      • Knowledge about Risk Factors for Breast Cancer and Having a Close Relative with Cancer Affect the Frequency of Breast Self-Examination Performance

        Background: Breast cancer incidence and mortality rates are increasing in North-Eastern Brazil and the patients with the disease often presented at advanced stages. The present study was focused on identifying variables that affect women's frequency of breast self- examination (BSE) performance. Materials and Methods: Data on BSE, socio-economic parameters and risk factors for breast cancer were obtained from 417 women from a community in North-Eastern Brazil by a self-informant method. To identify independent variables that affect frequency of BSE, nominal logistic regression analysis was performed. Results: Of 417 women, 330 (79.3%) reported performing BSE. Compared to high-income women, BSE performance by low-income women every month was 7.69 (OD=0.130; CI 95%: 0.044- 0.0386; p=0.000) times lower. Women who did not live in a stable union performed BSE each month 2.73 (OD=0.366; CI 95%: 0.171-0.782; p=0.010) less often than those living in a stable union. BSE performance every month and every six months or every year by women with poor knowledge about risk factors for breast cancer was 3.195 (OD=0.313; CI 95%: 0.141- 0.695; p=0.004) times and 2.028 (OD=0.493; CI 95%: 0.248- 0.979; p=0.043) times lower, compared to women with good knowledge. Participants who had a close relative with cancer performed BSE every month and every six months or every year 2.132 (OD=0.469; CI 95%: 0.220-0.997; p=0.049) times and 2.337 (OD=0.428; CI 95%: 0.219-0.836; p=0.013) times less often, compared to those women without close relatives with cancer. Conclusions: The results of this study indicated that income, marital status, knowledge about risk factors and having a close relative with breast cancer, affect the frequency of BSE performance. Information about risk factors in public health campaigns could additionally strengthen avoidance behaviour and also motivate BSE performance.

      • Dark-photon searches via Higgs-boson production at the LHC

        <P>Dark photons (gamma) over bar mediating long-range forces in a dark sector are predicted by various new physics scenarios, and are being intensively searched for in experiments. We extend a previous study of a new discovery process for dark photons proceeding via Higgs-boson production at the LHC. Thanks to the nondecoupling properties of the Higgs boson, BR(H ->gamma(gamma) over bar) values up to a few percent are possible for a massless dark photon, even for heavy dark-sector scenarios. The corresponding signature consists (for a Higgs boson at rest) of a striking monochromatic photon with energy E-gamma = m(H)/2, and a similar amount of missing energy. We perform a model-independent analysis at the LHC of both the gluon-fusion and vector-boson fusion (VBF) Higgs production mechanisms at 14 TeV, including parton-shower effects, and updating our previous parton-level analysis at 8 TeV in the gluon-fusion channel by a more realistic background modeling. We find that a 5 sigma sensitivity can be reached in the gluon-fusion channel for BR(H ->gamma(gamma) over bar) similar or equal to 0.1% with an integrated luminosity of L similar or equal to 300 fb(-1). The corresponding VBF reach is instead restricted to 1%. Such decay rates can be naturally obtained in dark-photon scenarios arising from unbroken U(1)(F) models explaining the origin and hierarchy of the Yukawa couplings, strongly motivating the search for this exotic Higgs decay at the LHC.</P>

        Exploratory Study of Epidermis, Basement Membrane Zone, Upper Dermis Alterations and Wnt Pathway Activation in Melasma Compared to Adjacent and Retroauricular Skin

        Background: Melasma is a chronic acquired focal hypermelanosis which pathogenesis has not been fully elucidated. Classical pathophysiologic studies have analysed the affected and perilesional areas, but little is known about the status of sun-protected skin, which is subjected to the same endogenous and genetic factors. Objective: To assess the histological characteristics of melasma compared to adjacent and retroauricular skin. Methods: Skin samples were collected from 10 female from: melasma, perilesional area and retroauricular. The samples were stained (haematoxylin-eosin, periodic acid-Schiff, Fontana-Masson, picrosirius red, toluidine blue and Verhoeff), immunolabelled for CD34 and Wnt1. The data from the skin sites were analysed simultaneously by a multivariate model. Results: Melasma skin exhibited noteworthy stratum corneum compaction, greater collagen heterogeneity, solar elastosis, higher number of mast cells, basement membrane zone (BMZ) damage, Wnt1 expression, pendulum melanocytes, higher cellularity and vascular proliferation at the superficial dermis. Stratum corneum compaction, collagen heterogeneity and BMZ abnormalities were variables associated to melasma that not follow a continuum through retroauricular to adjacent skin. Mast cell count was the variable that disclosed correlation with the most other abnormalities as well as had the greater contribution in the multivariate model. Conclusion: In addition to melanocyte hyperactivity, melasma skin exhibits alterations in the epidermal barrier, upper dermis and BMZ, which differ from the adjacent sun-exposed skin and retroauricular skin, indicating a distinct phenotype, rather than a mere extension of photoageing or intrinsic ageing.         SIMMER extension for multigroup energy structure search using genetic algorithm with different fitness functions

        The multigroup transport theory is the basis for many neutronics modules. A significant point of the cross-section (XS) generation procedure is the choice of the energy groups' boundaries in the XS libraries, which must be carefully selected as an unsuitable energy meshing can easily lead to inaccurate results. This decision can require considerable effort and is particularly difficult for the common user, especially if not well-versed in reactor physics. This work investigates a genetic algorithm-based tool which selects an appropriate XS energy structure (ES) specific for the considered problem, to be used for the condensation of a fine multigroup library. The procedure is accelerated by results storage and fitness calculation speedup and can be easily parallelized. The extension is applied to the coupled code SIMMER and tested on the European Sustainable Nuclear Industrial Initiative (ESNII+) Advanced Sodium Technological Reactor for Industrial Demonstration (ASTRID)-like reactor system with different fitness functions. The results show that, when the libraries are condensed based on the ESs suggested by the algorithm, the code actually returns the correct multiplication factor, in both reference and voided conditions. The computational effort reduction obtained by using the condensed library rather than the fine one is assessed and is much higher than the time required for the ES search.

