Composite multilayer thin films have been constructed on planar substrates by the layer-by-layer deposition of chitosan (CS) and two strong polyanions different by the density of sulfonic groups, i.e., poly(sodium 2-acrylamido-2-methylpropanesulfonate) (PAMPS) and a random copolymer of AMPS with acrylic acid, P(AMPS_55-co-AA_45); all deposition steps being performed at pH 5.5. The effect of the polyanion structure and number of adsorbed layers on the film morphology was examined by atomic force microscopy and contact angle measurements. The polyanion structure strongly affected the multilayer morphology, more swollen and less dense films being formed when P(AMPS_55-co-AA_45) was used compared to PAMPS. The sorption of two proteins, human serum albumin and bovine serum albumin, on the film surface with CS as the last layer adsorbed, as a function of the polyanion structure,indicates that a higher amount of protein had been sorbed mainly on the film surface when PAMPS was used as a polyanion in the film construction.

For an alloy to be suitable for use as an implant material, it must have a low specific weight and Young’s modulus, good mechanical properties that are similar to those of bone, and very good corrosion resistance and biocompatibility. In this study, we have developed a novel Ti-20Nb-10Zr-5Ta alloy that is composed of nontoxic, nonallergenic, corrosion-resistant elements. This alloy has low specific weight and Young’s modulus and good mechanical properties. It has a fine microstructure with a matrix that is mainly composed of the β phase and some α phase due to recrystallization during cooling. It shows elastoplastic behavior with a fairly linear elastic behavior and low Young’s modulus (59 GPa). In addition, its ultimate tensile strength, 0.2% yield strength, and hardness are higher than those of CP Ti, commercial Ti-6Al-4V,and similar β-type alloys. It exhibited a very stable passive state and its electrochemical parameters and corrosion and ion release rates were better than those of CP Ti in Ringer’s solutions of different pH values that simulate the severe functional conditions of an implant; this is attributable to the beneficial influence of the alloying elements and to the better protective properties of the coated passive film.
New Technique for the Synthesis of Polyphosphoesters

Polycondensation of arylphosphonic(phosphoric)dichlorides with various bisphenols was carried out using a new technique, namely, solid-liquid phase transfer catalysis. Potassium phosphate was used as the base. The synthesis of polyphosphonate by solid-liquid phase-transfer catalyst (PTC) polycondensation of phenylphosphonic dichloride with 4,4'-cyclohexylidenebisphenol is presented in detail. The structures of the polyphosphonates and fluorine-polyphosphate were studied using this technique. The polymers were characterized by gel permeation chromatography,Fourier transform infrared spectroscopy (FTIR), ^1H and ^31P nuclear magnetic resonance (NMR)spectroscopy, differential scanning calorimetry, and thermal analysis. Yields in the range of 80.2-86.5, and inherent viscosities in the range of 1.48-2.5 dL/g were obtained. High molecular weight polymers (M_n of ~11,800-21,600)were successfully prepared. The polymers were self-extinguishing and started losing weight at around 280-380 ℃. The glass-transition temperatures of the polymers were detected in the range of 85-180 oC. The flammability was investigated by measuring the limiting oxygen index values.
Cardiorenal Protection in Diabetic Kidney Disease

Over the last 5 years there have been many new developments in the management of diabetic kidney disease. Glucagon-like peptide-1 receptor agonists (GLP-1 RA) and sodium-glucose cotransporter-2 (SGLT2) inhibitors were initially used for glycemic control, but more recent studies have now shown that their benefits extend to cardiovascular and kidney outcomes. The recent additionof data on the novel mineralocorticoid receptor antagonist (MRA) gives us another approach to further decrease the residual risk ofdiabetic kidney disease progression. In this review we describe the mechanism of action, key studies, and possible adverse effects related to these three classes of medications. The management of type 2 diabetes now includes an increasing number of medicationsfor the management of comorbidities in a patient population at significant risk of cardiovascular disease and progression of chronickidney disease. It is from this perspective that we seek to outline the rationale for the sequential and/or combined use of SGLT2 inhibitors, GLP-1 RA and MRAs in patients with type 2 diabetes for heart and kidney protection.

