        중대산업사고 예방을 위한 종합위험관리체제(IRMS) 구축에 관한 연구

        The Process Safety Management (PSM) by the Law of Industry, Safety and Health has been performed for preventing major accidents of chemical plants since 1996. In terms of preventing chemical accidents more precisely, it is essential to develop a tool for quantitative risk assessment. For this, KOSHA (Korea Occupational Safety and Health Agency) developed an Integrated Risk Management System (IRMS) . The system is designed to assimilate data on chemical plant hazards from external database, to integrate these data with location information (topographic and demographic), and to make them user-friendly accessible. The system consists of several main functions: display of five major Korean petrochemical complex layout, display of equipment layout with its information utilizing the external database, zonation of the hazard effected area with consequence analyses, the most probable accident scenario generation, accident/incident database and calculation of frequency of accident using equipment reliability database, etc. The highlight of IRMS is to provide the risk contours using GIS(Geographical Information System) technology. IRMS is intended to manage hazardous installation more systematically and effectively, to reduce the number of accident remarkably, further minimizing production loss in the plant. The system is now under application to about 500 PSM sites as well as and emergency authorities in Korea by KOSHA (Korea Occupational Safety and Health Agency)

      • 양측성 천막상부 뇌동맥류에 대한 일측성 일회접근 결찰술 : 증례보고

        양측성으로 천막상부의 뇌동맥류가 발견된 경우, 제한된 경우에 한해서, 일측성 테리온 접근법으로 일회 수술로 뇌동맥류 결찰술을 시행할 수가 있다. 48세 여성에서 좌측 중뇌동맥과 우측 전맥락동맥에 두 개의 뇌동맥류가 발견되었다. 우측 전맥락동맥류는 뒤쪽 방향을 향하는 양상이었다. 좌측 테리온 접근법으로 뇌동맥류 결찰술을 시행하였다. 좌측 중뇌동맥에 위치한 뇌동맥류를 결찰한 다음에, 우측 전맥락동맥에 위치한 뇌동맥류를 좌측 테리온 접근법으로 반대쪽 경동맥-시신경 지주막하 공간을 통하여 성공적으로 뇌동맥류 결찰술을 시행하였다. 저자는 양측성으로 천막상부의 뇌동맥류가 있는 경우에 편측 테리온 접근법으로 양쪽 뇌동맥류를 성공적으로 결찰하여, 2차 수술이 필요없게 된 증례를 보고하면서 문헌을 고찰하였다. Patients harboring bilateral supratentorial aneurysms can be approached via the contralateral pterional approach in selected cases. A 48-year-old woman was found to have two aneurysms at the left middle cerebral artery and the right anterior choroidal artery. The main direction of the anterior choroidal artery aneurysm was posterior. A direct clipping was performed via the left pterional approach. After the left middle cerebral artery aneurysm was clipped, the contralateral anterior choroidal artery aneurysm was further exposed and successfully clipped through the contralateral carotico-optic space. The author reports a case of bilateral supratentoral aneurysms that was successfully clipped via unilateral pterional approach, thus avoiding a second craniotomy.

      • Multiple Spinal Intradural Schwannomas Combined with Sequestered Lumbar Disc Herniation

        In this report, we describe a case of multiple intradural schwannomas in the spine combined with a upward sequestered disc herniation at L4-L5. It is often difficult to differentiate this tumor from lumbar disc herniation, especially a sequestered hernia. Gadolinium-enhanced MR images were a useful preoperative examination modality for differentiating these lesions from other diseases. Microscopically, the intradural tumors were successfully removed. The dura maters of the L2 and S1 were opened microsurgically, allowing the nerve fibers involved in the tumor to be identified. The involved fibers were cut around the tumor, and the lesion was resected while the intact nerve fibers were preserved. An upward sequestered lumbar disc herniation at L4-L5 was also successfully removed. Based on histological examination of the resected specimen, the tumors were diagnosed as schwannomas. Microsurgery allowed the tumors to be removed with minimal impairment from cutting of nerve fibers in the each nerve root.

      • Reciprocation of perceived organizational support on organizational citizenship behaviors in local government

