      • 제4형 가족성 고중성지방혈증 임산부에서 발생한 급성 췌장염 1례

        Plasma levels of cholesterol (TC) and triglyceride(TG) physiologically increase during pregnancy. The lipid increment is 23-53% above the pregravidic level for TC and 2- to 3-fold of the prepregnancy level for TG. If the TC and TG are higher than normal values in pregnancy, the patient must be carefully monitored. Acute pancreatitis is the main consequence of hyperlipidemia and occurs either during pregnancy, in the third trimester, or in the puerperium. Mortality is high both for the mother (21%) and the fetus (20%). We report a case of 28 year-old pregnant woman at 29 weeks gestation with hypercholesterolemia (TC = 357 ㎎/dl) and severe hypertriglyceridemia (TG = 1300 ㎎/dl). The patient was admitted to the hospital because of severe epigastric pain, nausea and vomiting. Total serum cholesterol was increased and trigyleride was markedly increased. The electroporesis pattern of serum lipoprotein showed increase in pre-beta lipoprotein fraction, suggesting IV hypertriglyceridemia pattern. According to a review of the literature, the incidence of pancreatitis during pregnancy is 1 in 1000 to 3000 pregnancies. Severe hypertriglyceridemia in pregnancy should be treated with a careful restriction of calories and fat: for preventing acute pancreatitis, hospitalization for intravenous fluid therapy and plasma exchange must be required.

      • 근원세포 분화조절 유전자의 클로닝

        배아의 근원세포는 분화가 진행 됨에 따라 성체의 골격근 세포로 된다. 이러한 분화단계에는 특정 유전자가 관여할 것으로 추정되며 이전 실험에서 0.8Kb의 cDNA를 확인하였다 (Kang et al., 1992). 이러한 특이 유전자는 계배 부화 9일째부터 발현이 되었으며 성체에서는 발현이 되지 않았다. 특히 12일과 14일 계배의 뇌,간,신장조직에서는 유전자 발현이 되지 알았으며 단지 가슴근육조직에서만 유전자 발현이 되었다. 이 유전자는 계배 골격근 세포의 분화에서 중요한 역할을 할 것으로 추정된다. Embryonic myoblasts become determined to differentiate into distinctive types of muscle cells. Skeletal muscle cell is one of them. We studied the expression of muscle specific 0.8Kb cDNA related to the differentiation of chick embryonic myoblast in various stage of development and in organs. The gene was expressed from 9 day old chick embryo stage, but was not expressed at adult breast muscle. Also the gene was not expressed in brain, liber, kidney but expressed in 12 and 14 day old chick embryonic breast muscle. Therefore, this muscle specific gene(0.8Kb) may have an important role in regulating the diffrentiation of chick embryonic myoblast.

      • 도장작업시 유해인자들의 노출수준과 작업환경관리실태

        Objectives: For the purpose of preparing the fundamental data and assessing the status of exposure level of chemical substances in painting process. Methods and Materials: The author determined the concentration of organic solvents, lead(Pb), Chromium(Cr), and Toluene-2, 4-diisocyanate(TDI) in air of painting process to 90 industries classified by 15 types of manufacturing industry during a period from Jan. 1st 2001 to Dec. 31st 2001. Results: The results were as follows: 1. The geometric mean level of each chemical substance was 0.1706 ppm for exposure index in organic solvents, 0.0019㎎/㎥ for Pb, 0.0024 ㎎/㎥ for Cr, and 0.0004ppm for TDI, respectively. 2. The exposure level of each chemical substances by type of painting was the highest in dipping and brush painting for organic solvent, in spray-painting for TDI, and in spray gun-painting for Pb and Cr, respectively. 3. The exposure level of each chemical substances by type of local ventilation equipment was higher in enclosed and booth type for organic solvents. Cr, and TDI, and in exterior type for Pb, respectively than those of other types. 4. The exposure level of each chemical substances by type of hazardous material arresting media was higher in using water for organic substances, in using bag filter for Pb, TDI, respectively than those of other types. Conclusion: According to above results, this study suggests that the exposure level by type of industry exceeded the exposure level in wooden furniture and metal furniture manufacturing industry for organic solvents and Pb, respectively.

