        Effects of load height application and pre-bucklingdeflections on lateral buckling of thin-walled beams

        Based on a non-linear model taking into account flexural-torsional couplings, analytical solutions are derived for lateral buckling of simply supported I beams under some representative load cases. Aclosed form is established for lateral buckling moments. It accounts for bending distribution, load height application and pre-buckling deflections. Coefficients C1 and C2 affected to these parameters are then derived. Regard to well known linear stability solutions, these coefficients are not constant but depend on another coefficient k1 that represents the pre-buckling deflection effects. In numerical simulations, shell elements are used in mesh process. The buckling loads are achieved from solutions of eigenvalue problem and by bifurcations observed on non linear equilibrium paths. It is proved that both the buckling loads derived from linear stability and eigenvalue problem lead to poor results, especially for I sections with large flanges for which the behaviour is predominated by pre-buckling deflection and the coefficient k1 is large. The proposed solutions are in good agreement with numerical bifurcations observed on non linear equilibrium paths.

        Tea Flavonoids Induced Differentiation of Peripheral Blood-derived Mononuclear Cells into Peripheral Blood-derived Endothelial Progenitor Cells and Suppressed Intracellular Reactive Oxygen Species Level of Peripheral Blood-derived Endothelial Progenitor

        Endothelial dysfunction in atherosclerosis is associated with increasing oxidative stress that could be reversed by antioxidant. Therefore epigallocatechin gallate (EGCG), epicatechin gallate (ECG), epigallocatechin (EGC) and catechin (C) of tea flavonoids were investigated for their roles in regenerating endothelial cell. Peripheral blood mononuclear cells (PB-MNCs) were isolated, plated and cultured in medium with/without treatment of EGCG, ECG, EGC and C. Results showed that among all EGCG, ECG, EGC and C concentrations tested, 12.5 ?mol/L was not cytotoxic for peripheral blood-derived endothelial progenitor cells (PB-EPCs). Treatment of EGCG, ECG, EGC or C increased the percentages of CD34, CD133, VEGFR-2 expressions and suppressed hydrogen peroxide-induced percentages of reactive oxygen species (ROS) level in PB-EPCs. Taken together, our current results showed that EGCG, ECG, EGC or C of tea flavonoids could induce differentiation of PB-MNCs into PB-EPCs as well as protect PB-EPCs from oxidative damage by suppresing the intracellular ROS levels.

        Development of a High-Resolution Multi-Locus Microsatellite Typing Method for Colletotrichum gloeosporioides

        Colletotrichum gloeosporioides is an economically important fungal pathogen causing substantial yield losses indifferent host plants. To understand the genetic diversity and molecular epidemiology of this fungus, we have developed a novel, highresolution multi-locus microsatellite typing (MLMT) method. Bioinformatic analysis of C. gloeosporioides unannotated genome sequence yielded eight potential microsatellite loci, of which five, CG1 (GT)<sub>n</sub>, CG2 (GT1)<sub>n</sub>, CG3 (TC)<sub>n</sub>, CG4 (CT)<sub>n</sub>, and CG5 (CT1)<sub>n</sub> were selected for further study based on their universal amplification potential, reproducibility, and repeat number polymorphism. The selected microsatellites were used to analyze 31 strains of C. gloeosporioides isolated from 20 different host plants from India. All microsatellite loci were found to be polymorphic, and the approximate fragment sizes of microsatellite loci CG1, CG2, CG3, CG4, and CG5 were in ranges of 213-241, 197-227, 231-265, 209-275, and 132-188, respectively. Among the 31 isolates, 55 different genotypes were identified. The Simpson’s index of diversity (D) values for the individual locus ranged from 0.79 to 0.92, with the D value of all combined five microsatellite loci being 0.99. Microsatellite data analysis revealed that isolates from Ocimum sanctum, Capsicum annuum (chili pepper), and Mangifera indica (mango) formed distinct clusters, therefore exhibited some level of correlation between certain genotypes and host. The developed MLMT method would be a powerful tool for studying the genetic diversity and any possible genotype-host correlation in C. gloeosporioides.

        With the growing complexity of the U.S. health care system, highly motivated medical directors with strong leadership skills are vital to the success of health care facilities. Presently, there are no articles assessing a plastic surgeon's qualifications for the role of medical director. In addition, there is a paucity of literature comparing the responsibilities of medical directors across various types of health care institutions. Herein, we outline why plastic surgeons have the unique skillset to succeed in this role and highlight the differences between medical director positions across the vast landscape of health care. While the intricacies of this position vary greatly across different landscapes of the health care industry, successful medical directors lead by following a set of universal principles predisposing them for success. Plastic surgeons innately exhibit a subset of particular traits deeming them suitable candidates for the medical director position. While transitioning from the role of a surgeon to that of a medical director does require some show of adaptation, plastic surgeons are ultimately highly likely to find intrinsic benefit from serving as a medical director.

