        Diversity minimization through part combination – a Portuguese railway infrastructure case study

        With increasing market needs for product and service variety, companies struggle to provide diversity in cost-effective ways. Through standardization of components with a low perceived added value, companies can take advantage of economies of scale while maintaining product diversity. Railway infrastructure managers face similar challenges of providing economically sustainable services while dealing with the costs of maintaining the system diversity. Typically, unintended design diversity stems from design practices in which existing solutions are not reused for new problems and new solutions are rarely planned considering the dynamics of requirement changes. In this paper we provide a methodology to assess how to standardize different designs to minimize design diversity and to assess design divergence in a product family. The developed methodology is able to take into account any set of standardization compatibility constraints that the user can define. The methodology was applied in the context of a small-scale railway infrastructure manager using a dataset of 223 unique designs of functionally similar components from its electrification system. Depending on the activated compatibility constraints, results indicate that over 60% of components can be reduced to a set of 86 unique designs.

        Winkler spring behavior in FE analyses of dowel action in statically loaded RC cracks

        A nonlinear finite element modeling approach is developed to assess the behavior of a dowel bar embedded on a single concrete block substrate, subjected to monotonic loading. In this approach, a discrete representation of the steel reinforcing bar is considered, using beam finite elements with nonlinear material behavior. The bar is connected to the concrete embedment through nonlinear Winkler spring elements. This modeling approach can only be used if a new constitutive model is developed for the spring elements, to simulate the deformability and strength of the concrete substrate. To define this constitutive model, an extensive literature review was conducted, as well as 3 experimental tests, in order to select the experimental data which can be used in the calibration of the model. Based on this data, an empirical model was established to predict the global dowel response, for a wide range of bar diameters and concrete strengths. This empirical model provided the information needed for calibration of the nonlinear Winkler spring model, valid for dowel displacements up to 4 mm. This new constitutive model is composed by 5 stages, in order to reproduce the concrete substrate response.

        Constitutive Model for Aggregate Interlock in FEM Analyses of Concrete Interfaces with Embedded Steel Bars

        A nonlinear finite element model (FEM) is developed to assess the behaviour of a cracked concrete interface, reinforced with embedded steel bars and subjected to monotonic loading. A dowel action finite element modelling approach is conceived for that purpose. The bond between the steel bars and the surrounding concrete is also considered in the model and an interface finite element is included to simulate aggregate interlock. Then, the comparison of the model results with experimental values allowed the calibration of aggregate interlock constitutive relations for cracks in monolithic concrete restrained by embedded steel bars. New constitutive relations are also proposed for shear transfer by aggregate interlock in a concrete joint.

        Endoscopic Ultrasound-Guided Fine Needle Aspiration and Endoscopic Retrograde Cholangiopancreatography-Based Tissue Sampling in Suspected Malignant Biliary Strictures: A Meta-Analysis of Same-Session Procedures

        Background/Aims: The diagnosis of biliary strictures can be challenging. There are no systematic reviews studying same-sessionendoscopic retrograde cholangiopancreatography (ERCP)-based tissue sampling and endoscopic ultrasound-guided fine needleaspiration (EUS-FNA) for the diagnosis of biliary strictures. Methods: A systematic review was conducted on studies analyzing same-session EUS and ERCP for tissue diagnosis of suspectedmalignant biliary strictures. The primary outcome was the accuracy of each method individually compared to the two methodscombined. The secondary outcome was the accuracy of each method in pancreatic and biliary etiologies. In the meta-analysis, we usedForest plots, summary receiver operating characteristic curves, and estimates of the area under the curve for intention-to-treat analysis. Results: Of the 12,132 articles identified, six were included, resulting in a total of 497 patients analyzed. The sensitivity, specificity,positive likelihood ratio, negative likelihood ratio, and accuracy of the association between the two methods were: 86%, 98%, 12.50,0.17, and 96.5%, respectively. For the individual analysis, the sensitivity, specificity and accuracy of EUS-FNA were 76%, 100%, and94.5%, respectively; for ERCP-based tissue sampling, the sensitivity, specificity, and accuracy were 58%, 98%, and 78.1%, respectively. For pancreatic lesions, EUS-FNA was superior to ERCP-based tissue sampling. However, for biliary lesions, both methods had similarsensitivities. Conclusions: Same-session EUS-FNA and ERCP-based tissue sampling is superior to either method alone in the diagnosis of suspectedmalignant biliary strictures. Considering these results, combination sampling should be performed when possible.

