Drought is identified and quantified using a variety of methods. This study aims to evaluate the performance of several rain-based meteorological droughts, namely, Rainfall Anomaly Index (RAI), Standard Precipitation Index (SPI), China-Z Index (CZI), Percent of Normal (PN), Deciles Index (DI), and Z-Score Index (ZSI) on 1-, 3-, 6-, 9-, 12-, 18-, 24-, 48-, and 72-month time scales based on monthly precipitation totals for 24 Johor River Basin (JRB) stations from 1970 to 2015. After imputation of missing data with Multiple Imputation by Chained Equations (MICE), homogeneity test for the annual time series found that 21 stations were ‘useful’, 1 station ‘doubtful’ and 2 stations ‘suspect’, meanwhile monthly time series found 17 stations ‘useful’, 1 station ‘doubtful’ and 6 stations ‘suspect’. The performance of CZI, RAI, and SPI were found to be higher compared to others based on the correlation between drought indices, average correlation of a combination of drought indices for all stations (correlation plot and heatmap), scatter plot linearity between drought indices in capturing the wet/dry conditions, and simulating the historical drought events. However, based on the ability to simulate the historical event, spatial assessment, and frequency analysis, it was found that RAI is more responsive in identifying the ‘drought prone’ area to enable a better spatial mapping of the 3- and 6-months drought condition in JRB. ‘Moderately dry’ condition is expected to be temporally and spatially widespread as North East (NE) monsoon becomes drier. Meanwhile, a condition of ‘very dry’ and ‘extremely dry’ is expected to be more prevalent in the South East area of the basin, particularly due to the drier South West (SW) monsoon period. Study results indicate that RAI can perform comparatively better than other studied indices, and because the calculations of RAI are relatively simple, they can be used as better tools to monitor the conditions of drought in JRB.
Distributional Changes in Rainfall and River Flow in Sarawak, Malaysia

Climate change may not change the rainfall mean, but the variability and extremes. Therefore, it is required to explore the possible distributional changes of rainfall characteristics over time. The objective of present study is to assess the distributional changes in annual and northeast monsoon rainfall (November-January) and river flow in Sarawak where small changes in rainfall or river flow variability/distribution may have severe implications on ecology and agriculture. A quantile regression-based approach was used to assess the changes of scale and location of empirical probability density function over the period 1980-2014 at 31 observational stations. The results indicate that diverse variation patterns exist at all stations for annual rainfall but mainly increasing quantile trend at the lowers, and higher quantiles for the month of January and December. The significant increase in annual rainfall is found mostly in the north and central-coastal region and monsoon month rainfalls in the interior and north of Sarawak. Trends in river flow data show that changes in rainfall distribution have affected higher quantiles of river flow in monsoon months at some of the basins and therefore more flooding. The study reveals that quantile trend can provide more information of rainfall change which may be useful for climate change mitigation and adaptation planning.

Autosomal recessive congenital ichthyosis (ARCI) is a group of diseases presenting as collodion baby at birth. ARCI is categorized as Harlequin ichthyosis, lamellar ichthyosis, and non-bullous congenital ichthyosiform erythroderma (NBCIE), bathing suit icthyosis (BSI) and others. We describe the case of a male newborn with NBCIE whose whole exome sequencing revealed two variants of TGM1 gene (NM_000359.3) in a compound heterozygous state: c.790C>T (p.Arg264Trp) in exon 5 and c.2060G>A (p.Arg687His) in exon 13. In the literature, the Arg264Trp variant has been reported as homozygous or compound heterozygous with other variants in patients with BSI. In contrast, the Arg687His variant has been reported only as homozygous in patients with BSI. To the best of our knowledge, this is the first case whose two compound heterozygous variants, exhibiting the NBCIE phenotype, instead of the BSI.

Glucagon-Like Peptide 1 Therapy: From Discovery to Type 2 Diabetes and Beyond

The therapeutic benefits of the incretin hormone, glucagon-like peptide 1 (GLP1), for people with type 2 diabetes and/or obesity, are now firmly established. The evidence-base arising from head-to-head comparative effectiveness studies in people with type 2 diabetes, as well as the recommendations by professional guidelines suggest that GLP1 receptor agonists should replace more traditional treatment options such as sulfonylureas and dipeptidyl-peptidase 4 (DPP4) inhibitors. Furthermore, their benefits in reducing cardiovascular events in people with type 2 diabetes beyond improvements in glycaemic control has led to numerous clinical trials seeking to translate this benefit beyond type 2 diabetes. Following early trial results their therapeutic benefit is currently being tested in other conditions including fatty liver disease, kidney disease, and Alzheimer’s disease.

Background/Aims: The coronavirus disease-19 (COVID-19) pandemic forced endoscopy units to enact major changes on dailypractice and policy. The Chaim Sheba Medical Center is a tertiary referral center located in the center of Israel, and serves cities withhigh infection rates. Our aim was to review the policies enacted during this outbreak and study their influence on the performanceof endoscopic procedures. Methods: Following the revision of work protocols, personnel were divided into two permanent and physically separate workinggroups and screening procedures were rescheduled. Relevant data including the number of endoscopic examinations, type ofprocedure performed, and patient referrals and indications were taken from a computerized database and evaluated. The studyincluded data for January–March 2018–2020, and a comparison among the data from each year was performed. Results: As of March 2020, the total number of endoscopic examinations performed reduced by 44% (p<0.0001) as compared toprevious years, gastroscopy examinations reduced by 39% (p=0.02), and lower endoscopy procedures reduced by 57% (p<0.0001). Meanwhile, the number of advanced endoscopic procedures performed remained consistent with previous years. The indicationsfor performance of gastroscopy and lower endoscopy were different in March 2020, while these remained unchanged for advancedendoscopic procedures. Conclusions: The current policy appears to serve both our initial goals: protecting personnel and patients’ safety and minimizingpotential damage from delayed endoscopic procedures. A longer term follow-up study is needed in order to fully analyze our results.
Psychosocial Reaction and Trauma After a Natural Disaster: The Role of Coping Behavior

This study examined the relationship between psychosocial reaction, coping behavior and traumaamong natural disaster survivors in West Java, Indonesia. One hundred and one subjects werestudied. The roles of coping behavior as a moderating factor were also tested. Psychosocial reactionduring disaster and coping behavior were significantly correlated with trauma. Hierarchical regressionanalysis found that coping behavior was a significant potential moderating variable in termsof correlation between psychosocial reaction and trauma after a natural disaster.
