Imperfect Correlation of Mammographic and Clinical Breast Tissue Density

Background: Clinicians determine degree of mammographic density based on tissue firmness on breast examination. The study aimed to compare breast density in mammography and clinical breast examination. Materials and Methods: Six-hundred sixty three women 40 years of age or older were studied. The breast exam density was graded from 1 to 4 by two expert surgeons and the mammographic parenchymal density by two expert radiologists. Then for practical reasons, grades 1 and 2 were considered as low-density and grades 3 and 4 as high-density. Results: High and low densities were detected in 84.5% and 15.5% of clinical breast examinations and 59.7% and 40.3% of mammographies, respectively. The statistical analysis showed a significant difference between the breast tissue densities in breast examination with those in mammography. Conclusions: A clinically dense breast does not necessarily imply a dense mammographic picture.
Determining the Required Vitamin D Level for Bone Health Based on Bone Turnover Markers

To date, no clear threshold that has been established for defining an adequate store of vitamin D for bone health. Therefore, this study aims to determine the required level of vitamin D to maintain a healthy skeleton based on bone remodelling process among healthy adult population. This was a cross sectional study, involving a healthy adult population in Kota Bharu, Malaysia, aged 18∼50 years. We measured serum 25(OH)D (vitamin D), serum parathyroid hormone (PTH), serum C-terminal telopeptide of type 1 collagen (CTX), and Procollagen 1 Intact N-Terminal (P1NP) in 120 healthy adults selected via multi stage sampling (64 males, 56 females) from 6 subdistricts in Kota Bharu. The mean level of 25(OH)D was 23.50 (±8.74) nmol/L. There was a significant difference of the vitamin D level between genders (26.81±8.3 nmol/L vs 19.72±7.68 nmol/L in males and females respectively) (p value<0.001). More than 50% of female subjects had 25(OH)D less than 20 nmol/L, while only 20.3% of male subjects had 25(OH)D below 20 nmol/L. Based on the LOESS plot, the bone turnover markers showed a plateauing result, at the 25(OH)D level of 35 nmol/L for CTX and 20 nmol/L for P1NP. Contrastingly, PTH showed a step rise in the 25(OH)D level of 20 nmol/L. Based on the LOESS plot for CTX, P1NP and PTH versus 25(OH)D, level of vitamin D between 20 to 35 nmol/L is recommended to maintain healthy skeleton.

Compared to the risk-oriented approach to ovarian lesions that is presented in the Ovarian- Adnexal Reporting and Data System (O-RADS) US Risk Stratification and Management System as a guideline, a lesion-oriented flowchart is more user-friendly for radiologists. In the present article, the O-RADS guideline has been depicted in the form of a single flowchart to explain how to approach ovarian lesions in an algorithmic manner. In addition to providing a practical flowchart, this pictorial article contains some images of the related ovarian lesions.
On nonlinear vibration behavior of piezo-magnetic doubly-curved nanoshells

In this paper, nonlinear vibration behaviors of multi-phase Magneto-Electro-Elastic (MEE) doubly-curved nanoshells have been studied employing Jacobi elliptic function method. The doubly-curved nanoshell has been modeled by using nonlocal elasticity and classic shell theory. An exact estimation of nonlinear vibrational behavior of smart doubly-curved nanoshell has been obtained via Jacobi elliptic function method. This method can incorporate the influences of higher order harmonics leading to an exact estimation of nonlinear vibration frequency. It will be indicated that nonlinear vibrational frequency of doubly-curved nanoshell relies on nonlocal effect, material composition, curvature radius, center deflection and electro-magnetic field.

