Accumulating evidence indicates that lactic acid bacteria could improve host physiology and lipid metabolism. To investigate the effect of the gut microbiota on host lipid metabolism, a hyperlipidemic rat model was established by feeding rats a high-fat diet for 28 days, and the gut microbiota of the rats was analyzed using real-time PCR before and after administration of Lactobacillus rhamnosus hsryfm 1301 and its fermented milk for 28 days. The findings showed that the Lactobacillus spp., Bifidobacterium spp., Bacteroides spp., and Enterococcus spp. content in the hyperlipidemic rats gut was increased significantly (p < 0.05), while the Clostridium leptum and Enterobacter spp. content was decreased significantly after intervening with L. rhamnosus hrsyfm 1301 and its fermented milk for 28 days (p < 0.05). Furthermore, the lipid levels of the serum and the liver were decreased significantly (p < 0.05) and the fecal water content was increased significantly (p < 0.05) in the hyperlipidemic rats after the intervention, and hepatocyte fatty degeneration of liver tissues was also prevented. A positive correlation was observed between the Clostridium leptum content and the level of serum cholesterol, triglycerides, low-density lipoprotein, and high-density lipoprotein, and a negative correlation was observed between the Enterobacter spp. content and the Lactobacillus spp. and Bifidobacterium spp. content in the hyperlipidemic rats gut. These results suggest that the gut microbiota and lipid metabolism of hyperlipidemic rats could be improved by supplementation with L. rhamnosus hsryfm 1301 and its fermented milk.

Piliated Lactobacillus rhamnosus (pLR) strains possess higher adherent capacity than non-piliated strains. The objective of this study was to isolate and characterize probiotic pLR strains in human fecal samples. To this end, mouse polyclonal antiserum (anti-SpaA) against the recombinant pilus protein (SpaA) of L. rhamnosus strain GG (LGG) was prepared and tested for its reactivity and specificity. With the anti-SpaA, a method combining the de Man, Rogosa, and Sharpe (MRS) agar plating separation and colony immunoblotting (CIB) was developed to isolate pLR from 124 human fecal samples. The genetic and phenotypic characteristics of the resultant pLR isolates were compared by randomly amplified polymorphic DNA (RAPD) fingerprinting, and examination of adhesion to Caco-2 cells, hydrophobicity, autoaggregation, and in vitro gastrointestinal tolerance. Anti-SpaA specifically reacted with three pLR strains of 25 test strains, as assessed by western blotting, immunofluorescence flow cytometry, and immunoelectron microscopy (IEM) assays. The optimized MRS agar separation plus anti- SpaA-based CIB procedure could quantitatively detect 2.5 × 10<sup>3</sup> CFU/ml of pLR colonies spiked in 10<sup>6</sup> CFU/ml of background bacteria. Eight pLR strains were identified in 124 human fecal samples, and were confirmed by 16S RNA gene sequencing and IEM identification. RAPD fingerprinting of the pLR strains revealed seven different patterns, of which only two isolates from infants showed the same RAPD profiles with LGG. Strain PLR06 was obtained with high adhesion and autoaggregation activities, hydrophobicity, and gastrointestinal tolerance. Anti-SpaA-based CIB is a rapid and inexpensive method for the preliminary screening of novel adherent L. rhamnosus strains for commercial purposes.

