      • Poster Session : PS 0931 ; Lower GI Tract : A Case of Inverted Diverticulitis of Colon Mimicking Subepithelial Tumor

        Background: Inverted colonic diverticula (ICD) are rarely reported. ICD can be confused with elevated polypoid lesions and are occasionally complicated by bleeding and colonic perforation following biopsy or endoscopic resection. We report a case of inverted diverticulitis of ascending colon misdiagnosed as subepithelial tumor. Case report: A 61-year-old man, who had history of adenomatous colon polyps and an about 3.5 cm sized subepithelial tumor in the ascending colon 5 years ago, presented for surveillance colonoscopy. He had no symptoms. Physical examination and laboratory data were normal. On follow-up colonoscopy, subepithelial tumor without change in size showed a central necrosis communicating to surface in ascendingcolon. Biopsies revealed chronic infl ammation and elongated crypts without evidence of malignancy. The abdominal CT showed 4 cm sized segmental wall thickening with enhancement in proximal ascending colon. A laparoscopic right hemicolectomy was performed to rule out malignant transformation of subepithelial tumor. Intraoperative fi ndings revealed an infl ammatory mass in proximal ascending colon. Histopathologic evaluation revealed a 3.5x3.0 cm sized diverticulum with multiple mucosal invaginations containing inspissated mucoid materials, congested subserosa and fi brinous exudated serosa without malignancy -The fi nal diagnosis was inverted colonic diverticulitis of ascending colon. Two years ago, on further history taking, patient had an episode of right lower quadrant abdominal pain, which was misdiagnosed as an infectious enterocolitis and resolved with conservative management. Maybe it was thought to be due to colonic diverticulitis. Conclusion: Because ICD may resemble polypoid lesions in the colon, they should be considered in the differential diagnosis of unusual polypoid lesions by additional imaging studies such as EUS and CT. To our knowledge, this is the fi rst case of ascending ICD complicated with diverticulitis.

        Astragali Radix ( A. Radix ) is the dried root of Astragalus membranaceus var. mongholicus (Bge) Hsiao or Astragalusmembranaceus (Fisch.) Bge., belonging to the family Leguminosae, which is mainly distributed in China. A. Radix hasbeen consumed as a tonic in China for more than 2000 years because of its medicinal eff ects of invigorating the spleenand replenishing qi. Currently, more than 400 natural compounds have been isolated and identifi ed from A. Radix , mainlyincluding saponins, fl avonoids, phenylpropanoids, alkaloids, and others. Modern pharmacological studies have shown thatA. Radix has anti-tumor, anti-infl ammatory, immunomodulatory, anti-atherosclerotic, cardioprotective, anti-hypertensive,and anti-aging eff ects. It has been clinically used in the treatment of tumors, cardiovascular diseases, and cerebrovascularcomplications associated with diabetes with few side eff ects and high safety. This paper reviewed the progress of researchon its chemical constituents, pharmacological eff ects, clinical applications, developing applications, and toxicology, whichprovides a basis for the better development and utilization of A. Radix .

      • Risk Management Qualitatively on Railway Signal System

        Risk management is an important part of system assurance and it is widely used in safety-related system. Railway signal system is one kind of safety-related system and its most important goal is to guarantee the safety of railway system. The method based on risk management can find and solve the security issues of railway signal system more effectively. This paper introduces the basic conception of risk management, studies the whole process of risk management and related tools and techniques and discusses some key points qualitatively combining with the particularity of railway signal system.

        Lentivirus-mediated silencing of the PTC1 and PTC2 genes promotes recovery from spinal cord injury by activating the Hedgehog signaling pathway in a rat model

        This study aimed to investigate the effect of Patched-1 (PTC1) and PTC2 silencing in a rat model, on Hedgehog (Hh) pathwaymediated recovery from spinal cord injury (SCI). An analytical emphasis on the relationship between the sonic hedgehog (Shh) pathway and nerve regeneration was explored. A total of 126 rats were divided into normal, sham, SCI, negative control (NC), PTC1-RNAi, PTC2-RNAi and PTC1/PTC2-RNAi groups. The Basso, Beattie and Bresnahan (BBB) scale was employed to assess hind limb motor function. Quantitative real-time polymerase chain reaction and western blotting were performed to examine the mRNA and protein levels of PTC1, PTC2, Shh, glioma-associated oncogene homolog 1 (Gli-1), Smo and Nestin. Tissue morphology was analyzed using immunohistochemistry, and immunofluorescent staining was conducted to detect neurofilament protein 200 (NF-200) and glial fibrillary acidic protein (GFAP). The PTC1/PTC2-RNAi group displayed higher BBB scores than the SCI and NC groups. Shh, Gli-1, Smo and Nestin expression levels were elevated in the PTC1/PTC2-RNAi group. PTC1 and PTC2 mRNA and protein expression was lower in the PTC1/PTC2-RNAi group than in the normal, sham and SCI groups. Among the seven groups, the PTC1/PTC2-RNAi group had the largest positive area of NF-200 staining, whereas the SCI group exhibited a larger GFAP-positive area than both the normal and the sham groups. The Shh pathway may provide new insights into therapeutic indications and regenerative recovery tools for the treatment of SCI. Activation of the Hh signaling pathway by silencing PTC1 and PTC2 may reduce inflammation and may ultimately promote SCI recovery.