        조직시민행동(organizational citizenship behaviors)은 조직이 효과적으로 기능하는데 필요한 구성원의 자발적 조직행동으로서 조직의 공식적 보상체계에 의해 직접적 또는 명시적으로 인정되지 않지만 전체 조직의 효과적 운영을 촉진하는 역할을 의미한다. 이러한 행동은 조직에 의해 개인이 공식적으로 수행해야 할 업무와 관련된 행동은 아니지만, 조직의 입장에서 보면 조직의 발전을 위해 바람직한 행동들이라고 정의할 수 있다. 그리고 조직 구성원의 조직에 대한 보상 심리적 교환관계에 근거하여 개발된 조직지원인식(perceived organizational support)은 조직이 개인의 기여를 가치있는 것으로 받아들이고, 동시에 구성원에게 관심과 배려를 보이는 정도에 대하여 구성원이 개괄적으로 인지하는 신념이기 때문에 조직시민행동의 중요한 영향 요인으로 평가되고 있다. 본 연구는 지방정부 구성원들의 조직시민행동을 높이기 위해서는 리더의 역할을 포함하는 조직지원에 대한 구성원의 인식이 중요하다고 보고, 이들 변인들간의 인과 경로를 분석하는 데 목적을 둔다. 이를 위해 먼저, 이론적 논의를 통해 분석의 이론적 근거를 제시하고, 변인들간의 영향경로를 분석할 수 있는 인과모형을 작성한다. 그리고 실증분석을 위한 자료를 수집하고 경험적 검증을 한다. 마지막으로 분석 결과에 근거하여 효율적인 조직관리를 위한 시사점을 제시한다.

      • 횡문근융해증과 급성 신부전 및 범발성 혈관내 응고장애를 동반한 열사병 1예

        Heat stroke is a life-threatening illness characterized by an elevated core body temperature that rises above 40% and central nervous system dysfunctions that results in delirium, convulsions, or coma. Heat stroke is subdivided into two forms, classic and exertional. Since strenuous exercise is one of the major exacerbating and precipitating factors, the incidence of exertional heat stroke is high among young adult, especially military personnel undergoing military training. Exertional heat stroke commonly causes rhabdomyolysis, lactic acidosis, acute renal failure, shock and pulmonary edema. We report a case of heat stroke with rhabdomyolysis, acute renal failure, disseminated intravascular coagulopathy(DIC) after strenuous exercise. A 18 year old man was admitted to the hospital because of loss of consciousness. He was a wrestler and he had tried to lose 6kg in weight by jogging and strenuous exercise on that summer day with hot temperature. The development of this illness was favoured by much clothes inappropriate for that environmental condition with a purpose to enhance sweating, and by restricted intake of water and food. On admission, the blood pressure was 80/50mmHg, and body temperature was 39.4℃ with stuporous mentality. Laboratory finding were as follows : in complete blood cell count, hemoglobin was 16.7g/dL and platelet count was 158,000/ mm³. In blood chemistry, AST 1061U/L, ALT 201U/L, BUN 30mg/dL, Cr 2.2mg/dL, LDH 941IU/L, CK 696IU/L were resulted. In arterial blood gas analysis, pH 7.39, pCO₂ 30mmHg pO₂ 86mmHg, HCO₃ 18.4mEq/L, O₂ saturation 96.7% were resulted. In urinalysis, specific gravity 1.025, protein 3+, erythrocyte 2+ were resulted. In brain imaging study, brain CT finding was normal. After appropriate treatments with rapid cooling, hydration and supportive care, he improved with alert mental status. On 2nd hospital day, in laboratory result, thrombocytopenia and coagulopathy were appeared and renal insufficiency was aggravated. So he was treated with platelet concentrate and fresh frosen plasma for DIC, and enough hydration for appropriate urine volume preservation. After all, On 22nd hospital day, he fully recovered and discharged. Nowadays, with followed for some duration, he is wellbeing with no complications.

      • 거래 및 교환적 심리계약 이행 지각이 정서 및 지속적 헌신에 미치는 영향 분석 : 경남지역 고등학생들을 중심으로

        조직헌신의 영향 변인들에 대해 많은 연구가 이루어져 왔지만 심리적 계약과과의 연관성에 대한 연구는 소수에 불과하다. 본 연구는 학교 조직에서도 심리적 계약이 조직헌신의 중요한 영향 변인이라는 가정하에 이들 변인간의 관계를 경험적으로 분석하는데 목적을 두었다. 본 연구는 통제적 성과주의 가치가 지배하고 있는 현실의 학교조직에서 학생들의 단기적인 성과를 강조하는 거래적 계약보다는 교사와 학생들과의 심리적, 정서적 유대감을 통한 장기적인 성과 향상에 중점을 두는 교환적 계약이행의 지각을 고양시킴으로써 학생들의 조직헌신을 높일 수 있다는 것을 실증적으로 분석하였다는 점에서 의미를 가진다.

