        Immunonutrition in ovarian cancer: clinical and immunological impact?

        Objective: Malnutrition is frequent in ovarian cancer (OC) patients and may compromise post-operative outcomes. The aim of this study is to evaluate the impact of pre-operative immunonutrition on the surgical outcome of OC patients, and on their nutritional, inflammatory and peripheral blood immune status. Methods: A prospective study was performed between September 2016 and April 2020. Immune-enhancing enteral nutrition was administered to 42 patients before surgery according to their nutritional status assessed by the Malnutritional Universal Screening Tool. Biochemical and hematological monitoring was performed before and after immunonutrition. Post-operative outcomes were assessed and compared with those of a similar group of patients treated without nutritional support. Results: Of the 42 immune-nourished patients, 23 (54.8%) had a low, 11 (26.2%) an intermediate and 8 (19%) a high risk of malnutrition. After the immunonutritional intake, significant variations of prealbumin, creatinine and white blood cells were detected. All T cell populations had an increasing trend, in particular CD3+ T lymphocytes (p=0.020), CD3+CD8+ cytotoxic T lymphocytes (p=0.046) and lymphocyte with HLA-DR expression (p=0.012). The rate of grade II–III post-operative complications was lower (21.4% vs. 42.9%, p=0.035) and the time of hospitalization was shorter (7.5 vs. 9.2, p=0.009) in the immune-nourished group. Conclusion: Pre-operative immunonutrition improves the surgical outcome of OC patients. After immunonutrition, an increase of CD3+CD8+ cytotoxic T lymphocytes was observed.

        Impaired Colonic Contractility and Intestinal Permeability in Symptomatic Uncomplicated Diverticular Disease

        Background/Aims Impaired intestinal motility seems to play a crucial role in symptomatic uncomplicated diverticular disease (SUDD), although the mechanism is not clear. The aim of the present study is to explore the contractility patterns of colonic smooth muscle strips (MS) and smooth muscle cells (SMCs) and to assess mucosal integrity in SUDD patients. Methods MS or SMCs were isolated from specimens of human distal colon of 18 patients undergoing surgery for non-obstructive colonic cancer, among them 9 with SUDD. Spontaneous phasic contractions on strips and morpho-functional parameters on cells were evaluated in basal conditions and in response to acetylcholine (ACh). Mucosal integrity of SUDD colonic biopsies was evaluated by the Ussing Chamber system. Immunohistochemical staining for tight junction protein complex and for Toll-like receptor 4 (TLR4) was performed. Results Colonic MS of SUDD group showed a significant reduced basal tone and ACh-elicited contraction, compared to the control group (9.5 g and 47.0% in the SUDD group; 14.16 g and 69.0% in the control group; P < 0.05). SMCs of SUDD group showed a maximal contractile response to ACh significantly reduced compared to control group (8.8% vs 16.5%, P < 0.05). SUDD patients displayed lower transepithelial electrical resistance and increased paracellular permeability compared to control group. Immunohistochemical expression of TLR4 was not different in both groups, while tight junction protein complex expression was lower in SUDD patients compared to control group patients. Conclusion It could be hypothesized that in SUDD, in absence of severe inflammation, an increased intestinal mucosal permeability is related to altered colonic motility probably responsible for symptoms genesis.         Reported Speech : Towards a Definition as a Communicative and Linguistic Universal

        Reported speech is a structure that has quite always been investigated from a formal point of view, as the syntactical means by which we can construct the oratio, choosing between embedding or un-embedding sentences, respectively within oratio obliqua or oratio recta. The purpose of this essay is arguing that reported speech corresponds to a universal communicative function, the reporting function, which can realize and show itself as a multiplicity of forms, grouped around few main types, among which one in particulardirect speechcan be considered its formal universal side. So we will first define the traditional syntactical account of RS; next, we will turn to a wider perspective, in which functions are prior then forms. The functional and pragmatic account of RS will be considered the proper theoretical framework in order to identify the universalfeatures of reporting. Finally, we will present the results of some sperimental researches about the acquisition of RS both in L1 and L2. The results will allow us to confirm the main hypotheses: the universality of RS as a basic communicative function, and the universality of (at last) one formal fulfillment of it, the direct speech, related to its prototypical 2 Reported Speech: Towards a Definition as a Comunicative and~ narrative function.

