      • Predictors Affecting Breast Self-Examination Practice among Turkish Women

        Background: Breast cancer (BC) is the most common cancer among females in Turkey. Predictors affecting the breast self-examination (BSE) performance vary in developing countries. Objective: To determine the frequency of BSE performance and predictors of self-reported BSEs among women in the capital city of Turkey. Materials and Methods: This cross-sectional study was conducted on 376 Turkish women using a self-administered questionnaire covering socio-demographic variables and BSE-related features. Results: Of the participants, 78.7% (N=296) reported practicing BSE, whereas 9.5% (N=28) were implementing BSE regularly on a monthly basis, and only 5.7% (N=17) were performing BSE regularly within a week after each menstrual cycle. Multivariate logistic regression modeling revealed that BSE performance was more likely in younger age groups [20-39 years] (p=0.018, OR=3.215) and [40-49 years] (p=0.009, OR=3.162), women having a family history of breast disease (p=0.038, OR=2.028), and housewives (p=0.013, OR=0.353). Conclusions: Although it appears that the rates of BSE performers are high, the number of women conducting appropriate BSE on a regular time interval basis is lower than expected. Younger age groups, family history of breast diseases and not being employed were identified as significant predictors of practicing BSE appropriately. Older age and employment were risk factors for not performing BSE in this sample.

      • The New Architourist Destination: Dallas/Fort Worth, Texas

        Tourism has been an empowering phenomenon in our modern leisure oriented society, and become very important especially for politicians, economics and developers, as well as the urban planners and architects. Tourism has also a strong impact on architecture and urban space; approximately 63% of daily tourist activities are visiting famous places and urban spaces (Richter, 2010). In this context, this paper will discuss the relationships between the design of architecture, urban space and the contemporary practices of tourism. A relatively new term “Architourism” – Architecture as a Destination for Tourism –, which is coined by Ockman and Frausto in 2005, will also be studied.Ockman and Frausa suggested that architecture itself has become a marketable destination today, and now architourism has its own niche in the tourist industry. Frank Gehry s Guggenheim Bilbao Museum is a good example of how architecture can turn around a city s fortunes. All of these trends toward experiencing the architecture are allowing places such as Dallas and Fort Worth, Texasto augment and define tours that highlight the contributions that famous architects have made to the area, thus providing a valuable resource of revenue generation for the community.Dallas/Fort Worth Area Tourism Council (DFWATC) was formed in 1978 to be a comprehensive force within the tourism industry to jointly market the entire area as a single-destination. The Dallas/ Fort Worth Area Tourism Council has banded together all segments of the tourism industry. The Council represents area communities,area hotels, major area attractions, museums, restaurants, shopping/retail establishments, transportation companies and support services to the industry. In this sense,this paper will explore howDallas/Fort Worth Area is transformed or revitalized around architourism, how it stimulates the creation of new masterpieces of architecture, and some forms of “desire” that motivate architourism like authentic, exotic, escapist, and spectacular. It will also be questioned in what ways contemporary media culture,globalization, urbanism, and consuming economy have changed the relationship between architecture and tourism. This research discusses the relationships between the design of architecture, urban space and the contemporary practices of tourism. “Architourism” – Architecture as a Destination for Tourism –, which is defined by Ockman and Frausto in 2005, is also studied.       • An Alternative to Sea-Sun-Sand Tourism

        In the late 1970s Cohen argued that attention was being overly focused on tourism's negative effects. After that, several types of non-mass tourism such as 'alternative tourism' and 'ecotourism' were advocated as being less damaging to society and environment. In recent years, rural tourism has attracted a steadily increasing level of research attention, stimulated by two factors. First, tourism demand for rural areas is growing. Second, rural tourism has arrived on the political-economic agenda, the hope being that tourism business can alleviate the consequences of a decline in traditional means of rural employment. Varying degrees of rural areas tend to have four main qualities: relatively low physical densities of people, buildings and activities; less social and cultural heterogeneity; less economic diversity; and a comparative physical isolation from general economic, social and political networks. With the aim of improving the possibilities of alternative tourism in Antalya and accelerating the progress in rural areas by means of tourism, mountain villages are attempted to be brought in tourism guaranteed that all its local, social and cultural values kept. Mountain villages were settled by means of an architectural concern that gives priority to natural conditions and that is conscious of human-centered value judgments. By means of this study, necessity of regional protection that covers all the local, social and cultural entities is emphasized, and the importance of involvement within tourism in order to accelerate regional development is discussed.

