      • Exact Lagrangian submanifolds, Lagrangian spectral invariants and Aubry-Mather theory

        <B>Abstract</B><P>We construct graph selectors for compact exact Lagrangians in the cotangent bundle of an orientable, closed manifold. The construction combines Lagrangian spectral invariants, developed by Oh, and results, by Abouzaid, about the Fukaya category of a cotangent bundle. We also introduce the notion of Lipschitz-exact Lagrangians and prove that these admit an appropriate generalisation of graph selector. We then, following Bernard-Oliveira dos Santos, use these results to give a new characterisation of the Aubry and Mañé sets of a Tonelli Hamiltonian and to generalise a result of Arnaud on Lagrangians invariant under the flow of such Hamiltonians.</P>

        Effect of Glutamine, Glutamic Acid and Nucleotides on the Turnover of Carbon (δ<sup>13</sup>C) in Organs of Weaned Piglets

        Morphological and physiological alterations occur in the digestive system of weanling piglets, compromising the performance in subsequent phases. This experiment aimed at verifying the influence of glutamine, glutamate and nucleotides on the carbon turnover in the pancreas and liver of piglets weaned at 21 days of age. Four diets were evaluated: glutamine, glutamic acid or nucleotides-free diet (CD); containing 1% glutamine (GD); containing 1% glutamic acid (GAD) and containing 1% nucleotides (ND). One hundred and twenty-three piglets were utilized with three pigs slaughtered at day zero (weaning day) and three at each one of the experimental days (1, 2, 4, 5, 7, 9, 13, 20, 27, and 49 post-weaning), in order to collect organ samples, which were analyzed for the ${\delta}^{13}C$ isotopic composition and compared by means of time. No differences were found (p>0.05) among treatments for the turnover of the $^{13}C$ in the pancreas ($T_{50%}$ = 13.91, 14.37, 11.07, and 9.34 days; $T_{95%}$ = 46.22, 47.73, 36.79, and 31.04 days for CD, GD, GAD, and ND, respectively). In the liver, the ND presented accelerated values of carbon turnover ($T_{50%}=7.36$ and $T_{95%}=24.47days$) in relation to the values obtained for the GD ($T_{50%}=10.15$ and $T_{95%}=33.74days$). However, the values obtained for the CD ($T_{50%}=9.12$ and $T_{95%}=30.31days$) and GAD ($T_{50%}=7.83$ and $T_{95%}=26.03days$) had no differences (p>0.05) among other diets. The technique of $^{13}C$ isotopic dilution demonstrated trophic action of nucleotides in the liver.

        This study builds on prevalent approaches of extending the applications of importance performance analysis (IPA) tool. Thus, we analyse the influence of Performance expectancy on Customer Satisfaction. The instruments employed were adapted from previous studies and pilot tested with a group of master students to verify clarity of meaning and comprehension. Findings reveal the stronger influence of three factor that emerge from performance expectancy: Usability, Reliability, Information Quality.

        Comparative clinical and radiologic evaluation between patients undergoing standard reversed shoulder arthroplasty or bony increased offset

        Background: Modifications of the medialized design of Grammont-type reverse shoulder arthroplasty (RSA) using a bony increased offset (BIO-RSA) has shown better clinical results and fewer complications. The aim of this study is to compare the clinical results, complications, and radiological outcomes between patients undergoing standard RSA and BIO-RSA. Methods: A retrospective review was performed of 42 RSA procedures (22 standard RSA and 20 BIO-RSA). With a minimum of one year of follow-up, range of motion (ROM), Constant shoulder score (CSS), visual analog scale (VAS), and subjective shoulder score (SSS) were compared. Radiographs and computed tomography (CT) scan were examined for scapular notching, glenoid and humeral fixation, and graft healing. Results: At a mean follow-up of 27.6 months (range, 12–48 months), a significant difference was found for active-internal rotation (P=0.038) and for passive-external rotation (P=0.013), with better results in BIO-RSA. No other differences were found in ROM, CSS (P=0.884), VAS score, and SSS. Graft healing and viability were verified in all patients with CT scan (n=34). The notching rate was 28% in the standard RSA group and 33% in the BIO-RSA group, but the standard RSA had more severe notching (grade 2) than BIO-RSA (P=0.039). No other significative differences were found in glenoid and humeral fixation. Conclusions: Bone-graft lateralization is associated with better internal and external rotation and with less severe scapular notching compared to the standard RSA. Integration of the bone graft occurs effectively, with no relevant changes observed on radiographic evaluation. Level of evidence: Level III; Retrospective cohort design treatment study

        Improving User Satisfaction in Adaptive Multicast Video

        Adaptability is the most promising feature to be applied in future robust multimedia applications. In this paper, we propose the Direct Algorithm to improve the degree of satisfaction at heterogeneous receivers in multi-layered multicast video environments. The algorithm relies on a mechanism that dynamically controls the rates of the video layers and is based on feedback control packets sent by the receivers. The algorithm also addresses scalability issues by implementing a merging procedure at intermediate nodes in order to avoid packet implosion at the source in the case of large multicast groups. The proposed scheme is optimized to achieve high global video quality and reduced bandwidth requirements. We also propose the Direct Algorithm with a virtual number of layers. The virtual layering scheme induces intermediate nodes to keep extra states of the multicast session, which reduces the video degradation for all the receivers. The results show that the proposed scheme leads to improved global video quality at heterogeneous receivers with no cost of extra bandwidth.

