        Dual Mechanical Thrombectomy for Recanalization of a Resistant Acute Posterior Circulation Stroke

        A 71-year-old man with acute basilar artery occlusion was referred for endovascular treatment 6 hours after the onset of stroke with a Glasgow Coma Score of 3 and National Institutes of Health Stroke Scale of 27. A cerebral arteriogram revealed occlusion of the left vertebral artery proximally and thromboembolic occlusion of the basilar tip. Direct aspiration and mechanical thrombectomy with various stent retrievers failed to reconstitute arterial flow in the basilar artery. Thrombolysis in cerebral infarction 2b recanalization was achieved only after placement of double Catch Mini stent retrievers through 2 microcatheters, on both side branches of the basilar bifurcation in a kissing fashion and retrieving them simultaneously. It was possible to perform this maneuver through a single distal access catheter without any complications. On follow-up the patient awakened and was able to follow commands on his right side. To our knowledge, dual mechanical thrombectomy with stent retrievers has not been reported in the posterior circulation previously. This technique may be useful in retrieving thrombi located at major intracranial bifurcations of the posterior circulation which do not recanalize with standard mechanical thrombectomy procedures. Although bilateral access to the basilar artery through both vertebral arteries is an advantage in posterior circulation for this technique, dual mechanical thrombectomy can also be performed through a unilateral access.

        Methylphenidate and Central Precocious Puberty: A Probable Side Effect among Seven Children with the Attention Deficit Hyperactivity Disorder

        Methylphenidate (MPH) is the most preferred drug for treatment of the attention deficit hyperactivity disorder (ADHD). Here, we aimed to discuss the possible effects and mechanisms of MPH on precocious puberty (PP) via a case series with seven children who had normal body mass index. In this case series we evaluated seven children with ADHD, who had received MPH for at least 6 months (0.5 mg/kg/dose three times a day, maximum 60 mg) and admitted to Department of Pediatric Endocrinology with PP symptoms. The mean age was 8.16 years. Basal hormonal levels (luteinizing hormone [LH], follicle stimulating hormone, and estrogen/testosterone) were within normal range. Results of LH-releasing hormone stimulation tests demonstrated central pubertal responses. Glutamine, dopamine and noradrenaline are most important excitatory neurotransmitters that have a role at the beginning of puberty. The effect of MPH, cumulating dopamine and noradrenaline in the synaptic gap could be associated with the acceleration of puberty with the excitatory effect of dopamine’s gonadotropin-releasing hormone (GnRH) release, excitatory effect of noradrenaline’s GnRH release and the disappearance of GnRH receptor expression suppressor effect on prolactin disinhibitory effect.

        Thermodynamic Analysis of an Auto-Cascade Freezer Cycle at Different Loads of Evaporators

        In this study, a modified auto-cascade refrigerating cycle is proposed. The cycle includes two evaporators and one compressor, and a separator is located after the refrigerator evaporator. The vapor phase from the separator is condensed through an internal evaporator and is mixed with some amount of the liquid phase taken from the separator. The condensed liquid is sent to the freezer evaporator at low pressure. Thermodynamic properties of the mixture R290/R600a, which is a zeotropic mixture as a cycle fluid, were determined using the REFPROP database. The variation of the cycle performance and total cooling capacity is investigated by changing the ratios of refrigerator and freezer loads. The results show that the volumetric cooling capacity increases and the compressor pressure ratio decreases as the load ratio of the refrigerator increases.

        Evaluation of the Relationships between Irisin Levels and Cognitive Functions in Individuals with Schizophrenia

        Objective: Irisin is a myokine that is involved in neurogenesis, neuronal proliferation, and neuronal differentiation. Many research examine the relationship between irisin and schizophrenia. In this study, we aimed to evaluate the relationship between irisin levels and cognitive functions in individuals with schizophrenia. Methods: Ninety-six individuals who were diagnosed with schizophrenia were included. The Brief Psychiatric Rating Scale (BPRS) was used to assess disease severity. To evaluate the cognitive functions of the patients, the trail-making test was evaluated with the A and B forms and the verbal memory processes scale. After a 12-hour night fast, samples of fasting blood were obtained from the participants. Results: There was no significant correlation between irisin, duration of disease, and BPRS total score. In the analysis performed, a positive correlation was found between the plasma irisin level and the error score of the trail-making test form B. Other than that, no correlation was found between irisin level and cognitive performance in schizophrenia patients. In addition, in subgroup analysis between genders, it was determined that the duration of the trail-making test B was longer in female schizophrenia patients. Conclusion: In this study, there was a positive correlation between the trail-making test B-form error scores and the irisin levels. This relationship between impaired executive functions and irisin levels may suggest that the irisin level is increased as compensation for the impairment in executive functions. More research is needed to understand the role of irisin in cognitive impairment and schizophrenia.

