      • Temperamental predictors of developmental trajectories of inattention and hyperactivity–impulsivity problems in schoolchildren

        <P><B>Background</B></P><P>The current study aimed to examine the temperamental predictors of developmental trajectory subgroups of children's inattention and hyperactivity–impulsivity problems through a short-term longitudinal study.</P><P><B>Methods</B></P><P>Children (<I>n</I> = 1344) were divided into younger (age 6–8 years) and older (age 9–11 years) groups in order to observe changes in inattention and hyperactivity–impulsivity problems. Inattention and hyperactivity–impulsivity problems were measured three times at 5-month intervals and Cloninger's four temperaments (novelty seeking, harm avoidance, reward dependence, and persistence) were examined on the first occasion only. A cohort sequential design and growth mixture model were used for investigating trajectory subgroups and multiple logistic regression analysis to examine the temperamental predictors.</P><P><B>Results</B></P><P>Developmental trajectories of inattention and hyperactivity–impulsivity showed different subgroupings depending on the age group of children. Temperament (high score on novelty seeking and low score on persistence as well as high score on reward dependence) and gender predicted the likelihood of belonging to high-risk versus low-risk subgroups.</P><P><B>Conclusion</B></P><P>Suggestions taking into account the predictors of developmental trajectories in inattention and hyperactivity–impulsivity problems for future research are discussed along with the limitations of the current study.</P>

        한국 성인의 안정시 심박수와 우울과의 관계: 한국 국민건강영양조사(2015∼2018년)를 바탕으로

        본 연구의 목적은 한국 성인들을 대상으로 안정시 심박수와 우울과의 관계를 규명하는 것이다. 본 연구는 2015년부터 2018년까지 시행된 국민건강영양조사 자료를 이용하여 만19세 이상 성인 총 14,021명을 대상으로 하였다. 안정시 심박수는 10 bpm (beat per minute) 단위로 다섯 그룹으로 나누었으며 우울과의 관계를 분석하기 위해 공변량과 혼란변수를 통제하고 로지스틱 회귀분석을 실시하였다. 분석 결과, 안정시 심박수가 60 bpm 미만인 그룹과 비교하였을 때 80~89 bpm 또는 90 bpm 이상 그룹에서 우울 위험률이 통계적으로 유의하게 증가하였다(p<.05). 따라서, 본 연구를 통해 안정시 심박수는 우울과 유의한 관련성이 있음을 확인하였다. Background: The purpose of this study was to examine the association between resting heart rate (RHR) and symptoms of depression among Korean adults. Methods: This study used data of Korean adults obtained from the Korea National Health and Nutrition Examination Survey (KNHNES, 2015∼2018). After excluding participants with missing data, a total of 14,021 participants were included for analysis. Participants were classified as depressed if they had experienced depression within the past two weeks, or were currently depressed, or were diagnosed as depressed. For the main analysis, participants were classified into five groups by resting heart rate (RHR), i.e., <60 bpm (beats per minute), 60∼69 bpm, 70∼79 bpm, 80∼89 bpm, and ≥90 bpm. The correlation between RHR and symptoms of depression was examined using logistic regression analysis after controlling for the confounders. Results: Compared to the participants with RHR of <60 bpm, the participants with RHR of 80∼89 bpm or ≥90 bpm showed higher level of symptoms of depression. [Men: 80∼89 bpm by 1.77 (95% confidence interval [CI] 1.07∼2.93) and ≥90 bpm by 3.20 (95% CI, 1.68∼6.08), Women: ≥90 bpm by 1.71 (95% CI, 1.04∼2.81)]. Further stratified analyses were performed and yielded similar results regardless of the potential confounders such as age, BMI, income, marital status, and levels of physical activity. Conclusions: RHR was significantly associated with symptoms of depression. This study suggests the importance of RHR as a predictor of the risk of depression.