Aims: Survival after liver resection of hepatocellular carcinoma (HCC) remains poor due to a high incidence of recurrence. We sought to investigate risk factors, patterns, and long-term prognosis among patients with early and late recurrence after liver resection for hepatitis B virus (HBV)-associated HCC. Methods: Data of consecutive patients undergoing curative resection for HBV-associated HCC were analyzed. According to the time to recurrence after surgery, recurrence was divided into early (≤ 2 years) and late recurrence (> 2 years). Characteristics, patterns of initial recurrence and post-recurrence survival (PRS) were compared between patients with early and late recurrence. Risk factors of early and late recurrence, and predictors of PRS were identified by univariable and multivariable Cox-regression analyses. Results: mong 894 patients, 322 (36.0%) and 282 (31.5%) developed early and late recurrence, respectively. On multivariable analyses preoperative HBV-DNA>104 copies/ml was associated with both early and late recurrence, while postoperative no/ irregular antiviral therapy was associated with late recurrence. Compared with patients with late recurrence, patients with early recurrence had a lower proportion of intrahepatic only recurrence (72.0% vs. 91.1%, P<0.001), as well as a lower chance of receiving potentially-curative treatments for recurrence (33.9% vs. 50.7%, P<0.001) and a worse median PRS (19.1 vs. 37.5 months, P<0.001). Multivariable analysis demonstrated that early recurrence was independently associated with worse PRS (HR 1.361, 95%CI 1.094-1.692, P=0.006). Conclusions: Although risk factors associated with early recurrence and late recurrence were different, a high preoperative HBV-DNA load was an independent hepatitis-related risk for both early and late recurrence. Recently research in automated creation of skeleton model has been carried out actively by using Kinect developed by Microsoft. Using Kinect in the field of ergonomics simulation reduces time and costs in forming posture and movement of digital model. While Kinect provides advantages, there are some issues in "accuracy" of the coordinates acquired from it. In this paper, an algorithm that can provide ability of correcting joints and/or skeleton acquired from Kinect is proposed to correct unnatural postures and movements. The algorithm predicts the movement of a human by using movement-prediction-formula which takes moving average of previous patterns of movements and the coordinates corrected previously by ROM (Range Of Motion) into account. It leads to output that creates more natural postures and movements and that is similar to real human uses Kinect.

Aims: According to the BCLC treatment guidelines, surgery does not be recommended for intermediate/advanced hepatocellular carcinoma (HCC). In real world, however, liver resections are often performed in patients with intermediate/ advanced but resectable HCC, especially in the East. Methods: We retrospectively evaluated multicentric data of 1,325 patients newly diagnosed with intermediate/advanced HCC who underwent curative resection. We randomly divided the subjects into development (n = 875) and validation (n = 450) samples. Multivariate Cox proportional hazards models were developed and separately validated on the basis of patients’ clinicopathological variables assessed for associations with 1-year recurrence and 3-year mortality. The discriminatory accuracy of these models was compared with conventional tools by analyzing receiver operating characteristic (ROC) curves. Results: He statistical nomograms built based on performance status, Child-Pugh grade, portal hypertension, preoperative alpha-fetoprotein level, tumor rupture, largest tumor diameter, tumor number, macrovascular and microvascular invasion, and satellites had good calibration and discriminatory abilities, with c-indices of 0.70 (1-year recurrence) and 0.68 (3-year survival), respectively. These models showed satisfactory goodness-of-fit and discrimination abilities in the validation cohort (c-index, 0.68 for 1-year recurrence and 0.69 for 3-year survival). The areas under the ROC curve using these nomograms exceeded those of traditional staging systems, indicating superior discriminatory capability (c-indices, 0.60-0.63 and 0.56-0.62, respectively). Conclusions: Our proposed online nomograms, which present graphically postoperative prognostic models for recurrence and survival in patients with intermediate/advanced but resectable HCC, offer valuable guidance to surgeons and hepatologists for individually predicting survival benefits from surgery and planning recurrence surveillance and adjuvant therapy.

Drug-induced diabetes is widely reported inclinical conditions, and it is becoming a global issuebecause of its potential to increase the risk of severe cardiovascularcomplications. However, which drug mechanismsexert their diabetogenic effects and why the effectspresent significant inter-individual differences remain largelyunknown. Pharmacogenomics, which is the study ofhow genomic variation influences drug responses, providesan explanation for individual differences in drug-induceddiabetes. We highlight that pharmacogenomics can beinvolved in regulating the expression of genes in signalingpathways related to the pharmacokinetics or pharmacodynamicsof drugs or the pathogenesis of diabetes, contributingto the differences in drug-induced glucoseimpairment. The pharmacogenomics studies of the majordiabetogenic drugs are reviewed, including calcineurininhibitors, antipsychotics, hormones, and antihypertensivedrugs. We intend to elucidate the genetic basis of druginduceddiabetes and pave the way for the precise use ofthese drugs in the clinic.
