        舌側骨分割術에 依한 下顎埋伏智齒 拔去後 合倂症에 關한 硏究

        Critical evaluation of existing chisel and bur techniques for removal of impacted mandibular third molars reveals a high incidence of complications. Both approaches require creation of a buccal defect immediately distal to the second molar for the insertion of an elevator, which may cause pocketing to occur postoperatively. The lingual splitting technique described here minimizes periosteal reflection, almost completely avoids loss of bones. The operation is a modification of the split bone technique of Fry. This technique has been modified by many, recent contribution being in 1983 by Davis. The investigation examined the incidence of dry socket, labiomental paresthesia, lingual paresthesia and postoperative infection. The study reported here covered a total of 238 impacted mandibular third molar extractions performed at the Dept. of Oral and Maxillofacial Surgery, Won Kwang University Dental Hospital from April 1984 to September 1984. The results were follows; 1. 238 impacted mandibular third molar extractions consisted of 22 complete bony impaction and 216 partial bony impaction. 2. No significant correlations were found between the postoperative complications and the type of impaction, the etiology of extraction. After medical treatment was done, extraction was performed 12 cases under systemic diseases. 3. The incidence of dry socket is 9 cases (3.8%) out of 238 cases. 4. 4 cases (1.7%) of the total extractions resulted in labiomental paresthesia. All labiomental paresthesia resolved within 1 to 6 weeks. 5. Following 4 cases (1.7%) of the total extractions lingual paresthesia developed. All lingual paresthesia resolved within 2 to 8 weeks. 6. 2 cases (0.8%) of 238 extractions were complicated by postoperative infection.