        Depletion of Alloreactive T-Cells by Anti-CD137-Saporin Immunotoxin

        <P>Depletion of alloreactive T-lymphocytes from allogeneic bone marrow transplants may prevent graft-versus-host disease (GVHD) without impairing donor cell engraftment, immunity, and the graft-versus-leukemia (GVL) effect. Alloreactive T-cells may be identified by their expression, upon activation, of CD137, a costimulatory receptor and putative surrogate marker for antigen-specific effector T-cells. In this context, we tested the use of anti-CD137-saporin immunotoxin to selectively deplete mouse and human alloreactive T-cells. Anti-CD137 antibodies were internalized by cells within 4 h of binding to the cell surface CD137, and anti-CD137-saporin immunotoxin effectively killed polyclonally activated T-cells or antigen-stimulated T-cells. Transfer of donor T-cells after allodepletion with anti-CD137-saporin immunotoxin failed to induce any evident expression of GVHD; however, a significant GVL effect was observed. Targeting of CD137 with an immunotoxin was also effective in killing polyclonally activated or alloreactive human T-cells. Our results indicate that anti-CD137-saporin immunotoxin may be used to deplete alloreactive T-cells prior to bone marrow transplantation and thereby prevent GVHD and the relapse of leukemia.</P>

        A Web-Based Decision Aid for Informed Prostate Cancer Screening: Development and Pilot Evaluation

        Background: Prostate-specific antigen-based routine screening is not recommended for the general population due to conflicting results with mortality reduction. We aimed to develop a web-based decision aid (DA) for informed decision making for prostate cancer screening. Methods: Using the International Patient Decision Aid Standards (IPDAS) development process model, we developed our DA based on patient and clinician interviews and multidisciplinary expert discussions. The prototype consisted of predicting individual prostate cancer risk and informed decision-making, including knowledge, risk and benefit, cost, personal value, and decision making. We conducted a pilot study on 101 healthy men, evaluating the effectiveness of DA by measuring knowledge, attitude, and intention to screen before and after using the DA, as well as decisional conflict and usefulness after using the DA. Results: Of the 101 participants (median age 60 [50–69] years), 84% had not undergone screening for prostate cancer in the past two years. After using the DA, knowledge on prostate cancer screening increased (mean score [of 10] before versus after: 6.85 ± 1.03 versus 7.57 ± 1.25; P < 0.001), and intention to not screen increased from 27.7% to 51.5% (P < 0.001), but attitude toward screening did not change (P = 0.564). After use of the DA, 79 participants reported no decisional conflict, and the usefulness score was high (mean score [of 100] 77.35 ± 7.69), with 85% of participants reporting that the DA helped with decision making. Conclusion: Our web-based DA yielded increased knowledge, decreased screening intention, and high perceived usefulness. These findings indicate potential clinical relevance, especially among younger individuals.

        수술 직후 유방암 환자의 운동제약 및 촉진요인: 질적 내용분석

        PURPOSE: This study aimed to understand the barriers to exercise and facilitators of exercise for up to 4 weeks following breast cancer surgery. METHODS: A descriptive qualitative research method was used in this study. Twelve patients were recruited through purposive sampling immediately after breast cancer surgery. RESULTS: Physical aspects (pain at the surgery site, reduction in the range of motion, and decrease in fitness), environmental aspects (difficulty in movement due to drain, lack of information on exercise), and psychological aspects (concerns about side effects, fear of pain, and fear of injury during exercise) were identified as barriers to exercise. Expectation of positive effects (recovery from surgery, prevention of lymphedema, usefulness for future radiation therapy, prevention of cancer recurrence, and health management) of exercise and social support (hospital education, support from medical staff, and exercise information received via mass media) were identified as facilitators of exercise. CONCLUSIONS: We recognized different barriers to exercise and facilitators of exercise among patients who recently underwent breast cancer surgery. Future exercise intervention studies should consider minimizing such barriers and maximizing the facilitators identified in our study.
