        Acute Dislocation of the Metacarpal-Trapezoid Joint

        The trapezoid metacarpal dislocation is a rare event. In the literature, it is found in case reports. This injury is caused by direct or indirect high energy trauma. In most cases, the dislocation is dorsal and is difficult to reproduce because the joint is not very mobile. Given the low incidence and little evidence supported in the literature regarding the management, this injury can be treated by open or closed reduction; however, it has been published that most authors use Kirschner wire fixation with good results. Here we present our experience in the management of a male patient with acute trapezoid metacarpal dislocation handled with a splint with good functional results at 6 weeks.

      • Negotiating Resistance: Women, Peace and Leadership in Dayah in Post-conflict Aceh

        There is a current gap in knowledge on the important role of women grassroots actors in processes of peace building, and more specifically the possibilities and limits to women`s agency in supporting peaceful societies through Islamic education systems. Amidst the masculine aspects and maledominated sphere of post-conflict development, the agency of women educational leaders actually serves as potential agents of societal change (or of reproduction) in processes of peacebuilding and social justice. The paper explores the context of education, religion in dayah (Islamic traditional school), and its surrounding structures, with a close link to what extent and how an Islamic perception of women`s role in fostering peace, understanding and social justice is conceptualized and enacted. Adding a gender-specific lens to explore the contextual understanding of agency through the strategic relational approach and cultural political economy (Jessop, 2005), the key aim of the paper is to understand various ways in which female educators negotiate and navigate largely restricted patriarchal and religiously orthodox spaces (Mahmood, 2005; 2011). Committed to representing the often silenced and marginalized voice of female leaders, educators and grassroots actors, the paper explores their views and experiences in consciously or unconsciously transforming or reproducing existing inequalities and potential conflict-triggers in the cultural political economy context of contemporary Aceh.

        Wild birds and urban pigeons as reservoirs for diarrheagenic Escherichia coli with zoonotic potential

        In order to describe the role of wild birds and pigeons in thetransmission of shiga toxigenic Escherichia coli (STEC) andenteropathogenic Escherichia coli (EPEC) to humans and otheranimals, samples were collected from cloacae and oropharynxof free-living wild birds and free-living pigeons. Two STEC(0.8%) and five EPEC strains (2.0%) were isolated from wildbirds and four EPEC strains (2.0%) were recovered from pigeons. Serogroups, sequence types (STs) and virulence genes,such as saa, iha, lpfAO113, ehxA, espA, nleB and nleE, detectedin this study had already been implicated in human and animaldiseases. Multidrug resistance (MDR) was found in 25.0%of the pigeon strains and in 57.0% of the wild bird strains;the wild birds also yielded one isolate carrying extended-spectrumβ-lactamases (ESBLs) gene blaCTX-M-8. The high variabilityshown by PFGE demonstrates that there are no prevalentE. coli clones from these avian hosts. Wild birds and pigeonscould act as carriers of multidrug-resistant STEC andEPEC and therefore may constitute a considerable hazard tohuman and animal health by transmission of these strainsto the environment.