Objective Recently, the association between periodontal diseases and polycystic ovary syndrome (PCOS) has been established, and it has been revealed that visfatin levels increase in chronic periodontitis (CP) and PCOS. However, there was no study comparing the mean visfatin levels between advanced CP and PCOS. Therefore, the probable role of visfatin linking these diseases remains unknown, and this study was conducted to address this gap. Methods In this age- and weight-matched case-control study (cases with PCOS disease and controls without it), 110 female participants were divided into 4 groups based on clinical findings. The 1st group participants had both PCOS and advanced CP (n=30), 2nd group participants had only PCOS (n=25), 3rd group participants had only advanced CP (n=23), and 4th group comprised of healthy participants (n=32). Enzyme-linked immunosorbent assay was used to investigate visfatin levels in the gingival crevicular fluid (GCF). Data were collected and analyzed using Stata software (version 11). Results The results revealed the significant effect of both PCOS and advanced CP on visfatin levels in the GCF (P<0.05). Conclusion According to the results of this study, the visfatin level in the GCF could be the probable link of association between PCOS and advanced CP.

The reaction of $LnCl_3{\cdot}7H_2O$ [Ln = La (1), Ce (2)] with salicylic acid (HL) and 1,10-phenanthroline (Phen) at $20^{\circ}C$ in $H_2O$/ethanol gave after work-up and recrystallization two novel lanthanide complexes with general formula $[Ln(Phen)_2(L)_3(HL)]{\cdot}H_2O$. Compounds 1 and 2 were characterized by IR and UV-Vis spectroscopy, TGA, CHN as well as by X-ray analysis. According to these results, compounds 1 and 2 are isostructural and contain $Ln^{3+}$ ions with coordination number nine. Complexes 1 and 2 consist of two Phen, one neutral HL and three L anions (two L anions act as monodentate ligands and the third one is chelating to $Ln^{3+}$). Thermal decomposition led to primary loss of the Phen molecules. Then HL molecules and finally L moieties left the material to give $Ln_2O_3$.

Levels of Serum 25-Hydroxy-Vitamin D in Benign and Malignant Breast Masses

Background: The true association between breast cancer and vitamin D is currently under investigation. We compared serum 25-hydroxy-vitamin D levels in women with benign and malignant breast masses and controls. Materials and Methods: Levels of vitamin D were measured by electrochemiluminescense. Serum levels >35 ng/ml, 25-35 ng/ml, 12.5-25 ng/ml and <12.5 ng/ml were considered as normal, mild, moderate and severe vitamin D deficiency, respectively. Results: Overall, 364 women were included in the control, 172 in the benign and 136 in the malignant groups. The median serum vitamin D level was significantly lower in breast cancers than controls. Levels were also lower in malignant than benign cases and in benign cases than controls although statistically non-significant. Conclusions: Multinomial logistic regression analysis showed that severe vitamin D deficiency causes a three-fold increase in the risk of breast cancer while this was not the case for moderate and mild deficiency.
Clinical and Ultrasonographic Changes of the Breast after Use of Soy Isoflavones

Background: Phytoestrogens may be an alternative therapy in control of menopausal symptoms but their definite effects on breast tissue must be determined. Our study aimed to define the clinical and ultrasonographic changes of the breast after use of soy isoflavones in menopausal women. Materials and Methods: Menopausal women with hot flashes were randomly grouped as cases and controls and cases received soy isoflavones for 12 weeks. Breast examination (BE) and ultrasonography (US) were done at 0, 6 and 12 weeks. Tenderness and nodularity on BE were graded 1-4 by breast surgeons. Results: There were 30 women in the case and 26 in the control group. The mean age was 51.3 years and the mean age of menopause was 49.2 years. There was no change in the BE and US at 6 weeks in controls. In the case group, 10% had grade 1 tenderness and 13.3% grade 2 tenderness and grade 1 nodularity in BE accompanied with diffuse small cysts in US. At 12 weeks, there was no change in BE and US in the 2 groups. Conclusions: There was no statistically significant difference in the BE of the 2 groups at 6 and 12 weeks (p value=0.36 and 0.41 for nodularity and tenderness respectively) and in the US results. Although the literature contains many facts concerning PEs and the breast, further prospective studies are needed to identify structural breast changes produced by PEs in order to identify the appropriate dosage and indications of use.
