        Learners’ perspective: where and when pre-residency trainees learn more to achieve their core clinical competencies

        Purpose: While it is known that effective clinical education requires active involvement of its participants, regular feedback, communication skills and interprofessional training, limited studies have been conducted in Korea that demonstrate how pre-residency trainees acquire their core clinical skills. This is a cross-sectional study of interns and students across a third-tier university hospital in Korea to examine where and when they acquire core clinical skills. Methods: A total of 74 students and 91 interns were asked to participate in a closed-ended questionnaire, and 50 participants (20 students and 30 interns) were involved in semistructured individual interviews. The questionnaire was based on the Accreditation Council for Graduate Medical Education core competencies. Results: The majority of core clinical skills were acquired during their rotations in emergency medicine, general surgery, and cardiothoracic surgery. The semistructured interviews revealed that these departments required their trainees to be highly involved and analytical, and participate in clinical discourse. Conclusion: The common factor among the three departments is an environment in which trainees are highly involved in clinical duties, and are expected to make first-contact patient encounters, participate in clinical discourse, interpret investigative results and arrive at their own conclusions. Work-based learning appear to be key to the trends observed, and further study is warranted to determine whether these findings are indicative of true acquisition of clinical competence.

        한국의 의사상: 좋은 의사양성

        Much of the behavior of doctors reflects the influences from the social, cultural, historical, and economic environment of the time. Therefore, it is very important for future doctors to understand the practice environment in an ever changing world. Traditionally, doctors' competence has been based on the doctor-patient relationship. However, the social practice of medicine in the contemporary era asks future doctors to have social competencies, which often are defined as non-clinical competencies. As a global project, the World Federation for Medical Education has urged every country to define the future role of doctors to encompass global roles based on the duty toward and value of clinical as well as non-clinical competencies. In the past four years, Korean medical professional societies have coordinated to set forth the global role of Korean doctors. Five domains of clinical competence, professionalism, social accountability, communication and collaboration, and education and research have been chosen. The current version of the "global role of Korean doctors" can be used not only for the common objectives of medical education, but also for translating into the competencies of doctors that can be achieved through life-long learning. If we all want to improve medical education in order to produce more qualified and competent doctors as the public desires, then it may be the most urgent task to produce doctors who are equipped with social competencies to persuade, negotiate, and engage in constructive dialogues with society for better health care for a better society.

        Perception of interprofessional conflicts and interprofessional education by doctors and nurses

        Purpose: This study aimed to collect information that is needed to develop interprofessional education curricula by examining the current status of interprofessional conflicts and the demand for interprofessional education. Methods: A total of 95 doctors and 92 nurses in three university hospitals in Seoul responded to a survey that comprised questions on past experience with interprofessional conflicts, the causes and solutions of such conflicts, past experience with interprofessional education, and the demand for interprofessional education. Results: We found that 86% of doctors and 62.6% of nurses had no interprofessional education experience. Most of them learned about the work of other health professions naturally through work experience, and many had experienced at least one interprofessional conflict. For doctors, the most popular method of resolving interprofessional conflicts was to let the event pass; for nurses, it was to inform the department head. Further, 41.5% of doctors and 56.7% of nurses expressed no knowledge of an official system for resolving interprofessional conflicts within the hospital, and 62.8% of doctors and 78.3% of nurses stated that they would participate in interprofessional education if the opportunity arose. Conclusion: In Korean hospital organizations, many doctors and nurses have experienced conflicts with other health professionals. By developing an appropriate curriculum and educational training system, the opportunities for health professionals to receive interprofessional education should expand.

