        Cervical xanthogranuloma in a case of postmenopausal pyometra

        Xanthogranuloma is a non-neoplastic presentation of chronic inflammation commonly seen in gallbladder, kidney and rarely seen in genital organs. Only one case has been reported in cervix. Here, we report a case of 60-yearold postmenopausal lady who presented with history of fever and purulent discharge per-vaginum. On speculum examination, cervix had an ulcer extending from 3 to 5 o’clock position. Uterus was bulky. On probing the ulcer, a 1-cm deep sinus was identified. Ultrasound showed enlarged uterus and fluid collection suggestive of pyometra. Pyometra was drained and cervical biopsy was taken from the ulcerated lesion; histopathology revealed granulomatous inflammation with predominantly xanthous cells suggestive of tuberculosis. High index of clinical suspicion needs to be maintained in abnormal cervix. It is a perplexing and rare entity for a clinician and also a diagnosis of exclusion; only histopathology can help for diagnosis. It mimics like malignancy and chronic infections.

        Thank you for giving me this opportunity. I shall begin with a James Madison's quote - "A people who mean to be their governors must arm themselves with power which knowledge gives." This knowledge, most would agree, comes easily with a right circulation of news which only a free press can ensure. We in India have been blessed with an independent press which scores of other nations crave for. Newspapers in India may have been bracketed as 'pro-government', 'anti-government', pro-one political party or other, but one aspect that no one can dispute is that they have been largely free from state control. During the now infamous period of emergency (1975-77) under the Prime Minister Indira Ghandi, most of the mainstream newspapers used to keep their editorial pages blank to protest censorship which came as part of emergency. However, the same can not be said about broadcasting with all India Radio and Doordarshan(television) being under state control ever since their inception. Though there has been much talk in recent years about granting autonomy to AIR and Doordarshan to help them cope up with the competition thrown up by the boom in 'satellite television', it may still take months till a stop is put to 'state broadcasting' in the country. Satellite boom has also kicked off a debate not only in India but in most of the other Asian countries about "how much openness is just enough" and "how much ground should be offered to foreign TV networks to in this side of the globe."

        Plant molecular systematics relies on the use of chloroplast-based DNA barcodes for studying evolutionary relationship between species. Aloe is a large genus of around 400 species and, most species are widely used for traditional medicinal purposes throughout the world viz., A. vera, A. ferox, A. arborescens, and A. maculate. In this study, the plastid genes matK (Maturase K) and rbcL (Ribulose-bisphosphate carboxylase/oxygenase large subunit) were used to demonstrate their efficacy in identifying Aloe species. The evolutionary relatedness of A. vera was inferred using the Neighborjoining method based on matK and rbcL sequences retrieved from NCBI database. The 48 Aloe samples were divided into four major clades based on matK sequence: Clade I is the largest and represented by vera; Clade II is represented by A. boylei, A. suprafoliata, A. welwitschii, and A. verecunda; Clade III is represented by A. ciliaris; and Clade IV is represented by A. ferox and A. arborescens. Similarly, the 29 Aloe samples were distributed into two major clades based on rbcL sequence: Clade I is represented by A. vera; and Clade II is represented by A. arborescens and A. maculate. The study concludes the possibility of using these genes as a DNA barcode to correctly identify, discriminate and documentation of the species belongs to genus Aloe.

      • Skin Cancer Concerns in People of Color: Risk Factors and Prevention

        Background: Though people of color (POC) are less likely to become afflicted with skin cancer, they are much more likely to die from it due to delay in detection or presentation. Very often, skin cancer is diagnosed at a more advanced stage in POC, making treatment difficult.The purpose of this research was to improve awareness regarding skin cancers in people of color by providing recommendations to clinicians and the general public for early detection and photo protection preventive measures. Methods: Data on different types of skin cancers were presented to POC. Due to limited research, there are few resources providing insights for evaluating darkly pigmented lesions in POC. Diagnostic features for different types of skin cancers were recorded and various possible risk factors were considered. Results: This study provided directions for the prevention and early detection of skin cancer in POC based on a comprehensive review of available data. Conclusions: The increased morbidity and mortality rate associated with skin cancer in POC is due to lack of awareness, diagnosis at a more advanced stage and socioeconomic barriers hindering access to care. Raising public health concerns for skin cancer prevention strategies for all people, regardless of ethnic background and socioeconomic status, is the key to timely diagnosis and treatment.