        Bovine Aortic Arch and Bilateral Retroesophageal Course of Common Carotid Arteries in a Symptomatic Patient

        Anatomical variations of carotid arteries may be related to their development(agenesis, aplasia, hypoplasia) or course (coiling, kinking, tortuosity). Partial ortotal aberrancies in carotid vessel anatomy rarely occur. We describe the case of a95-year-old woman presented with sudden onset of confusion and disorientationtogether with upper limb clonus. Computed tomography (CT)-scan revealed aleft frontal brain injury with a not conclusive carotid doppler ultrasound. CTangiography reported a bovine aortic arch with bilateral retroesophageal courseof both common carotid arteries and left severe (>70%) internal carotid arterystenosis. The knowledge of anatomical variations of the course of carotid arteries isrelevant for possible surgical or endovascular repair or in case of otolaryngology orintubation procedures.

      • Seasonal patterns of heavy metal deposition to the snow on Lambert Glacier basin, East Antarctica

        Al, V, Mn, Fe, Cu, As, Cd, Ba, Pb, Bi and U were determined in a continuous series of 46 snow samples from a 2.3-m snow pit, covering the time period from austral spring 1998 to summer 2002, at a site on the east side of the Lambert Glacier basin in East Antarctica. Concentrations are very low for all metals and differ by orders of magnitude from one metal to another, with the mean concentrations ranging from 0.028pgg<SUP>-1</SUP> for Bi to 165pgg<SUP>-1</SUP> for Al. It is estimated that anthropogenic contributions are dominant for Cu, Pb and probably As, in the snow in our study area while the natural contributions from rock and soil dust, sea-salt spray and volcanic emissions account for most of the measured concentrations of the other metals. Our snow profiles show pronounced seasonal variations for Mn, As, Ba, Pb and Bi throughout the year, but a very different situation is observed between different metals. These observations suggest that heavy metals determined in our samples are controlled by different transport and deposition mechanisms related to physical and chemical alterations in the properties and sources of aerosol.

        Two-Layer Hand-Sewn Esophagojejunostomy in Totally Laparoscopic Total Gastrectomy for Gastric Cancer

        Purpose: Different esophagojejunostomy (EJ) reconstruction methods are used after totally laparoscopic total gastrectomy (TLTG), and none is considered a standard technique. This report describes a 2-layer hand-sewn EJ technique during TLTG; we also evaluated postoperative morbidity associated with this technique. Materials and Methods: This retrospective cohort study included all consecutive patients who underwent TLTG for gastric cancer (GC) from 2012 to 2016 at 2 affiliated teaching hospitals. All participating surgeons performed standardized intracorporeal 2-layer hand-sewn EJ. Results: We included 51 patients who underwent TLTG for GC and standardized EJ anastomosis. Twenty-seven (53%) were male, and the median age was 60 (36–87) years. The average operative time was 337±71 minutes and intraoperative bleeding was 160±107 mL. There were no open conversions related to EJ. Postoperative morbidity was observed in 9 (17.0%) patients. There was no postoperative mortality. EJ leakage was observed in 2 patients (3.8%) and 1 patient (1.9%) developed EJ stenosis. Patients with leakage were managed non-operatively and the patient with stenosis required endoscopic dilation. The median length of hospital stay was 8 (6–29) days. Conclusions: Two-layer hand-sewn EJ during TLTG for GC is a feasible and safe technique. This method avoids a laparotomy for reconstruction and the disadvantages associated with laparoscopic introduction of mechanical staplers for EJ, and provides an alternative for alimentary tract reconstruction after TLTG.

        Alveolar ridge augmentation with the perforated and nonperforated bone grafts

        Purpose: Autogenous bones are frequently used because of their lack of antigenicity, butgood osteoconductive and osteoinductive properties. This study evaluated the biologicalbehavior of perforated and nonperforated cortical block bone grafts. Methods: Ten nonsmoking patients who required treatment due to severe resorption of thealveolar process and subsequent implant installation were included in the study. The inclusioncriteria was loss of one or more teeth; the presence of atrophy of the alveolar processwith the indication of reconstruction procedures to allow rehabilitation with dental implants;and the absence of systemic disease, local infection, or inflammation. The patientswere randomly divided into two groups based on whether they received a perforated (innersurface) or nonperforated graft. After a 6-month healing period, a biopsy was performedand osseointegrated implants were installed in the same procedure. Results: Fibrous connective tissue was evident at the interface in patients who receivednonperforated grafts. However, full union between the graft and host bed was visible inthose who had received a perforated graft. Conclusions: We found that cortical inner side perforations at donor sites increased thesurface area and opened the medullary cavity. Our results indicate an increased rate ofgraft incorporation in patients who received such perforated grafts.
