        Introduction: Adult Still`s Disease is a systemical disease which its both etiology and pathogenic structure is not known clearly. In its etiology, genetic and infectious agents are blamed. During its medical treatment, non steroid anti infi ammatory drugs, steroids and immunal modulator drugs are usable. In our case, a 25 Y. O. man has been considered. After getting ecartationed by clinic, laborotary and radiological methods, this disease was considered as still disease diagnosis. Resutls of laboratory searches were as; erythrocyte sedimentation speed 64/ hour, creactive protein 12, AST 65 U/L, ALT 74 U/L, hemoglobin 14. 1 g/dl, blood platelet 210. 000/mm3, leucocyte 9100/mm3, neutrophil 7240 mm3. CK level 1850 U/l. Atypical cells were not observed in his peripheral spread, neutrophil ratio was obtained as (% 80 neutrophil, % 13 leucocyte, % 6 monocyte, % 1 eosinophil). At his controls, transamination enzymesand CK (creatine kinase) level began to rise while they were declining. Simultaneously, pain and crampsnearly takes half an hour occured at his knee join. There was no patalogic result at his sacroiliac graphy. A fi gure fi tting with a cyst on left tibia was determined at his bilateral knee graphy. It was reported as fi tting with fi broma according to his magnetic resonance watch and scintigraphy. The sick person had run to a temperature reaching nealy 40 degrees at night and at the same time casts were seen on his body. Regarding to collagenose disease and vascolyte, ANA (Anti NuclearAntibody), anti ds DNA (antidoublestrandeddeoksiribonucleicacid), RF (Rheumatoid Factor), p-ANCA (perinuclear anti-neutrophilcytoplasmicantibodies), c-ANCA (cytoplasmicanti-neutrophilcytoplasmicantibodies) were sent. Result was negative. Ferritin blood test was taken because he was supposed as ESR (erythrocyte sedimentation rate). He was taken under medical treatment by getting >2000 level and relating to Yamaguchi criteria he has 4 major, 2 minor standarts with ESR diagnose.

        Cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider, and the One G glide path instruments in double-curvature canals

        Objectives The aim of this study was to compare the cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider and One G glide path instruments in artificial double-curvature canals. Materials and Methods This study included 15 WaveOne Gold Glider (size 15/0.08), 15 ProGlider (size 16/0.08), and 15 One G (size 16/0.06) nickel titanium files. The files were used in accordance with the manufacturer's instructions until they were broken in artificial double-curvature canals made of stainless steel. The time to fracture was recorded via a digital stopwatch and the number of rotations until fracture was also calculated. The data were statistically analyzed via the Kruskal-Wallis test. Results The highest average number of rotations until fracture of the files was found for the WaveOne Gold Glider, followed by ProGlider and One G in order. Statistically significant differences were present between all groups of files (p < 0.05). Conclusions In our study, the resistance of the WaveOne Gold Glider nickel-titanium (Ni-Ti) file to cyclic fatigue in S-shaped curved canals was found to be higher than that of the ProGlider and One G Ni-Ti files.

        Chemokine and Chemokine Receptor Polymorphisms in Bipolar Disorder

        ObjectiveaaBipolar disorder (BD) is a debilitating psychiatric disease with unknown etiology. Recent studies have shown inflammation as a potential contributing factor of BD pathogenesis. However, potential associations between chemokine and chemokine receptor polymorphisms and BD have been fundamentally understudied. To identify participation of chemokines in BD pathogenesis, we examined genetic variants of several chemokine and chemokine receptor genes. MethodsaaThe study population comprised 200 patients with BD and 195 age- and sex-matched healthy controls. Genotyping of monocyte chemotactic protein 1 (MCP-1) A2518G, CCR2 V64I, CCR5 Δ32, CCR5 A55029G, stromal cell-derived factor 1 (SDF-1) 3’A, and CXCR4 C138T polymorphisms was performed using polymerase chain reaction and restriction enzyme digestion. ResultsaaWe found that CCR5-Δ32 II and CXCR4-C138T C+ genotype frequencies contributed to an increased risk for BD. However, no statistical significance could be obtained with these genotypes after Bonferroni correction. A significant asssociation was only found with MCP-1 GG and G+ genotypes, which were markedly more prevalent in patients with BD and these genotypes seemed to significantly increase the risk for BD even after Bonferroni correction. ConclusionaaOur findings indicate an association between genetic variants of certain chemokine and chemokine receptor (especially MCP-1) genes and BD. The exact mechanisms by which these variants contribute to BD pathogenesis and their clinical implications need to be further investigated.

