        경골 내과에 발생한 견인 골단염(2예 보고)

        9 and 10 years old boys presented with pain and swelling without history of trauma around medial malleolar right and left ankle. The swelling was diffuse with tenderness on anterior aspect of medial malleolus. The X-rays revealed fragmented accessory ossification center of medial malleolus an symptomatic side. Traction apophysitis was diagnosed because MRI revealed multiple foci of hypointensity in T1 and T2 weighted images of symptomatic medial malleolus apophysis. Patient was treated in conservative treatment by short leg cast for three or four weeks with restriction of sports activity and improved symptoms.

        Ball-and-socket deformity of the ankle joint is a rare entity that is usually associated with inequality of leg length, fibular hyperplasia, coalition of the ankle, and ray deficiency. Etiology is unknown, congenital itself or secondary to congenital conditions in the ankle. Nonunion of medial malleolar is rare in bilateral ball and socket ankle joint without lower leg deformity. We report upon this case, and include brief reviwe of the literature.

        경비 인대 결합 나사못의 이상적인 삽입부위

        Purpose: The purposes of the present study were to find the ideal insertion position syndesmosis screw and relation to the surrounding anatomical structures when indirectly inserting the screw anteriorly at a 30 angle at 3 cm proximal portion of the ankle join t. Materials and Methods: We performed computed tomography from the axial view in 20 normal individuals at 3 cm proximal portion of the ankle joint and divided the lateral side of the fibula into 4 sections. We drew a line from the middle of each of these 4 sections to the posterolateral tibia at a 30 angle and determined the relationship between each of these lines and the surrounding anatomical structures and confirmed the site at which each of these 4 lines passed through the posterolateral tibia which divided into 3 sections and the site of the fibula at which each of these lines passed through the middle 1/3 portion of the tibia. Results: The posterior 3/4 portion of the fibula that passed through the middle 1/3 portion of the posterolateral tibia in 18 cases. The portion did not approach the surrounding anatomical structures(Peroneal vessels and the muscular portion of FHL). Conclusion: The ideal insertion position of syndesmosis screw at 3 cm proximal portion of the ankle joint at a 30 angle is the posterior 3/4 portion of the lateral side of the fibula, and injury to the surrounding anatomical structures could be avoided when the screw passes through the middle 1/3 portion of the posterolateral tibia.

        심장이식 후 부신피질호르몬 투여로 인한 거골의 무혈성 괴사 - 1예 보고 -

        A vascular necrosis of the talus has frequently been reported following trauma because talus has no muscle insertions, sixty percent of the surface of the talus is covered by hyaline cartilage, takes only a small area for entrance of a blood supply. Osteonecrosis is also associated with a variety of nontraumatic disorders. There are many indications for steroid usage, patient with rheumatoid arthritis, systemic lupus erythematosus, chronic obstructive pulmonary disease, and status- post renal or cardiac transplantation may be on long- term steroid usage, osteonecrosis may develop. A vascular necrosis of the talus secondary to chronic steroid usage is an unusual case. Delay in detection of osteonecrosis may lead to fragmentation and collapse of the talar body. When pain on range of motion is present and conservative treatment have been exhausted, surgical treatment is indicated, that is, fusion of the ankle joint. However it is important that conservative treatment may prevent its various sequelae with early diagnosis because steroid - treated patients have a more operative risk and increased risk for postoperative infection. We report a rare case of corticosteroid induced avascular necrosis of talus after cardiac transplantation.

