      • Poster Session : PS 0389 ; Infectious Disease ; Vasculitis Caused by Infectious Agent or Allergic Reaction

        The clinical case is about a 44 years old male patient who was transferred to our hospital due to vasculitis. He was living in Angola for 8 years, being healthy until 2 weeks before being transferred. He started to feel abdominal cramps, muscular and joint pain over a period of 3 days. He was admitted in a clinic suspecting of malaria, starting antimalarial medication. On the 3rd day, the blood results for infectious diseases and maliaria were negative. He stooped the treatment and was discharged. After 4 days the patient started to complain of the same symptoms aggravated with asthenia, tiredness and diarrhea. He was readmitted and treated with antibiotics. Three days later some petequial lesions appeared in the lower limbs, getting spread arms and ears. These lesions got bigger over the next 2 days becoming macules and then changed to penfi - goid lesions with central necroses. The lesions covered the all lower limbs and part of the upper limbs, spearing the trunk. Corticosteroids were added to the treatment but he got worse, being later transferred to our hospital. The fi rst evaluation revealed a severe lower limbs edema, hypoalbuminemia and renal failure. All the treatment was stooped apart of pain killers and done some skin biopsies. The result was positive for leukocytoclastic vasculitis. Because of the GIT symptoms an endoscopy and colonoscopy was performed. The gut was covered with extense ulcerative lesions and was identifi ed inside them CMV. The body CT and Abdominal ultrasound were normal. The patient started the treatment with Valaciclovir during 15 days, followed with 6 weeks of Ganciclovir. This case still covered in dough, because it`s impossible to know if the hypersensibility reaction to the anti-malary drugs started the vasculites, triggering the CMV infection or the CMV developed the vasculitis.

        Numerical investigation of effects of buoyancy around a heated circular cylinder in parallel and contra flow

        "Two-dimensional, steady, incompressible Navier-Stokes and energy equations are expressed in the stream function/vorticity formulation and solved numerically by finite difference method to study effects of buoyancy on fluid flow and heat transfer from a horizontal circular cylinder. The cylinder is exposed to approaching flow stream, for parallel (parallel flow) and opposing (contra flow) directions to the buoyant force. Two different thermal boundary conditions were considered at the cylinder surface: constant temperature (CT) and constant heat flux (CHF). The results elucidating the dependence of the flow and heat transfer characteristics on the Richardson number 0≤ Ri ≤2, Prandtl number 0≤ Pr ≤100 and Reynolds number 0 ≤ Re ≤ 40 are presented. Overall, for parallel flow regime, an increase in the Ri led to a raise in both Nusselt number and drag coefficient. However, for contra flow regime, these trends were reversed. For both regimes, the aforementioned behaviors were more pronounced for CT boundary condition than that for the CHF boundary condition."

        A 67-year-old patient was sent to the internal medicine consultation due to dyspnea, orthopnea and lower limb edemas for 6 month, which got worse in the past 2 weeks, with diffi cult stabilization and control, even with intense therapy for heart failure. The therapy was improved and an echocardiogram was requested. One month later she was reevaluated showing no clinical improvement, so she was admitted in the internal medicine ward. The echocardiogram showed an ejection fraction of 21%, increased right chambers, severe pulmonary hypertension and left ventricle dilatation with depressed cardiac function. The therapy for heart failure was optimized and a ventilation-perfusion scyntigraphy was performed to evaluate the respiratory system. It revealed areas of pulmonary collapse and pulmonary embolism. On the ward she suffered a sudden episode of dyspnea and left leg edema and experienced severe pain. An ultrasound of the lower limbs proved the existence of bilateral severe deep venous thrombosis. For a further investigation the patient did a body CT-scan and cancer markers were searched. The CA19.9 was 253679 and there were several metastatic lesions on the liver and an infracentrimetic mass on the pancreatic tail on the CTScan. She started anticoagulation for prevention of other pulmonary embolism, while waiting for a pancreatic biopsy. The patient died 3 month after the diagnosis due to a sudden Myocardial infarction. The symptoms of heart failure dominated the whole clinical picture, being the DVT the wind of change to establish the diagnosis of pancreatic cancer, a scary disease that was silent for so long. This case shows the importance of the paraneoplastic symptoms such the bilateral DVT which was the important milestone for a further investigation and the fi nal diagnose.

        BRAIN DRAIN AND BRAIN WASTE

        When skilled workers migrate, they face the brain waste risk, i.e., they can end up employed as unskilled. We analyze the effects of brain waste on brain drain, resulting from low international transferability of skills. We show that this type of brain waste: (1) reduces education incentives; (2) weakens the chances for a positive self-selection; and (3) decreases the possibility of a brain gain. In addition, the effectiveness of education policies that subsidize students is reduced under the presence of brain waste. Results are robust to introducing different migration costs for the skilled and the unskilled.

