      • 한부모가족 반편견교육의 모형 연구

        한부모가족지원팀(Single-parent family service t) 연세대학교 상담코칭지원센터 2015 연세상담코칭연구 Vol.3 No.-

        본 연구는 한국 사회 내 한부모가족이 증가함에 따른 한부모가족 반편견교육의 필요성을 이해하고 적절한 반편견교육의 모형을 제시하고자 진행되었다. 반편견교육이란 특정 영역에 대한 고정관념, 편견을 없애고 차이와 다양성 및 그 가치를 인정할 수 있는 능력을 함양 하는 데 목적이 있다. 국내에서는 자살예방 인식개선 교육, 다문화가족 교육 등과 같은 반편견교육이 시행되고 있으며, 한부모가족 반편견교육 역시 공교육의 장에서 시행될 필요가 있다. 체계적 사고의 관점에서 보았을 때, 우리는 한부모가족이 사회와 문화가 반영된 유기체적인 체계 속에 있음을 이해해야 한다. 그리고 이들이 체계 내에서 편견의 장벽을 넘어 창조적 역할을 발휘할 수 있도록하기 위해서는 체계의 변화가 필요하다. 한부모가족 반편견교육의 체계적 접근 모형은 체계의 구성요소인 개인과 대인관계, 사회문화, 제도에 대한 각각의 인식 개선에 대한 접근이 필요함을 보인다. This study is conducted to understand the necessity of the education of anti-bias for single parent family and suggest the proper educational model. Anti-bias education is to help people be more aware of bias and the stereotypes in the society and value diversity. Currently public schools provide anti-bias educations such as a suicidal prevention program and a multicultural family program. An anti-bias program for single parent family also needs to be conducted in the public education. In respect of the systematic thought, the single parent family should be understood in the organic system whose social and cultural factors are reflected. Also should be changed to have a sigle parent family surpass the barrier of bias and use their own creative ability. The education model of anti-bias for a single parent family represents the necessity of improving recognition of each componant of the systems containg person, personal relations, social culture, and institution.

        The purpose of this study is to arrange the ground to give the old a substantial aid, which can allow them to spend their senescence with comfort. For this, it was accomplished to analyze the basic factors, that is, food, clothing, shelter and family relationship and to examine the actual conditions of the old's life. This study is likely to be considered as an opportunity to understand their family life, to furnish essential data on the field of business related the old, and to present the direction to develop various educational programs and social welfare policies for them. The subject of this study was 400 female women who are over 60 years. By the method of interview research, general facts and matters on food, clothing, shelter and family relationship were inquired of them. Among the questionnaires, 278 were taken back and used in collecting final statistics. The whole findings show that role forfeiture, degradation of authority, weakening health, and economical disability most of old people have experienced in senescence have influenced their lives. The support to cope with these matters accompanied in senescence will be able to improve their contentment. Therefore as the subject of the old it is necessary to educate a fundamental knowledge to maintain food, clothing, shelter and family relationship and to develop various kinds of business to solve the basic matters, and to enlarge the opportunities to use with convenience.

        말기 암 환자에서 감염에 대한 항생제 사용은 의학적 문제에 대해 해결해야 하는 의무와 임종을 앞둔 시점에 생명 연장으로 인한 환자의 고통증가에 대한 부담으로 명확하게 결정하기 어려운 문제이다. 대한가정의학회 완화의학연구회 세미나에서는 65세 남자환자로 직장암 말기에 악취를 동반한 광범위한 피부감염증으로 입원 후 국소 항생제 치료로 증상 호전 중 임종을 맞이한 증례를 토의하였고, 이를 통해 말기 암 환자에서 항생제 치료에 대한 문헌고찰과 토론 후 다음과 같이 의견을 제시하고자 한다. 항생제 치료 목적에 있어 증상조절을 고려해야 하며, 특히 요로계 감염이 있는 경우는 증상조절을 위해 항생제를 사용한다. 또한 감수성 검사를 통해 적절한 항생제를 처방해야 한다. 무엇보다도 우선적으로 고려되어야 할 점은 환자입장에서 무엇이 가장 득이 되는지를 생각하고, 항생제 사용에 대해 환자와 가족들의 의견을 존중하여 판단하며, 환자나 가족이 적극적으로 치료를 요구하는 경우에는 치료로 인한 득실에 대해 환자와 토의를 한 후 사용한다. Decision-making of antibiotics use in infected patients with terminal stage of cancer was difficult for physicians, because of responsibility of solving a medical problem and burden on patients distressed by worthless life expansion. Korean Family Medicine Palliative Medicine Research Group discussed this subject using a case of a 65 year-old male having terminal stage of sigmoid colon cancer with extended cutaneous infection who was treated local antibiotics, improved but expired at the 12th hospital day. We reviewed related literatures and proposed a guide for antibiotics use in inferred patients with terminal stage of cancer. Antibiotics should be used for symptom control as major indication, especially when patients suffered from urinary symptoms. Appropriate antibiotics should be chosen based or sensitivity test. the most important considering factor should be patient and family members' wish about antibiotics use.

