      • Video Evaluation of Self-Regulation Skills in Preschool Aged Children with Developmental Delays: Can the impact of Tumbling TogetherTM be Measured?

        Objectives: This study evaluated the feasibility of video recording to evaluate an innovative, multi-disciplinary exercise medicine intervention, called Tumbling TogetherTM, on the self-regulation skills required for successful group participation. Methods: A photographer, who did not interact with participants and maintained a consistent position outside of the program area, used a digital video camera to record the program activities. Response to a stop command (attempts after the signal), ability to wait for a turn (yes/no), and ability to line up (correct/incorrect position) were the indicators of self-regulation. Video recordings were analyzed to determine each child’s baseline (first two sessions) and post-program (last two sessions) abilities. Chi-square and t-tests identified differences in performance from beginning to end of the program. Results: Video recordings were analyzed for 27 children (3 (11%) female, 3.1 to 4.8 years of age) with complex communication/developmental challenges. Response to a stop command improved by 38% (Pre: 2.1 ± 0.9; Post: 1.3 ± 0.4; p<.001). Correct response to a line up signal improved from 26% to 89% (p<.001). The ability to wait for a turn when required increased from 7% to 80% (p<.001). Conclusions: Video recordings suitable for analysis were obtained during all sessions. Children’s skills performed during the first two classes were similar. Their skills improved between the start and end of the program. Video recordings accurately reflected the improved performance subjectively reported by therapists and parents. Therefore, video recording appears to be an appropriate method of evaluating the impact of exercise medicine interventions, such as Tumbling TogetherTM, that are designed to enhance the self-regulation skills of preschool children. Research comparing Tumbling participants to children not offered the program, and examining whether the self-regulation skills improved during Tumbling participation would generalize to other settings is required.

        Tenant compte de Ia theorie du skopos, du debat sur Ia visibilite du traducteur et des specificites de Ia traduction des livres dits pour enfants, cet article compare Ia traduction de Harry Potter and the Philosopher's Stone vers le franyais avec celles vers six autres langues, trois germaniques proches de I'anglais, trois latines plus semblables au franyais. Un corpus aligne constitue de !'original et des traductions d'elements lexicaux ou d'extraits plus longs a servi pour Ia comparaison. L'analyse de ce corpus permet de deduire que si les choix de traduction sont parfois contraints par Ia langue-cible, des facteurs extralinguistiques, a savoir culturels et commerciaux, ainsi que le skopos et les commanditaires jouent un role determinant sur le marche de Ia traduction. Malgre !'affirmation de J.K. Rowling qu'elle n'ecrivait pas pour un lectorat specifique, les maisons d'edition ont classe le premier Harry Potter « litterature jeunesse ». Les traductions vers differentes langues refletent forcement cette categorisation, celle vers le franyais notamment presentant de nombreux ecarts par rapport a !'original. Afin de redonner au texte-source sa vraie valeur, des traductions revisees de quelques courts extraits du premier Harry Potter sont proposees a Ia fin.

        The Adverse Events and Hemodynamic Effects of Adenosine-Based Cardiac MRI

        Objective: We wanted to prospectively assess the adverse events and hemodynamic effects associated with an intravenous adenosine infusion in patients with suspected or known coronary artery disease and who were undergoing cardiac MRI. Materials and Methods: One hundred and sixty-eight patients (64 ± 9 years) received adenosine (140 μg/kg/min) during cardiac MRI. Before and during the administration, the heart rate, systemic blood pressure, and oxygen saturation were monitored using a MRI-compatible system. We documented any signs and symptoms of potential adverse events. Results: In total, 47 out of 168 patients (28%) experienced adverse effects, which were mostly mild or moderate. In 13 patients (8%), the adenosine infusion was discontinued due to intolerable dyspnea or chest pain. No high grade atrioventricular block, bronchospasm or other life-threatening adverse events occurred. The hemodynamic measurements showed a significant increase in the heart rate during adenosine infusion (69.3 ± 11.7 versus 82.4 ± 13.0 beats/min, respectively; p < 0.001). A significant but clinically irrelevant increase in oxygen saturation occurred during adenosine infusion (96 ± 1.9% versus 97 ± 1.3%, respectively; p < 0.001). The blood pressure did not significantly change during adenosine infusion (systolic: 142.8 ± 24.0 versus 140.9 ± 25.7 mmHg; diastolic: 80.2 ± 12.5 mmHg versus 78.9 ± 15.6, respectively). Conclusion: This study confirms the safety of adenosine infusion during cardiac MRI. A considerable proportion of all patients will experience minor adverse effects and some patients will not tolerate adenosine infusion. However, all adverse events can be successfully managed by a radiologist. The increased heart rate during adenosine infusion highlights the need to individually adjust the settings according to the patient, e.g., the number of slices of myocardial perfusion imaging.       • Alternative Transcarotid Approach for Endovascular Treatment of Acute Ischemic Stroke Patients: A Case Series

