        Salivary exosomal microRNAs as biomarkers for head and neck cancer detection—a literature review

        Background: MicroRNAs (miRs) are small, non-coding mRNA molecules which regulate cellular processes in tumorigenesis. miRs were discovered in extracellular environment and biological fluids, carrying marks of head and neck squamous cellcarcinoma (HNSCC). They were also identified in abundance in salivary exosomes, in which they are protected by exosomelipid barrier against enzymatic injuries and therefore, the accuracy of exosomal miR-based cancer detection increase. Thissystematic review aimed to reveal and inventorize the most reliable exosomal miRNAs in saliva samples which can be usedas novel biomarkers for early detection of HNSCC. Materials and methods: A systematic literature search, according to PRISMA guideline, was performed on Pubmed andGoogle Academic libraries, based on specific keywords. Original articles published between 2010 and 2021 were selected. The quality of each paper was assessed using the Quality Evaluation Scoring Tool. Results: At the end of selection process, five studies met the inclusion criteria. These studies analyzed twelve salivaryexosomal miRs, presenting different methods of exosome and miR identification for HNSCC detection. A comprehensiveexplanation of the miR pathways of action was drawn and illustrated in this review. Conclusion: Exosomal miRs are promising biomarkers for oral cavity and oropharyngeal cancer detection. miR-10b-5p, miR486-5p, miR-24-3p and miR-200a stand as the most useful ones in saliva sample examination

        Influence of Electroacupuncture on Thermal Changes in a Soft Tissue Defect

        This study investigated thermal changes in the skin at locations where soft tissue defects existed and acupuncture needles stimulated by using bipolar electroacupuncture (EA) had been inserted. Under general anesthesia (GA), experimental defects were made at the dorsum site of five New Zealand rabbits. Bipolar EA was used for 20 minutes to stim- ulate the experimental defects, and the skin temperature was monitored at the sites where the acupuncture needles had been inserted and the soft tissue defects existed. The initial thermography of those defects had the same trend as that of the negative pole of EA stimulation at the first acupoint. Skin thermography during the first 3 minutes of bipolar EA indicated a centrifugal vasoconstriction and a vasodilatation at the negative and positive poles, respectively. After that, the thermal change in soft tissue undergoing EA stimulation was not modified by a different EA polarity. The local temperature at the defect and its surroundings under both positive and negative electric loads was increased by 0.2e0.3 ℃ for vasodilatation. This study indicates that EA influences sympathetic modulation of soft tissue defects and that selective sympathetic modulation caused by bipolar EA is responsible for the clinical perception.

        Paraclinical Investigations of Electroacupuncture Analgesia in a Rabbit Ovariohysterectomy

        The aim of this study is to investigate paraclinical changes in glycemia, serum cortisol, and adrenocorticotropic hormone (ACTH) due to electroacupuncture analgesia (EAA) in a rabbit undergoing an ovariohysterectomy. Seven clinically healthy New Zealand white rabbits were used in this study. Local and systemic analgesia was represented by EAA. Blood samples were collected at preoperative, operative, and postoperative times. At the operative time, increased cortisolemia does not promote a proportional increase in glycemia even though it is correlated with an increased level of ACTH. EAA follows the stress mechanism, which is coordinated by neuroendocrine activity. EAA in a rabbit ovariohysterectomy underscores the crucial involvement of the hypothalamus–pituitary–adrenal axis with a direct influence on paraclinical changes, including changes in glycemia, cortisol, and ACTH, which are specific for stress changes.