      • 동종의 동결건조 악관절원판 매식에 관한 실험적 연구

        In occasions when articular disk is perforated or removed, restoring injured or resected disk and a normal bony structure of the joint is undoubtedly a choice of treatment. In such a case, various disk substitutes are being used, but considering the advantages, disadvantages and long-term prognosis of individual substitute materials for disc, there is still no ideal material closely resembling the original disk both functionally and histologically. Besides, owing to the unique characteristic disk in anatomic structure & tissue nature, material closely resembling the disk should be able to withstand the masticatory force and play a active role in jaw movement. From this point of view, lyophilized allogeneic articular disk could be the superior substitute in various aspects only if the problem of immunologic rejection can be solved. There upon the author grafted lyophilized allogeneic articular disks into the left TMJ of 15 male rabbits weighing about 3.0 ㎏. And also autogenous articular disks harvested from left TMJ were placed in to the defects of right articular disk and histological observation was performed to evaluate the efficiency of the lyophilized allogeneic TMJ disk as a disk substitute. The results were as follows ; 1. In groups where lyophilized allogeneic disks were grafted, most of the graft material resorbed in early stage whereas the one adjacent to synovial tissue relatively longer. From the 3rd week, synovial tissue proliferated actively, and chondrocyte like cell appeared in remaining graft material adjacent to synovial tissue at 6th week. 2. While lyophilized allogeneic disk was resorbed, new disk having similar form to that of the fibrous cartilage which grew from adjacent synovial membrane was partially formed owing to its action as space maintainer & matrix instead of removed disk. 3. New disk tissue formed from a mold of lyophilized allogeneic disk was functionally superior enough to protect the articular surfaces of the articular fossa & condyle well histologically. 4. Autogenous articular disk demonstrated a similar histological picture to normal articular disk at 1 week after grafting and almost normal articular disk & joint structures were seen at post-operative interval of 8 week. In this manner autogenous disk was shown to be superior to lyophilized allogeneic disk at early stage, but lyophilized allogeneic disk substitute was also evaluated to have sufficient efficiency as articular disk substitute because it was transformed into the new disk with time.

        The performance of real-time systems depends upon how well the tasks are scheduled within a cycle time and how fastly the response is made according to the occurrence of an external event. This paper presents the design of an effective real-tie data acquisition system in order to gather the data from an automobile engine. This paper investigates an estimation and a restriction method of execution for periodic data. Through the experiments, the hard real-time guarantee problem of periodic data is studied and the damage problem of periodic data according to the increase of aperiodic tasks is analyzed.

      • 간암과 간세포이형성 부위의 철침착 저항성에 대한 조직학적 연구

        Liver cell dysplasia(LCD) is commonly associated with some liver diseases such as liver cirrhosis or hepatocellular carcinoma and has been regarded as a premaligant condition. In order to clarify the question of whether or not LCD is a premalignant liver lesion, iron special staining, Prussian blue, was done. The specimens were collected at the department of pathology, Chungnam National University College of Medicine from January 1983 to September 1992. The results were as follows: 1. Of observed 48 cases, 34 were hepatocellular carcinoma(70.8%), 11 were metastatic carcinoma(22.9%), and 3 were hepatoblastoma(6.3%). 2. LCD was present in 26 of 34 cases(76.5%) of hepatocellular carcinoma, 5 of 11 cases(45.5%) of metastatic carcinoma, and 1 of 3 case(33.3%) of hepatoblastoma. 3. The carcinoma cells were free of iron deposition(iron excluding foci) in contrast to the neighboring dysplastic or nonneoplastic normal hepatocytes. 4. The presence of iron deposition in LCD cases was similar to or more severe than that of normal hepatocytes. In summary, the results obtained from the present study indicate that LCD reflects neither a regenerating condition nor a premalignant lesion but suggests a reactive change or accompaning lession.

      • SCM415강의 침탄 표면처리 공정과 기계적 성질

        The influence of plasma carburizing process on the surface hardness of SCM415 low-alloy steel (0.15% C) was investigated under the various process conditions of gas composition, gas pressure, plasma current density, temperature and time. The effective case depth was found to depend on the amount of methan gas containing carbon, thus the deepest case depth and the uniformity of hardeness were obtained under the 100% methan gas condition. The case depth increased with the higher plasma current density which could be increased in the higher gas pressure, because the surface carbon content depends on plasma current density. The effective plasma carburizing temperature of SCM415 steel was found to be higher than 850℃, and the case depth was proportional to the square root of carburizing time under the same current density. The bending fatigue strength of plasma carburized specimen is higher than those of as-received specimen and reheat-quenched specimen. High cycle fatigue and dry wear characteristics of vacuum carburized low alloy steel, SCM415(0.15% C), were evaluated after various heat treatments including direct quenching, single reheat quenching and sub-zero treatment. Comparing the fatigue and wear characteristics of carburized specimens to those of hot annealed or reheat quenched specimens, the wear resistant property as well as bending fatigue limits of vacuum carburized steels were substantially superior to other heat treated steels. Test results indicated that the carburized and reheat treated specimens have higher fatigue limits than the carburized and direct-quenched specimens and the specimens followed by sub-zero treatment. The wear volumes of sub-zero treated specimens with higher surface hardness show slightly lower values than those of non sub-zero treated specimens. This is attributed to the reduction of retained austenite amount of the surface and an increase of surface hardness by the sub-zero treatment effect.
