A 40-year-old male presented an episode of severe thunderclap headache with right hemiparesis. Imaging revealed severe stenosis in the left middle cerebral artery, associated with increased mean velocities detected by Transcranial Doppler Ultrasound (TCD). Notably, a distinctive "tin whistle" sound, known as a doppler harmonic effect, near the highest velocity point was observed during the examination. Extensive workup was performed, including CSF analysis and laboratory investigations, and the diagnosis of Reversible Cerebral Vasoconstriction Syndrome (RCVS) was established. Treatment involving medication and daily monitoring led to a decline in the left MCA's velocity, enabling the patient's discharge with no recurrent headaches or neurological deficits. Notably, a follow-up MRI exhibited the disappearance of the left MCA stenosis. This report discusses the rare occurrence of "musical murmurs" in cerebrovascular diseases, underscoring the potential for early diagnosis using techniques like TCD in emergency settings, particularly in conditions like RCVs.

The search for new structural systems capable of associating performance and safety requires deeper knowledge regarding the mechanical behavior of structures subject to different loading conditions. The Strut-and-Tie Model is commonly used to structurally designing some reinforced concrete elements and for the regions where geometrical modifications and stress concentrations are observed, called “regions D”. This method allows a better structural behavior representation for strength mechanisms in the concrete structures. Nonetheless, the topological model choice depends on the designer’s experience regarding compatibility between internal flux of loads, geometry and boundary/initial conditions. Thus, there is some difficulty in its applications, once the model conception presents some uncertainty. In this context, the present work aims to apply the Strut-and-Tie Model to nonlinear structural elements together with a topological optimization method. The topological optimization method adopted considers the progressive stiffness reduction of finite elements with low stress values. The analyses performed could help the structural designer to better understand structural conceptions, guaranteeing the safety and the reliability in the solution of complex problems involving structural concrete.
Differential brain angiotensin-II type I receptor expression in hypertensive rats

Blood-borne angiotensin-II (Ang-II) has profound effects in the brain. We tested the hypothesis that Ang-II-dependent hypertension involves differential Ang-II type I (AT1) receptors expression in the subfornical organ (SFO) and the rostral ventrolateral medulla (RVLM). Male Wistar rats were implanted with 14-day osmotic minipump filled with Ang-II (150 ng/kg/min) or saline. AT1 receptor mRNA levels were detected in the SFO and RVLM by reverse transcription-polymerase chain reaction (RT-PCR). Ang-II caused hypertension (134 ± 10 mmHg vs. 98 ± 9 mmHg, n = 9, p < 0.05). RT-PCR revealed that Ang-II infusion induced increased AT1 receptor mRNA levels in RVLM and decreased in SFO. Our data suggest that Ang-II-induced hypertension involves differential expression of brain AT1 receptors.
Role and effect of traditional markets: The internationally awarded case of Barcelos

Social, economic, and environment sustainability are more than ever hot topics. Because of their importance in this context, we examine how and why traditional markets play such a critical role in local as well as regional development and sustainability, along with their respective effects. We use a single case methodology based on the internationally awarded case of Barcelos (a northern region of Portugal). The main results show that the traditional market of Barcelos plays a critical role in the region, especially with respect to socioeconomic, environmental, and political and administrative effects. To optimize the role and influence of this traditional market, authors suggest the application of artificial intelligence.
The influence of saliva pH value on the retention and durability of bar-clip attachments

PURPOSE The aim of this study was to compare the durability and retention of 4 types of attachments placed over computer aided design/computer aided manufacturing (CAD/CAM) titanium bars when subjected to different pH conditions. MATERIALS AND METHODS Four commercially available attachments were investigated: Hader Yellow, Hader Red, Ackerman Gold and Ackerman Stainless Steel. These attachments and Ackerman CAD/CAM titanium bars were placed in 2 vessels containing different artificial saliva solutions (pH 7/pH 4) at 37℃ for one month to simulate corrosion conditions, and they were then subjected to mechanical testing (5400 cycles of insertion and removal). RESULTS The results revealed that there were significant differences in the average values of insertion/removal force due to the pH (F (1, 24)=9.207, P<.05) and the type of attachment (F (3, 24)=11.742, P<.05). CONCLUSION More acidic pH values were found to have a negative influence on the retention capacity of the attachments.

Background/Aims: Argon plasma coagulation (APC) is the most commonly used endoscopic treatment for gastric antral vascularectasia (GAVE). Endoscopic band ligation (EBL) has emerged as an alternative therapy. Our goal was to evaluate the feasibility,efficacy, and safety of APC and EBL for the treatment of GAVE. This is the first systematic review that included only randomizedcontrolled trials (RCTs) on this topic. Methods: A comprehensive search was performed using electronic databases to identify RCTs comparing APC and EBL for thetreatment of GAVE following the Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. Results: Four RCTs were included, with a total of 204 patients. EBL was related to higher endoscopic eradication rates risk difference[RD], 0.29; 95% confidence interval [CI] [0.14, 0.44]; I2=0%) and less bleeding recurrence than APC (RD, 0.29; 95% CI [0.15, 0.44];I2=0%). Patients treated with EBL required fewer blood transfusions (mean difference [MD], 1.49; 95% CI [0.28, 2.71]; I2=96%) andhospitalizations (MD, 0.29; 95% CI [0.19, 0.39]; I2=0%). The number of sessions required for the obliteration of lesions was higherwith APC. There was no difference in the incidence of adverse events. Conclusions: EBL is superior to APC in the treatment of GAVE in terms of endoscopic eradication rates, recurrence of bleeding, andtransfusion requirements.

The island of S.Miguel is part of the Azores, an archipelago composed of nine volcanic islands situated between the european and american continents. Aligned in a general WNW -ESE trend, between latitudes 37$^{\circ}$ -40$^{\circ}$ N and longitude 25$^{\circ}$ -31$^{\circ}$ W, the Azores islands show a very special geotectonic setting, at the triple junction of the Eurasian, African and North American lithospheric plates.(omitted)