      • Bile Acids and Intestinal Dysbiosis in Alcoholic Hepatitis

        Aims: Alcoholic liver disease is associated with dysbiosis, impaired gut barrier and inflammation. Intestinal microbiota (IM) plays an important role in bile acids (BA) homeostasis and impact the gut barrier and promotes inflammation. The aim of our study was to study the structure of the IM and its function in BA homeostasis in alcoholic patients according to the severity of alcoholic liver disease. Methods: We included in a prospective study 4 groups of active alcoholic patients (N=97): two non-cirrhotic (nc) without or with alcoholic hepatitis (AH) (noAHnc, N=54 or AHnc N=14, respectively) and two cirrhotic groups without or with AH (noAHc, N=16 or sAHc, N=13, respectively). Serum and fecal BA profiles, as well as IM composition, using high-throughput 16s sequencing, were assessed. Results: In sAHc patients compared to noAHc patients, there was an increase in serum total BA, primary BA (total CA and total CDCA), conjugated BA and tauro-glycoconjugated ratio and a decrease in the UDCA/total BA and secondary/primary ratio. In feces, there was a decrease in total BA and an increase in secondary BA (total LCA and DCA). These 2 groups had a different IM structure. At the phyla level, there was an increase in Actinobacteria and a decrease in Bacteroidetes; 7 genera were increased and 4 were decreased. Moreover, in sAHc patients compared to noAHc patients, there was an increase in 4 and a decrease in 11 metabolic pathways (eg increase in glutathion and nucleotid metabolism, phosphotransferase system). In AHnc patients as compared to noAHnc, there was an increase in serum total conjugated BA. The IM of AHnc patients was characterized by an increase in Wolbachia and Dorea as compared to noAHnc. Conclusions: Disruption of BA homeostasis associated with alcoholic hepatitis is correlated to a specific IM signature that may lead to liver disease progression.

        Influence of Electroacupuncture on the Soft Tissue Healing Process

        The aim of this study was to determine the effect of bipolar electroacupuncture (EA) on a soft tissue defect in rabbits. Ten clinically healthy New Zealand white rabbits were divided into two groups: the control group (Group C, n = 5) and the experimental (EA) group (Group T, n = 5). During neuroleptanalgesia, defects of soft tissue (skin and mus- cle) were made at the dorsum site on the rabbits in both groups, and those defects were stimulated using EA. The biopsy samples were collected on Day 2, Day 4, and Day 6, pre- pared for histology, and examined microscopically. On the 2 nd day, in Group C, the inflam- matory degree was higher than it was in Group T; on subsequent days, low or identical degrees of inflammation were observed in both groups. Proliferative fibrous activity was increased on Day 4 for Group T and identical for both groups on Day 6. The dynamics of the epidermal thickness were characterized by a high rate on Day 2, Day 4, and Day 6 for Group T. EA facilitates a low tissue mechanical stress and has a positive effect on the healing of muscular defects. EA enhances the healing process, with no side effects.

        LI-RADS ancillary features on contrast-enhanced ultrasonography

        The Liver Imaging Reporting and Data System (LI-RADS) was created to standardize liver imaging in patients at high risk for hepatocellular carcinoma (HCC), and it uses a diagnostic algorithm to assign categories that reflect the relative probability of HCC, non-HCC malignancies, or benign focal liver lesions. In addition to major imaging features, ancillary features (AFs) are used by radiologists to refine the categorization of liver nodules. In the present document, we discuss and explain the application of AFs currently defined within the LI-RADS guidelines. We also explore possible additional AFs visible on contrast-enhanced ultrasonography (CEUS). Finally, we summarize the management of CEUS LI-RADS features, including the role of current and potential future AFs.

      • Different Intestinal Microbiota Profile in Alcoholic Pancreatitis as Compared to Alcoholic Hepatitis

