        Acute Osteomyelitis of the Mandible by Extended-Spectrum β-Lactamase Producing Klebsiella Pneumoniae: A Case Report

        Acute osteomyelitis caused by Klebsiella pneumoniae is rare in the oral and maxillofacial region. Klebsiella pneumoniae is a Gram-negative bacillus and the normal flora of the human body, but it can cause pneumonia, urinary tract infection, meningitis, and osteomyelitis in patient with compromised immune systems. These infections are mainly caused by nosocomial infection. Microbacterial osteomyelitis was developed by clinical cause such as tooth extraction, fracture, and surgical history, which requires long-term antibiotic administration and surgical treatment. This report describes that a 56-year-old male patient with acute osteomyelitis caused by Klebsiella pneumoniae infection after implant placement was treated with intravenous administration of ertapenem without open surgery treatment. Through this case, we report that antibiotic susceptibility test is essential for the treatment of acute osteomyelitis caused by a bacterial infection resistant to empirical antibiotics, and early administration of appropriate antibiotics can reduce the possibility of extensive bone destruction or additional open surgery.

        북한 전통해금 개방현 주법 연구

        본 연구는 1950~1960년대 북한의 악기개량사업이 시행되는 과정에서 발생한 전통해금 개방현 주법의 사용 양상과 음악적 효과를 확인하는데 그 목적이 있다. 당시 북한에서는 전통 악기의 개선점으로 음역, 음량, 전조 기능에 대해 논의하였으며, 이를 주법적으로 해결하기 위해 개방현을 사용하였을 것으로 추론하였다. 개방현은 남한의 전통음악에서는 사용된 바 없는 주법이며, 분단 이후 북한 해금 연주자들이 다양한 목적을 가지고 사용한 주법임을 당시 사료들을 통해 확인할 수 있다. 『조선예술』에 나타난 논의를 바탕으로 령산회상 및 산조에 출현한 개방현의 사용 유형을 세 가지로 분류하여 음악적 효과를 도출한 결과는 다음과 같다. 첫째, 개방현의 사용으로 음역이 확장되었다. 해금이 발음 가능한 최저음을 연주하는 개방현은 주변 음과 관계되어 최소 8도에서 최대 17도까지 도약진행을 뚜렷하게 나타내는 효과를 가져왔다. 둘째, 개방현의 사용으로 음량이 확대되었다. 같은 활의 압력을 가지고 연주한다고 가정했을 때, 개방현은 줄을 짚는 왼손에 의해 진동이 감소되지 않기 때문에 상대적으로 큰 음량을 발음한다. 개방현은 합장단 및 장고점에 출현하여 셈여림을 더욱 대비시킬 수 있다. 셋째, 개방현의 사용으로 운지 유형이 확대되었다. 운지를 벗어난 저음의 출현 시 개방현으로 대체하여 운지의 이동을 최소화하였고, 현을 운지하는 경우에 비해 상대적으로 왼손에 휴지부가 생기는 개방현을 활용하여 음정 간격이 넓은 운지로의 이동 시 음정의 정확도를 상승시킨다. This study is to unveil usage patterns and musical effects of the playing method of open strings of the traditional haegeum that occurred during the musical instrument modifying project conducted in the 1950s-1960s in North Korea. At that time, North Korea discussed pitch range, volume, and transposition functions as a way to improve traditional musical instruments, and open strings were used to resolve the issue concerning playing method. The technique of open strings has never been used in the field of traditional music in South Korea, and it can be confirmed through historical sources at that time that the technique was used for various purposes by North Korean haegeum players since the division of the country. This study classifies the usage patterns of open strings appearing in ryeongsanhoesang and sanjo into three based on the discussion on the improvement points of traditional musical instruments shown in Joseon Art . The results on the musical effects derived from the study are as follows. First, the vocal range was expanded by the use of open strings. The open string, which plays the lowest note that the haegeum can express, has the effect of clearly showing the leaping progress from a minimum of 8 degrees to a maximum of 17 degrees in relation to the surrounding notes. Second, the volume was enlarged by the use of open strings. Assuming playing with the same bow pressure, the open strings produce a relatively large volume because the vibration is not reduced by the left hand holding the string. Open strings appear on the strong beat of the rhythm, further contrasting the dynamics. Third, the types of fingering were expanded with the use of open strings. When a low note outside of fingering appears, it is replaced with an open string to minimize fingering movement, and compared to the case of fingering, an open string having a rest part on the left hand is used to increase the pitch acceracy when moving to a fingering with a wide pitch interval.

        A Case Report on the Risk of Enterobacteriaceae Infection in the Oral and Maxillofacial Region

        Osteomyelitis is an inflammatory condition of the bone caused by pathogenic bacteria. The causative pathogen is usually oral residing bacteria, but this is a report of patients with osteomyelitis infected with Enterobacteriaceae, which is not common. Enterobacteriaceae has been known to cause in-hospital infections for over last 30 years and is known to have multiple antibiotic resistances. Both cases in this study developed osteomyelitis after removal of the dentigerous cyst. Enterobacter aerogenes was cultured in one patient and Serratia marcescens in the other. After changing antibiotics through antibiotic susceptibility testing, clinical symptoms subsided and radiographic images confirmed that the callus formed and recovered at the same time.

        A Literature Review of Infection with ESKAPE Pathogens in Oral and Maxillofacial Region

        Odontogenic infection in the oral and maxillofacial regions caused by bacteria (mostly of oral origin) is one of the most common diseases encountered by dentists. Localized infection can easily be treated with incision and drainage followed by antibiotics. Emergence of multidrug resistant (MDR) bacteria called "Superbacteria" has become one of the serious problems in modern society, due to its small window of opportunity for treatment and high casualty. The acronym "ESKAPE", encompassing the common and serious MDR pathogens stand for Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa and Enterobacter spp. Literature search was performed in Medline, PubMed and Google Scholar ranging from 2012 to 2020. ESKAPE patient's infection period was longer than that of non-ESKAPE group, and the treatment method due to antibiotic resistance was also complicated. The purpose of this study is to investigate infection caused by ESKAPE pathogens in the oral and maxillofacial regions through literature review and to inform dental surgeons of the danger of ESKAPE pathogens and to suggest viable treatment options. Many studies worldwide reported infections associated with ESKAPE pathogens, but only limited number of studies targeted infection in oral and maxillofacial regions. Further research is required with more data on ESKAPE bacteria and their infection, especially in oral and maxillofacial regions.
