        Narrative differences between Orkhon inscriptions

        This study aims to demonstrate some of the discourse features of Tunyukuk scripts, a part of Orkhon inscriptions and compare it to the other parts, which are Bilge Kagan and Kul Tigin, in terms of its basic narrative discourse. The data will be analyzed using descriptive tools provided by narrative analysis and the rhetorical structure theory with a synchronic point of view. Tunyukuk will be the main text to be analyzed. and it will be compared to Kul Tigin to specify the discursive differences among them. Since Kul Tigin and Bilge Kagan have similar contents, Bilge Kagan will be excluded from this analysis. This study claims that even though these two scripts seem to report the same sequence of events, there is a drastic difference between them in terms of the presentation of their macro structures.

        Efficiency of Ultrasound and Ultrasound-Guided Fine Needle Aspiration Cytology in Preoperative Assessment of Axillary Lymph Node Metastases in Breast Cancer

        Purpose: We performed this study to detect preoperative axillary metastases with ultrasound (US)-guided fine needle aspiration biopsy (FNAB), to eliminate the need for time-consuming and costly sentinel lymph node (SLN) scintigraphy and biopsy steps in the treatment of breast cancer patients, and in that of with suspicious US findings, and to evaluate the accuracy of preoperative US-guided FNAB for patients with suspicious lymph node metastases on US. Methods: Patients with a suspicious breast lump or histopathologically proven breast cancer underwent breast-axillary US. Increase in lymph node size, cortical thickening, non-hilar cortical flow, and hilar changes were evaluated with gray scale-color Doppler US. FNAB was performed if US results were suspicious for malignancy. Results: Thirty-eight axillary lymph nodes (ALN) underwent FNAB. ALN dissection, SLN scintigraphy, and biopsy steps were bypassed in 23 axillas with positive ALN FNAB (60.5%). The sensitivity of ALN FNAB was 88.46%; specificity and positive predictive value were 100%; and negative predictive value was 66.6% (inadequate cytology included; 76.7%, 100%, 100%, 53.3%, respectively). Asymmetrical cortical thickening, non-hilar cortical flow, and increase in hypoechogenity were only detected in metastatic nodes. Cortical thickening, and lymph node and breast mass size was higher in the metastatic group. Conclusion: By performing FNAB on suspicious lymph nodes, the routine, high-cost SLN scintigraphy and intraoperative gamma probe steps may be skipped, and axilla dissection can be performed directly. This leads to the elimination of the need for SLN investigation in more than half of the patients. The assessment of ALN metastases with preoperative US-guided FNAB is a cost-effective method with high specificity, that eliminates the need for costly and time-consuming SLN scintigraphy and biopsy steps, and helps in preoperative staging.

        Heavy metal profiles of agricultural soils in Sakarya, Turkey

        Sakarya is famous for cucurbit productions in Turkey and cucurbits can grow as big as 560 kg of weight per fruit in its agricultural areas. There is no or limited information about contaminant levels and profiles of the agricultural fields in Sakarya. The purpose of this study is to investigate the levels of polycyclic aromatic hydrocarbons (PAHs) (naphthalene, phenanthrene, pyrene, and fluoranthene) and heavy metal (As, Cd, Cu, Cr, Ni, Pb, Zn) concentrations of the selected fields. Total 33 soil samples were collected from 12 counties of Sakarya where both cucurbits have been produced and organochlorine pesticides have been applied to the fields for more than 30 y during the historical plantation periods. Heavy metal and PAH contents in the soil samples were measured by an Inductively Coupled Plasma Emission Spectroscopy and a Gas Chromatography-Mass Spectrometry. The highest phenanthrene, pyrene, and fluoranthene concentrations were measured as 63.50 ng/g, 134.34 ng/g, 140.0 ng/g, respectively in the soil samples from Geyve County. Cu, Ni, and Cr concentrations were measured as 108.2 ㎎/㎏, 219.9 ㎎/㎏, and 173.1 ㎎/㎏, respectively in Geyve’s samples which were also the highest and 2-7 times more than the limit values given in the Turkish Soil Pollution Control Regulation. Precautions need to be taken for Sakarya’s agricultural fields which are an important milestone of Turkey’s cucurbit and fruit productions since the contaminants can be accumulated in the fruits and edible parts of the plants.