        Effects of Ammonium and Calcium on Lisianthus Growth

        The objective of the present study was to determine the influence of ammonium (NH4+) (0% and 50% of total nitrogen, N) in combination with calcium (Ca) on the growth of lisianthus (Eustoma grandiflorum) in perlite cultures. Ammonium significantly improved leaf area, stem diameter, flower buds, and chlorophyll content (SPAD index) while increasing levels of Ca resulted in decreased leaf area, and chlorophyll contents. Application of NH4+ increased dry weight (DW) of stems, leaves and flowers with a reduction in root DW. In contrast, increased Ca caused a decrease in DW of all plant parts, except flowers. Decreased photosynthesis in NH4+-fed plants was associated with a decrease in magnesium (Mg) and potassium (K) uptake, and an increase in phosphorus (P) and sulfur (S) uptake. The potentially deleterious effect of this nutrient imbalance was counteracted by allocating more K and Mg to the shoot and flowers. The ability of lisianthus to shift the allocation of nutrients to sensitive plant parts may explain the tolerance of this species to high NH4+. Ammonium was associated with decreased peroxidase and catalase activities, suggesting that plants were under oxidative stress. Plants typically respond to high NH4+ nutrition by decreasing the uptake of Ca; however, in our study, increasing Ca and NH4+ resulted in increased Ca uptake, reaching toxic concentrations in shoot tissues.

        Micro-Cogeneration Systems

        Micro-cogeneration systems have been increasingly used in buildings as a means of generating energy with higher global efficiency and reduced carbon emissions. The decentralised way of generating electricity together with useful heat, is one of the priorities of the European Union energy policy. In this paper, different available processes! equipments for micro-cogeneration are reviewed, including those that may be driven by solar energy. Their main features and some economic results are discussed. The possibility of their use for cooling is also addressed. The situation in some European countries is analysed.

        Human Acellular Dermis versus Submuscular Tissue Expander Breast Reconstruction: A Multivariate Analysis of Short-Term Complications

        Background Acellular dermal matrix (ADM) allografts and their putative benefits have been increasingly described in prosthesis based breast reconstruction. There have been a myriad of analyses outlining ADM complication profiles, but few large-scale, multi-institutional studies exploring these outcomes. In this study, complication rates of acellular dermis-assisted tissue expander breast reconstruction were compared with traditional submuscular methods by evaluation of the American College of Surgeon’s National Surgical Quality Improvement Program (NSQIP) registry. Methods Patients who underwent immediate tissue expander breast reconstruction from 2006-2010 were identified using surgical procedure codes. Two hundred forty tracked variables from over 250 participating sites were extracted for patients undergoing acellular dermisassisted versus submuscular tissue expander reconstruction. Thirty-day postoperative outcomes and captured risk factors for complications were compared between the two groups. Results A total of 9,159 patients underwent tissue expander breast reconstruction; 1,717 using acellular dermis and 7,442 with submuscular expander placement. Total complications and reconstruction related complications were similar in both cohorts (5.5% vs. 5.3%, P=0.68 and 4.7% vs. 4.3%, P=0.39, respectively). Multivariate logistic regression revealed body mass index and smoking as independent risk factors for reconstructive complications in both cohorts (P<0.01). Conclusions The NSQIP database provides large-scale, multi-institutional, independent outcomes for acellular dermis and submuscular breast reconstruction. Both thirty-day complication profiles and risk factors for post operative morbidity are similar between these two reconstructive approaches.

        Velocity Control of an Omnidirectional Wheeled Mobile Robot Using Computed Voltage Control with Visual Feedback: Experimental Results

        In this paper, we propose a novel computed voltage based control law for an omnidirectional wheeled mobile robot equipped with four mecanum wheels. First, the dynamical model for the mechanical part is originally introduced by means of the Euler-Lagrange formulation. The motion constraints are added to the dynamic model using the Lagrange multipliers. Then, the dynamic model is completed by incorporating the dynamics of the actuators. Therefore, the dynamical model input signals are the armature voltage of the motors. The control law requires only the feedback of position and velocity of the whole robot, unlike most controllers in the literature that also need wheel speed feedback. The position and velocity are obtained by means of a multicam vision system, so measurements from motor encoders are not needed. A high-order sliding modes differentiator is included to estimate linear and angular velocities in a finite time. A stability proof is presented by means of the direct Lyapunov method, and furthermore, an analysis about parametric uncertainties in the mechanical parameters is introduced. The experimental results validate the theoretical proposal and show the good performance of the approach.
