        The role of neutrophil lymphocyte ratio to leverage the differential diagnosis of familial Mediterranean fever attack and acute appendicitis

        Background/Aims: Familial Mediterranean fever (FMF) is an autosomal recessive disorder characterized by attacks of fever and diffuse abdominal pain. The primary concern with this presentation is to distinguish it from acute appendicitis promptly. Thus, we aimed to evaluate the role of neutrophil lymphocyte ratio (NLR) to leverage the differential diagnosis of acute FMF attack with histologically proven appendicitis. Methods: Twenty-three patients with histologically confirmed acute appendicitis and 88 patients with acute attack of FMF were included in the study. NLR, C-reactive protein and other hematologic parameters were compared between the groups. Results: Neutrophil to lymphocyte ratio was significantly higher in patients with acute appendicitis compared to the FMF attack group (8.24 ± 6.31 vs. 4.16 ± 2.44, p = 0.007). The performance of NLR in diagnosing acute appendicitis with receiver operating characteristic analysis with a cut-off value of 4.03 were; 78% sensitivity, 62% specificity, and area under the curve 0.760 (95% confidence interval, 0.655 to 0.8655; p < 0.001). Conclusions: This study showed that NLR, the simple and readily available in-flammatory marker may have a useful role in distinguishing acute FMF attack from acute appendicitis.

        Some properties of a Ricci quarter symmetric metric connection on Lorentzian para-Sasakian manifold

        We study the Lorentzian Para-Sasakian Manifold with respect to Ricci Quarter symmetric metric connection. We find the existence of the Ricci Quarter symmetric metric connection on Lorentzian Para-Sasakian Manifold. We show that there is a relations between the Levi-Civita connection and the Ricci Quarter symmetric metric connection. We deduce the projective cur-vature tensor of Lorentzian para-Sasakian manifold is skew symmetric with respect to Ricci Quarter symmetric metric connection. Finally we obtain con-ditions under which the conformal curvature with respect to Ricci Quarter symmetric metric connection is equal to that with respect to Levi-civita con-nection.

        Theoretical Approach to the Structural, Electronic, and Morphological Properties of Poly(ε-caprolactone) Grafted Polypyrroles

        Poly(ε-caprolactone) grafted polypyrroles (PPy-g-PCL) are an important member of hairy-rod copolymers which have many industrial applications. PPy-g-PCL cooligomers were studied for their electronic, structural and morphological properties by density functional theory (DFT), molecular dynamics and mesoscale dynamics simulation methods. The band gaps of the cooligomers were calculated at the B3LYP/6-31g(d,p) level by varying the grafting position of PCL group. UV spectra was studied by time-dependent DFT methods. The solubility parameters and interaction parameters between the PCL and PPy were calculated via molecular dynamics simulations. The morphological behaviors were studied by the molecular dynamics and dissipative particle dynamics (DPD) methods to understand the role of rigidity of polypyrrole backbone, the chain lengths of PPy and PCL and the incompatibility of the polymers on the formation of layered phase domains observed experimentally for high weight percentages of PCL side chains.

        Facial Emotion Recognition in Schizophrenia: The Impact of Gender

        Objective Previous studies reported gender differences for facial emotion recognition in healthy people, with women performing better than men. Few studies that examined gender differences for facial emotion recognition in schizophrenia brought out inconsistent findings. The aim of this study is to investigate gender differences for facial emotion identification and discrimination abilities in patients with schizophrenia. Methods 35 female and 35 male patients with schizophrenia, along with 35 female and 35 male healthy controls were included in the study. All the subjects were evaluated with Facial Emotion Identification Test (FEIT), Facial Emotion Discrimination Test (FEDT), and Benton Facial Recognition Test (BFRT). Patients' psychopathological symptoms were rated by means of the Positive and Negative Syndrome Scale (PANSS). Results Male patients performed significantly worse than female patients on FEIT total, and negative scores. Male controls performed significantly worse than female controls on FEIT total and negative scores. On all tasks, female patients performed comparable with controls. Male patients performed significantly worse than controls on FEIT, and FEDT. Conclusion Women with schizophrenia outperformed men for facial emotion recognition ability in a pattern that is similar with the healthy controls. It could be claimed that male patients with schizophrenia need special consideration for emotion perception deficits.