      • An artificial intelligence-based design model for circular CFST stub columns under axial load

        This paper aims to use the artificial intelligence approach to develop a new model for predicting the ultimate axial strength of the circular concrete-filled steel tubular (CFST) stub columns. For this, the results of 314 experimentally tested circular CFST stub columns were employed in the generation of the design model. Since the influence of the column diameter, steel tube thickness, concrete compressive strength, steel tube yield strength, and column length on the ultimate axial strengths of columns were investigated in these experimental studies, here, in the development of the design model, these variables were taken into account as input parameters. The model was developed using the backpropagation algorithm named Bayesian Regularization. The accuracy, reliability, and consistency of the developed model were evaluated statistically, and also the design formulae given in the codes (EC4, ACI, AS, AIJ, and AISC) and the previous empirical formulations proposed by other researchers were used for the validation and comparison purposes. Based on this evaluation, it can be expressed that the developed design model has a strong and reliable prediction performance with a considerably high coefficient of determination (R-squared) value of 0.9994 and a low average percent error of 4.61. Besides, the sensitivity of the developed model was also monitored in terms of dimensional properties of columns and mechanical characteristics of materials. As a consequence, it can be stated that for the design of the ultimate axial capacity of the circular CFST stub columns, a novel artificial intelligence-based design model with a good and robust prediction performance was proposed herein.

        Te Efect of Repetitive Transcranial Magnetic Stimulation on Spasticity in Patients with Multiple Sclerosis

        Background and Purpose Tis randomized controlled study examined the efect of repetitive transcranial magnetic stimulation (rTMS) on spasticity in patients with multiple sclerosis (MS). Methods Tis study included 16 patients with a history of MS and spasticity in the adductor hip muscles according to the Modifed Ashworth Scale (MAS). Te participants were randomized into the active group (n=10) and control group (n=6), in which active rTMS and sham rTMS were applied in 10 sessions, respectively. A physical therapy and rehabilitation program was applied along with rTMS sessions in both groups. Te evaluation parameters were assessed at baseline and then 1 week and 1 month afer applying rTMS. Results Statistical analyses with post-hoc correction revealed statistically signifcant improvements in the active group compared to the control group in the bilateral MAS score, Penn Spasm Frequency Scale score, patient satisfaction, amount of urine leakage, actual health status, perceived health status, energy and fatigue, role limitations due to physical problems, social function, overall quality of life, cognitive functioning, physical health composite score, mental health composite score, and total score on the Multiple Sclerosis Quality of Life-54 (MSQOL-54) (p< 0.05). Statistically signifcant changes were detected in the MSQOL-54 social function and physical health composite scores of patients in the control group (p<0.05). Conclusions Active rTMS combined with a physical therapy program reduced spasticity in MS patients compared to the control group that received only physical therapy. Further comprehensive and more advanced studies are needed to confrm the present fndings.

        Development of metal organic framework filled PDMS/PI composite membranes for biobutanol recovery

        Butanol is a second generation biofuel with high potential to replace fossil fuels in the future. The main drawbacks in biobutanol production are the low yields of operation and high cost of separation. In this study, n-butanol recovery from its aqueous solutions was carried out by pervaporation as an alternative separation process using polyimide supported polydimethylsiloxane membranes, which were especially prepared for this purpose. To enhance the separation efficiency of the membrane, metal organic framework fillers of MOF-199 were included in its structure. Several combinations of polyimide supported PDMS active layered membranes were prepared with the fillers in each layer of the membrane, in both layers of membrane and membranes without fillers. Thus, the effect of the fillers on the separation performance was investigated. It was found that the inclusion of MOF-199 enhances the membrane performance; meanwhile, the support layer also contributes to mass transfer of n-butanol. Furthermore, the effect of the process parameters on the pervaporation performance was investigated. Results of pervaporation tests indicate that the developed membranes have a good potential for n-butanol recovery from aqueous mixtures.

