        Process of Regaining Self-Efficacy in Cancer Survivors Undergoing Gastrectomy

        Purpose: Patients with gastric cancer experience several challenges in regaining self-efficacy, including postoperative gastric dysfunction and psychosocial problems, after undergoing gastrectomy. This study aimed to describe the process of regaining self-efficacy in cancer survivors who underwent gastrectomy. Methods: This study enrolled adults, employed a modified grounded theory approach, and targeted participants by theoretical sampling. Data were collected via semi-structured interviews and analyzed by qualitative and inductive methods. Results: The participants included 10 post-gastrectomy patients with gastric cancer. The analysis yielded 28 concepts, eight categories, and four core categories. Regaining self-efficacy in cancer survivors began with “Facing gastric cancer and surgery.” After a period of being “At the mercy of gastrectomy and eating habits,” it changed to “Reclaiming life while accepting the loss of gastric function” through trial-and-error, and finally reached the stage of “Aiming for a better future than that before gastric cancer” after a long period. Conclusion: Despite a crisis and repeated trial-and-error circumstances that could change abruptly, the patients demonstrated a flexible and resilient attitude in regaining self-efficacy. This study presented a time axis of the difficult process of regaining self-efficacy and explained the stages at which nursing intervention becomes important.

        Comparison of three type of muscle glycogen loading interventions using a very-high-carbohydrate diet in an elite male racewalker: a case report

        [Purpose] Muscle glycogen storage before a race is necessary for endurance athletes to achieve the best performance. Generally, the recommended carbohydrate intake for preparation over 90 min of the race is 10–12 g·kg-1·day-1. However, it remains unclear whether an elite athlete with an already high-carbohydrate diet can further increase muscle glycogen through a very-high-carbohydrate intake. Therefore, we compared the effects of three types of glycogen loading in a 28-year-old male athlete who belongs to the top 50 racewalkers in the world, consuming a daily energy intake of 4507 kcal and a carbohydrate intake of 12.7 g·kg-1·day-1. [Methods] The racewalker consumed very-high-carbohydrate diets three times for 2 days each, 13.7 g·kg-1·day-1 for trial 1, 13.9 g·kg-1·day-1 for trial 2, and 15.9 g·kg-1·day-1 for trial 3. Muscle glycogen concentrations in the anterior (vastus lateralis and vastus intermedius) and posterior thighs (semimembranosus, semitendinosus, and biceps femoris) were measured using carbon-13 magnetic resonance spectroscopy. [Results] Muscle glycogen concentrations in both the anterior and posterior thighs increased in all trials, particularly in trial 3. Body mass also increased by 1.5 kg in trials 1 and 2 and by 1.8 kg in trial 3 before and after the trials. The participant felt satiated throughout the day and experienced stomach discomfort during trial 3. [Conclusion] We found that a 2-day very-high-carbohydrate diet and tapering of training could further increase the muscle glycogen concentration in athletes. However, we speculated that 15.9 g·kg-1·day-1 carbohydrate intake was unrealistic for the actual pre-race diet.

        General anesthesia with a transcutaneous pacemaker for a Noonan syndrome patient with advanced atrioventricular block discovered in the remote period after open-heart surgery: a case report

        We provided general anesthesia management to a patient with advanced atrioventricular block, which was discovered in the remote period after open-heart surgery. A 21-year-old man with Noonan syndrome was scheduled to undergo excision of a median intramandibular tumor. At 2 months of age, the patient underwent endocardial repair for congenital heart disease. During our preoperative examination, an atrioventricular block was detected, which had not been previously noted. Emergency drugs were administered, and a transcutaneous pacemaker was placed. During anesthesia induction, mask ventilation was easy, and intubation was performed smoothly using a video laryngoscope. The transcutaneous pacemaker was activated in demand mode at a pacing rate of 50 cycles/min approximately throughout the anesthesia time, and the hemodynamic status remained stable. The effect of intraoperatively administered atropine was brief, lasting only a few seconds. Although body movements due to thoracoabdominal muscle spasm were observed during pacemaker activation, they did not interfere with surgery. In postoperative patients with congenital heart disease, an atrioventricular block may be identified in the remote period, and preoperative evaluation should be based on this possibility. In addition, during anesthesia management, it is important to prepare multiple measures to maintain hemodynamic status.

        Risk factors for postoperative nausea and vomiting in patients of orthognathic surgery according to the initial onset time: a cross-sectional study

        Background: A high incidence (40–73%) of postoperative nausea and vomiting (PONV) has been reported following orthognathic surgery, and various risk factors have been associated with it. Identifying PONV risk factors based on initial onset time will help establish preventive measures. This study aimed to identify factors that are significantly related to PONV based on the initial onset time after orthognathic surgery. Methods: This study included 590 patients who underwent orthognathic surgery. Multivariate logistic regression analysis was performed to identify the risk factors that are significantly related to PONV. The objective variables were classified into three categories: no PONV, early PONV (initial onset time: 0–2 h after anesthesia), and late PONV (initial onset time: 2–24 h after anesthesia). The explanatory variables included relevant risk factors for PONV, as considered in previous studies. Results: Total intravenous anesthesia with propofol was a significant depressant factor for early PONV (adjusted odds ratio [aOR] = 0.340, 95% confidence interval [CI] = 0.209–0.555) and late PONV (aOR = 0.535, 95% CI = 0.352–0.814). The administration of a combination of intraoperative antiemetics (vs. no administration) significantly reduced the risk of early PONV (aOR = 0.464, 95% CI = 0.230–0.961). Female sex and young age were significant risk factors for late PONV (aOR = 1.492, 95% CI = 1.170–1.925 and unit aOR = 1.033, 95% CI = 1.010–1.057, respectively). Conclusion: We identified factors that are significantly related to PONV based on the initial onset time after orthognathic surgery. Total intravenous anesthesia with propofol significantly reduced the risk of PONV not only in the early period (0–2 h after anesthesia) but also in the late period (2–24 h after anesthesia).

