Treatment Outcome with Brachytherapy for Recurrent Nasopharyngeal Carcinoma

Background: To evaluate the treatment outcome and major late complications of all patients with recurrent nasopharyngeal carcinoma (NPC) treated with intracavitary brachytherapy (ICBT) in Hospital Kuala Lumpur. Materials and Methods: This retrospective study was conducted at the Department of Radiotherapy and Oncology, Hospital Kuala Lumpur, Malaysia. All patients with histologically confirmed recurrent NPC in the absence of distant metastasis treated in the period 1997-2010 were included in this study. These patients were treated with ICBT alone or in combination with external beam radiotherapy (EBRT). Treatment outcomes measured were local recurrence free survival (LRFS), disease free survival (DFS) and overall survival (OS). Results: Thirty three patients were eligible for this study. The median age at recurrence was 56 years with a median time to initial local recurrence of 27 months. Majority of patients were staged as rT1-2 (94%) or rN0 (82%). The proportion of patients categorised as stage III-IV at first local recurrence was only 9%. Twenty one patients received a combination of ICBT and external beam radiotherapy while 12 patients were treated with ICBT alone. Median interval of recurrence post re-irradiation was 32 months (range: 4-110 months). The median LRFS, DFS and OS were 30 months, 29 months and 36 months respectively. The 5 year LRFS, DFS and OS were 44.7%, 38.8% and 28.1% respectively. The N stage at recurrence was found to be a significant prognostic factor for LRFS and DFS after multivariate analysis. Major late complications occurred in 34.9% of our patients. Conclusions: Our study shows ICBT was associated with a reasonable long term outcome in salvaging recurrent NPC although major complications remained a significant problem. The N stage at recurrence was a significant prognostic factor for both LRFS and DFS.
Shared Single Display Application : An Interactive Patterns Approach

Interactive patterns are able to promote the notion of interaction in groupware application development. Hence, these patterns improve the students’ engagement and promote problem solving collectively. This paper reports on the usability of the interactive patterns in designing and developing a collaborative language application. With interactive patterns, we are able to design and develop two different interactive shared single display education applications. RimbaIlmu is an after school programme to allow students to learn Bahasa Malaysia during their free time. RICLAT is an authoring tool for collaborative presentations. The patterns are useful to enrich the interaction among both education applications. Also, it facilitates more effective communication among the group members and enriches the interaction for students based on the qualitative analysis.

The objective of present work is to use numerical simulation to investigate the complex three-dimensional and secondary flow structures developed at the inlet and impeller in a centrifugal pump at design and off-design points. The pump impeller is shrouded with 6 backward swept blades and with a specific speed of 0.8574. The characteristic of the pump is measured experimentally with straight and curved intake sections. Numerical computation is carried out to investigate the pump inlet flow structures and subsequently the flow field within the centrifugal pump. The numerical results showed that strong interaction between the impeller eye and intake section. Secondary flow structure occurs upstream at the pump inlet has great influence on the pump performance and flow structure within the impeller.
Unsteady Analysis of Impeller-Volute Interaction in Centrifugal Pump

An unsteady numerical analysis has been carried out to study the strong impeller volute interaction of a centrifugal pump with six backward swept blades shrouded impeller. The numerical analysis is done by solving the three-dimensional Reynolds Averaged Navier-Stokes codes with standard k-${\varepsilon}$ two-equations turbulence model and wall regions are modeled with a scalable log-law wall function. The flow within the impeller passage is very smooth and following the curvature of the blade in stream-wise direction. However, the analysis shows that there is a recirculation zone near the leading edge even at design point. When the flow is discharged into volute casing circumferentially from the impeller outlet, the high velocity flow is severely distorted and formed a spiraling vortex flow within the volute casing. A spatial and temporal wake flow core development is captured dynamically and shows how the wake core diffuses. Near volute tongue region, the impeller/volute tongue strong interaction is observed based on the periodically fluctuating pressure at outlet. The results of existing analysis also proved that the pressure fluctuation periodically is due to the position of impeller blade relative to tongue.
Colorectal Carcinoma in Malaysians: DNA Mismatch Repair Pattern in a Multiethnic Population

Background: The interesting preponderance of Chinese with colorectal carcinoma (CRC) amongst the three major ethnic groups in Malaysia prompted a study to determine DNA mismatch repair (MMR) status in our CRC and attempt correlation with patient age, gender and ethnicity as well as location, grade, histological type and stage of tumour. Histologically re-confirmed CRC, diagnosed between $1^{st} $January 2005 and $31^{st}$ December 2007 at the Department of Pathology, University of Malaya Medical Centre, were immunohistochemically stained with monoclonal antibodies to MMR proteins, MLH1, MSH2, MSH6 and PMS2 on the Ventana Benchmark XT autostainer. Of the 142 CRC cases entered into the study, there were 82 males and 60 females (M:F=1.4:1). Ethnically, 81 (57.0%) were Chinese, 32 (22.5%) Malays and 29 (20.4%) Indians. The patient ages ranged between 15-87 years (mean=62.4 years) with 21 cases <50-years and 121 ${\geq}50$-years of age. 14 (9.9%) CRC showed deficient MMR (dMMR). Concurrent loss of MLH1 and PMS2 occurred in 10, MSH2 and MSH6 in 2 with isolated loss of MSH6 in 1 and PMS2 in 1. dMMR was noted less frequently amongst the Chinese (6.2%) in comparison with their combined Malay and Indian counterparts (14.8%), and was associated with right sided and poorly differentiated tumours (p<0.05). 3 of the 5 (60.0%) dMMR CRC cases amongst the Chinese and 1 of 9 cases (11.1%) amongst the combined Malay and Indian group were <50-years of age. No significant association of dMMR was noted with patient age and gender, tumour stage or mucinous type.
Exploring Factors Influencing Smoking Behaviour in Malaysia

Objective: The objective of present study is to investigate the determinants of smoking behaviour among adults in Malaysia. Method: Findings of the Third National Health and Morbidity Survey (NHMS-3) by the Ministry of Health, Malaysia, were used. The sample consisted of 34,539 observations. A logistic regression model was thus applied to estimate the probability to participate in smoking. Results: Age, income, gender, marital status, ethnicity, employment status, residential area, education, lifestyle and health status were statistically significant in affecting the likelihood of smoking. Specifically, youngsters, low income earners, males, unmarried individuals, Malays, employed individuals, rural residents and primary educated individuals were more likely to smoke. Conclusion: In conclusion, socio-demographic, lifestyle and health factors have significant impacts on smoking participation in Malaysia. Based on these empirical findings, several policy implications are suggested.