        Is Hearing Loss in Infants Associated With Risk Factors? Evaluation of the Frequency of Risk Factors

        Objectives. To evaluate the frequency of risk factors and their influence on the evoked otoacoustic emission (OAE) of in- fants. Methods. All newborns between November 2009 and June 2012 in Haydarpaşa Numune Education and Research Hospi- tal were tested on distortion evoked OAE screening test. Total of 2,284 infants were examined. Sex, maternal infec- tious disease, birth type (vaginal birth or caesarean sectio), birth weight, familial hearing loss, intermarriage of par- ents, hyperbilirubinemia, intensive care were analyzed as risk factors. Results. Total of 2,284 neonates were screened (1,220 males and 1,064 females) for the presence of OAE in both ears. Vagi- nal delivery, maternal infections during pregnancy, intermarriage of parents relative, low birth weight(<1,500 g) are related risk factors to failure of screening with OAE in our study. There was no statistically significant difference in sex ratios, birth weight, familial hearing loss, hyperbilirubinemia, and intensive care stay. Conclusion. Risk factors are only as useful as their predictive power. Not enough is known about which risk factors are rel- evant, which babies have the risk factors, or which babies will fail to attend follow-up, the effectiveness of targeted hearing loss testing is questionable at this point in time. A system needs to be developed to clarify which risk factors are discoverable, predictive and useful.

        Osseous outgrowth on the buccal maxilla associated with piezosurgery-assisted en-masse retraction: A case series

        Piezoelectric surgery is a novel surgical approach used in orthodontic treatment for rapid tooth movement. This paper presents a case series wherein osseous outgrowths were observed in response to piezosurgery-assisted en-masse ret-raction. Sixteen patients requiring upper premolar extractions were treated with miniscrew-supported en-masse retraction and received minimally invasive decortication via piezosurgery. Computed tomography (CT) of the maxillary anterior region was performed to investigate the nature of the outgrowths. In 8 of the 16 patients, hemispheric or disc-shaped osseous outgrowths were observed on the sites where piezosurgery was performed during retraction. CT images revealed that these outgrowths were alveolar bone. This case series presents a previously unreported osseous response to piezosurgery-assisted tooth movement during orthodontic treatment. The response is mostly transient and is observed in 50% of the treated patients, suggesting a bone turnover that can be assessed clinically and radiographically.[Korean J Orthod 2018;48(1):57-62]

        Electromyographic and Clinical Investigation of the Effect of Platelet-Rich Plasma on Peripheral Nerve Regeneration in Patients with Diabetes after Surgery for Carpal Tunnel Syndrome

        Background Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy. Studies have shown that results of CTS surgery are poorer in patients with diabetes. In this study, the effect of platelet-rich plasma (PRP) on nerve regeneration was investigated through clinical and electromyographic findings in patients with diabetes who underwent CTS surgery.Methods A retrospective analysis of 20 patients with diabetes who had surgically decompressed CTS was conducted. Patients were divided into two groups. The study group received PRP treatment following surgery. The control group did not receive any treatment. Patients were assessed using electromyography and the Boston Carpal Tunnel Syndrome Questionnaire preoperatively as well as postoperatively at 3-month, 6-month, and 1-year follow-ups visits.Results There was a decrease in complaints and an improvement in sensory and motor examinations in both groups. The Boston Carpal Tunnel Syndrome Questionnaire scores did not show any statistically significant differences between the two groups. However, electromyographic findings showed that there were statistical differences between preoperative and postoperative (3 months, 6 months, and 1 year) results in both groups. When the two groups were compared using preoperative and postoperative (3 months, 6 months, and 1 year) electromyographic values, no statistically significant differences were seen.Conclusion Single injections of PRP did not have a significant impact on median nerve regeneration following CTS surgery in patients with diabetes. The effectiveness of multiple PRP injections can be investigated in patients with diabetes in future studies.