        Numerical Analysis on Distortional Failure of Cold-Formed Steel Hat-Section Beams Under Non-uniform Bending

        Cold-formed steel (CFS) members stand out among steel structures notably due to their lightness, structural efficiency (high strength-to-weight ratio) and versatility, however, given their high width-to-thickness ratio, are highly susceptible to instability phenomena (buckling). The objective of this research is to analyze the structural behavior of cold-formed steel hat-section beams under non-uniform bending about the major and minor-axis, regarding the risks of distortional failure. Through the Generalized Beam Theory, using the computational program GBTUL, the geometries where the distortional failure is predominant were selected and through the computational program ABAQUS, a shell finite element model was developed to perform the buckling and post-buckling analysis on the selected elements. The results obtained from the developed model—which include critical buckling moments, failure moments, equilibrium paths, deformed configurations and collapse mechanisms—show that the developed model adequately simulates the distortional buckling and post-buckling behavior of the considered CFS beams and demonstrate how loading affects such behaviors.

        Acupuncture as Adjuvant Therapy for Sleep Disorders in Parkinson’s Disease

        There are few studies which attest the efficacy of acupuncture on treatment of sleep disturbs in Parkinson disease. The aimed of this randomized clinical trial was to evaluate the effects of acupuncture on sleep disturbs of 22 patients with diagnosis of idiopathic Parkinson disease (Hoehn–Yahr 1 to 3) who have assistance on the Pro-Parkinson Program of Clinical Hospital at Federal University of Pernambuco in Brazil. All participants were evaluated by Parkinson Disease Sleep Scale (PDSS) before and after 8 weeks. The experimental group was submitted to 8 sections (once a week) which had duration of 30 minutes. The control group had no intervention. The intervention was executed using the acupuncture points LR3 (Taichong), SP6 (Sanyinjiao), LI4 (Hegu), TE5 (Wai-Guan), HT7 (Shenmen), PC6 (Neiguan), LI11 (Quchi), GB20 (Fengchi). Paired analyses were obtained by Wilcoxon test and independent analyses were made according to Mann–Whitney test. This study presented a potential therapeutic benefit of acupuncture on sleep disturbs of Parkinson's disease patients. This study showed a possible therapeutic benefit through acupuncture in sleep disorders in patients with PD. However, we propose new studies related to the effects of acupuncture on the clinical symptoms and evolution of the disease.

        Body composition and functional performance of older adults

        Objectives: To determine if anthropometric variables, body composition, medication and gender are associated with functional performance and to compare these variables between octogenarians with high and low functional performance. Methods: Observational, cross-sectional study. Weight, height, body mass index (BMI), waist circumference (WC), and waist-to-height ratio (WHtR) were evaluated. Handgrip strength (HGS) was assessed. Participants’ body composition was assessed by dual-energy X-ray absorptiometry (DXA) and functional performance by Short Physical Performance Battery (SPPB). A binomial logistic regression was performed. Results: One hundred and twenty-two octogenarians were included and separated into high and low function groups. The high function group showed lower values of WHtR (mean difference [MD] = 0.047, P = 0.025) and body fat (BF%) (MD = 3.54, P = 0.032) and higher values of apendicular skeletal muscle mass (ALM) (MD = 3.03, P = 0.001), HGS (MD = 6.11, P = 0.001) and SPPB score (MD = 4.20, P = 0.001). Women were more likely to be classified as low function (OR = 3.66, P = 0.002) and males showed 5.21 odds ratio (P = 0.021) of having high functional performance compared to females. Also, each decrease in age and medication use displayed 1.30 (P = 0.007) and 1.26 odds ratio increases (P = 0.008) in high functional performance. Conclusions: Older males display better functional performance than women, and decrements in age and medications increase the high functional performance odds ratio. Octogenarians with high functional performance displayed lower BF measurements and higher values of muscle mass and strength.

        Analysis of electroencephalogram-derived indexes for anesthetic depth monitoring in pediatric patients with intellectual disability undergoing dental surgery

        Background: Patients with intellectual disability (ID) often require general anesthesia during oral procedures. Anesthetic depth monitoring in these patients can be difficult due to their already altered mental state prior to anesthesia. In this study, the utility of electroencephalographic indexes to reflect anesthetic depth was evaluated in pediatric patients with ID. Methods: Seventeen patients (mean age, $9.6{\pm}2.9years$) scheduled for dental procedures were enrolled in this study. After anesthesia induction with propofol or sevoflurane, a bilateral sensor was placed on the patient's forehead and the bispectral index (BIS) was recorded. Anesthesia was maintained with sevoflurane, which was adjusted according to the clinical signs by an anesthesiologist blinded to the BIS value. The index performance was accessed by correlation (with the end-tidal sevoflurane [EtSevo] concentration) and prediction probability (with a clinical scale of anesthesia). The asymmetry of the electroencephalogram between the left and right sides was also analyzed. Results: The BIS had good correlation and prediction probabilities (above 0.5) in the majority of patients; however, BIS was not correlated with EtSevo or the clinical scale of anesthesia in patients with Lennox-Gastaut, West syndrome, cerebral palsy, and epilepsy. BIS showed better correlations than SEF95 and TP. No significant differences were observed between the left- and right-side indexes. Conclusion: BIS may be able to reflect sevoflurane anesthetic depth in patients with some types of ID; however, more research is required to better define the neurological conditions and/or degrees of disability that may allow anesthesiologists to use the BIS.