        The Spectral Analysis and Exponential Stability of a 1-d 2x2 Hyperbolic System with Proportional Feedback Control

        In this paper, the well-posedness and stability of a 1-d 2×2 hyperbolic system under proportional feedback control is addressed. The semigroup approach and norm equivalence theorem is adopted to obtain the explicit dissipative condition for control parameters. The asymptotic spectral analysis of the system is presented. It is shown that the real part of the eigenvalues goes to some negative constant as the modulus of eigenvalues goes to +∞. Moreover, we find that there is a group of generalized eigenfunctions which forms a Riesz basis of the Hilbert state space. Hence, the spectrum determined growth condition holds, and then the exponential stability is established.

        The Characteristics and Survival Outcomes in Patients Aged 70 Years and Older with Nasopharyngeal Carcinoma in the Intensity-Modulated Radiotherapy Era

        Purpose We aim to examine nasopharyngeal carcinoma (NPC) characteristics and survival outcomes in patients aged 70 years and older in the intensity-modulated radiotherapy (IMRT) era. Materials and Methods From 2006 to 2013, 126 non-metastatic NPC patients aged  70 years who were treated with IMRT +/ chemotherapy were included. Adult Comorbidity Evaluation 27 (ACE-27) was used to measure patient comorbidities. The overall survival (OS) and cancer-specific survival (CSS) were calculated with the Kaplan-Meier method, and differences were compared using the log-rank test. The Cox proportional hazards model was used to carry out multivariate analyses. Results For the entire group, only two patients (1.6%) presented stage I disease, and up to 84.1% patients had stage III-IVB disease. All patients had a comorbidity score of 0 in 24 (19.0%), 1 in 45 (35.7%), 2 in 42 (33.3%), and 3 in 15 (11.9%) patients. The main acute grade during radiotherapy was 3-4 adverse events consisting of mucositis (25.4%), bone marrow suppression (16.7%), and dermatitis (8.7%). After treatment, four patients (3.2%) developed temporal lobe injury. Five-year CSS and OS rates were 67.3% (95% confidence interval [CI], 58.6% to 77.4%) and 54.0% (95% CI, 45.6% to 63.9%), respectively. Five-year OS was significantly higher for ACE-27 score 0-1 than ACE-27 score 2-3 (72.9% and 39.9%, respectively; p < 0.001). Multivariate analyses showed ACE-27 score 0-1 was significantly associated with superior OS (hazard ratio [HR], 3.02; 95% CI, 1.64 to 5.55; p < 0.001). In addition, the rate of OS was higher for stage I-III than that of stage IV, with borderline significance (HR, 1.67; 95% CI, 0.99 to 2.82; p=0.053). But no significant advantage was observed in OS when chemotherapy was used (p > 0.05). Conclusion Our findings suggest IMRT +/– chemotherapy has a manageable toxicity and provides an acceptable survival in patients aged  70 years with NPC. ACE-27 score was significantly associated with survival outcomes in this group population.

Piao Qijia is one of a North Korea poets, who has been sent on a diplomatic mission to Beijing four times, and he also make important contributions to China-North Korean cultural exchange. The research of the friendship between Li Dingyuan and PiaoQijia helps restore China-North Korea cultural exchange in Qing dynasty and explore the China-North Korean literati in the important position in the cultural exchange and contribution. This paper unfold the study from the background, methods and influence, in order to reproduce the real exchanges conditions of Chinese and North Korean scholar in the Qing dynasty.

        <P>Cold roll-forming of metal sections is a significant field in advancing forming of strip metal, and the forming processes are influenced by many factors. The scientific design of passes is worked out by combining theoretical analysis with finite element analysis, in accordance with the principles of cold roll-forming; thus the desired high-quality bending sections are achieved through a combination of theory and practice. This study mainly addresses the velocity of the rolls for sheets that have angle sides, and the spring-back of SGARC 440 alloy sheets in the roll forming process, where we use DEFORM to simulate the sheet.</P>
