        Nutritional Status and Risk of Sarcopenia among Hospitalized Older Adults Residing in a Rural Region in Turkey

        Background: This study aimed to determine the prevalence of sarcopenia and its associated factors in community-dwelling older adults at risk of malnutrition based on the Mini Nutritional Assessment (MNA), Prognostic Nutritional Index (PNI), and Geriatric Nutritional Risk Index (GNRI). Methods: The study participants were 345 adults aged >65 years who visited Geriatric Internal Medicine outpatient clinics. The study included people without acute illness for whom the risk of malnutrition could be assessed and appropriate measurements taken. At the baseline visit, participants’ data and measurements were gathered. The primary data included sociodemographic details, anthropometric measurements, malnutrition screening tests, and functional assessments. Results: The participants’ mean age was 76.21±5.59 years, and 57.1% were men (n=97). The prevalence rate of sarcopenia was 45.5%. Compared to individuals without sarcopenia, those with it were older; had lower MNA, PNI, and GNRI scores; and had lower muscle mass, muscle strength, and lower leg circumferences (p<0.001). After adjusting for potential confounding factors, we found that sarcopenia, advanced age, male sex, high risk of malnutrition, calf circumference, and a low PNI score were all significantly associated with a low GNRI score (p<0.001). Conclusion: Sarcopenia was significantly associated with advanced age, male sex, and high risk of malnutrition. Patients’ nutritional and functional status should always be assessed for therapeutic interventions and lifestyle changes.

        A comparative assessment of bagging ensemble models for modeling concrete slump flow

        In the last decade, several modeling approaches have been proposed and applied to estimate the high-performance concrete (HPC) slump flow. While HPC is a highly complex material, modeling its behavior is a very difficult issue. Thus, the selection and application of proper modeling methods remain therefore a crucial task. Like many other applications, HPC slump flow prediction suffers from noise which negatively affects the prediction accuracy and increases the variance. In the recent years, ensemble learning methods have introduced to optimize the prediction accuracy and reduce the prediction error. This study investigates the potential usage of bagging (Bag), which is among the most popular ensemble learning methods, in building ensemble models. Four well-known artificial intelligence models (i.e., classification and regression trees CART, support vector machines SVM, multilayer perceptron MLP and radial basis function neural networks RBF) are deployed as base learner. As a result of this study, bagging ensemble models (i.e., Bag-SVM, Bag-RT, Bag-MLP and Bag-RBF) are found superior to their base learners (i.e., SVM, CART, MLP and RBF) and bagging could noticeable optimize prediction accuracy and reduce the prediction error of proposed predictive models.

        Axon Count and Sympathetic Skin Responses in Lumbosacral Radiculopathy

        Background and Purpose Electrodiagnostic studies can be used to confirm the diagnosis oflumbosacral radiculopathies, but more sensitive diagnostic methods are often needed to measure the ensuing motor neuronal loss and sympathetic failure. Methods Twenty-six patients with lumbar radiculopathy and 30 controls were investigatedusing nerve conduction studies, motor unit number estimation (MUNE), testing of the sympathetic skin response (SSR), quantitative electromyography (QEMG), and magnetic resonancemyelography (MRM). Results Using QEMG as the gold standard, the sensitivity and specificity of MUNE for the abductor hallucis longus muscle were 71.4% and 70%, respectively. While they were 75% and 68.8%, respectively, when used MRM as gold standard. The sensitivity and specificity ofMUNE for the extensor digitorum brevis muscle were 100% and 84.1%, respectively, when theperoneal motor amplitude as the gold standard. The SSR latency was slightly longer in the patients than in the controls. Conclusions MUNE is a simple and sensitive test for evaluating autonomic function and for diagnosing lumbosacral radiculopathy in patients. MUNE could be used routinely as a guide for the rehabilitation of patients with radiculopathies. SSR measurements may reveal subtle sympathetic abnormalities in patients with lumbosacral radiculopathy.

        Changes in phenolic acids, flfl avonoids, anthocyanins, and antioxidant activities of Mahonia aquifolium berries during fruit development and elucidation of the phenolic biosynthetic pathway

        It is important to determine the most suitable ripening period of fruit regarding bioactive compounds for evaluating as anindustrial and in human nutrition. The aims of this study were to determine the changes in non-anthocyanin phenolics, anthocyanins,and antioxidant activity of Mahonia aquifolium fruit throughout ripening and to determine the biosynthetic pathwayof the phenolic compounds. The fruit was collected from shrubs at 10 diff erent ripening stages between June 21 and August17, 2016. The samples were analyzed for individual anthocyanins, non-anthocyanin phenolics, and antioxidant activity. There were signifi cant decreases in gentisic, chlorogenic, p -coumaric, ferulic acids, rutin, and isorhamnetin 3- O -glucosideand increases in protocatechuic, syringic acid, caff eic acid, myricetin, and luteolin. The concentrations of delphinidin-3- O -glucoside, cyanidin-3- O -glucoside, and pelargonidin-3- O -glucoside rose progressively from the beginning to the end ofripening, while decreases occurred from the 6th stage in the amounts of malvidin-3- O -glucoside, peonidin-3- O -glucoside,cyanidin-3- O -rutinoside, and delphinidin-3- O -rutinoside. A decrease followed by an increase in the antioxidant activityindicated that the phenolic profi le of the fruit changed toward the end of ripening, including for the compounds with higherantioxidant potential.       • Healthy Life-Style Promoting Behaviour in Turkish Women Aged 18-64