        HS-SPME-GC/MS Analysis of the Volatile Compounds of Achillea collina: Evaluation of the Emissions Fingerprint Induced by Myzus persicae Infestation

        A Headspace Solid-phase Microextraction (HSSPME)method combined with Gas Chromatography-Mass Spectrometry (GC/MS) was developed and optimized to extrat and analyze the volatile compounds of aerial parts of Achillea collina Becker ex Rchb. and to investigate the effect of the phlem feeding aphid Myzus persicae Sulzer on the Volatile Organic Compounds (VOCs) emitted by the infested plants. The extraction of 1 g of powdered freeze dried plant samples for 120 min at 30oC using divinylbenzene-carbowaxpolydimethylsiloxane (DVB/CAR/PDMS) fiber showed the highest area counts for the majority of the volatile compounds. Overall, 62 and 80 volatile compounds were detected in control and infested plant samples respectively. In A. collina infested plants, we observed a great increase in both monoterpenes and sesquiterpenes fractions. Several changes among alcohols also occurred, particularly regarding Z-3-hexen-1-ol, E-3-hexen-1-ol and E-2-hexen-1-ol proposing these compounds as herbivore-induces plant volatiles (HIPVs). New perspective for agricultural practice may derive from the opportunity to identify novel herbivores-induced plant VOCs active as plant protection agents.

        Detection of a concentrated damage in a parabolic arch by measured static displacements

        The present paper deals with the identification of a concentrated damage in an elastic parabolic arch through the minimization of an objective function which measures the differences between numerical and experimental values of static displacements. The damage consists in a notch that reduces the height of the cross section at a given abscissa and therefore causes a variation in the flexural stiffness of the structure. The analytical values of static displacements due to applied loads are calculated by means of the principle of virtual work for both the undamaged and damaged arch. First, pseudo-experimental data are used to study the inverse problem and investigate whether a unique solution can occur or not. Various damage intensities are considered to assess the reliability of the identification procedure. Then, the identification procedure is applied to an experimental case, where displacements are measured on a prototype arch. The identified values of damage parameters, i.e., location and intensity, are compared to those obtained by means of a dynamic identification technique performed on the same structure.

        Melatonin’s Effect on the Efficacy of Desmopressin in the Treatment of Enuresis

        Purpose: This study aims to evaluate and compare the efficacy of exogenous melatonin associated with desmopressin (dDAVP) and dietary recommendations. Methods: A total of 189 patients were enrolled from the Service of Pediatrics, Campus Bio-Medico University Hospital of Rome, from January 2013 to June 2015. Of the 189 original patients, 153 children, aged between 5 and 14 years (mean age, 8.7 years) were included in the study. After clinical evaluation and a 3-month period of observation without treatment, children were assigned to receive treatment in one of 3 groups: group 1, dDAVP at a dose of 120 mcg a day (Minirin); group 2, dDAVP at a dose of 120 mcg and dietary recommendations; or group 3, dDAVP at a dose of 120 mcg, dietary recommendations, and melatonin at a dose of 1 mg a day (Melamil plus). Each patient was treated for 3 months. Results: After the 3 months of therapy, a desiderable response was achieved in 30 of 51 patients (58.82%) treated with dDAVP, 35 of 53 patients (66.04%) treated with dDAVP and dietary recommendations, and 35 of 49 patients (71.43%) treated with dDAVP, dietary recommendations, and melatonin. Conclusions: Although not statistically significant, the results show that the association between dDAVP treatment with dietary recommendations and melatonin could be considered a safe and effective treatment of NE. Considering that the statistically insignificant results might be due to the small sample size, the study will be continued to increase the number of subjects.

      • The family, communication and the new technologies

        What has become of communication in the family after the widespread diffusion of the New technologies (that is, mobile phone, computer and internet)? If in the past television was often accused of having killed conversation, what changes has the advent of the computer, internet and mobile phone produced inside the family? What is the nature of communicative modalities today, the circulation and quality of communication, the range of discourse in families? What is the usefulness of the New Technologies in the family as regards communication and what are the obstacles perceived in their diffusion? In this paper we aim to answer these questions, availing ourselves above all of the results obtained in two research projects conducted in Italy. The first is a telephone survey in 1998 of a random sample of the Italian population and sponsored by Telecom Italia (margin of error ± 2.6%). In all, 1,400 people between the ages of 15 and 64 were interviewed. This was a very wide-ranging research that investigated several aspects linked to the diffusion of the new technologies. One section was specifically dedicated to inquiring into the effects of the New technologies on the communication atmosphere inside the family. The second was a face-to-face survey conducted in the spring of 2003 in North-eastern Italy in two secondary schools, a classical lyc?e and a scientific one (Cianchi, D'Alessio, Fortunati, Manganelli, 2003). The data collected consist of 716 questionnaires with open and closed questions, which all the young people, between the ages of 14 and 21, of these schools answered (questionnaires valid: 714). In this case, the aim was to investigate the changes that had taken place in the control of young people's communication by parents. The data were analysed by means of descriptive techniques (frequency analysis) and the construction of contingency tables originated by the crossing of observed variables with socio-demographic variables such as gender, age, family typology and so on. Then inferential analysis was conducted by means of x² test and log-linear models. In some cases, models of multivaried analysis of variance were used. The results of the first research underline the assessments by the 1,400 respondents of the quality of family communication in a moment of intense diffusion of new technologies. Those of the second one are interesting because they reveal a series of problems in family communication which are reflected in control practices applied by parents following on the widespread use of the mobile phone by young people.