        여성정책 거버넌스 유형에 대한 분석 : 여성부의 협력사업 사례

        이 글은 여성단체와 여성부가 2001년부터 추진하고 있는 공동협력사업의 내용과 재정지원 규모 등의 내용을 통해 여성부와 여성단체의 관계를 거버넌스 관점에서 분석하고 그 특징을 밝히고자 하였다. 분석을 위해 과거 5년간(2001-2005년) 진행한 협력사업에 대해 단체가 사업의 내용에 얼마나 주체적으로 개입하였으며, 재정분담 측면에서 어느 정도 비율로 참여하는지를 살펴보았다. 여성부 협력사업은 외형적으로 사업내용 선정의 주체성과 재정부담에 있어서 여성부가 주도하고 있어 여성단체는 대리자 유형에 속하는 것으로 보이나, 여성단체가 사업내용의 결정에 크게 개입하고 있어 궁극적으로는 동반자 유형의 거버넌스 특징을 갖는 것으로 나타났다. 이러한 결과는 여성정책을 수행하는데 있어서 여성정책 거버넌스가 갖는 매우 특징적인 현상이라 할 수 있다. 여성단체는 비록 재정적인 측면에서 자율성은 없으나, 사업내용에 있어서는 주체성을 가지고 있다는 의미이다. 이 같은 동반자형 거버넌스의 특징을 보이는 원인은 여성부와 여성단체의 관계, 정부부처 내 여성정책 전문인력의 부재 등 여성 정책기구의 위상과 밀접한 관련이 있다.

        2022 Consensus statement on the management of familial hypercholesterolemia in Korea

        Familial hypercholesterolemia (FH) is the most common monogenic disorder. Due to the marked elevation of cardiovascular risk, the early detection, diagnosis, and proper management of this disorder are critical. Herein, the 2022 Korean guidance on this disease is presented. Clinical features include severely elevated low-density lipoprotein cholesterol (LDL-C) levels, tendon xanthomas, and premature coronary artery disease. Clinical diagnostic criteria include clinical findings, family history, or pathogenic mutations in the LDLR, APOB, or PCSK9. Proper suspicion of individuals with typical characteristics is essential for screening. Cascade screening is known to be the most efficient diagnostic approach. Early initiation of lipid-lowering therapy and the control of other risk factors are important. The first-line pharmacological treatment is statins, followed by ezetimibe, and PCSK9 inhibitors as required. The ideal treatment targets are 50% reduction and < 70 or < 55 mg/dL (in the presence of vascular disease) of LDL-C, although less strict targets are frequently used. Homozygous FH is characterized by untreated LDL-C > 500 mg/dL, xanthoma since childhood, and family history. In children, the diagnosis is made with criteria, including items largely similar to those of adults. In women, lipid-lowering agents need to be discontinued before conception.

      • Minimization of Voltage and Current Total Harmonic Distortion for a Single-Phase Multilevel Inverter Staircase Modulation

        The paper addresses optimal voltage and current quality (minimal Total Harmonic Distortion - THD) for a singlephase multilevel inverter staircase modulation. The previous research mostly addressed optimal THD problem in frequency domain accounting for a limited harmonics count. The novelty of this research is that voltage and current minimal THD problems are formulated in time domain as a constrained optimization ones in order to find theoretically optimal switching angles accounting for all switching harmonics. The optimal switching angles and respective minimal THD values are obtained for different inverter level counts and the whole fundamental voltage (modulation index) range. Suggested are smooth ripplefree approximations for accurate voltage and current THD for arbitrary level counts and modulation indices. While these simple hyperbolic formulas accurately approximate the average trend, local maxima and minima of voltage and current THD may be considered on separate. In fact, the obtained current THD is frequency weighted THD that assumes inductance dominated RL-load. For grid-connected applications, normalized current fundamental harmonic is smaller than voltage modulation index and current THD is essentially larger than frequency weighted voltage one.

        Applications of Metabolic Modeling to Drive Bioprocess Development for the Production of Value-added Chemicals

        Increasing numbers of value added chemicals are being produced using microbial fermentation strategies. Computational modeling and simulation of microbial metabolism is rapidly becoming an enabling technology that is driving a new paradigm to accelerate the bioprocess development cycle. In particular, constraint-based modeling and the development of genome-scale models of industrial microbes are finding increasing utility across many phases of the bioprocess development workflow. Herein, we review and discuss the requirements and trends in the industrial application of this technology as we build toward integrated computational/experimental platforms for bioprocess engineering. Specifically we cover the following topics: (1) genome-scale models as genetically and biochemically consistent representations of metabolic networks; (2) the ability of these models to predict, assess, and interpret metabolic physiology and flux states of metabolism; (3) the model-guided integrative analysis of high throughput 'omics' data; (4) the reconciliation and analysis of on- and off-line fermentation data as well as flux tracing data; (5) model-aided strain design strategies and the integration of calculated biotransformation routes; and (6) control and optimization of the fermentation processes. Collectively, constraint-based modeling strategies are impacting the iterative characterization of metabolic flux states throughout the bioprocess development cycle, while also driving metabolic engineering strategies and fermentation optimization.