        페루의 동: 현황과 미래의 도전

        페루의 동 광화작용은 주로 반암 동 광상과 밀접하게 관련되며, 팔레오세, 에오세-올리고세, 마이오세 동 광화대로 세분된다. 현재까지 반암 동 광화대로부터 생산된 동은 총 2,800만 톤에 이르며, 또께빨라(Toquepala), 꾸아호네(Cuajone), 쎄로베르데(Cerro Verde) 광상들을 포함하고 있는 팔레오세 광화대로부터 생산된 동은 페루 총 동 생산량의 57%를 차지한다. 그러나 최근 들어 페루 중남부(에오세-올리고세)와 북서부(마이오세) 지역으로 탐사활동의 집중은 연간 동 생산량이 20만 톤 이상인 라 그란하(La Granja), 엘 갈레노(El Galeno), 라스 밤바스(Las Bambas), 또로모초(Toromocho), 리오 블랑꼬(Rio Blanco) 등의 발견을 가져왔다. 이들 광상들을 제외하고, 총 13개의 신규 동 광상들이 팔레오세, 에오세-올리고세, 마이오세 동 광화대로부터 발견되었다. 따라서 페루는 2014년부터 이들 광상들로부터 순차적으로 생산을 시작할 계획이며, 2012년 기준 연간 동 생산량 143만 톤을 2019년에 490만 톤 규모로 증가시킬 계획이다. 또한 이러한 신규광상의 발견으로 인하여, 앞으로 페루 광물자원탐사는 팔레오세의 동 광화대에서 에오세-올리고세와 마이오세 동 광화대로 이동될 것으로 예상된다. The copper mineralization in Peru is intimately associated with porphyry Cu deposits and subdivides into three porphyry Cu belt as Paleocene, Eocene-Oligocene, and Miocene. Up to now, the total copper production from them reach 28 Mt Cu. The total copper production from the Paleocene Cu belt, including Toquepala, Cuajone, and Cerro Verde, accounts for approximately 57% of total copper production from Peru. But focusing mineral exploration on middle southern (Eocene-Oligocene) and northwestern part (Miocene) of Peru results in new discoveries, including La Granja, El Galeno, Las Bambas, Toromocho, and Rio Blanco, which have an estimated annual production more than 200,000 t Cu. In addition to them, thirteen Cu deposits are discovered from the Paleocene, Eocene-Oligocene, and Miocene Cu belts. Thus, Peru is supposed to produce Cu production from 2014 and increases annual production from 143 Mt Cu in 2012 to 490 Mt Cu in 2019. Due to new discoveries, it is expected that mineral exploration activities in Peru are likely to move from Paleocene Cu belt to Eocene-Oligocene and Miocene Cu belts.

        Incidence and risk factors for the development of cerebral metastasis in cervical cancer patients

        Objective: Cerebral metastasis (CM) in cervical cancer (CC) cases, although rare, results in high lethality rates. The present study aimed to assess CM incidence in a Brazilian reference CC center and evaluate the risk factors for CM development. Retrospective observational study of patients diagnosed with CC between 2010 and 2017. Methods: Cumulative CM incidence and incidence density were evaluated. Characteristics associated to CM development risks were identified using crude (cOR) or adjusted (aOR) odds ratios. Results: A total of 3,397 patients were included in this study. Patient age ranged from 18 to 101 years, with a mean age of 48.8±14.0. After a mean follow-up time of 3.2±2.1 years, 51 CM cases were identified, resulting in a cumulative incidence of 1.5% (95% confidence intervals [CI]=1.12–1.97) and an incidence density at the end of the 6th year of 27.4 per 1,000 women/ year. Advanced clinical stage (aOR=3.15; 95% CI=1.16–8.58; p=0.025), the presence of previous lung metastasis (aOR=4.04; 95% CI=1.82–8.94; p=0.001) and the adenocarcinoma (aOR=2.90; 95% CI=1.46–5.76; p=0.002), adenosquamous carcinoma (aOR=7.33; 95%CI=2.87–18.73; p<0.001), undifferentiated carcinoma (aOR=14.37; 95% CI=3.77–54.76;p<0.001) and neuroendocrine carcinoma (aOR=21.31; 95% CI=6.65–68.37, p<0.001) histological types were associated with a higher risk for CM development. CM risk was higher in the first years of follow-up, with no cases observed after the 6th year. Conclusion: CC patients in advanced clinical stages, displaying previous lung metastasis and non-squamous histological types are at high risk of developing CM.