      • Predictors of Knowledge Level and Awareness towards Breast Cancer among Turkish Females

        Background: Breast cancer (BC) is the most-common malignancy of women worldwide. Though there are differences among developed and developing countries, BC remains the most common cancer type of women in Turkey. Objective: This study aimed to identify the level of knowledge, awareness, and their potential predictors towards BC in Ankara, Turkey. Materials and Methods: The present descriptive study was conducted on 376 females attending a breast health outpatient clinic. A self-administered questionnaire was designed to evaluate knowledge level about BC and predictors effecting its level. Data analysis was performed using the chi-square test. A value of p<0.05 was considered statistically significant. Results: Mean age of the participants was $46.2{\pm}9.93$ (22-75). The majority (92.6 %) were married; 41.5% were educated less than nine years. Most of the women were housewives (82.7%) and, were living in an urban region (86.4%). Predictors of effecting responses to seven knowledge and awareness questions about BC varied from demographic features including older age groups, higher educational levels, being married, living in an urban area, being employee, smoking, having greater BMI to additional attributes associated breast health such as the increased number of births, applying for the purpose of control, positive family history of breast diseases, any diagnoses of breast diseases and performing BSE practice. Conclusions: It was determined that females in Turkey have better knowledge of BC than other developing countries even though it is not at the desired level. These findings revealed that females should be more informed about BC risk factors, prognosis and treatments by primary health-care providers to counteract the ascending burden of this disease.

      • Making a Case for Adaptive Reuse as a Strategy to Revitalize and Generate Urban Tourism in Post-industrial Cities

        The Post Industrial era produced urban environments where what were once vibrant parts of the city became blighted as traditional industries closed. Just over ten years ago, the city of Bilbao was an unknown port in Spain that was on the decline due to the loss of shipping contracts. The city commissioned Frank Gehry to design and build the Guggenheim Museum in Bilbao. The museum became such an iconic structure that the city of Bilbao is now a tourist magnet with millions of visitors every year. In 2000, the architecture firm of Herzog and De Meuron was commissioned to design the museum UK Tate Modern to display the national collection of international modern art. The site selected was an abandoned power plant on the south bank of the river Thames, an area that was neglected. The area is now a vibrant tourist destination. The object of this paper is to investigate how the relationship between architecture, urban tourism and adaptive reuse can play a key role in the revitalization of areas impacted by urban decay. While the idea of using tourism as a vehicle for economic development has been approached in different ways, Judd (1991) focused and developed ideas around the economic role of tourist precincts that had been curved out of urban decay. Based on the findings of this study, a strong case can be made for the concept of adaptive reuse not as a panacea, but as a vehicle for revitalization and growth based on urban tourism.

      • Trastuzumab-based Retreatment after Lapatinib in Heavily Pretreated HER2 Positive Metastatic Breast Cancer: an Anatolian Society of Medical Oncology Study