        Comparing the effectiveness of ultrasound guided versus blind genicular nerve block on pain, muscle strength with isokinetic device, physical function and quality of life in chronic knee osteoarthritis: a prospective randomized controlled study

        Background: The genicular nerve block (GNB) is demonstrated from several reports to alleviate pain and improve knee functionality in patients with chronic knee osteoarthritis (OA). Ultrasound (US)-guided GNB has been the most used imaging method. This study aimed to compare the effectiveness of US-guided versus blind GNB in the treatment of knee OA. Methods: This prospective, randomized clinical trial included patients with knee OA based on American College of Rheumatology diagnostic criteria. The patients were evaluated for clinical and dynamometer parameters at the baseline, 4 weeks after treatment, and 12 weeks after treatment. The patients underwent blind injection or US-guided injection. Results: When compared with the baseline, both groups showed significant improvement in pain, physical function, and quality of life parameters. Significant differences were observed between the groups for clinical parameters (30-second chair stand test, 6-minute walk test) in favor of the US-guided group. On the other hand, blind injection was more significantly effective on some parameters of the Nottingham Health Profile. There wasn’t any significant improvement in isokinetic muscle strength for either group. Conclusions: This study demonstrated that both US-guided and blind GNB, in the treatment of knee OA, were effective in reducing symptoms and improving physical function. GNB wasn’t an effective treatment for isokinetic muscle function. USguided injections may yield more effective clinical results than blind injections.

        Maxillary sinus aspergilloma of odontogenic origin: Report of 2 cases with cone-beam computed tomographic findings and review of the literature

        Aspergilloma of the maxillary sinus is considered rare in immunocompetent patients, but a considerable increase has recently been seen in the incidence of reported cases. Dental procedures involving the antral region are thought to predispose individuals to this form of aspergillosis. Because aspergilloma shares similar clinical features with other sinus pathologies, its diagnosis may be delayed. Thus, an early diagnosis confirmed by a histopathological examination plays a crucial role in the adequate management of aspergilloma. This article provides a concise review of the reported cases of aspergilloma associated with dental procedures and reports 2 new cases of aspergilloma in middle-aged female patients, with a presentation of their cone-beam computed tomographic findings.

        COMBINATORIAL SUMS INVOLVING STIRLING, FUBINI, BERNOULLI NUMBERS AND APPROXIMATE VALUES OF CATALAN NUMBERS

        The aim of this paper is to derive some identities and formulas associated with the Bernoulli numbers of negative order, the Stirling numbers of the second kind, the Fubini numbers, and the Catalan numbers. Furthermore, we give some inequalities including the binomial coecients, the Stirling numbers of the second kind, the Catalan numbers and the Bernoulli numbers of negative order. Finally, we give further remarks and observations for not only on spline curves and the Fubini numbers and polynomials, but also on approximate value of the Catalan numbers.

        Edible Packaging Film Derived from Mechanically Deboned Chicken Meat Proteins: Effect of Transglutaminase on Physicochemical Properties

        In this study, effect of transglutaminase (TGase) addition on physical, water barrier, optical and mechanical properties of mechanically deboned chicken meat protein (MDCM-P) films was investigated. When TGase was added to the films, the thickness increased, but the solubility decreased. Films treated with TGase exhibited higher water vapor permeability than control film (p<0.05). When TGase concentration increased, the L* values of films decreased, but a* and b* values increased. All films showed very good barrier properties against UV light. The highest tensile strength was obtained in MDCM-P films containing 3% TGase (p<0.05). The elongation at break values increased with the TGase concentration increasing from 1 to 3%, but decreased at higher enzyme concentration (p<0.05). The addition of TGase altered molecular organization and intermolecular interaction in the film matrix. TGase treated films showed smoother and ordered surface structure and homogeneous and compact microstructure. The results indicated that TGase use can be an effective approach in improving the solubility and mechanical properties of MDCM-P films.

        Factors associated with paravertebral muscle cross-sectional area in patients with chronic low back pain

        Background: This study was performed to reveal the relationships between the cross-sectional areas (CSAs) of the paraspinal muscles and the severity of low back pain (LBP), including the level of disability. Methods: This single-center cross-sectional study was conducted on 164 patients with chronic LBP. The effects of demographic characteristics, posture, level of physical activity, disc herniation type, and sarcopenia risk on the CSAs of paraspinal muscles were evaluated along with the relationship between the CSAs and severity of pain and disability in all patients. The CSAs of paraspinal muscles were evaluated using the software program Image J 1.53. Results: A negative significant correlation was found between age and the paraspinal muscle’s CSA (P < 0.05), whereas a positive correlation was present between the level of physical activity and the CSA of the paraspinal muscle at the L2-3 and L3-4 levels. The CSAs of paraspinal muscles in patients with sarcopenia risk was significantly lower than those in patients without sarcopenia risk (P < 0.05). The CSAs of paraspinal muscles at the L2-3 and L3-4 levels in obese patients were significantly higher than those in overweight patients (P = 0.028, P = 0.026, respectively). There was no relationship between the CSAs of paraspinal muscles and pain intensity or disability. Conclusions: Although this study did not find a relationship between paraspinal CSAs and pain or disability, treatment regimens for preventing paraspinal muscles from atrophy may aid pain physicians in relieving pain, restoring function, and preventing recurrence in patients with chronic LBP.