        Aim: In this study we aimed to investigate the healthy life-style behaviour of Turkish women and establish influencing features. Methods: This descriptive study performed by a questionnaire method was conducted in a primary health care centre, in an urban region in Kayseri, Turkey. Every midwife region belonging to the health care centre was accepted as a cluster, and a sample of 450 women between ages 18-64, was gathered from 9 midwife regions. The Health Promotion Life-style Profile (HPLP) was applied to evaluated the healthy lifestyle behaviour of 421 women that could be reached. T test, Tukey HSD with ANOVA, and chi square tests were used for analysis. Results: The mean total HPLP was $126.8{\pm}19.2$ (interpersonal support subscale, $74.3{\pm}14.1$; nutrition subscale, $73.6{\pm}12.6$; self-actualisation subscale, $70.6{\pm}11.9$; stress management subscale, $63.4{\pm}13.0$; health responsibility subscale, $61.2{\pm}13.2$; and exercise subscale, $47.1{\pm}15.0$). There was no statistically significant variation when evaluated for age, marital state, family type, economic status, and perception of self-health, smoking, and BMI. HPLP was high in people with an education of primary school and lower in university graduates, in people who lived mostly in the city centre and in individuals with chronic diseases. In conclusion, it was established that the health promoting behaviour in Turkish women is, in general, at a medium level, and women should be enlightened in order to develop and increase the habit of health preservation and promotion.

        A new biomarker for the early diagnosis of ovarian torsion: SCUBE-1

        Objective: Prompt diagnosis and management are essential for saving the adnexal organs from infarction in cases of ovarian torsion (OT). This study aimed to determine the diagnostic significance of signal peptide, complement C1r/C1s, Uegf, and Bmp1 (CUB), and epidermal growth factor-like domain-containing protein-1 (SCUBE-1) levels in cases of OT, an emergent ischemic condition, and the relationship of SCUBE-1 with oxidative stress parameters. Methods: This prospective study was conducted among 15 OT patients and 20 age- and gravidity-matched healthy women. SCUBE-1 serum concentrations were determined by using enzyme-linked immunosorbent assays. In addition, oxidative stress was evaluated by measuring the serum levels of advanced oxidation protein products (AOPP), ferric reducing ability of plasma (FRAP), and glutathione (GSH). Results: The SCUBE-1 titers were significantly higher in the patients with OT than in the controls (p=0.008). In addition, serum FRAP and GSH levels were significantly lower in the OT patients than in the controls (p<0.001 for both). Serum AOPP levels were higher in the OT patients, but this trend was not statistically significant (p>0.05). Furthermore, there were no correlations between SCUBE-1 levels and age, gravidity, parity, cyst size, and AOPP, FRAP, or GSH levels (p>0.05). Conclusion: We believe that SCUBE-1 may be a promising biomarker for the early diagnosis of OT.

        A spontaneous pregnancy and live birth in a woman with primary infertility following the excision of an ovarian adrenal rest tumor: A rare case

        Adrenal rest tumors are a rare extra-adrenal complication of congenital adrenal hyperplasia (CAH) in women although they are more commonly found in the testes of male patients with CAH. An ovarian adrenal rest tumor (OART) may coexist with CAH or imitate its symptoms without CAH. In this case report, we present the case of a woman with OART without CAH, whose main complaint was infertility and who had a baby after successful surgical treatment.

        Two Stage Deep Learning Based Stacked Ensemble Model for Web Application Security

        Detecting web attacks is a major challenge, and it is observed that the use of simple models leads to low sensitivity or high false positive problems. In this study, we aim to develop a robust two-stage deep learning based stacked ensemble web application firewall. Normal and abnormal classification is carried out in the first stage of the proposed WAF model. The classification process of the types of abnormal traffics is postponed to the second stage and carried out using an integrated stacked ensemble model. By this way, clients’ requests can be served without time delay, and attack types can be detected with high sensitivity. In addition to the high accuracy of the proposed model, by using the statistical similarity and diversity analyses in the study, high generalization for the ensemble model is achieved. Within the study, a comprehensive, up-to-date, and robust multi-class web anomaly dataset named GAZI-HTTP is created in accordance with the real-world situations. The performance of the proposed WAF model is compared to state-of-the-art deep learning models and previous studies using the benchmark dataset. The proposed two-stage model achieved multi-class detection rates of 97.43% and 94.77% for GAZI-HTTP and ECML-PKDD, respectively.

        Nâmık Kemal and His Utopian Dream About Freedom

        Born in 1840, Nâmık Kemal left his mark on Turkish and world literature. He was one of the pioneers of the Ottoman Reform era. Due to Nâmık Kemal’s pioneering endeavors and his writings that purported to enlighten the society and expostulate on the political descension occurring during his time under the rule of Abdulaziz, the 32nd Sultan of the Ottoman Empire, Kemal was twice sent into exile. During these exiles, he deepened his knowledge and academic background further, explored new worlds, and wrote Dream. This study deduces how, in contrast to its apparent meaning, Nâmık Kemal’s choice of title for his “utopia” was meant to suggest a sarcastic condition, indeed one that he might have intentionally created while he was ostracized in Famagusta, Cyprus. Nâmık Kemal’s utopia, Dream, consists of a “dream” that he claims to have had while in a mansion overlooking Bosphorus in Istanbul. Dream, in an ironic way, is actually Nâmık Kemal’s collection of thoughts designed to agitate the Ottoman nation. This study subsumes Dream as a euchronia or a homotopical utopia that portrays a better society created in the same place in Istanbul and the Ottoman Empire. The study also reveals how Nâmık Kemal posited the social and local environments in Dream with the intent to influence future political, cultural, and social connotations and reasoning in his contemporary world. Through a comparative study of history and literature, this essay thus propounds how Nâmık Kemal actually intended to “shake” the people to awaken them from their long-lasting irresponsible sleep.