        Endoscopic Ultrasound Fine-Needle Aspiration versus Fine-Needle Biopsy for Lymph Node Diagnosis: A Large Multicenter Comparative Analysis

        Background/Aims: Endoscopic ultrasound fine-needle aspiration (EUS-FNA) is preferred for sampling of lymph nodes (LNs) adjacentto the gastrointestinal wall; however, fine-needle biopsy (FNB) may provide improved diagnostic outcomes. This study aimed toevaluate the comparative effcacy and safety of FNA versus FNB for LN sampling. Methods: This was a multicenter retrospective study of prospectively collected data to evaluate outcomes of EUS-FNA and EUS-FNBfor LN sampling. Characteristics analyzed included sensitivity, specificity, accuracy, the number of needle passes, diagnostic adequacyof rapid on-site evaluation (ROSE), cell-block analysis, and adverse events. Results: A total of 209 patients underwent EUS-guided LN sampling. The mean lesion size was 16.22±8.03 mm, with similar sensitivityand accuracy between FNA and FNB ([67.21% vs. 75.00%, respectively, p=0.216] and [78.80% vs. 83.17%, respectively, p=0.423]). Thespecificity of FNB was better than that of FNA (100.00% vs. 93.62%, p=0.01). The number of passes required for diagnosis was notdifferent. Abdominal and peri-hepatic LN location demonstrated FNB to have a higher sensitivity (81.08% vs. 64.71%, p=0.031 and80.95% vs. 58.33%, p=0.023) and accuracy (88.14% vs. 75.29%, p=0.053 and 88.89% vs. 70.49%, p=0.038), respectively. ROSE was asignificant predictor for accuracy (odds ratio, 5.16; 95% confidence interval, 1.15–23.08; p=0.032). No adverse events were reported ineither cohort. Conclusions: Both EUS-FNA and EUS-FNB are safe for the diagnosis of LNs. EUS-FNB is preferred for abdominal LN sampling. EUS-FNA+ROSE was similar to EUS-FNB alone, showing better diagnosis for EUS-FNB than traditional FNA. While ROSE remained asignificant predictor for accuracy, due to its poor availability in most centers, its use may be limited to cases with previous inconclusivediagnoses.

        Ossifying fibroma in the maxilla and orbital floor: report of an uncommon case

        Benign fibro-osseous lesions occur when normal bone is replaced by cellular fibrous connective tissue and mineralized structures. One rare type of these lesions is the ossifying fibroma (OF). The aim of this study is to report an unusual case of OF in a 57-year-old female. Physical examination showed facial asymmetry without any tenderness, fluctuation, ocular pain, or ophthalmoplegia. Imaging exams revealed a solid mass involving the left maxilla and orbital floor. Surgical resection was performed without any complications or sequelae, and the histopathological results confirmed OF. Although recurrence is rare in this condition, the patient remains under follow-up.

        Role of Cardiac Septal Occluders in the Treatment of Gastrointestinal Fistulas: A Systematic Review

        Treating gastrointestinal (GI) fistulas endoscopically is challenging owing to an established epithelial tract. The variety of endoscopicapproaches is transforming endoscopy into a first-line therapy. However, many sessions are often required, with variable success rates. Owing to these limitations, the off-label use of cardiac septal occluders (CSOs) has been reported. We searched for articles related to CSOs in the MEDLINE, EMBASE, Cochrane Library, and LILACS databases and gray literature. Theprimary outcomes included technical success, clinical success, and safety of CSOs in GI fistula management. A total of 25,574 records were identified, and 19 studies ultimately satisfied the inclusion criteria. Technical success was achieved inall cases. Of the 22 fistulas, 77.27% had successful closure, with a mean follow-up period of 32.02 weeks. The adverse event rate was22.72%, with no associated mortality. Univariable and multivariable regression analyses showed no significant difference in the successof closure and adverse events in relation to several variables among the subgroups. The use of CSOs appeared to be technically feasible, effective, and safe in the treatment of GI fistulas. The satisfactory results derived fromthis sparse literature suggest that it can be an option in the management of GI fistulas.

        How to deepen the single market and at the same time respect the European goal brought by the Treaty of Lisbon of building a "highly competitive social market economy"? This question comes at a time when Europe`s economy and financial markets are still severely affected by the 2008 financial crisis. The European Commission established a European Systemic Risk Board that started its functions on January 2011. This aims at correcting regulatory weaknesses by building a European System of Supervision and Crisis Management to improve macroeconomic analysis and regulatory policy in Europe. In this paper I will analyze this policy and compare it to other national and international policies with the same objectives.

        Masquerade as Memory: Conflict as Heritage in Lavras do Abade, Brazil

        This article presents a discussion about the memory of an environmental conflict that occurred in the mining village of Lavras do Abade in mid-western Brazil at the end of the nineteenth century. I introduce a study of the phenomenon called Mascarados, part of the religious festival of the present-day town, and its relationship to the previous conflict in the neighbourhood. I report several interviews I conducted about this historical disagreement with the oldest residents in the present-day town of Pirenópolis, an old rival village formerly called Meia Ponte. Finally, I discuss theories about the individual and group implications of this ‘masked memory’ and its impact on the community's archaeological, historical and environmental heritage.