        Aims: Chronic excessive alcohol consumption may cause alcoholic liver disease (ALD) or alcoholic pancreatitis (AP) in only a subset of patients. We have shown that individual susceptibility to ALD is substantially driven by intestinal microbiota (IM). However, factors related to tissue predilection (liver or pancreas) to alcohol toxicity are unknown. We aimed to characterize the IM profile in alcoholic patients according to the presence and the nature of the complication ie severe alcoholic hepatitis (sAH) or AP. Methods: Eighty-two alcoholic patients were included into 3 groups according to their complications: AP (N=24), sAH (N=13) and no complication despite a similar amount of alcohol consumption (alcoholic controls, N=45). IM was analyzed using high-throughput sequencing of the 16S Ribosomal RNA (16S RNA) gene. Results: Patients with AP had a reduced bacterial diversity (p=0.001) and a different global microbial composition as compared to alcoholic controls (p=0.001). 17 taxa at the genus level were different between the 2 groups; among them, 8 were increased in AP (Klebsiella, Enterococcus, Aquabacterium and Sphingomonas). When compared to sAH there was no difference in bacterial diversity between the 2 groups. However, 16 taxa were increased in sAH and 10 in AP. After adjusting for confounding factors (age, sex, BMI, alcohol intake, diabetes and proton-pump inhibitors) there was a marked increase in Haemophilus in sAH patients. Conclusions: Patients with AP have a specific dysbiosis as compared to alcoholic controls. Specific microbiome signatures are associated with AP and sAH.

        Quasi-Baer module hulls and applications

        <P><B>Abstract</B></P> <P>Let <I>V</I> be a module with S = End ( V ) . <I>V</I> is called a quasi-Baer module if for each ideal <I>J</I> of <I>S</I>, <SUB> r V </SUB> ( J ) = e V for some <SUP> e 2 </SUP> = e ∈ S . On the other hand, <I>V</I> is called a Rickart module if for each ϕ ∈ S , Ker ( ϕ ) = e V for some <SUP> e 2 </SUP> = e ∈ S . For a module <I>N</I>, the quasi-Baer module hull qB ( N ) (resp., the Rickart module hull Ric ( N ) ) of <I>N</I>, if it exists, is the smallest quasi-Baer (resp., Rickart) overmodule, in a fixed injective hull E ( N ) of <I>N</I>. In this paper, we initiate the study of quasi-Baer and Rickart module hulls. When a ring <I>R</I> is semiprime and ideal intrinsic over its center, it is shown that every finitely generated projective <I>R</I>-module has a quasi-Baer hull. Let <I>R</I> be a Dedekind domain with <I>F</I> its field of fractions and let { <SUB> K i </SUB> | i ∈ Λ } be any set of <I>R</I>-submodules of <SUB> F R </SUB> . For an <I>R</I>-module <SUB> M R </SUB> with <SUB> Ann R </SUB> ( M ) ≠ 0 , we show that <SUB> M R </SUB> ⊕ <SUB> ( <SUB> ⨁ i ∈ Λ </SUB> <SUB> K i </SUB> ) R </SUB> has a quasi-Baer module hull if and only if <SUB> M R </SUB> is semisimple. This quasi-Baer hull is explicitly described. An example such that <SUB> M R </SUB> ⊕ <SUB> ( <SUB> ⨁ i ∈ Λ </SUB> <SUB> K i </SUB> ) R </SUB> has no Rickart module hull is constructed. If <I>N</I> is a module over a Dedekind domain for which N / t ( N ) is projective and <SUB> Ann R </SUB> ( t ( N ) ) ≠ 0 , where t ( N ) is the torsion submodule of <I>N</I>, we show that the quasi-Baer hull qB ( N ) of <I>N</I> exists if and only if t ( N ) is semisimple. We prove that the Rickart module hull also exists for such modules <I>N</I>. Furthermore, we provide explicit constructions of qB ( N ) and Ric ( N ) and show that in this situation these two hulls coincide. Among applications, it is shown that if <I>N</I> is a finitely generated module over a Dedekind domain, then <I>N</I> is quasi-Baer if and only if <I>N</I> is Rickart if and only if <I>N</I> is Baer if and only if <I>N</I> is semisimple or torsion-free. For a direct sum <SUB> N R </SUB> of finitely generated modules, where <I>R</I> is a Dedekind domain, we show that <I>N</I> is quasi-Baer if and only if <I>N</I> is Rickart if and only if <I>N</I> is semisimple or torsion-free. Examples exhibiting differences between the notions of a Baer hull, a quasi-Baer hull, and a Rickart hull of a module are presented. Various explicit examples illustrating our results are constructed.</P>