        Hypoglycemia Fear and Self-efficacy of Turkish Patients Receiving Insulin Therapy

        Purpose: The purpose of this study was to determine hypoglycemia fear and self-efficacy levels of patients receiving insulin and the factors affecting these levels. Method: In total, 345 diabetic patients who met the inclusion criteria participated in this descriptive,correlational study. Patients were invited to participate in the study during their regular visits to the diabetes outpatient clinic of Istanbul University, Istanbul Medicine Faculty. Data were collected using a patient-information form as well as the Hypoglycemia Fear Survey and Confidence in Diabetes Self-care Scale. Results: It was found that patients who had type 1 diabetes, received intensive insulin therapy and experienced more frequent and severe hypoglycemia had more hypoglycemia worry and fear. It was also determined that patients who had type 1 diabetes and received intensive insulin therapy had higher selfefficacy levels than patients who had type 2 diabetes and received conventional therapy. Conclusion: The effects of experiences of frequent and severe hypoglycemia in patients with diabetes emphasize the need for programs that support diabetes-specific self-efficacy and also guide and teach hypoglycemia prevention.

      • Hedging against bunker price fluctuations by using derivative products

        Recent approaches and opinions support that shipping companies must take precautionary measures against financial risks and design steadier steps for financial management. The key to developing, implementing, and managing a successful hedging strategy is to use effective forecasting systems and appropriate financial derivative products. The key objective of this study is to control risks from bunker cost fluctuations using financial derivative products. To do so, a time-series analysis is conducted using a dataset derived from a bunker index system. The artificial neural network method is used for time-series analysis with a mean absolute percentage error of 0.9182105. Next, progress predictions of bunker costs and hedging strategies are determined to use financial derivative products against their risks. Finally, this study concludes that forward agreements can serve as the perfect protection mechanism against bunker risks in tramp shipping.

        Mediterranean Diet and Overactive Bladder

        Purpose: The relationship between nutrition and overactive bladder (OAB) has yet to be elucidated. Therefore, this study investigated the relationship between the Mediterranean diet and OAB. Methods: The 14-item Mediterranean Diet Adherence Screener (MeDAS) and Overactive Bladder-Validated 8-question Screener (OAB-V8), validated in Turkish, were administered to 500 patients over the age of 18 who presented to outpatient clinics other than urology outpatient clinics. Of those patients, 174 with chronic diseases and urinary tract infections (based on urinalysis and a detailed medical history) were excluded. Therefore, 326 patients’ data were analyzed. Results: There was a negative correlation between the MeDAS and OAB-V8 scores. High OAB-V8 scores were associated with obesity (body mass index ≥30 kg/m2 ), being single, and a low education level. Conclusions: Dietary patterns represent a broader perspective on food and nutrient consumption and may therefore be more predictive of disease risk. The Mediterranean type should be recommended in the first-line treatment of patients with OAB symptoms. It is easily possible to determine the compliance of patients with this diet by using the 14-item MeDAS.

        Association of Fluorodeoxyglucose Positron Emission Tomography Radiomics Features with Clinicopathological Factors and Prognosis in Lung Squamous Cell Cancer

        Aim To evaluate the role of fluorine-18 fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography(PET/CT) radiomics features (RFs) for predicting clinicopathological factors (CPFs) and prognosis in patients with resectedlung squamous cell cancer (LSCC). Material and Methods Patients with early-stage (stage I–III) LSCC who underwent 18F-FDG PET/CT before surgical resectionbetween August 2012 and February 2020 were analyzed. Patients who received neoadjuvant chemotherapy or radiotherapywere excluded from the study. The maximum standard uptake value (SUVmax) and RFs were extracted from PETimages for primary tumors. The diagnostic performances of PET parameters in groups of tumor differentiation, stage, andmediastinal lymph node metastasis (MLNM) status were evaluated. The study endpoints were overall survival (OS) andprogression-free survival (PFS). Univariate and multivariate analyses were performed with RFs, SUVmax, and CPFs to findindependent predictors of PFS and OS. Results A total of 77 patients (5 female, 72 male) were included in the study. SUVmax and GLCM entropy were independentlyassociated with tumor differentiation. The only parameter with significant diagnostic performance for MLNM wasGLZLM-SLZGE. Tumor diameter and NGLDM busyness were independently associated with the stage. MLNM and tumordifferentiation were found to be independent predictors of PFS. NGLDM contrast and MLNM were independently associatedwith OS. Conclusion Using radiomic features in addition to CPFs to predict disease recurrence and shorter overall survival can guideprecision medicine in patients with LSCC.
