        New Representations of Pascal Matrix via Operational $q$-calculus

        Comparison of the effect of different metal alloys on the esthetic appearance of dentin porcelain

        The purpose of this study was to examine the effect of aurofilm masking agents applied to various metal alloys on the colorof porcelain in metal-ceramic restorations (MCRs). The study was conducted with 2 different base-metal alloys (Ni-Cr, Co-Cr)and 2 different noble alloys (Pd-based, Au-Pd) used for MCRs, as well as 1 high noble alloy (Au-based) that served as a controlgroup. Eight experimental groups (n=7) and 1 control group were used in this study. An aurofilm masking agent was appliedto 4 groups (AuPdM, PdM, CoCr, NiCrM). Opaque porcelain and dentin body porcelain were applied to all groups. CIEL*a*b* color coordinates were measured. The Pd group had the highest mean a* value (-5.82); however, in comparisonto the control group, the differences in a* values were statistically significant only for the Cr-Co alloy groups (CoCr andCoCrM). The Pd group had the highest mean b* value (7.89). The ΔE value (2.13) of the CoCr group was significantly higherthan all other alloy groups. Metal alloy substrate and aurofilm masking agents significantly affected the color of porcelainMCRs. However, color differences between base-metal and noble alloys and the control group were within clinically acceptablelimits (ΔE<3.5).

        Semigroup presentations for congruences on groups

        We consider a congruence p on a group G as a subsemigroup of the direct product G × G. It is well known that a relation on G is a congruence if and only if there exists a normal subgroup N of G such that p={(s, t) : st−1 ∈ N}. In this paper we prove that if G is a finitely presented group, and if N is a normal subgroup of G with finite index, then the congruence p={(s, t) : st−1 ∈ N} on G is finitely presented.

        Aripiprazole Improved Obsessive Compulsive Symptoms in Asperger’s Disorder

        There are many comorbid disorders associated with autism spectrum disorders in child and adolescent population. Although obsessive compulsive disorder and autism spectrum disorders (ASD) comorbidity has common in clinical practice, there are few reports about psychopharmacological treatment for obsessive compulsive symptoms in children with ASD in the literacy. We report a successful treatment case with aripiprazole in Asperger’s Disorder with obsessive compulsive symptoms. The Yale Brown Obsessive Compulsive Scale was performed to assess symptom variety. This case report supports the effectiveness of aripiprazole in treatment of obsessive compulsive symptoms in Asperger’s Disorder or ASDs. Aripiprazole may be beneficial to obsessive compulsive disorder comorbid autism spectrum disorders in child and adolescent age group.

        Palliative treatment of Kaposi sarcoma with radiotherapy: a single center experience

        Purpose: The aim of this study is to evaluate the treatment responses of Kaposi sarcoma patients treated with radiotherapy (RT). Materials and Methods: The data of 18 patients (40 different regions) who were treated for Kaposi sarcoma in Department of the Radiation Oncology, Ankara City Hospital, Turkey between March 23, 2010 to February 13, 2018 were evaluated retrospectively. The primary endpoint of the study was the clinical-subjective response after RT, and the secondary endpoint was the visual response assessment after RT. Results: In evaluating the patients’ reported response of the lesions: 25 (62.5%) of complete response (CR), 12 (30%) of partial response (PR), and stable response was seen in 3 patients (7.5%). Patient reported response after RT was significantly higher in male sex (p = 0.002; odds ratio [OR] = 13.8, 95% confidence interval [CI], 2.7–70.0). Physician reported response rates were available for 28 lesions and CR was detected in 12 lesions (30%); PR was observed in 16 (40%). The relationship between physician reported outcome and RT techniques (electron, bolus, or water bolus) is close to the limit of statically significance (p = 0.052). Fewer lesions disappeared in patients with photon preference than electrons (p = 0.036; OR = 0.093; 95% CI, 0.009–0.950). Patients’ reported complete response rates were significantly higher in the 20 Gy per 5 fractions treatment arm (p = 0.042; OR = 1.75; 95% CI, 1.1–2.7). Conclusion: RT is an effective local treatment with high response rates in the treatment of Kaposi sarcoma. The subjective-clinical response rate was higher in male sex and the visual response was higher in the 20 Gy per 5 fractions arm. Additional studies are needed to standardize RT dose and techniques.

