Clinical Efficacy of Hormonal and Nonhormonal Agents in the Treatment of Vulvovaginal Atrophy

Objectives: Symptomatic local treatment of vaginal atrophy (VA) in menopausal women includes hormonal and nonhormonal preparations. Some women may be reluctant to use vaginal estradiol preparations because of the concern for developing breast cancer and endometrial hyperplasia. Therefore, it is necessary to compare the therapeutic effectiveness of alternative vaginal drugs, such as promestriene, an estrogen agonist, and sodium hyaluronate (NaH), a nonhormonal, water-based agent. Methods: Ninety-one postmenopausal women diagnosed with symptomatic VA were divided into three groups and treated for 12 weeks; 30 women with vaginal estradiol (VE), 30 women with promestriene, and 31 women with NaH. Composite scoring, vaginal maturation index (VMI), pH, frequency of sexual activity, serum hormone levels, and endometrial thickness were evaluated VA before and after treatment. Results: In the comparison of VA examination findings composite scoring, VMI, and vaginal pH values, three different drugs were found to be effective in the treatment (P < 0.05). The VMI following VE treatment was significantly higher than that after NaH treatment (P = 0.031), whereas the promestriene group had a more positive change than the others in terms of increase in after treatment composite scoring and sexual activity frequency (P = 0.031, P = 0.020). There were no differences between the groups in terms of pre and after treatment serum E2 levels and endometrial thickness. Conclusions: Based on these findings, we can conclude that the use of promestriene or NaH can prove to be as effective and well tolerated as vaginal estradiol in the symptomatic treatment of vaginal atrophy.
Methylphenidate Induced Lip and Tongue Biting

Attention deficit hyperactivity disorder (ADHD) is a life-long neurodevelopmental disorder and treatment depends on pharmacotherapy because of its biological origin. Stimulant drugs are the most commonly used treatment for ADHD and they have various side effects. Herein, we report a case who bit off the tip of her tongue with Osmotic Release Oral System methylphenidate (OROS MPH) 36 mg/day, bit the tip of her lower lip with immediate release (IR) MPH 10 mg/day and lateral part of her tongue with IR MPH 20 mg/day. A diagnosis of epilepsy was unlikely because of the normal neurological examination and electroencephalography findings. This case was considered as an atypical side effect of MPH such as perseverative/compulsive behaviours and movement disorders. Clinicians should be aware of that stimulant medications may cause lip and tongue biting behavior and this may effect treatment compliance tremendously.

Background: Human papilloma virus (HPV) occurs in women of all age groups, and causes cervical, anal, vaginal, vulvar, penile and oropharyngeal cancers. The aim of the study was to discover what nurses know about HPV infection, testing and vaccination and to determine vaccine practice of their daughters and perceived barriers. Materials and Methods: This cross-sectional and prospective study was carried out nurses who have worked in a hospital between January and June 2014. Pre-test and post-test were used to evaluate the nurses' knowledge about HPV infection, testing and vaccination. This study was performed with nurses who had girls between 9 and 26 years of age for evaluating the behavior of vaccination after three months of education. Results: The mean of pre-test and post-test scores about HPV infection, which included 22 items, were $8.2{\pm}5.6$ and $19.2{\pm}1.5$, respectively. Before education the HPV testing knowledge score was remarkably poor ($1.9{\pm}1.7$ over 5), after education it increased to $4.8{\pm}0.5$. The mean HPV vaccine knowledge score were $3.7{\pm}2.7$ (pre-test) $7.3{\pm}0.8$ (post-test) on a 0-8 scale. The difference between mean total pre-test ($13.9{\pm}9.1$) and post-test ($31.3{\pm}1.9$) scores was statistically significant (p<0.001). After three months of education, only two of the nurses' daughters were vaccinated. The main reason was noted by nurses were not willing to be vaccinated was cost, doubts about safety and efficacy related to the vaccine. About one-third of nurses declared that they would receive the vaccine for their daughter later. Conclusions: Nurses have a crucial role in the prevention, treatment, increasing public awareness and care for population. The education of the nurses about HPV infection, test and vaccination will play an important part decreasing cancer mortality and morbidity.
THE GRAM AND HANKEL MATRICES VIA SPECIAL NUMBER SEQUENCES

In this study, we consider the Hankel and Gram matrices which are defined by the elements of special number sequences. Firstly, the eigenvalues, determinant, and norms of the Hankel matrix defined by special number sequences are obtained. Afterwards, using the relationship between the Gram and Hankel matrices, the eigenvalues, determinants, and norms of the Gram matrices defined by number sequences are given.
Use of UVP to Investigate the Evolution of Bed Configuration

Ultrasonic Velocity Profiler (UVP) is a widely used instrument which allows the determination of the complete velocity profile. UVP can also measure the echo amplitude in the mean time. In this study, it is intended to use the UVP transducers in an indirect way to detect the bottom surface by orienting them vertically downward. Preliminary experiments were conducted with immobile bed by using uniform, non-uniform sediment as well as wooden and plastic surface. It was appeared that the first remarkable peak corresponds to the bottom surface. The time variations of local bed elevations were registered by applying this technique in order to determine the erosion and deposition around a bridge pier and local scour around a horizontal submerged pipe. It was revealed that the so registered scour depths were in good agreement with those observed during the experiments. Consequently, it is demonstrated that time varied bed elevations can be measured via the first peak value of echo amplitudes by placing UVP transducers vertically.

Objective: Although attention deficit hyperactivity disorder (ADHD) is a disease with high genetic transition, our knowl-edge about the mechanism of the disease is limited. In this study, it was aimed to evaluate the levels of miR-132-3p and miR-942-5p that are associated with the dopamine carrier protein gene (DAT1) and dopamine receptor 5 (DRD5) genes, which have been shown to play a role in the development of ADHD. Methods: According to the Diagnostic and Statistical Manual of Mental Disorders 5th edition, 50 children diagnosed with ADHD and 48 healthy controls were included in the study. Affective Disorders and Schizophrenia Interview Sche-dule-Now and Lifetime Version-Turkish Adaptation was used to evaluate ADHD and the diagnoses accompanying ADHD. Quantitative Real-Time Polymerase Chain Reaction was used to evaluate miR-132-3p and miR-942-5p expression levels. Results: It was observed that miR-132-3p level (p = 0.001) was significantly higher with children with ADHD compared to the control group, and the level of miR-942-5p (p = 0.181) was higher in ADHD but did not reach statistically significant level. Conclusion: In our study, we found that the increase in the miR-132-3p levels of children with ADHD may be a ther-apeutic target of the disease.

The objective of this study was to investigate the effects of partial beef fat replacement (0, 30, 50, 100%) with gelled emulsion (GE) prepared with olive oil on functional and quality properties of model system meat emulsion (MSME). GE consisted of inulin and gelatin as gelling agent and characteristics of gelled and model system meat emulsions were investigated. GE showed good initial stability against centrifugation forces and thermal stability at different temperatures. GE addition decreased the pH with respect to increase in GE concentration. Addition of GE increased lightness and yellowness but reduced redness compared to control samples. The results of the study showed that partial replacement of beef fat with GE could be used for improving cooking yield without negative effects on water holding capacity and emulsion stability compared to C samples when replacement level is up to 50%. The presence of GE significantly affected textural behaviors of samples (p<0.05). In conclusion, our study showed that GE have promising impacts on developing healthier meat product formulations besides improving technological characteristics.
