Clofarabine in the Treatment of Elderly Patients with Acute Myeloid Leukemia

Background: Elderly patients with acute myeloid leukemia (AML) have a poor outcome because of co-morbidities, poor tolerance to intensive chemotherapy and inherently more resistant disease. Clofarabine is a second generation nucleoside analogue which has shown promising activity in elderly patients with AML. This study was conducted to review the outcome of treatment with clofarabine in a group of such patients. Methods: The records of 5 elderly patients who were diagnosed to have AML and treated with clofarabine over a 12 month period were reviewed retrospectively. Results: There were 2 female and 3 male patients with a median age of 68 years (range 65-82). At the time of treatment, 2 patients had newly diagnosed AML not considered suitable for intensive therapy, while 3 patients had partial or no response to conventional chemotherapy. The overall response rate was 100%, all patients achieving a complete remission. Induction and consolidation were well tolerated. All patients developed neutropenia with a median duration of 20 days (range 17-42). One patient developed hand and foot syndrome and a generalized rash but recovered. There was no mortality and all patients remained in remission after a median follow-up of 5.2 months (Range 3-10). Conclusion: Clofarabine (alone or in combination) is active in elderly AML patients with an acceptable safety profile and should be considered a potential option in this group.

Pigmented extramammary Paget’s disease (PEMPD) is an uncommon intraepithelial adenocarcinoma and a rare variant of Paget’s disease affecting skin that is rich in apocrine sweat glands such as the axilla, perianal region and vulva. It most commonly occurs in postmenopausal women and presents as a superficial pigmented scaly macule, mimicking a melanocytic lesion. The histological presentation is adenocarcinoma in situ with an increased number of melanocytes scattered between the Paget’s cells. Therefore, PEMPD may be misdiagnosed as a melanocytic tumour both clinically and histologically. The tumour cells are usually positive for cytokeratin 7, epithelial membrane antigen, Cam 5.2, HER2, and mucicarmine stain while S100 and human melanoma black-45 highlight the processes of reactive dendritic cells. The association between Paget’s cells and intratumoural reactive melanocytes is still unclear. We report our first case of PEMPD associated with an intradermal naevus involving the axilla in a 63-year-old woman.
Parallel operation of inverter using trans-Z-source network

<P>There is strong demand for high-power application for photovoltaic and wind power generation systems. The realisation of a high power conversion system is usually limited by the available switching devices because of the maximum voltage and current ratings, losses and switching frequency. Thus, utilising parallel-connected power converters rated at more minute power levels are highly desirable. A robust approach for such a realisation would result in many advantages, such as modularity, ease of maintenance, (<I>n</I> + 1) redundancy and high reliability. The parallel operation of a voltage-fed (VF) trans-Z-source inverter (ZSI) is presented. A two parallel connected VF-trans-ZSI is introduced and a similar concept is extended to N-parallel connected inverter. A 2.9 kW prototype inverter is built and tested to verify the performance of the proposed inverter.</P>
Frequency, Clinical Pattern and Outcome of Thrombosis in Cancer Patients in Saudi Arabia

Objectives: Thrombotic risk is increased in patients with cancer and there are important implications for those who suffer a venous thromboembolism (VTE). We undertook this study to determine the frequency, clinical patterns, and outcome of VTE in Saudi patients with cancer. Methods: Cancer (solid tumors and lymphoma) patients who developed VTE from January 2004 to January 2009 were studied retrospectively. Demographics and clinical characteristics related to thrombosis and cancer were evaluated. Results: A total of 701 patients with cancer were seen during the study period. VTE was diagnosed in 47 (6.7%) patients (median age 52, range 18-80 years). Lower limb DVT was the most common type, seen in 47% patients, followed by PE in 19%, and 19% patients had both DVT & PE. Thrombosis was symptomatic in 72% patients while it was an incidental finding on routine workup in 28%. Cancer and VTE were diagnosed at the same time in 38% of patients, and 47% patients developed VTE during the course of disease after the cancer diagnosis. The majority of VTE post cancer diagnoses occurred during the first year (median 4 months, range 1-14). Additional risk factors for VTE were present in 22 (47%) patients and 14 (30%) of these patients were receiving chemotherapy at the time of thrombosis. Only 5 (10.6%) patients were receiving thrombo-prophylaxis at the time of VTE diagnosis. Most common types of tumors associated with thrombosis were breast cancer, non-Hodgkin's lymphoma and lung cancer. The majority of the affected patients (79%) had advanced stage of cancer. After a median follow-up of 13 (range 0.5-60) months, 38 (81%) patients had died. There was no difference in the mortality of patients with symptomatic or asymptomatic thrombosis (82% vs 78.6%). Conclusions: Thrombotic complications can develop in a significant number of patients with cancer, and almost half of the patients have additional risk factors for VTE. Thrombosis is usually associated with advanced disease and can be asymptomatic in more than a quarter of cases. Thromboprophylaxis in cancer patients is under-utilized. Community based studies are needed to accurately define the extent of this problem and to develop effective prophylactic strategies.
Review of Analytical and Empirical Estimations for Incident Blast Pressure

Recently, the blast load has become more recognized in the structural engineering field because the blast load can result in not only disproportionate structural failure but also tremendous casualties of lives and injuries. As an effort to overcome this problem, blast resistant analyses and designs have been developed, and the methodology would be incorporated into the conventional construction design. Analysis of structures exposed to blast load is the first step for the design, and it requires good understanding of blast phenomena and the following dynamic response of structures. This paper provides an up-to-date comprehensive review of the incident blast wave and its parameters for air and ground blasts. Considering the Unified Facilities Criteria (UFC 3-340-02) as a benchmark, a quantitative comparison between the empirical results presented by researchers and the result given in UFC charts is conducted for a span of scaled distances. We discuss the appropriate use of empirical or analytical equations for precise prediction of blast pressures, and recommend equations that match well the results presented in UFC.
Impact of Anti-Reflective Coating on Silicon Solar Cell and Glass Substrate : A brief review

The most important factor in enhancing the performance of an optical device is to minimize reflection and increasing transmittance of light for a broad wavelength range. The choice of appropriate coating material is crucial in decreasing reflection losses at the substrate. The purpose of this review is to highlight anti-reflection coating (ARC) materials that can be applied to silicon solar cell and glass substrate for minimizing reflection losses. The optical and electrical behavior of ARC on a substrate is highly dependent on thickness and refractive index (RI) of ARC films that are being deposited on it. The coating techniques and performance of single and multi-layered ARC films after coated on a substrate in a wide range of wavelength spectrum will be studied in the paper.
Review on the Progress in Building Integrated Photovoltaic Materials and Module Technology

A building integrated photovoltaic (BIPV) is an innovative, promising and practical technology for buildings with net zero CO2 emissions. This Review paper addresses the development of a curtain wall module, color module, lightweight module and anti-soiling techniques in BIPV Technology. Selecting the best material permits a high conversion efficiency and bright color of the module at the same time. Curtain wall and light weight solar PV cells fabricated using an established procedure could increase the applications of solar PV cells.