        SPOT 영상을 위한 번들조성에서 외부표정의 신뢰성

        항공사진보다 경제적인 SPOT 위성의 입체영상으로부터 지형도를 제작하는 것이 가능하게 되었다. 많은 학자들이 국가기본도 수정에의 적용가능성과 결과의 정확도를 분석하여 SPOT 영상은 1/50,000에서 1/100,000까지의 지형도에 적합하다고 발표하였다. SPOT 영상으로부터 지형도, 정사사진 및 수치지형모델을 제작하기 위해서는 정확한 외부표정요소를 알아야 하지만, 동적인 선형주사 영상인 SPOT 영상의 특성 때문에 사진측량에서 이용되어온 종래의 번들조정은 직접 사용되지 못한다. 신뢰성은 내적신뢰성으로 불리는 과대오차의 검출능력과 외적신뢰성으로 불리는 검출되지 않은 과대오차가 외부표정 결과에 미치는 영향을 나타낸다. 본 연구에서는 좌표계, 궤도방향 좌표의 표현방법, 외부표정 요소의 차수 및 기준점 배치에 따라 SPOT 영상의 신뢰성이 어떻게 달라지는가를 밝히고, SPOT 영상을 이용한 작업에서의 기초자료로 제공될 수 있는 외부표정에서의 이론적인 신뢰성을 분석하였다. Topographic mapping from economic SPOT stereo imagery than aerial photographs has become possible. Many of authors have studied the possibility of base map revisions and the accuracy of results. They have concluded that the SPOT image is suitable for 1/50,000 to 1/100,000 topographic map. For topographic map, orthophoto and DTM generation from SPOT imagery, accurate exterior orientation parameters are needed. But since the geometric characteristic of SPOT image is dynamic linear array imagery, the conventional bundle adjustment for photogrammetry can not be directly applied. Reliability is the ability to detect gross error, which is called the internal reliability, and the effect of non-detectable gross error on the results of exterior orientation, which is called the external reliability. This paper shows how the reliability of SPOT imagery depends on the different coordinate systems, presentations of coordinate for flight direction, orders of exterior orientation parameters and distribution of control points, and thus analyses the theoretical reliability of the exterior orientation, which can provide a basis for the planning of SPOT projects.

        수계에 존재하는 곰팡이속 분리동정 및 관리방안 조사

        병원성과 독성을 포함하는 곰팡이속은 사람의 건강에 직접적인 영향을 끼치며, 수계에 분포할 경우 수처리 공정에서 소독과정을 방해할 뿐만 아니라 잔류염소의 유지를 어렵게 하여 여러 가지 맛과 냄새문제를 야기 시키는 것으로 알려져 있다. 본 연구에서는 부산시 상수원으로 사용하고 있는 물금, 매리 지점을 대상으로 수계에 존재하는 곰팡이속에 대한 분포실태를 2009년 3월부터 10월까지 조사하였다. 곰팡이속은 4월에 최소, 9월에 최대 농도를 보이는 특성을 보였으며, 검출농도는 5~64 CFU/100 ㎖ 범위로 분포하였다. 그리고 곰팡이 농도는 여러 가지 수질인자 가운데 수소이온농도, 수온 및 총대장균군과 상관관계를 보였으며 지점별로는 물금 지점이 매리 지점보다 개체수가 높게 나타났다. 검출되어진 곰팡이속을 대상으로 종 동정을 실시한 결과 Aspergillus fumigus, Aspergillus flavus, Aspergillus niger, Mucor pusillus, Penicillum notatum 등 9종이 분리되었으며, 이중 Aspergillus flavus, Penicillum notatum이 우점종으로 조사되었다. 그리고 지점별로는 물금에서 6종, 매리에서 9종이 검출되어 매리 지점이 물금 지점보다 종 수는 많은 것으로 확인되었다. Fungi, including allergenic, pathogenic or toxigenic species, may have direct effects on human health, interfere with the disinfection process and maintenance of chlorine residual, and add to taste and odor problems. We surveyed distribution of waterborne fungi from raw water in Busan metropolitan city using the EPA standard method. According to the survey, conducted from March 2008 to October 2008, the number of fungi in raw water was high in September and low in April. Quantitative enumeration of fungi per 100 ㎖ of raw water ranged from 5 to 64 CFU, and a higher number was detected in the Mulgum region compared to the Maeri region. Total fungi numbers highly correlated with pH, water temperature and total coliform in this area. Predominant fungi genera grown in medium were identified as Aspergillus, Penicillum, Histoplasma, Rhizopus, Trichophyton, Mucor and Sporthrix species. Aspergillus flavus and Penicillum notatum were particularly dominant in both areas. There were a total of 6 and 9 species detected in the Mulgum and Maeri regions respectively, and the number of species was more verified in the Maeri region than the Mulgum region.