      • 도금사업장의 작업환경의 유해인자 노출수준에 관한 연구

        Objective : For the purpose of preparing the fundamental data and assessing the status of exposure level of chemical substances and physical agents. Methods and Material : Such as noise, two types of dust, four types of metals, seventeen types of organic solvents, and four types of other materials classified by specific chemical substances in 21 metal product plating factories classified by 28 processes. The author determined exposure level on those hazardous materials from Jan. to Dec. 2001. Results : The results were as follows: 1. The exposure level for noise was exceeded to Threshold Limit Value(TLV) in the process of 6 cases of Machining, 5 and 4 cases of pre and post-buffing, and 6 cases of waterdrop-removing, the mean noise level of all of processes was not exceeded to compliance level. 2. The exposure level for dust was not exceeded to TLV at all. 3. The exposure level for Chromium(Cr) was exceeded to TLV in Chromium plating process, metal exposure level including Zinc(Zn), Manganese(Mn), and Lead(Pb) was not exceeded to TLV. 4. The exposure level of toluene in case of organic solvents was exceeded to TLV in 1 case of finishing/cleaning, coating/developing process, respectively. 5. The exposure level of four types of other materials classified by specific chemical substances were not exceeded to TLV. 6. The exposure level for dust and metal including Zn, Mn, and Pb was decreased in comparison with two results of air monitoring performed. There was no difference of the exposure level for Cr, noise, and four types of other materials classified by specific chemical substances, and the exposure level for organic solvents was slightly increased. 7. The exposure level for noise in factories employing more than 21 workers was higher than that for noise in foadtories employing less than 20 workers. The more workers, the lower exposure level in metal including Zn, Mn. The exposure level for Cr was increasing in factories employing more than 51 workers, and the more workers the lower exposure level in organic solvents. Conclusion : According to above results, this study suggests positive work environment managements in processes exceeding exposure limit value as well as continuously consecutive work environment management in processes not exceeding exposure limit value and revealing no chronological difference of exposure level should be needed.

        슬괵건을 이용한 후방 십자 인대 재건술시 횡고정 핀을 이용한 대퇴 터널 고정술

        목적 : 슬괵건을 이용한 후방 십자 인대 재건술 시행시 대퇴 터널을 횡고정 핀으로 고장하는 술식을 시행하고 그 결과를 보고자 한다. 대상 및 방법 : 2001년 9월부터 2002년 3월까지 동반손상이 없는 grade Ⅲ 이상의 후방 십자 인대 손상으로 네 겹의 슬괵건을 이용한 후방 십자 인대 재건술을 받고 1년이상 추시가 가능하였던 8예를 대상으로 하였다. 전 예에서 대퇴 터널을 횡고정 핀으로 고정하였으며 경골 터널은 Intrafix(Mitek_(?), USA)를 이용하여 고정하였다. 기능 평가는 Lysholm 점수, International Knee Documentation Committee(IKDC) 평가 기준과 KT-2000 관절계를 이용한 최대 도수 부하 검사를 이용하였다. 결과 : Lysholm 점수가 술전 평균 54점에서 술후 1년 추시상 89점으로 향상되었다. IKDC 평가 기준상 정상이 2예, 거의 정상이 5예였으며 비정상이 1예 있었다. KT-2000 관절계를 이용한 최대 도수 부하 검사상 술전 평균 12.2 ㎜에서 3.7 ㎜로 호전되었다. 결론 : 슬괵건을 이용한 후방십자인대 재건술시 횡고정 핀을 이용하는 고정방법은 좋은 술식으로 사료된다. Purpose: The purpose of this study is to introduce new fixation technique for posterior cruciate ligament (PCL) reconstruction using quadruple hamstring tendon autograft. Materials & Materials : From September 2001 to March 2002, eight patients received PCL reconstruction using quadruple hamstring autograft for PCL injury without associated procedures for posterolateral complex injures. All femoral tunnels were fixed with cross pins and all tibial tunnels were fixed with Intrafix(Mitek_(?), USA). Clinical assessments consited of Lysholm knee scores, International Knee Documentation Committee(IKDC) evaluation form and manual maximal side to side difference using KT-2000 arthrometer. Result : The average Lysholm knee score improved from 54 preoperative to 89 postoperatively. At the final IKDC evaluation, 2 cases were graded as normal, 5 nearly normal, 1 abnormal. Mean side to side difference of manual maximum posterior displacement with knee in 70° flexion using the KT-2000 arthrometer was 3.7 ㎜. Conclusion : In PCL reconstruction using quadruple hamstring autograft, cross pins are good fixation method with high strength and stiffness.

        Cross Pins versus Endobutton Femoral Fixation in Hamstring Anterior Cruciate Ligament Reconstruction; Minimum 4-Year Follow-Up

        Purpose: We aimed to compare cross-pin fixation and Endobutton femoral fixation for hamstring anterior cruciate ligament (ACL) reconstruction with respect to clinical and radiographic results, including tunnel widening and the progression of knee osteoarthritis (OA). Materials and Methods: Between August 2002 and August 2005, 126 autogenous hamstring ACL reconstructions were performed using either cross pins or Endobutton for femoral fixation. Fifty-six of 75 patients in the cross-pin group and 35 of 51 patients in the Endobutton group were followed up for a minimum of 4 years. We compared the clinical and radiological results between the groups using the International Knee Documentation Committee (IKDC) evaluation form, the KT-2000 arthrometer side to side difference, the amount of tunnel widening and the advancement of OA on radiographs. Results: There were no significant differences in the IKDC grades between the groups at the 4 year follow-up. There was no significant difference in the side to side difference according to KT-2000 arthrometer testing. Also, there were no significant differences in terms of tunnel widening or advancement of OA on radiographs. Conclusions: Endobutton femoral fixation showed good results that were comparable to those of cross pins fixation in hamstring ACL reconstruction.