      • Breast Cancer Risk and Early Diagnosis Applications in Turkish Women Aged 50 and Over

        Background: The aim of the study was to determine breast cancer risk and early diagnosis applications in women aged ${\geq}50$. Materials and Methods: This cross-sectional, descriptive field study focused on a population of 4,815 in Mansuro$\breve{g}$lu with a 55.1% participation rate in screening. In the study, body mass index (BMI) was also evaluated in the calculation of breast cancer risk by the Breast Cancer Risk Assessment Tool (BCRA) (also called the "Gail Risk Assessment Tool"). The interviewers had a three-hour training provided by the researchers, during which interactive training methods were used and applications were supported with role-plays. Results: The mean age of the women participating in the study was $60.1{\pm}8.80$. Of these women, 57.3% were in the 50-59 age group, 71.7% were married, 57.3% were primary school graduates and 61.7% were housewives. Breast-cancer development rate was 7.4% in the women participating in the study. When they were evaluated according to their relationship with those with breast cancer, it was determined that 73.0% of them had firstdegree relatives with breast cancer. According to the assessment based on the Gail method, the women's breast cancer development risk within the next 5 years was 17.6%, whereas their calculated lifetime risk was found to be as low as 0.2%. Statistically significant differences (P=0.000) were determined between performing BSE-CBE and socio-demographic factors. Conclusions: It was determined that 17.6% of the participants had breast cancer risk. There was no statistically significant difference between the women with and without breast cancer risk in terms of early diagnosis practices, which can be regarded as a remarkable finding. It was planned to provide training about the early diagnosis and treatment of breast cancer for people with high-risk scores, and to conduct population-based breast cancer screening programs.

        Birt-Hogg-Dubé Syndrome: Diagnostic Journey of Three Cases from Skin to Gene

        Birt-Hogg-Dube syndrome (BHDS) is a rare disorder characterized by the triad of cutaneous lesions, renal tumors, lung cysts and inactivation of the gene folliculin (FLCN). Here, we present three female patients diagnosed with BHDS. First case a 55-year-old female had flesh moles histopathology compatible with angiofibroma, multiple cysts in the lung and kidneys, FLCN gene mutations (‘c.1285dupC [p.His429Profs*]’ 11th exon and ‘c.653G>A [p.Arg258His]’ 7th exon). The second case a 76-year-old female had trichodiscoma on her skin, multiple cysts in the lung, spontaneous pneumothorax, FLCN gene mutation ‘c.1285dupC (p.His429Profs*27) 11th exon’ and, her son had renal carcinoma history under 50 years of age. Our third case, also the daughter of case 2, had dermal papules histopathology compatible with trichodiscoma, spontaneous pneumothorax, FLCN gene mutation ‘c.1285dupC (p.His429Profs*27) 11th exon’ and, parotid oncocytoma. Through our cases, we document the first case of two mutations (‘c.1285dupC [p.His429Profs*]’ 11th exon and ‘c.653G>A [p.Arg258His]’ 7th exon) in the same FLCN gene and the 11th known case of parotid oncocytoma associated with BHDS in the light of the literature.

      • Assessment of Cervical Cancer Risk in Women between 15 and 49 Years of Age: Case of Izmir

        Background: The aim of the study was to determine risk factors for cervical cancer for women in Izmir. Methods: This cross-sectional, descriptive field covered a population of 4319 women of reproductive age (15-49) (household registration in the Mukhtar's office-2007). A total of 1,637 women were included in the sample given a four-part questionnaire through face-to-face interview by visiting the women in their homes in order to determine socio-demographic factors, obstetric history, genital hygiene and the use of family planning methods. In addition, during the data collection process, the women were given group training in order to raise awareness of cervical cancer. The number and percentage distributions of the data were calculated. Results: While the average age of the women was $31.9{\pm}9.77$ (Min: 15.00-Max: 49.00), education level of 43.4% of them was elementary school only. It was determined that 70.3% of the women experienced at least one pregnancy, 71.0% had vaginal delivery and 75.9% used a contraceptive method. In the study it was determined that among the cervical cancer related risks vaginal delivery, vaginal lavage and having three or more pregnancies had the highest rates, while having sexual intercourse before 16 years of age and having more than one sexual partner constituted lower rates. The rate of the women who stated not having a smear in the last three years was 82.4%. Conclusions: Considering the case in terms of having Pap smear test, women's awareness on the risk factors and early diagnosis of cervical cancer was found to be low. Due to this reason, awareness of women has to be raised through education.
