Vascular dementia (VaD) is associated with the loss of cognitive function. The pharmacotherapy for dementia is cholinesterase inhibitor (ChEI). However, currently available ChEIs produce side efects. Therefore, new anticholinesterase agents are required for treating dementia. In vitro assays showed that Syzygium polyanthum leaf extract (SPLE) inhibit acetylcholinesterase (AChE) and butyrylcholinesterase (BuChE) activities. Here, we evaluated the nootropic efect of SPLE, mediated via a cholinergic system with long-term hippocampal plasticity (LTP) in chronic cerebral hypoperfusion (CCH) rats. Male Sprague Dawley (SD) rats were subjected to permanent bilateral occlusion of common carotid arteries (POBCCA) or sham groups. An automated open feld test (AOFT) was conducted to examine motor and exploratory functions. Cognitive functions were evaluated using Novel object recognition (NOR) and Morris water maze (MWM) tests. At the end of the behavioral task, the brain was harvested to determine cholinesterase, choline acetyltransferase (ChAT), acetylcholine (ACh), and brainderived neurotrophic factor (BDNF) activities. Hippocampal synaptic plasticity was measured under urethane anesthetized rats. The study revealed SPLE (100, 200, and 300 mg/kg) (i) improved both non-spatial and spatial memories in NOR and MWM tests, respectively; (ii) promoted long-lasting LTP enhancement in CA3-CA1 synapses; (iii) increased the ACh, ChAT and BDNF activities in the frontal cortex, hippocampus and cerebral cortex. Interestingly, the elevated AChE activity in the hippocampus was signifcantly inhibited by SPLE extract. SPLE executes its cognitive functions by restoration of the cholinergic system. The fndings support the therapeutic potential of SPLE in the treatment of VaD.

This paper presents a simple, consistent and reliable technique to identify detected partial discharges (PD) using an acoustic ultrasonic method. A new reliable algorithm named ‘Simple Partial Discharge Identifier’ (SPDI) is proposed to perform identification process of the detected ultrasonic signals of PD. Experimental works based on recommended practices were setup and the ultrasonic signals of the PD were recorded. The PD data is then employed as the reference data. The SPDI developed has been tested against commonly used models in Neural Network (NN). Results from the SPDI algorithm shows more reliable results compared to NN models results. Comparison made on the mean square error (MSE) results shows SPDI produces the desired outcome with lower MSE in 97.17% of trials. Low error of SPDI indicates a high reliability to be applied in the identification of PD.
Canine biphasic synovial sarcoma:case report and immunohistochemical characterization

The clinical, radiological and pathologic features of a biphasic synovial sarcoma in the left elbow joint of a twoyear- old male Rottweiler are presented. The tumor showed positive immunoreactivity for vimentin, Epithelial Membrane Antigen (EMA), p53 and PCNA, while it was negative for the cytokeratin used, S-100, Rb and p21. Immunohistochemistry for EMA allowed the identification of epithelioid components of synovial sarcoma, and may, therefore, contribute in establishing a diagnosis of biphasic synovial sarcoma. Intratumoral variation in PCNA immunoreactivity was minimal, indicating that the various tumor components proliferate at more or less similar rates. Overall, the characterized immunohistochemical profile for canine synovial sarcoma, not defined previously, may provide clues to the histogenesis of the phenotypically mesenchymal and epithelial elements of the tumor, and may be of value in the differential diagnosis of challenging cases, decreasing the risk of under- and mis-diagnosis. Although more cases need to be studied to determine whether there is a consistent pattern of immunostaining in canine synovial sarcoma, its potential significance is discussed in relation to the histogenesis, molecular pathology and differential diagnosis of canine synovial sarcoma.
Sexual Dysfunction in Patients with Polycystic Ovary Syndrome in Malaysia

Background: Polycystic ovary syndrome (PCOS) is a combination of chronic anovulation, obesity, and hyperandrogenism and can affect sexual function in women of reproductive age. It is also associated with endometrial cancer. Our aim was to evaluate the frequency and predisposing factors of sexual dysfunction in PCOS patients. Materials and Methods: In this cross-sectional study, 16 married women with a definite diagnosis of PCOS were recruited. Sexual function was assessed in the domains of desire, arousal, lubrication, orgasm, satisfaction and pain using the female sexual function index (FSFI) questionnaire. Patients were also assessed for mental health using the depression, anxiety and stress (DASS-21) questionnaire. Presence of hirsutism was assessed using the Ferriman-Gallwey (FG) scoring system. Demographic data were obtained from patients during in-person interview. Results: Sexual dysfunction was present in 62.5% of patients with the domains of arousal and lubrication particularly affected (93.8% and 87.5%, respectively). Patients with symptoms of depression and anxiety were significantly more likely to suffer sexual dysfunction than those without these symptoms (p=0.04 and p=0.03 respectively). Patients with stress symptoms reported higher orgasm dysfunction than those without (p=0.02). No significant difference in any of the FSFI score domains was observed between patients with and without hirsutism. Conclusions: PCOS patients markedly suffer from sexual dysfunction and therefore it seems appropriate to be screened for intervention. Poor mental health conditions that may be the result of infertility or other complications of PCOS should also be considered as curable causes of sexual dysfunction in these patients.

