Huge subcapsular hematoma caused by intrahepatic sarcomatoid cholangiocarcinoma

Intrahepatic sarcomatoid cholangiocarcinomais is a very rare disease with a poor prognosis due to its biologically aggressive tumor behavior. We report a patient who presented with subcapsular hemorrhage and a rapidly growing liver mass. A 57 year-old man was admitted with severe abdominal pain. CT and MRI images showed the presence of a 10 cm-sized subcapsular hemorrhage connected with a multi-lobulated mass with hemorrhage and necrotic foci in the right liver. The patients underwent right hemihepatectomy with caudate lobectomy and lymphadenectomy. The operation findings revealed metastatic nodules to the diaphragm and omentum. Detailed histopathological analysis through immunohistochemistry confirmed the diagnosis of sarcomatoid cholangiocarcinoma with a poorly undifferentiated sarcomatous component. The patient underwent chemotherapy. To date, the patient is doing well for 8 months after initial diagnosis. (Korean J Hepatobiliary Pancreat Surg 2012;16:70-74)

With increased use of magnetic resonance imaging(MRI), diagnosis of cavernous ma formations has become easy. These malformations are often small and can be difficult to localize during surgery. Small deep lesions within the brain. and/or near the eloquent areas are difficult to localize and resect without significant morbidity. But using the interactive Image Guided System, provides the 3-dimensional view of the actual lesions and it also allows the surgeon to plan a surgical strategy that takes into account the relationship of the lesion. Therefore Image Guided Surgery for deep seated or brain stem lesions represent a very accurate and safe method.
Compressive Strength Properties of Fly Ash-Based Geopolymer Concrete

<P>This study intends to investigate experimentally the mechanical characteristics of the compressive strength and elastic modulus of concrete using 3 types of binder that are ordinary Portland cement, fly ash and physically milled fly ash. The test results show that the compressive strength and elastic modulus of the cement-zero concrete reached respectively 30.0 MPa and 19.1 GPa, and indicated that more than 90% of the strength was developed at early age. In addition, a comparison of the geopolymer concrete with ordinary concrete enabled to derive and suggest formulaeexpressing the elastic modulus in function of the compressive strength.</P>

<P>Repair materials for concrete structures are often required to exhibit high bond strength at a concrete substrate, and it typically consists of ordinary Portland cement (OPC) incorporating expensive admixtures at a low water-to-binder ratio. Meanwhile, cementless mortar employs alkali-activation of cementless and pozzolanic precursors such as ground granulated blast-furnace slag (GGBFS) and fly ash (FA). The final product develops strength relatively faster than OPC, and its strength can be flexibly talyored by controlling the type and dosage of the activator. The present study investigates the strength development characteristic of cementles mortar for use in repair of concrete structures. Independent variables include mix proportions and curing conditions, which were chosen to optimize the performance of the cementless mortar.</P>

A sixteen years old male patient was treated with the fractionated stereotactic radiosurgery(FSR) using Novalis system. The patient had surgery initially at other hospital before FSR and suffered from visual and endocrine dysfunctions which was required hormone replacement. Seven months after FRS, we found marked decrease of tumor volume. Now he is followed for 1 year. There is a still controversy regarding the ideal management of craniopharyngiomas. Radical resection is hazardous and can produce unacceptable mortality and morbidity. Many authors noted that postoperative conventional radiotherapy or Gamma knife radiosurgery after limited excision achieved better results than total excision of tumor for survival and postoperative complications. FRS delivered precise radiation to a target volume in close proximity to surrounding critical structures resulted in tumor regression. FRS using the relocatable mask frame may have a good role for adjuvant treatment of recurred or relapsed craniopharyngiomas.