        The Impact of Service Orientation on Organizational Performance in Public Sectors: Empirical Evidence from Indonesia

        The importance of the public sector’s role in fostering a positive business climate has prompted public sector organizations to consistently enhance their performance. The study aims to develop service orientation dimensions for public sectors and examine the relationship between service orientation and organizational performance. A field survey was employed in this study. Six hundred questionnaires were distributed, and four hundred and eighty-eight were returned and analyzed. Factor analysis and multiple regression analysis were used in the dataset. This study identifies five dimensions of organizational service orientation in public sector service organizations: technologyservice standard-communication, service vision, service delivery, service training and powering, and servant leadership. The result also concludes that service orientation influences organizational performance, such as corporate growth, service quality image, IT effectiveness, service innovation, and public complaint. This study’s findings imply that public sector organizations should rectify service orientation factors to increase corporate growth, service quality image, IT effectiveness, service innovation, and public complaint reduction. Managerial guidelines are presented for developing a service orientation.

        Long-Term Efficacy of Anti-Tumor Necrosis Factor Agents in Pediatric Luminal Crohn’s Disease: A Systematic Review of Real-World Evidence Studies

        Purpose: To determine the long-term efficacy of the anti-tumor necrosis factor (TNF) agents, infliximab (IFX) and adalimumab (ADA), in pediatric luminal Crohn's disease (CD) by performing a systematic literature review. Methods: An electronic search was performed in Medline, Embase, and the Cochrane Library from inception to September 26, 2019. Eligible studies were cohort studies with observation periods that exceeded 1 year. Studies that reported time-to-event analyses were included. Events were defined as discontinuation of anti-TNF therapy for secondary loss of response. We extracted the probabilities of continuing anti-TNF therapy 1, 2, and 3 years after initiation. Results: In total, 2,464 papers were screened, 94 were selected for full text review, and 13 studies (11 on IFX, 2 on ADA) met our eligibility criteria for inclusion. After 1 year, 83–97% of patients were still receiving IFX therapy. After 2 and 3 years the probability of continuing IFX therapy decreased to 67–91% and 61–85%, respectively. In total, 5 of the 11 studies subgrouped by concomitant medication consistently showed that the probabilities of continuing IFX therapy in patients with prolonged immunomodulator use were higher than those in patients on IFX monotherapy. Conclusion: This review of real-world evidence studies confirms the long-term therapeutic benefit of IFX therapy in diverse cohorts of children with luminal CD. Moreover, it supports the view that combination therapy with an immunomodulator prolongs the durability of IFX therapy in patients who previously failed to recover following first-line therapy. The limited number of time-to-event studies in patients on ADA prevented us from drawing definite conclusions about its long-term efficacy.

        Vibrations of long repetitive structures by a double scale asymptotic method

        In this paper, an asymptotic two-scale method is developed for solving vibration problem of long periodic structures. Such eigenmodes appear as a slow modulations of a periodic one. For those, the present method splits the vibration problem into two small problems at each order. The first one is a periodic problem and is posed on a few basic cells. The second is an amplitude equation to be satisfied by the envelope of the eigenmode. In this way, one can avoid the discretisation of the whole structure. Applying the Floquet method, the boundary conditions of the global problem are determined for any order of the asymptotic expansions.

        Feasibility of Translaminar Screw Placement in Korean Population: Morphometric Analysis of Cervical Spine

        Purpose: To analyze the feasibility of unilateral and bilateral translaminar screw placement in Koran population, and compare the acceptance rate using previously reported data in American population. Materials and Methods: The translaminar lengths, thickness, heights, and sagittal-diagonal measurements were performed. The feasibility analysis was performed using unilateral and bilateral 3.5 mm cervicalscrew placement on the CT scans within 0.5 mm of safety margin. We also performed radiographic analysis of the morphometric dimensions and the feasibilityof unilateral and bilateral translaminar screw placement at C3‒C7. Results: Korean population had similar or significantly shorter translaminar lengths and thickness (lengths and thickness in C7 among males; lengths in C6‒C7 and thicknessin C4 among females) than American population, but had similar or significantlylonger translaminar heights and sagittal-diagonal measurements (heights in C3‒C7 and sagittal-diagonal measurements in C3‒C6 among males; heights in C7 and sagittal-diagonal measurements in C3‒C7 among females). Unilaterally, translaminarscrew acceptance rates in C3‒C7 were similar between Korean and Americanmale population, but the rates in C4‒C6 were significantly smaller between Korean and American female population. Bilaterally, translaminar screw acceptancerates in C3 and C5‒C6 were significantly larger between Korean and Americanmale population, but the rates in C3‒C7 were similar between Korean and American female population. Conclusion: The feasibility of unilateral and bilateraltranslaminar screw placement is different depending on different ethnics. Subaxialcervical unilateral translaminar screw placement among Korean male populationand bilateral placement at C4‒C7 among Korean female population are more acceptable than American population.