The H1N1 pandemic in 2009 and the H5N1 outbreak in 2005 have shocked the world as millions of people were infected and hundreds of thousands died due to the infections by the influenza virus. Oseltamivir, the most common drug to block the viral life cycle by inhibiting neuraminidase (NA) enzyme, has been less effective in some resistant cases due to the virus mutation. Presently, the binding of 10 chalcone derivatives towards H5N1 and H1N1 NAs in the non-catalytic and catalytic sites was studied using molecular docking. The in silico study was also conducted for its drug-like likeness such as Lipinski Rule, mutagenicity, toxicity and pharmacokinetic profiles. The result demonstrates that two chalcones (1c and 2b) have the potential for future NA inhibitor development. Compound 1c inhibits H5N1 NA and H1N1 NA with IC50 of 27.63 μM and 28.11 μM, respectively, whereas compound 2b inhibits NAs with IC50 of 87.54 μM and 73.17 μM for H5N1 and H1N1, respectively. The in silico drug-like likeness prediction reveals that 1c is 62% better than 2b (58%) in meeting the criteria. The results suggested that 1c and 2b have potencies to be developed as non-competitive inhibitors of neuraminidase for the future development of anti-influenza drugs.

Background: The current standard treatment for patients with newly diagnosed diffuse large B cell lymphoma (DLBCL) is rituximab combined with cyclophosphamide, doxorubicin, vincristine and prednisolone (CHOP). A significant number of patients were not treated with recommended dose of rituximab due to limited financial resources in Malaysia. This study evaluates the efficacy of R-CHOP like chemotherapy in Malaysian patients with DLBCL. Materials and Methods: The study comprised a retrospective analysis of patients with DLBCL treated at a single centre. The outcome was compared with patients who were treated with R-CHOP like and CHOP like chemotherapy. Patients who were treated with lower dose of rituximab was subanalysed for outcome. Results: A total of 86 patients who had CHOP-like chemotherapy were included. Only 39 (45%) patients had rituximab and only 12 (29%) patients had the recommended dose. The overall response (OR) and complete response (CR) rates were 88% and 81% respectively. There was no significant difference in OR and CR in patients who had rituximab and those without rituxmab. Those with International Prognostic Index (IPI) score of ${\leq}2$ had significant higher CR rate, progression free survival (PFS) and overall survival (p<0.001). Conclusions: The lack of significant improvement in CR and DFS in our patients may be due to an inadequate dose of rituximab.

<P>Asthma is characterized by aberrant airways including epithelial thickening, goblet cell hyperplasia, and smooth muscle hypertrophy within the airway wall. The current study examined whether kaempferol inhibited mast cell degranulation and prostaglandin (PG) release leading to the development of aberrant airways, using an <I>in vitro</I> model of dinitrophenylated bovine serum albumin (DNP-BSA)-sensitized rat basophilic leukemia (RBL-2H3) mast cells and an <I>in vivo</I> model of BSA-challenged asthmatic mice. Nontoxic kaempferol at 10–20 μM suppressed β-hexosaminidase release and cyclooxygenase 2 (COX2)-mediated production of prostaglandin D2 (PGD2) and prostaglandin F2α (PGF2α) in sensitized mast cells. Oral administration of ≤20 mg/kg kaempferol blocked bovine serum albumin (BSA) inhalation-induced epithelial cell excrescence and smooth muscle hypertrophy by attenuating the induction of COX2 and the formation of PGD2 and PGF2α, together with reducing the anti-α-smooth muscle actin (α-SMA) expression in mouse airways. Kaempferol deterred the antigen-induced mast cell activation of cytosolic phospholipase A2 (cPLA2) responsive to protein kinase Cμ (PKCμ) and extracellular signal-regulated kinase (ERK). Furthermore, the antigen-challenged activation of Syk-phospholipase Cγ (PLCγ) pathway was dampened in kaempferol-supplemented mast cells. These results demonstrated that kaempferol inhibited airway wall thickening through disturbing Syk-PLCγ signaling and PKCμ-ERK-cPLA2-COX2 signaling in antigen-exposed mast cells. Thus, kaempferol may be a potent anti-allergic compound targeting allergic asthma typical of airway hyperplasia and hypertrophy.</P>