        Effect of acute intradialytic strength physical exercise on oxidative stress and infiammatory responses in hemodialysis patients

        Background: Oxidative stress and in.ammation are common .ndings in chronic kidney disease (CKD) patients, and they are directly related to the increased risk of developing cardiovascular disease, which is the major cause of death in these patients, particularly for those undergoing hemodialysis (HD). Strength physical exercise is a new therapeutic approach to reduce these complications in CKD patients. Following this, the purpose of this study was to assess the effect of acute intradialytic strength physical exercise on oxidative stress and in.ammatory responses in HD patients. Methods: Sixteen HD patients were studied (11 women; 44.4714.6 years; body mass index 23.374.9 kg/m2; 61.6743.1 months of dialysis) and served as their own controls. Acute (single session) intradialytic physical exercise were performed at 60% of the onerepetition maximum test for three sets of 10 repetitions for four exercise categories in both lower limbs during 30 minutes. Blood samples were collected on two different days at exactly the same time (30 minutes and 60 minutes after initiating the dialysis-with and without exercise). Antioxidant enzymes activity [superoxide dismutase (SOD), catalase, and glutathione peroxidase], lipid peroxidation marker levels (malondialdehyde), and in.ammatory marker levels (high-sensitivity C-reactive protein) were determined. Results: SOD plasma levels were signi.cantly reduced after acute physical exercise from 244.8740.7 U/mL to 222.4728.9 U/mL (P¼0.03) and, by contrast, increased on the day without exercise (218.2726.5 U/mL to 239.4738.6 U/mL, P¼0.02). There was no alteration in plasma catalase, glutathione peroxidase, malondialdehyde, or highsensitivity C-reactive protein levels in on either day (with or without exercise). Additionally, there was no association between these markers and clinical, anthropometric, or biochemical parameters. Conclusion: These data suggest that acute intradialytic strength physical exercise was unable to reduce oxidative stress and in.ammation, and in addition, it seems to reduce plasma SOD levels, which could exacerbate the oxidative stress in HD patients.         Influence of Adding Recovered Protein from Processing Wastewater on the Quality of Mechanically Separated Chicken Meat Surimi Like-Material

        Functional and nutritional soluble proteins can be recovered from surimi (and surimi-like material) processing wastewater, reducing environmental problems and the cost of an irresponsible waste disposal. Recovered proteins may be added back at a low percentage to surimi products. The aim of this work was to evaluate the effect of the addition of soluble recovered proteins (RP), obtained from mechanically separated chicken meat surimi-like material (MSCM-SLM) processing wastewater by acidic pH-shifting, on the composition and texture of RP-MSCM-SLM, with RP contents of 0, 10, 20 and 30% (w/w) in the mixture. For that, proximate composition and gel properties were evaluated. The fat content of the MSCMSLM was significantly reduced to 11.98% and protein increased to 83.64% (dry basis) after three washing cycles. The addition of 30% RP in the MSCM-SLM significantly augmented the protein content to 93.45% and reduced fat content from to 2.78%. On the other hand, the addition of RP was responsible for a drastic decrease in texture parameters, reaching 252.36 g, 185.23 g.cm, and 6.97 N for breaking force, gel strength and cutting strength, respectively, when 30% of RP was included in the MSCM-SLM. It was concluded that the obtained intermediary product (RP-MSCM-SLM) is a good option to applications in processed meat products where high texture parameters are dispensable, e.g., emulsified inlaid frankfurter-type sausages, but high protein content and low fat content desired.

        Metabolic and Hemodynamic Results and Early Complications in Simultaneous Bilateral versus Unilateral Hip Arthroscopy

        Background: To compare the hemodynamic parameters—electrolyte concentration, D-dimer level, creatine phosphokinase level—and the incidence of early complications of simultaneous bilateral versus unilateral hip arthroscopy. Methods: A prospective study was conducted on patients (> 18 years of age) undergoing unilateral or bilateral hip arthroscopy under the same anesthetic between 2013 and 2015. Patients were followed up for 30 days after surgery. In all cases, data were collected before, during, and after the surgical procedure. Results: One hundred cases of hip arthroscopy (51 unilateral and 49 bilateral) were included in this study. There was a greater variation in systolic blood pressure and heart rate in the unilateral group. The sodium levels were higher in the bilateral group with an adjusted mean difference of 5.31 mmol/L (p < 0.001). During the first 24 hours after the procedure, the proportion of patients with an altered D-dimer of > 500 ng/mL was 85.7% in the bilateral group and 56.9% in the unilateral group. There was no significant difference in the incidence of complications between the groups (bilateral, 8.2%; unilateral, 9.8%; crude odds ratio, 0.83; 95% confidence interval, 0.24 to 2.92; p = 0.526). Conclusions: The variations of hemodynamic parameters in patients undergoing hip arthroscopy remained within normal ranges. The findings of this study suggest that bilateral hip arthroscopy be selected according to the patient’s condition, considering that the risk of complications and metabolic alterations in bilateral hip arthroplasty are similar to those in unilateral arthroscopy.