        Transient paraplegia after neurolytic splanchnic block in a patient with metastatic colon carcinoma

        We present a patient with metastatic colon carcinoma who developed paraplegia following a neurolytic splanchnic block. A 41-year old man with metastatic adenocarcinoma of the colon received a splanchnic neurolytic block using alcohol because of severe abdominal pain. Bilateral motor weakness and a sensorial deficit in both legs developed after the procedure. Diffusion magnetic resonance imaging revealed spinal cord ischemia between T8 and L1. The motor and sensorial deficits were almost completely resolved at the end of the third month. We think that anterior spinal artery syndrome due to reversible spasms of the lumbar radicular arteries using alcohol have resulted in transient paraplegia. The retrograde spread of alcohol to neural structures may have also contributed. (Korean J Pain 2018; 31: 50-3)

        Antiarrhythmic activity of a new spiro-cyclic benzopyran activator of the cardiac mitochondrial ATP dependent potassium channels

        ‘Compound A’ (4ı-(N-(4-acetamidobenzyl))-2,2- dimethyl-2,3-dihydro-5ıH-spiro[chromene-4,2ı-[1,4]oxazinan]- 5ı-one) is a new spiro-cyclic benzopyran activator of the mitochondrial ATP-dependent potassium channels (mitoKATP). We researched the effect of compound A on ischemia/reperfusion (I/R)-induced ventricular arrhythmias. We also tested the hypothesis that the application of the activation of mitoKATP in combination with the inhibition of sarcolemmal ATP-dependent potassium channels (sarcKATP) may produce a stronger antiarrhythmic effect. In anesthetized rats, myocardial ischemia was performed by ligating the left main coronary artery followed by reperfusion. At a dose of 10 mg/kg, compound A significantly decreased arrhythmia scores and the total length of arrhythmias, whereas this was found to be ineffective at a dose of 3 mg/kg. Pre-treatment with 5-HD, a selective mitoKATP blocker, abolished the antiarrhythmic effect of compound A. Both diazoxide, a selective mitoKATP opener and HMR 1098, a selective sarcKATP blocker, significantly decreased the total length of arrhythmias. However, the combination of neither diazoxide nor compound A with HMR 1098 showed no additional therapeutic benefit. These results reveal that compound A may have a dose-dependent antiarrythmic effect, which is more pronounced than the antiarrhythmic effect of diazoxide. The antiarrhythmic effect of compound A may possibly depend on mitoKATP activation.

        A modified presurgical alveolar molding technique for treatment of cleft in Down syndrome

        Craniofacial clefts are extremely rare deformities. Tessier’s classification is a widely accepted system that is based on clinical, radiographical, and surgical observations. The Tessier No. 0 cleft most commonly affects the upper lip, nose, and palate. This case presentation aims to report the outcome of a modified presurgical alveolar molding (PAM) appliance used in the treatment of an infant with Tessier No. 0 cleft as an alternate approach to mold such defects before surgery. The modified PAM appliance consisted of reciprocal parts connected by a helix. The segments were approximated by stripping the appliance at the midline in a V-shaped manner and the force was exerted by the extraoral elastics. The procedure gave results in 8 weeks, which may be regarded as a reasonable duration. The anterior cleft gap, which was 13 ㎜ before the treatment, was reduced to 3 ㎜ after the treatment by using modified PAM appliance. On a 21-month follow-up period, oral reshaping was regarded successful due to stability of the improved oral mold.

        Happle-Tinschert Syndrome: Report of a Case with Hemimegalencephaly

        Happle-Tinschert syndrome is a disorder causing unilateral segmentally arranged basaloid follicular hamartomas of the skin associated with ipsilateral osseous, dental and cerebral abnormalities including tumors. Although a case with hemimegalencephaly was previously described, this is the first report of Happle-Tinschert syndrome with discrepant short left leg, ipsilateral skin lesions, hemimegalencephaly and frontal polymicrogyria.
