      • Genomic Common Data Model for Seamless Interoperation of Biomedical Data in Clinical Practice: Retrospective Study

        <P><B>Background</B></P><P>Clinical sequencing data should be shared in order to achieve the sufficient scale and diversity required to provide strong evidence for improving patient care. A distributed research network allows researchers to share this evidence rather than the patient-level data across centers, thereby avoiding privacy issues. The Observational Medical Outcomes Partnership (OMOP) common data model (CDM) used in distributed research networks has low coverage of sequencing data and does not reflect the latest trends of precision medicine.</P><P><B>Objective</B></P><P>The aim of this study was to develop and evaluate the feasibility of a genomic CDM (G-CDM), as an extension of the OMOP-CDM, for application of genomic data in clinical practice.</P><P><B>Methods</B></P><P>Existing genomic data models and sequencing reports were reviewed to extend the OMOP-CDM to cover genomic data. The Human Genome Organisation Gene Nomenclature Committee and Human Genome Variation Society nomenclature were adopted to standardize the terminology in the model. Sequencing data of 114 and 1060 patients with lung cancer were obtained from the Ajou University School of Medicine database of Ajou University Hospital and The Cancer Genome Atlas, respectively, which were transformed to a format appropriate for the G-CDM. The data were compared with respect to gene name, variant type, and actionable mutations.</P><P><B>Results</B></P><P>The G-CDM was extended into four tables linked to tables of the OMOP-CDM. Upon comparison with The Cancer Genome Atlas data, a clinically actionable mutation, p.Leu858Arg, in the <I>EGFR</I> gene was 6.64 times more frequent in the Ajou University School of Medicine database, while the p.Gly12Xaa mutation in the <I>KRAS</I> gene was 2.02 times more frequent in The Cancer Genome Atlas dataset. The data-exploring tool GeneProfiler was further developed to conduct descriptive analyses automatically using the G-CDM, which provides the proportions of genes, variant types, and actionable mutations. GeneProfiler also allows for querying the specific gene name and Human Genome Variation Society nomenclature to calculate the proportion of patients with a given mutation.</P><P><B>Conclusions</B></P><P>We developed the G-CDM for effective integration of genomic data with standardized clinical data, allowing for data sharing across institutes. The feasibility of the G-CDM was validated by assessing the differences in data characteristics between two different genomic databases through the proposed data-exploring tool GeneProfiler. The G-CDM may facilitate analyses of interoperating clinical and genomic datasets across multiple institutions, minimizing privacy issues and enabling researchers to better understand the characteristics of patients and promote personalized medicine in clinical practice.</P>

        Method for the Analysis of Temporal Change of Physical Structure in the Instrumentation and Control Life-cycle

        The design of computer-based instrumentation and control (I&C) systems is determined by the allocation of I&C functionsto I&C systems and components. Due to the characteristics of computer-based technology, component failures can negativelyaffect several I&C functions, so that the reliability proof of the I&C systems requires the accomplishment of I&C systemdesign analyses throughout the I&C life-cycle. On one hand, this paper proposes the restructuring of the sequential IEC 61513 I&C life-cycle according to the V-model,so as to adequately integrate the concept of verification and validation. On the other hand, based on a metamodel for themodeling of I&C systems, this paper introduces a method for the modeling and analysis of the effects with respect to thesuperposition of failure combinations and event sequences on the I&C system design, i.e. the temporal change of physicalstructure is analyzed. In the first step, the method is concerned with the modeling of the I&C systems. In the second step, themethod considers the analysis of temporal change of physical structure, which integrates the concepts of the diversity anddefense-in-depth analysis, fault tree analysis, event tree analysis, and failure mode and effects analysis.

        Started upon Its discovery by Wright et al in 1973, studies on the solid polymer electrolyte are being carried out vigorously. So. models of Ll-polymer battery have been developed through R-L-C components combination and P-splce functional block in this paper. The Impedance characteristics of Li-polymer battery with R-L-C components are presented. Simulation results using P-spice functional model are compared with measured charge/discharge characteristics.

        In the System-integrated modular advanced reactor (SMART), the motor for Control element drive mechanism(CEDM) requires high density power and simple drive mechanism to reduce volume because of restriction by install-space and must satisfy the reactor operating circumstances with high pressure and temperature.<br/> To Maximize the characteristics of the TFM, we chose the asymmetric bridge converter as the driving system for TFM. Because two switching devices are connected in series with the stator winding of each phases in the asymmetric bridge converter all the phases are not affected by another phase but controlled independently. Also, this converter has many advantages that the various control methods can be adopted, it is easy to control, and that in case that the switching devices of a phase are damaged, the affects can be minimized.<br/>

        Variation in radial head fracture treatment recommendations in terrible triad injuries is not influenced by viewing two-dimensional computed tomography

        Background: We analyzed association between viewing two-dimensional computed tomography (2D CT) images in addition to radiographs with radial head treatment recommendations after accounting for patient and surgeon factors in a survey-based experiment. Methods: One hundred and fifty-four surgeons reviewed 15 patient scenarios with terrible triad fracture dislocations of the elbow. Surgeons were randomized to view either radiographs only or radiographs and 2D CT images. The scenarios randomized patient age, hand dominance, and occupation. For each scenario, surgeons were asked if they would recommend fixation or arthroplasty of the radial head. Multi-level logistic regression analysis identified variables associated with radial head treatment recommendations. Results: Reviewing 2D CT images in addition to radiographs had no statistical association with treatment recommendations. A higher likelihood of recommending prosthetic arthroplasty was associated with older patient age, patient occupation not requiring manual labor, surgeon practice location in the United States, practicing for five years or less, and the subspecialties “trauma” and “shoulder and elbow.” Conclusions: The results of this study suggest that in terrible triad injuries, the imaging appearance of radial head fractures has no measurable influence on treatment recommendations. Personal surgeon factors and patient demographic characteristics may have a larger role in surgical decision making.