        Inhibitory Effect of Insulin Treatment on Apoptosis of Intervertebral Disc Cells in a Streptozotocin-Induced Diabetic Rat Model

        Study Design: Experimental study using a streptozotocin (STZ)-induced diabetic rat model. Purpose: This study aims to investigate whether insulin treatment could attenuate disc cell apoptosis and matrix degradation in a STZ-induced diabetic rat model. Overview of Literature: Diabetes is a significant risk factor for disc degeneration due to excessive apoptosis of disc cells and matrix degradation. However, no studies were noted to demonstrate the inhibitory effect of insulin treatment on the apoptosis of disc cells and matrix degradation in diabetic patients. Methods: Rats were allocated randomly into one of three groups: control, STZ, and STZ-insulin. Diabetes was induced by a single intraperitoneal injection of STZ (65 mg/kg) in the STZ and STZ-insulin groups. The blood glucose level was consistently above 400 mg/dL in the STZ and STZ-insulin groups 2 weeks after STZ injection. After 2 weeks of STZ injection, the STZ-insulin group was administered insulin treatment (1.5 unit/100 g) daily for up to 4 weeks. Blood glucose of the STZ-insulin rats significantly decreased to normal levels 4 weeks after insulin treatment. The rats were sacrificed 6 weeks after STZ injection, and disc cells and tissues were harvested to investigate the expression of apoptosis markers and matrix metalloproteinases (MMPs). Results: Fas and caspase-8, -9, and -3 expressions were significantly increased in the STZ group, along with increased expressions of MMP-2 and -3. On the contrary, insulin treatment significantly decreased the expressions of Fas, caspase-8, -9, and -3 as well as MMP-2 and -3 in the STZ-insulin group. Conclusions: The results of the current study demonstrated that insulin treatment attenuates excessive apoptosis of disc cells and matrix degradation in the diabetic rat model. Accordingly, strict blood glucose control should be recommended to prevent disc degeneration in diabetic patients.

        Transition Metal Induces Apoptosis in MC3T3E1 Osteoblast Evidence of Free Radical Release

        <P> Transition metal ions including Se<SUP>2⁢</SUP>, Cd<SUP>2⁢</SUP>, Hg<SUP>2⁢</SUP> or Mn<SUP>2⁢</SUP> have been thought to disturb the bone metabolism directly. However, the mechanism for the bone lesion is unknown. In this study, we demonstrated that MC3T3E1 osteoblasts, exposed to various transition metal ions; selenium, cadmium, mercury or manganese, generated massive amounts of reactive oxygen species (ROS). The released ROS were completely quenched by free radical scavengers-N-acetyl cysteine (NAC), reduced glutathione (GSH), or superoxide dismutase (SOD). First, we have observed that selenium (10μM), cadmium (100μM), mercury (100μM) or manganese (1 mM) treatment induced apoptotic phenomena like DNA fragmentation, chromatin condensation and caspase-3-like cysteine protease activation in MC3T3E1 osteoblasts. Concomitant treatment of antioxidant; N-acetyl-L-cysteine (NAC), reduced-form glutathione (GSH), or superoxide dismutase (SOD), prevented apoptosis induced by each of the transition metal ions. Catalase or dimethylsulfoxide (DMSO) has less potent inhibitory effect on the apoptosis, compared with NAC, GSH or SOD. In line with the results, nitroblue tetrazolium (NBT) stain shows that each of the transition metals is a potent source of free radicals in MC3T3E1 osteoblast. Our data show that oxidative damage is associated with the induction of apoptosis in MC3T3E1 osteoblasts following Se<SUP>2⁢</SUP>, Cd<SUP>2⁢</SUP>, Hg<SUP>2⁢</SUP> or Mn<SUP>2⁢</SUP> treatment.

        Effect of Cytokines and bFGF on the Osteoclast Differentiation Induced by 1α,25-(OH)<SUB>2</SUB>D<SUB>3</SUB> in Primary Murine Bone Marrow Cultures

        <P> Bone is a complex tissue in which resorption and formation continue throughout life. The bone tissue contains various types of cells, of which the bone forming osteoblasts and bone resorbing osteoclasts are mainly responsible for bone remodeling. Periodontal disease represents example of abnormal bone remodeling. Osteoclasts are multinucleated cells present only in bone. It is believed that osteoclast progenitors are hematopoietic origin, and they are recruited from hematopoietic tissues such as bone marrow and circulating blood to bone. Cells present in the osteoclast microenvironment include marrow stromal cells, osteoblasts, macrophages, T-lymphocytes, and marrow cells. These cells produce cytokines that can affect osteoclast formation. In vitro model systems using bone marrow cultures have demonstrated that IL-1β, IL-3, TNF-α, bFGF can stimulate the formation of osteoclasts. In contrast, IL-4 inhibits osteoclast formation. Knowledge of cytokines and bFGF that affect osteoclast formation and their capacity to modulate the bone-resorbing process should provide critical insights into normal calcium homeostasis and disorders of bone turnover such as periodontal disease, osteoporosis and Paget s disease.
