeutic effect on the cognitive function and depressive symptoms in the elderly and whether such effect can be maintained. The experimental group was composed of 37 elderly patients attending the day care service at the mental health center located in Kwangju county, Kyonggi province. The control group was composed of 22 elderly patients who either refused intervention or who dropped out of the program. Cognitive function was assessed with the Korean version of the Mini-Mental State Exam(MMSE-K), and depressive symptoms were assessed with the Short-form Geriatric Depression Scale(SGDS). The experimental group and the control group were compared on the score of on each test, which was administered before and after the program and 10 weeks after discontinuation of program. In the experimental group, the scores acquired immediately after the program and at 10 weeks of follow-up were also compared. The results showed that psychosocial intervention at a geriatric day care service was significantly associated with the change in SGDS scores dependent on time but not significantly associated with the change in MMSE-K scores. In the experimental group whose follow-up assessment was possible, it was further shown that 43% of patients had increased SGDS scores 10 weeks after the program ended, indicating that depressive symptoms had worsened. This study suggests that psychosocial therapeutic intervention in the geriatric day care service improve depressive symptoms but not cognitive functions in the elderly. In addition, for a considerable percentage of subjects in the experimental group, the improvements in depressive symptoms were not sustained after the intervention was withdrawn. These findings proposes a need of strengthening therapeutic intervention to maintain such effect.

      • 등고선 지도에서의 등고선 복원/인식에 관한 연구

        In this paper, we proposed the algorithm which extracts the contour lines in the contour map and reconstructs the broken ones. The contour lines is extracted by the algorithm using fairing method. Then the broken lines by symbols and the height number datum are reconstructed by using the Bezier polynomial function. In the reconstruction process, the control points which is searched by tracking the extracted contour lines are used. The reconstruction results is affected by the selection of the control points. The best selection is the choosing respectively the position of 6/10 between starting point and the cross point and between ending point and the cross one. The proposed algorithm has solved the problems with the conventional method by the division of the processing steps between the main contour lines and others. In the simulation results, the proposed method can be used in the digital elevation map generation. Also because the proposed one is consist of the automatic processing step, all the results can be obtained by the processing of computer.

      • 폐기물 회수 농약병의 전기 화학적 잔류농약 처리

        By electrochemical oxidation remaining pesticides treatment of recovered pesticide bottle wastes were oxidized and decomposed in a particle packed-bed electrode cell that it was increasing in an anodic reaction area by the proper packing materials. Air flotation oxidation treatment was used to compare organic oxidation by oxygen being generated at the anode with organic oxidation by electrolysis. Obtained the data from the results of batch style particle packed-bed electrode cell were applied to flow style and investigated necessary the some parameters to treat the remaining pesticides wastewater of recovered pesticide bottle wastes. At the removal COD of remaining pesticides wastewater of recovered pesticide bottle wastes PbO₂ was the best anode packing particle and optimum current condition was 3.33A/dm² at the batch style cell. When this condition was applied continuous flow style, the current was 0.69A/dm² at the 30 ml/min flow rate. Also incase of circulation COD was more decreased than not. Removal COD appeared a good tendency, when not only NaCl was added into the wastewater, but also wastewater was a high temperature. At performance comparison of reactor type rectangle type appeared a good tendency and special per-treatment of wastewater was unnecessary. Degradation of TOC was about 70% and COD was obtained about 92% after 85min, when this experiment were carried out electrolysis condition of 0.69A/dm², current density, and 30ml/min, flow rate.

        CO₂아크 용접 근로자의 뇌 MRI 고신호강도 3례

        We experienced three cases of high signal intensity observed by Brain Magnetic Resonance Imaging in CO₂arc welders of steel-frame manufacturing industry. Case 1 was a 35 years old man who has been an CO₂arc welder for 10 years, admitted a sudden onset of tonic clonic seizure. He complained fever, chilling and myalgia since 3 days before admission. On admission, in the test of manganese exposure indices, manganese concentrations of blood, urine and scalp hair were 5.17μg/dl, 22.00μg/l and 31.25ppm respectively. Case 2 was a 35 years old man who has been an CO₂arc welder for 20 years. On admission, he complained fatigue, numbness and weakness of extremities, and decrease of libido. In the test of manganese exposure indices, manganese concentrations of blood, urine and scalp hair were 6.34μg/dl, 14.62μg/l and 57.87 ppm respectively. In neurologic examination, Palmo-mentle reflex and Myerson sign were observed. Case 3 was a 33 years old man who has been an CO₂arc welder for 16 years. On admission, He complained loss of appetite, numbness of extremities, fatigue and decrease of attention. In the test of manganese exposure indices, manganese concentrations of blood, urine and scalp hair were 5.14μg/dl, 13.79μg/l and 50.08 ppm respectively. In neurologic examination, Myerson sign was observed. In brain magnetic resonance imaging. TIWI showed symmetrical high signal intensity in basal ganglia and midbrain of three cases. Authors argued that they were developed by manganese exposure, and we considered that follow up study would be necessary.

        직업적 아크릴아미드 폭로에 의한 다발성 신경병증

        Authors report a first Korean case of polyneuropathy in an acrylamide producing factory together with an environmental and epidemiologic survey for the ten workers of the same workplace and 46 workers in the production and manufacturing acrylamide in Ulsan, Korea. The case is 30 years old man who worked in the production of the acrylamide for two years. In August, 1997, he had a weakness of the lower extremity with loss of body weight of 15 Kg which progressed to paresthesia of the limbs, the loss of vibration sense, difficulty in walking, speech disturbance, dysuria, constipation, difficulty in erection and swallowing difficulty. Neurologic examination showed a complete loss of vibration and position sense, and loss of deep tendon reflex in all the limbs. There was a contact dermatitis-like skin lesion on the feet. Electrodiagnostic test showed a generalized polyneuropathy of moderate degree involving motor and sensory component of peripheral and cranial nerves. After 8 months' cessation from exposure and supportive care, his symptoms were completely resolved, but abnormality in electrodiagnostic test and vibrotactile perception threshold still persisted, although much improved than before. There were 11 workers in the same workplace of the case, producing acrylamide by microorganism as a catalyst. Environmental monitoring of the workplace revealed air borne concentration of the acrylamide to be from 0.05 to 0.23 mg/m3 by personal sampling and from 7.65 to 11.65 mg/m3 by area sampling during filter exchanging process, which far exceeds TLV-TWA of 0.03 mg/m3. However, acrylamide was not detected from the plasma of the workers. Symptom questionnaire showed a markedly higher complain of neurologic symptoms, compared to the workers of the other acrylamide producing companies. Electrodiagnostic test showed a several workers are in the lower normal range without definite abnormality. Vibrotactile perception threshold by Vibratron Ⅱ showed a significantly increased threshold in the workers of the same workplace compared to the other acrylamide workers and significantly decreased test score of color vision compared to normal controls. After improvement of the production process and strict wearing of the protective device, most workers reported the improvement of clinical symptoms, but vibrotactile perception threshold was not improved. These result suggests the need for the effective environmental monitoring together with a periodic biological monitoring. Development of effective screening test is urgently needed to control and assess the skin absorption of acrylamide.