        Nodulation Experiment by Cross-Inoculation of Nitrogen-Fixing Bacteria Isolated from Root Nodules of Several Leguminous Plants

        Root-nodule nitrogen-fixing bacteria are known for being specific to particular legumes. This study isolated the endophytic root-nodule bacteria from the nodules of legumes and examined them to determine whether they could be used to promote the formation of nodules in other legumes. Forty-six isolates were collected from five leguminous plants and screened for housekeeping (16S rRNA), nitrogen fixation (nifH), and nodulation (nodC) genes. Based on the 16S rRNA gene sequencing and phylogenetic analysis, the bacterial isolates WC15, WC16, WC24, and GM5 were identified as Rhizobium, Sphingomonas, Methylobacterium, and Bradyrhizobium, respectively. The four isolates were found to have the nifH gene, and the study confirmed that one isolate (GM5) had both the nifH and nodC genes. The Salkowski method was used to measure the isolated bacteria for their capacity to produce phytohormone indole acetic acid (IAA). Additional experiments were performed to examine the effect of the isolated bacteria on root morphology and nodulation. Among the four tested isolates, both WC24 and GM5 induced nodulation in Glycine max. The gene expression studies revealed that GM5 had a higher expression of the nifH gene. The existence and expression of the nitrogen-fixing genes implied that the tested strain had the ability to fix the atmospheric nitrogen. These findings demonstrated that a nitrogen-fixing bacterium, Methylobacterium (WC24), isolated from a Trifolium repens, induced the formation of root nodules in non-host leguminous plants (Glycine max). This suggested the potential application of these rhizobia as biofertilizer. Further studies are required to verify the N<sub>2</sub>-fixing efficiency of the isolates.

        Coenzyme Q10: a progress towards the treatment of neurodegenerative disease

        Coenzyme Q10 (CoQ10, or ubiquinone) is an electron carrier of the mitochondrial respiratory chain (electron transport chain) with antioxidant properties. In view of the involvement of CoQ10 in oxidative phosphorylation and cellular antioxidant protection a deficiency in this quinone would be expected to contribute to disease pathophysiology by causing a failure in energy metabolism and antioxidant status. Indeed, a deficit in CoQ10 status has been determined in a number of neuromuscular and neurodegenerative disorders. Primary disorders of CoQ10 biosynthesis are potentially treatable conditions and therefore a high degree of clinical awareness about this condition is essential. A secondary loss of CoQ10 status following HMG-CoA reductase inhibitor (statins) treatment has been implicated in the pathophysiology of the myotoxicity associated with this pharmacotherapy. CoQ10 and its analogue, idebenone, have been widely used in the treatment of neurodegenerative and neuromuscular disorders. These compounds could potentially play a role in the treatment of mitochondrial disorders, Parkinson’s disease, Huntington’s disease, amyotrophic lateral sclerosis, Friedreich’s ataxia, and other conditions which have been linked to mitochondrial dysfunction. This article reviews the physiological roles of CoQ10, as well as the rationale and the role in clinical practice of CoQ10 supplementation in different neurological diseases, from primary CoQ10 deficiency to neurodegenerative disorders. These will help in future for treatment of patients suffering from neurodegenerative disease.

        Anticataract activity of rhamnocitrin isolated from Bauhinia variegata stem bark

        A cataract is any clouding or opaque area in the eye's natural lens, which is normally crystal clear, is the leading cause of blindness worldwide. It accounts for approximately 42% of all blindness. Approximately 25% of the population over 65 and about 50% over 80 age has serious loss of vision because of cataract. Most cataracts progress and eventually hamper vision. Multiple mechanisms have been implicated in the development of cataract. But the exact pathogenesis which leads to opacification is not clearly known. Oxidative damage to the lens has been recognized as the initiating event in the pathogenesis of cataracts. In cataract, the oxidation-antioxidant equilibrium shift more towards oxidative stress. Accordingly, antioxidants could prove potential anticataract agents. Rhamnocitrin, a flavonoid, possess strong antioxidant effects; can be used effectively to manage the cataract. Therefore, anticataract activity of rhamnocitrin (10, 20, 40 and 80 ${\mu}g$), isolated from Bauhinia variegata (Leguminosae) stem bark, was studied in ovine and chick embryo lens model. It showed a significant protection against cloudiness in lenses induced by hydrogen peroxide and hydrocortisone in a dose dependent manner. The findings suggest that rhamnocitrin possess significant anticataract activity and act most likely due to its antioxidant property.