        대학선수의 진로결정 자기효능감과 은퇴불안의 관계

        본 연구는 대학선수의 인구통계학적 특성에 따른 진로결정 자기효능감과 은퇴불안의 차이를 확인하고, 진로결정 자기효능감과 은퇴불안의 관련성을 규명하는 데 목적이 있다. 총 223명(남 145, 여 78)의 대학선수를 대상으로 인구통계학적 특성, 진로결정 자기효능감(Betz, Klein, & Taylor, 1996), 은퇴불안(원영인, 김지태, 1996)을 조사하였다. 수집된 자료는 통계 프로그램을 활용해 분석하였으며, 그 결과는 다음과 같다. 첫째, 남성선수는 여성선수보다 진로결정 자기효능감 하위요인 중 자기평가, 미래계획수립에서 유의미하게 높은 반면 여성선수는 남성선수보다 은퇴불안 하위요인 중 경기력 저하에서 유의미하게 높았다. 둘째, 입상 경험이 있는 선수는 입상 경험이 없는 선수보다 진로결정 자기효능감 하위요인 중 자기평가, 목표 선택, 문제해결에서 유의미하게 높은 반면 입상 경험이 없는 선수는 입상 경험이 있는 선수보다 은퇴불안 하위요인 중 심리적 상태, 동료은퇴, 경기력 저하에서 유의미하게 높았다. 셋째, 부상 경험이 있는 선수는 부상 경험이 없는 선수보다 진로 결정 자기효능감 하위요인 중 자기평가와 은퇴불안 하위요인 중 부상발생에서 유의미하게 높았다. 넷째, 4학년 선수는 1학년 및 3학년 선수보다 진로결정 자기효능감 하위요인 중 미래계획수립에서 유의미하게 높았다. 다섯째, 진로결정 자기효능감 하위요인과 은퇴불안 하위요인은 부분적으로 부적인 상관관계가 나타났다. 여섯째, 진로결정 자기효능감 하위요인 중 목표선택과 자기평가는 은퇴불안 하위요인 중 종목특성과 동료은퇴에 각각 부적인 영향을 미치는 것으로 나타났다. 이상의 결과는 대학선수의 은퇴와 진로에 대한 기초자료를 제공할 것이다. The purpose of the study was to investigate the differences in career decision-making self-efficacy according to demographics and retirement anxiety, and to analyze relationship to the variables in university athletes. A total of 223 university athletes(male 145, female 78) participated in the study. It was used to measure Demographics(e.g., gender, grade, performance, and injury), CDMSF-SF(a short form of the Career Decision-Making Self-Efficacy Scale; Betz, Klein, & Taylor, 1996), and Retirement Anxiety(Won, & Kim, 2017). It was carried out that confirmatory factor analysis, reliability of items, descriptive statistics, t-test, one-way ANOVA, correlation analysis, and multiple regression analysis. The result was summarized as follow. First, male athletes were significant high in terms of self appraisal, making plans for the future, on the other hand, female athletes were significant high in terms of decreased performance, Seconds, prize athletes were significant high in self appraisal, goal selection, problem solving, on the other hand, non-prize athletes were significant high in psychological state, teammate retirement, decreased performance. Third, injured athletes were significant higher than non-injured athletes in self appraisal, and injury. Forth, the 4th grade were significant higher than the 1th and the 3th grade in making plans for the future. Fifth, there was significant relationship career decision-making self-efficacy and retirement anxiety. Sixth, goal selection had negative effect on type's uniqueness, and self appraisal negative influenced teammate retirement. The study will contribute to build base data about university athlete’s retirement and career.

        비판적 사고와 글쓰기에 기초한 의료윤리와 전문직업성 교육

        본 논문은 먼저 의학교육에서 의료윤리와 전문직업성을 가르치는 것이 어려운 이유를 밝히고, 다음으로 비판적 사고와 글쓰기에 의거한 의료윤리의 한 교육 모형을 제시하고 있다. 의료윤리와 전문직업성을 교육하는데 있어 가장 어려운 점은 그것의 목적, 절차 그리고 내용이 혼재되어 있기 때문이다. 따라서 의학교육에서 의료윤리와 전문직업성 교육은 의학적 문제에 대한 문제해결 역량을 기르는 것이 되어야 한다. 비판적 사고와 학술적 글쓰기에 기초한 도덕적 또는 의학적 추론은 그러한 문제해결 능력을 기르는 한 좋은 방편이 될 수 있다. 의학교육에서 적합하고 적실성 있는 의료윤리와 전문직업성 교육은 의학적 주제와 관련된 교육적 방법에 초점이 맞추어져야 할 뿐만 아니라 윤리, 비판적 사고, 글쓰기가 융합되어야 한다. 의료윤리와 전문직업성에 내재된 많은 현안 문제들은 생명윤리, 분배적 정의, 동물 실험, 의무의 충돌 그리고 자율규제 등과 같이 전통적인 도덕적 문제들과 관련되어 있기 때문이다. The main purpose of this paper is to explain the reason for the difficulty in teaching medical ethics and professionalism and to make a proposal for an educational model of improving moral inference in teaching medical ethics in virtue of critical thinking and writing. Above all else, the difficulty in teaching medical ethics and professionalism lies in that its educational purpose, educational procedure and contents are confused. I argue that the improvement of ethical problem-solving competency is the core component. So, I propose that the moral and ethical inference in medical ethics must be based on critical thinking and academic writing. Since many current issues in medical ethics and professionalism are related to traditional moral issues ― such as bioethics, distributive justice, animal experiment, conflict of obligations and self-regulation and so forth ― a proper and suitable teaching of medical ethics and professionalism in medical education should focus on medical subject-related educational methods and should be integrated with ethics, critical thinking and writing.

        의학전문직업성의 개념과 발전 방향

        This paper summarizes the concept of medical professionalism and presents its direction. Toward this end, the authors compare the characteristics of a professional with those of a craftsman and a technician. The authors describe the construct of medical professionalism on the assumption that knowledge and technical competence are necessary but not sufficient conditions for the professional. The construct of medical professionalism could be defined variously, depending on the time, place, or collectivity. The authors furthermore summarize cases for the development process of medical professionalism and then compare Korean medical professionalism with that of the West. Finally, the authors present the development direction of medical professionalism in Korea.
