        Endodontic approach in a replanted tooth with an immature root apex and chronic apical periodontitis: a case report

        This study describes the clinical steps taken in the treatment of a patient who had an avulsed right upper central incisor that presented with incomplete root development and chronic apical periodontitis. A 7-year-old boy was referred from a private dentist to a dental office specializing in endodontics. The tooth had remained in a dry environment for 20 minutes, and tooth replantation was performed at an emergency appointment. After clinical and radiographic examinations, root canal decontamination was performed, followed by several changes in intracanal calcium hydroxide medication. Blood clot formation was attempted, but bleeding within the root canal was insufficient; therefore, we opted for an intracanal medication change to stimulate mineralized tissue formation in the apical region. Root obturation was performed 45 days after the last change of intracanal medication, and clinical, radiographic, and tomographic follow-up examinations were performed at 3, 6, 18, and 40 months after the endodontic intervention. The increase in thickness and length of the root structure and the absence of root resorption were verified through follow-up examinations. Therefore, it was concluded that the procedures used were successful for tooth replantation.

        CBCT-based assessment of root canal treatment using micro-CT reference images

        Purpose: This study compared the root canal anatomy between cone-beam computed tomography (CBCT) and micro-computed tomography (micro-CT) images before and after biomechanical preparation and root canal filling. Materials and Methods: Isthmus-containing mesial roots of mandibular molars(n=14) were scanned by micro-CT and 3 CBCT devices: 3D Accuitomo 170 (ACC), NewTom 5G (N5G) and NewTom VGi evo (NEVO). Two calibrated observers evaluated the images for 2-dimensional quantitative parameters, the presence of debris or root perforation, and filling quality in the root canal and isthmus. The kappa coefficient, analysis of variance, and the Tukey test were used for statistical analyses(α=5%). Results: Substantial intra-observer agreement (κ=0.63) was found between micro-CT and ACC, N5G, and NEVO. Debris detection was difficult using ACC (42.9%), N5G (40.0%), and NEVO (40%), with no agreement between micro-CT and ACC, N5G, and NEVO (0.05<κ<0.12). After biomechanical preparation, 2.4%-4.8% of CBCT images showed root perforation that was absent on micro-CT. The 2D parameters showed satisfactory reproducibility between micro-CT and ACC, N5G, and NEVO (intraclass correlation coefficient: 0.60-0.73). Partially filled isthmuses were observed in 2.9% of the ACC images, 8.8% of the N5G and NEVO images, and 26.5% of the micro-CT images, with no agreement between micro-CT and ACC, and poor agreement between micro-CT and N5G and NEVO. Excellent agreement was found for area, perimeter, and the major and minor diameters, while the roundness measures were satisfactory. Conclusion: CBCT images aided in isthmus detection and classification, but did not allow their classification after biomechanical preparation and root canal filling.(

        Seminário de música brasileira na aula de português língua adicional: análise interacional da construção conjunta da aprendizagem

        In this paper, we investigate how institutional identity relations and the selection of learning objects can be interactionally conceived in an additional language classroom oriented to language use as social action (Clark, 1996). Informed by Ethnomethodological Conversation Analysis and Interactional Sociolinguistics, we analyse an eight-minute sequence of interaction featuring participants engaged in the accomplishment of a Portuguese-as-an-additional-language classroom activity, namely a seminar on Brazilian music, in which each student was supposed to lecture on a different musical style. In order to co-construct learning, participants orient to various identities and understandings that are built and negotiated in the course of interaction, configuring different participant frameworks. The analyses show (1) the rearrangement of the lecturer’s speech through the construction of new concepts in interaction (emergent products), and (2) the participants’ efforts to sustain the seminar frame, through the negotiation of conflicting identities of lecturer/audience, teacher/student and Brazilian national/foreigner to Brazil. This paper evidences that, more than requiring the use of the additional language, pedagogical activities such as the one analysed are opportunities for the participants to negotiate identities and modes of participation, define relevant learning objects in the course of interaction, and locally deal with the unexpected.