We present an 8 years old girl who was diagnosed at 6 months of age of Progressive Familial Intrahepatic Cholestasis type 2. Although liver transplantation (LT) was classically considered curative for these patients, cholestasis recurrence with normal gamma-glutamyl transpeptidase (GGT), mediated by anti-bile salt export pump (BSEP) antibodies after LT (auto-antibody Induced BSEP Deficiency, AIBD) has been recently reported. Our patient underwent LT at 14 months. During her evolution, patient presented three episodes of acute rejection. Seven years after the LT, the patient presented pruritus with cholestasis and elevation of liver enzymes with persistent normal GGT. Liver biopsy showed intrahepatic cholestasis and giant-cell transformation with very low BSEP activity. Auto-antibodies against BSEP were detected therefore an AIBD was diagnosed. She was treated with Rituximab and immunoadsorption with resolution of the AIBD. As a complication of the treatment she developed a pneumocystis infection successfully treated with corticoids, cotrimoxazol and anidulafungin.
Accuracy of the 2008 Simplified Criteria for the Diagnosis of Autoimmune Hepatitis in Children

Purpose: Classical criteria for diagnosis of autoimmune hepatitis (AIH) are intended as research tool and are difficult to apply at patient’s bedside. We aimed to study the accuracy of simplified criteria and the concordance with the expert diagnosis based on the original criteria.Methods: A cohort of children under study for liver disorder was selected through consecutive sampling to obtain the prevalence of AIH within the group of differential diagnoses. AIH was defined, based on classical criteria, through committee review of medical reports. Validity indicators of the simplified criteria were obtained in an intention to diag-nose approach. Optimal cut-off and the area under the receiver operating characteristic (ROC) curve were calculated. Results: Out of 212 cases reviewed, 47.2% were AIH. For the optimal cut-off (6 points), the simplified criteria showed a sensitivity of 72.0% and a specificity of 96.4%, with a 94.7% positive and a 79.4% negative predictive value. The area under the ROC curve was 94.3%. There was a good agreement in the pre-treatment concordance between the classical and the simplified criteria (kappa index, 0.775). Conclusion: Simplified criteria provide a moderate sensitivity for the diagnosis of AIH, but may help in indicating treatment in cases under suspicion with 6 or more points.

This paper describes the synthesis of a novel Ti-15Ta-5Zr alloy with an + β stable, homogeneous, biphasicmicrostructure (by optical microscopy, X-ray diffraction and scanning electron microscopy) and the determinationof its mechanical properties, hardness and a Young’s modulus of 42.2 GPa. The alloy passive film compositionand thickness were analyzed by X-ray photoelectron spectroscopy; a thick, compact native passive filmcontaining the protective oxides of all constituent elements (TiO2, Ta2O5, ZrO2) was identified. The electrochemicalparameters confirmed a nobler behaviour and a more capacitive, resistant passive film on thealloy surface compared to Ti and other existing implant alloys; these facts are due to the alloying elements that,by their oxides stabilize and reinforce the alloy passive film. This passive film thickened in time, which increased itsprotective capacity. The new alloy had no susceptibility to galvanic or local corrosion. Ti-15Ta-5Zr alloyis recommended as an advanced orthopaedic implant material.

In this study, the synthesis and characterization of functionalized flax fabric with adequate titanium and aluminiumprecursors is presented (AlCl3 or Ti(C4H9O)4). The modified flax fabrics were analysed both visually and instrumentally byIR spectroscopy and microscopy, scanning electron microscopy (SEM) and thermal analysis (DTA-TG). The data highlightthat the nature and amount of functionalizing agent induce important morphological changes of the surface. These flaxfabrics decorated with alumina or titanium dioxide nanoparticles are desired to be used as reinforcing agents of differentrecycled polyethylene terephthalate. The properties will be designed by the nature, processing route and amount of theprecursors used for the surface modification of these cellulosic fibres/fabrics, the surface decoration increasing their thermalproperties (especially thermal stability) but also surface properties (especially roughness, hydrophilic/hydrophobic ratio,etc.). All these factors will finally induce improved compatibility between phases and thus improved physico-mechanical andthermal properties. Taking into account the large amount of natural cellulosic fibres and recycled thermoplastic polymers,added-value products will be obtained and at the same time some environmental issues are solved. The expected applicationscan include the fabrication of automotive parts, protective panels for highways and railways, etc.
