Successful Perforator-Based Island Flap Placement Using the Push Technique

Background: Several surgical options are available for covering soft tissue defects. Perforator-based flaps can be utilized without skeletonization of the perforator, and surgery is usually straightforward, albeit with limited arcs of rotation. To overcome this limitation, we present our experience of the push technique, whereby the flap is manually pushed to release soft tissue attachments, allowing circumvention of the meticulous dissection required to enhance arc rotation. This technique allows faster flap elevation with minimal complications. Methods: The records of 37 patients that underwent perforator-based flap surgery using the push technique from September 2020 to June 2021 at our institution were retrospectively reviewed. Results: All 38 flaps survived completely though one flap developed congestion resulting in partial necrosis and revision. Mean operation time was less than 80 minutes. No dehiscence or recurrence was encountered at any donor or recipient site during follow-up. Conclusion: We present the “push technique” used during perforator-based flap surgery. This method allows for efficient, minimal, and rapid flap elevation without meticulous dissection and helps the operator determine which parts of soft tissue attachments require further release. The push technique can be performed safely and easily without jeopardizing flap survival due to venous ischemic or congestive change. It serves as a good practice model for inexperienced surgeons wishing to cover moderate-to large-sized defects.
적외선 열화상기법을 이용한 원공노치를 갖는 금속의 피로한도 측정

열화상기법을 이용한 피로한도 측정은 시편 형상에 따른 불확실성이 있다. 본 연구에서는 비접촉식 열화상기법을 이용하여 Al6061과 STS304 소재의 원공노치 시편의 피로시험 시 소산열 거동을 분석하고 피로한도 측정법을 제안하였다. 원공노치의 선단부에서 측정위치를 변수로 소산열 거동을 측정하였으며 완전교번응력 하에서 1 Hz, 5 Hz의 하중주파수로 피로시험을 수행하였다. 하중주파수 5 Hz, 원공노치 앞 1 mm에서 가장 뚜렷한 소산열의 변화거동을 보였으며, 이렇게 구한 최적실험조건을 고정하고 Hayabusa 방법을 개선하여 OCM(one curve method)으로 피로한도를 구한 결과 1.1%의 낮은 오차를 보여 열화상기법에 의한 피로한도 측정의 정확성을 제시하였다. The fatigue limit measurement using the thermography method has uncertainties depending on the shape of the specimen. In this study, we analyzed the behavior of heat dissipation during the fatigue tests of circular hole notched specimens of Al6061 and STS304 using a non-contact thermography method and proposed an adequate measuring method of the fatigue limit. Dissipated heat behavior was measured according to various measurement locations ahead of the hole notch tip, and fatigue tests were performed with full alternating stress and load frequencies of 1 Hz and 5 Hz. The fatigue limit measurement was the most accurate at the load frequency of 5 Hz and 1 mm in front of the hole notch. Based on the optical experimental conditions and by improving the Hayabusa method, the fatigue limit was calculated using the one curve method that showed a low error of 1.1%, suggesting an excellent accuracy of the fatigue limit measurement by the thermography method.

Metal Fatigue-Limit Estimation Based on Intrinsic Dissipated Energy

Fatigue-limit estimation of engineering materials through thermographic methods has been improved for reliable prediction. In this study, the fatigue limit for STS304 austenitic stainless steel was estimated at load frequencies of 1 and 5 Hz on the basis of the surface temperature change and intrinsic dissipated energy. The sensitivity of STS 304 steel to fatigue damage, the deviation of the measurement data, and the determination coefficient of the predicted fatigue limit were measured at various stress amplitudes through a fatigue test. When the intrinsic dissipated energy was adopted as an indicator, it significantly improved the damage sensitivity and the reliability coefficient in comparison to the surface-temperature change. The intrinsic dissipated energy behavior at 5 Hz predicted a reliable fatigue limit, in which the predicted limit value was approximately 10% lower than that obtained by the staircase method.

The result shows that the need for play (entertaining) and image as social reputation affect the current amount of use, while the needs for information search, building relationship, and entertainment would make them continue to use.
