Reduced-Dose Systemic Fibrinolysis in Massive Pulmonary Embolism: A Pilot Study

Objective: Severe pulmonary embolism (PE) has a high mortality rate, which can be lowered by thrombolytic therapy (TT). However, full-dose TT is associated with major complications, including life-threatening bleeding. The aim of this study was to explore the efficacy and safety of extended, low-dose administration of tissue plasminogen activator (tPA) on in-hospital mortality and outcomes in massive PE. Methods: This was a single-center, prospective cohort trial at a tertiary university hospital. A total of 37 consecutive patients with massive PE were included. A peripheral intravenous infusion was used to administer 25 mg of tPA over 6 hours. The primary endpoints were in-hospital mortality, major complications, pulmonary hypertension, and right ventricular dysfunction. The secondary endpoints were 6-month mortality and pulmonary hypertension and right ventricular dysfunction 6 months after the PE. Results: The mean age of the patients was 68.76±14.54 years. Purpose: The aim of this study was to evaluate the relationship of seminal parameters with the neutrophil-to-lymphocyte ratio (NLR) and the platelet-to-lymphocyte ratio (PLR), which are inflammatory markers, in men with an abnormal semen analysis and men with normozoospermia.Materials and Methods: Fifty-seven men with abnormal semen analyses and 59 men with normozoospermia were included in this cross-sectional study. A complete blood count was recorded, and the NLR and PLR were calculated from the hematologic parameters.Results: The NLR was 1.84±0.78 in the normozoospermic group and 1.80±0.75 in the abnormal semen analysis group. The PLR was 95.32±35.47 in the normozoospermic group and 93.57±28.09 in the abnormal semen analysis group. No significant differences were found between the normozoospermic and the abnormal semen analysis group in the NLR (p=0.828) or the PLR (p=0.768). In addition, no seminal parameters were correlated with the NLR or the PLR (p>0.05).Conclusions: We do not recommend using the NLR or the PLR as markers to screen for abnormal semen parameters or male infertility.
NO<SUB>x</SUB> emission modeling at cement plants with co-processing alternative fuels using ANN

The use of wastes as alternative fuel (AF) at the cement plants for clinkerization processes has increased in recent years as sustainable waste management. Such co-processing of AFs at cement plants causes some changes in the composition of the plant emissions, depending on waste type, kiln thermal power, thermal substitution rate, etc. Emissions of nitrogen oxides (NOx) are among the major environmental concerns in these plants. The paper includes a modeling study of NOx emissions at a cement plant during the co-processing of AFs. Due to the non-linear characteristic of the relationship between operational parameters and NOx emissions, the artificial neural network (ANN) approach was applied and studied. The study showed that NOx emissions can be predicted satisfactorily by using ANN at cement plants. Therefore, the model proposed may be used by cement plant operators to estimate their emission levels before starting the use of a new fuel source.

Patent foramen ovale (PFO) is a frequent condition. It usually does not pose any threat to life, but in certain circumstances, it can open and serve as a conduit for thrombi causing paradoxical embolism. Herein, we report a case of PFO in a patient with ischemic cerebrovascular accident, due to paradoxical embolism, which could not be passed via general practice; hence it was crossed with coronary sinus catheter,through which the delivery sheath was placed into the left atrium, and it was successfully closed.