        페루의 금: 현황과 미래의 도전

        페루의 금 자원량은 1억 9200만 온스에 달한다. 페루의 금 생산량은 2013년 현재 490만 온스로 라틴아메리카 지역 1위, 전 세계 6위에 해당한다. 페루는 현재까지 금 생산량이 1억 1800만 온스이며 이 중에 84%가 마이오세 천열수 광화대, 석탄기-페름기의 조산대, 백악기 관입암 광화대에서 생산되었다. 대부분의 금 생산지는 페루의 북쪽 지방에 몰려있으며 이 북쪽지방은 자국 내에 총 생산량의 63.5%를 차지한다. 2013년의 경우 야나코챠가 백만 온스로 자국 내에 금 생산량이 가장 많으며 알또치까마가 60.6만 온스로 그 뒤를 이었다. 페루의 금 생산량은 2017년에 650만 온스에 달할 것으로 전망한다. 이를 가능케 하기 위해서는 현재 시행중인 14개 프로젝트가 향후 3년 내에 생산 단계로 개발되어야 한다. Total reported gold resources in Peru is about 192 Moz. Gold production in Peru was 4.9 Moz in 2013, which was ranked first in Latin America and sixth in the world. Historic cumulative gold production in Peru is 118 Moz, and production from main gold belts including Miocene epithermal belt, Carboniferous-Permian orogenic gold belt and Upper Cretaceous intrusion-related gold belt corresponds to 84%. Most of production areas are located in Northern part of Peru, which corresponds to 63.5% of the total domestic production. Annual production onces in Yanacocha mine and Alto Chicama were 1 Moz and 0.606 Moz in 2013, which were ranked first and second in Peru, respectively. Gold production in Peru is expected to be 6.5 Moz in 2017. To accomplish the expected production, ongoing 14 projects should be developed to the production stage in three years.

        Macroscopic;Histological;and Microbiological Characterization of Contact Lesions at the Tibiotarsal Region of Broilers

        Brazil is considered as a great broiler feet exporter;especially for the Chinese trade. Contact lesions at the tibiotarsal region are responsible for economic losses and there is no model for its classification;thereby this study presents a fast and practical grade system to be used in the poultry industry and proposes these lesion characterizations into three different grades. For this;correlation was made between macroscopic;histological findings and microbiological quantification (Escherichia coli;Staphylococcus spp.;Streptococcus spp. and sulphite-reducing clostridia) from contact lesions in the tibiotarsal region of 112 broiler carcasses;divided in four groups (n=28);accordingly to the lesion’s intensity. There were no significant differences in microbiological quantification among the groups (p>0.05) except for the grade 3 group;as grade 1 and 2 lesions were in the early stages and histopathological changes such as ulceration were not observed. In grade 3 lesion group;it was observed bacterial cocci grume and ulceration at the articular region and significantly higher microbiological count (p<0.05) for E. coli and Staphylococcus spp. In conclusion;the visual standard proposed in this work;correlated and confirmed by the histopathologic;and microbiologic characterization;allows to precise and fast ascertainment of the contact lesion grade in the tibiotarsal regions of broiler carcasses. Moreover;it should be highlighted that grades 1 and 2 alterations are not caused by an inflammatory process caused by pathogenic agents and should not be considered a public health risk.