        Designing and Evaluating an EAP Reading Textbook: English for Political Science and Economics

        This study aims to report an attempt to design and organise a content-based EAP reading course in political science and economics at a university in Japan, and to shed some light on learners` actual practices and preferences through the use of survey- and interview-based research participated in by 438 students. A principal axis of this project was the collaborative production of a textbook by language and subject teachers. A posthoc course evaluation from students and teachers highlighted a number of important pedagogical implications. First, learners, particularly higher-proficiency learners who read conceptually challenging articles, found easier units significantly more interesting and more difficult units significantly less interesting. Further analysis suggested that a good balance between the level of discipline specificity and that of language has to be maintained in order to arouse their interest. Another interesting outcome was that both higher- and lower-proficiency learners found word lists with L2 definitions significantly more useful than those with L1 definitions, which contradicts earlier findings with Japanese students. This seems to suggest that the use of the target language as a medium of instruction facilitated learners` thinking processes vis-a-vis L2, and that this affected their preferences. Moreover, higher-proficiency learners found group work, class discussion and listening to the teacher significantly more useful than did lower-proficiency learners, whereas the latter found translating significantly more useful than the former. This implies that the transition from a teacher-centred translation approach to a learner-centred communicative approach should be introduced gradually to lower-proficiency learners.

        Future Outlook of Refractories for Iron and Steelmaking

        Refractory industry in technologically advanced countries has long been on gradual decline due to leveled-off steel production and decreasing unit consumption of refractories for steel. Notable technological achievements by refractory industry that contributed significantly to steel production are briefly reviewed covering from blast furnace, basic oxygen furnace to continuous casting. Future possibility to revitalize the refractory industry is discussed on the basis of the review, taking into account opportunities available in environment and energy related sector of industries.

        Acute effects of endurance exercise on nocturnal autonomic functions in sedentary subjects : a pilot study

        Nocturnal heart rate variability (HRV) is thought to reflect healthy recov-ery function of the autonomic nervous system. Although exercise is recommended for health promotion, exercise itself decreases HRV. We studied acute effect of daytime exercise on nocturnal HRV in 5 healthy adults (age, 22–40 years; 2 female subjects) without regular exercise habit. Using a treadmill, they performed 30-min walking at 4 km/hr and 30-min running at 9 km/hr from 11 a.m. on different days at an interval of 2 weeks. On these days and a day without exercise (control), Holter electrocardiograms were recorded from 9 a.m. for 24 hr. The amplitudes of low-frequency (LF, 0.04–0.15 Hz) and high-frequency (HF, 0.15–0.45 Hz) components of HRV were measured continuously by complex de-modulation and were averaged over periods of 11:00–11:30 a.m., 3 hr af-ter going to bed, and time in bed at night. Exercise intensities of the walking and running were at 10% to 44% and 55% to 67% of heart rate reserve, respectively. During exercise, heart rate increased and LF and HF amplitudes decreased with exercise intensity. Nocturnal heart rate and LF and HF amplitude, however, showed no consistent changes with exercise intensity and their averages on the days of walking and run-ning did not differ significantly from those of the control day. In conclu-sion, 30-min walking and running exercises performed in the morning had no significant acute effects on nocturnal heart rate or HRV.

        A Cohen syndrome patient whose muscle-relaxant effect may have been prolonged during general anesthesia: a case report

        Cohen syndrome is a rare genetic disorder associated with mutations in the VPS13B gene. Individuals with this disorder present with diverse clinical manifestations, including muscle hypotonia, intellectual disabilities, and typical facial characteristics, such as prominent upper central incisors and micrognathia. General anesthesia was administered to a 23-year-old man with Cohen syndrome. Although we observed prominent upper central incisors, an overjet of 10 mm, micrognathia, and thyromental distance of 4 cm, hypotonia was not observed in the patient. Intubation was rendered difficult when performing a direct laryngoscopy. However, smooth intubation was achieved using a video laryngoscope. The patient’s train of four (TOF) count remained zero close to 60 min after rocuronium administration, suggesting that the drug’s muscle-relaxant effect may have been prolonged. A TOF ratio of 0.79 was confirmed 130 min after rocuronium administration, and a TOF ratio of 1.0 was confirmed after administration of 150 mg of sugammadex. The patient’s respiration remained stable after extubation, and no recurarization of muscle relaxation was observed. As demonstrated in this case report, it is important to closely monitor recovery from muscle relaxation and prepare multiple techniques for airway management in general anesthesia management of patients with Cohen syndrome.