        Purpose: Mechanical thrombectomy has become the standard of care for acute stroke caused by large vessel occlusion. As more patients are treated endovascularly, the number of older patients with tortuous vessels has risen. In these patients, catheterizing the internal carotid artery via a transfemoral approach can be very difficult or even impossible. Therefore, in selected patients, alternative strategies to the transfemoral approach have to be applied. Materials and Methods: We report a case series of six patients undergoing mechanical thrombectomy via a combined transfemoral and transcarotid approach. Puncture of the carotid artery was conducted using roadmap guidance after an unsuccessful transfemoral attempt. Technical aspects and outcomes with this alternative approach were analyzed. Results: Direct puncture of the carotid artery was achieved in five out of six patients (83%). In three out of six patients (50%), revascularization (modified Thrombolysis in Cerebral Infarction score ≥2b) was restored. No complications related to endovascular therapy were documented. One patient showed good neurological outcome (modified Rankin Scale [mRS] 5 at admission, mRS 1 at discharge). Conclusion: A combined transfemoral/transcarotid approach can be an alternative vascular access in patients with problematic vessel anatomy.

        microRNA-mediated regulation of mTOR complex components facilitates discrimination between activation and anergy in CD4 T cells

        <P>T cell receptor (TCR) signals can elicit full activation with acquisition of effector functions or a state of anergy. Here, we ask whether microRNAs affect the interpretation of TCR signaling. We find that Dicer-deficient CD4 T cells fail to correctly discriminate between activating and anergy-inducing stimuli and produce IL-2 in the absence of co-stimulation. Excess IL-2 production by Dicer-deficient CD4 T cells was sufficient to override anergy induction in WT T cells and to restore inducible Foxp3 expression in <I>Il2</I>-deficient CD4 T cells. Phosphorylation of Akt on S473 and of S6 ribosomal protein was increased and sustained in Dicer-deficient CD4 T cells, indicating elevated mTOR activity. The mTOR components Mtor and Rictor were posttranscriptionally deregulated, and the microRNAs Let-7 and miR-16 targeted the <I>Mtor</I> and <I>Rictor</I> mRNAs. Remarkably, returning Mtor and Rictor to normal levels by deleting one allele of <I>Mtor</I> and one allele of <I>Rictor</I> was sufficient to reduce Akt S473 phosphorylation and to reduce co-stimulation–independent IL-2 production in Dicer-deficient CD4 T cells. These results show that microRNAs regulate the expression of mTOR components in T cells, and that this regulation is critical for the modulation of mTOR activity. Hence, microRNAs contribute to the discrimination between T cell activation and anergy.</P>

      • Expression of DDR1 and DVL1 in Invasive Ductal and Lobular Breast Carcinoma does not Correlate with Histological Type, Grade and Hormone Receptor Status

        Background: Invasive ductal (IDC) and lobular (ILC) carcinomas are the common histological types of breast carcinoma which are difficult to distinguish when poorly differentiated. Discoidin domain receptor (DDR1) and Drosophila dishevelled protein (DVL1) were recently suggested to differentiate IDC from ILC. Objectives: To assess the expression of DDR1 and DVL1 and their association with histological type, grading and hormonal status of IDC and ILC. Materials and Methods: This cross sectional study was conducted on IDC and ILC breast tumours. Tumours were immunohistochemically stained for (DDR1) and (DVL1) as well as estrogen receptor (ER), progesterone receptor (PR) and C-erbB2 receptor. Demographic data including age and ethnicity were obtained from patient records. Results: A total of 51 cases (30 IDCs and 21 ILCs) were assessed. DDR1 and DVL1 expression was not significantly associated with histological type (p=0.57 and p=0.66 respectively). There was no association between DDR1 and DVL1 expression and tumour grade (p=0.32 and p=1.00 respectively), ER (p=0.62 and 0.50 respectively), PR (p=0.38 and p=0.63 respectively) and C-erbB2 expression (p=0.19 and p=0.33 respectively) in IDC. There was no association between DDR1 and DVL1 expression and tumour grade (p=0.52 and p=0.33 respectively), ER (p=0.06 and p=0.76 respectively), PR (p=0.61 and p=0.43 respectively) and C-erbB2 expression (p=0.58 and p=0.76 respectively) in ILC. Conclusions: This study revealed that DDR1 and DVL1 are present in both IDC and ILC regardless of the tumour differentiation. More studies are needed to assess the potential of these two proteins in distinguishing IDC from ILC in breast tumours.

        Aggregation mechanism of bovine serum albumin based on collision factor by the population balance conservation law

        This paper discusses a novel approach for exploring the aggregation mechanism of bovine serum albumin using collision factor. The population balance equation consisting of aggregation term was developed and solved by the moment method. Different experiments were implemented to account for effective parameters on protein aggregation and to measure variations in average size of aggregates formed in a time interval. This was done by taking pictures with a CCD camera on a sterio microscope. The pictures were exported to image processing software to analyze average number and size of aggregates. The collision factor appearing in population balance equation was optimized and declared as a global term.