        Background: For HER2 positive metastatic breast cancer (MBC), continuing anti-HER2 therapy beyond progression is associated with improved outcome. However retreatment with trastuzumab after lapatinib progression is controversial. We retrospectively analyzed the efficacy of trastuzumab-based chemotherapy in HER2+ metastatic breast cancer patients whose disease progressed after lapatinib. Materials and Methods: Between October 2010 and May 2013, 54 patients whose disease progressed after lapatinib were retreated with trastuzumab-based chemotherapy. Efficacy and toxicity results were evaluated retrospectively. Results: The median age of patients was 46 (range 27-67). Fourteen patients (26%) had metastases at the time of diagnosis. All of the patients had received trastuzumab in an adjuvant or metastatic setting, while 16 (30%) had received two lines of trastuzumab. All patients had received lapatinib plus capecitabine. The median chemotherapy line for the metastatic setting was 2 (range 1-7). Cranial metastases were identified in 27 (50%) patients. 53 patients received trastuzumab-based chemotherapy following lapatinib progression while one patient received trastuzumab monotherapy. Combination chemotherapy consisted of navelbin (n=33), taxane (n=10), gemcitabine (n=2), platinum (n=2) and platinum with taxane (n=6). The median treatment cycle was 5 (range 1-44). Among 49 patients assessed for response 2 (4%) showed CR, 12 (25%) PR, 11 (22%) SD and 24 (49%) disease progression. Asymptomatic cardiotoxicity was reported in 2 (4%) of the patients. At a median follow-up of 9 months (1-39), median progression-free survival was 5 months (95% CI 4.1-5.9) and median overall survival was 10 months (95% CI 6.9-13.0). PFS and OS were not affected by the absence/presence of cranial metastases. Conclusions: Retreatment with trastuzumab-based therapy after lapatinib progression showed efficacy in heavily treated MBC patients.

        Solving design optimization problems via hunting search algorithm with Levy flights

        This study presents a hunting search based optimum design algorithm for engineering optimization problems. Hunting search algorithm is an optimum design method inspired by group hunting of animals such as wolves, lions, and dolphins. Each of these hunters employs hunting in a different way. However, they are common in that all of them search for a prey in a group. Hunters encircle the prey and the ring of siege is tightened gradually until it is caught. Hunting search algorithm is employed for the automation of optimum design process, during which the design variables are selected for the minimum objective function value controlled by the design restrictions. Three different examples, namely welded beam, cellular beam and moment resisting steel frame are selected as numerical design problems and solved for the optimum solution. Each example differs in the following ways: Unlike welded beam design problem having continuous design variables, steel frame and cellular beam design problems include discrete design variables. Moreover, while the cellular beam is designed under the provisions of BS 5960, LRFD-AISC (Load and Resistant Factor Design-American Institute of Steel Construction) is considered for the formulation of moment resisting steel frame. Levy Flights is adapted to the simple hunting search algorithm for better search. For comparison, same design examples are also solved by using some other well-known search methods in the literature. Results reveal that hunting search shows good performance in finding optimum solutions for each design problem.

      • Clinicopathological Characteristics of Hepatocellular Carcinoma in Turkey

        Background: Hepatocellular carcinoma (HCC), the main malignant tumor of the liver, is very common and highly lethal. The aim of this study was to determine its clinicopathologic characteristics and risk factors in Turkey. Materials and methods: In this study, patients who were diagnosed as suffering from HCC in the period between August 2004 and December 2011 were evaluated retrospectively. Results: A total of 98 patients were included, with a median age 61 (range: 16 to 82). Seventy nine (80.6%) were male 59 (60.2%) were infected with hepatitis B virus (HBV) and 15 (15.3%) with HCV, another 15 (15.3%) being alcohol abusers. Seventy two (73.5%) were at advanced stage and 54 (55.1%) had elevated serum alpha-fetoprotein (AFP). Surgery, chemoembolization, systemic chemotherapy and application of the tyrosine kinase inhibitor sorafenib were the major treatment options. Conclusions: According to our findings HCC is mostly diagnosed in advanced stage and age, being five times more common in males than females. Main risk factors of HCC are HBV infection, HCV infection and alcohol abuse. Elevation in AFP may facilitate early diagnosis of HCC in high risk groups.

        Effects of anisotropy and curvature on free vibration characteristics of laminated composite cylindrical shallow shells

        This paper presents effects of anisotropy and curvature on free vibration characteristics of cross-ply laminated composite cylindrical shallow shells. Shallow shells have been considered for different lamination thickness, radius of curvature and elasticity ratio. First, kinematic relations of strains and deformation have been showed. Then, using Hamilton's principle, governing differential equations have been obtained for a general curved shell. In the next step, stress-strain relation for laminated, cross-ply composite shells has been given. By using some simplifications and assuming Fourier series as a displacement field, differential equations are solved by matrix algebra for shallow shells. The results obtained by this solution have been given tables and graphs. The comparisons made with the literature and finite element program (ANSYS).
