        뇌 혈관검사 시 적정 조영제량에 관한 연구

        Recently, the use of contrast agents has been increasing as a broader range of tests and dynamic tests have be-come common due to the development of equipment and imaging techniques such as Multi-Detector CT. However, the side effects of using contrast agents have been reduced by the development of non-ionic contrast agents, but they are still occurring often. The purpose of this study was to propose a method to minimize the side effect of contrast agent by using the amount of contrast agent injected to the brain angiography test to suppress excessive use of contrast agent and analyze the amount of contrast agent. Patients who were prescribed Brain Angiography due to cerebrovascular disease, According to the results of the compar-ison of the results obtained by dividing into 4 groups of 10ml each according to the amount of contrast medium injected with contrast agent according to the BMI of the patient, BA and SNR were not different between groups, and even if the amount of contrast injection was reduced, there was no problem in the evaluation of CT angiography through 3D reconstruction. This result shows that even if the contrast medium is injected into the blood vessels of the patient first and then the contrast medium is used as the physiological saline solution, the contrast medium is reduced by 40% it can be expected to minimize.

        방사선검사 시 의료방사선 안전성에 대한 인식도 조사

        Abstract Most patients and parents and guardians display frequent anxiety due to radiation exposure during outpatient, ward, and pediatric general radiographic examinations. This is a behavior that perceives only the harmfulness of radiation. For the recognition of medical radiation, we conduct surveys on outpatients, inpatients, and pediatric parents and guardians to identify their awareness, and then use the radiation dose promotional materials After providing accurate information on the use of radiation, the outpatient, inpatient, and pediatric parents and guardians were asked to explain the change in awareness. The questionnaire items were classified into five categories: repetitive radiation awareness for diagnosis, awareness of exposure dose, availability of exposure information, awareness of radiation risk, and awareness of health problems caused by radiation. There was a statistically significant difference in the items of recognition result of medical radiation, although there was a slight difference in the individual items in the pre and post-recognition results of providing information about the radiologists of the protector and the outpatient(p<0.05). Therefore, through the installation of these promotional materials, we will improve our awareness of medical radiation safety during general radiography surveillance in the Department of Radiology to provide better quality medical information and medical services, thereby contributing to strengthening the competitiveness of the hospital.

        위장조영검사에 대한 의료서비스 만족도에 관한 연구

        건강검진을 위해 병원을 내원한 수검자 중 위장조영검사를 선택한 수검자를 대상으로 서비스 만족도 개선을 위해 본 연구를 시행하였다. 수검자를 대상으로 설문조사를 시행하였으며, 2014년 5월부터 2개월 동안 내원한 수검자 147명을 교육 전 대상자로, 2014년 7월부터 2개월 동안 내원한 수검자 164명을 교육 후 대상자로 선정하였다. 교육 전 대상자에게 불안감, 설명필요, 설명방식, 조영검사 만족도 등을 조사하여 이를 바탕으로 동영상을 제작하였으며, 교육 후 대상자에게 제공하여 불안감 변화, 동영상 설명 만족도, 도움정도, 재 시청 의향 등을 조사하였다. 교육 전 측정한 평균값은 불안감 2.62, 설명필요성 2.52로 측정 되었으며, 설명방식은 대상자의 78.2%가 설명 안내 동영상을 선택하였고, 조영검사 만족도는 1.55로 조사되었다. 교육 후 측정한 평균값에서 불안감의 변화는 1.45, 동영상설명만족 2.80, 도움정도 2.79, 재 시청 의향은 2.80으로 측정되었다. 교육 전, 후 집단 간 T-test에서 불안감 변화, 만족도 등에서 통계적으로 유의한 차이를 보였으며(p<0.05), 위장조영검사에 대한 동영상 교육을 통한 사전 정보제공이 환자의 불안감을 감소시켰고, 검사만족도는 증가 시켰음을 알 수 있었다. This study was conducted with examinees who chose upper gastrointestinography (UGI) among those who visited the hospital for health examination in order to improve the visitors’ satisfaction with services. A questionnaire survey was conducted with the examinees, and 147 examinees who visited the hospital during the two months from May 2014 participated in the survey as the subjects of pre education examination while 164 examinees who visited during the months from July 2014 participated as the subjects of post education examination. The pre education subjects were surveyed about anxiety, need of explanation, method of explanation, satisfaction with UGI, etc., and a video was prepared based on the results and provided to the post education subjects and then they were surveyed about change in anxiety, satisfaction with the explanation in the video, helpfulness of the video, intention to watch the video again, etc. In the pre education survey, the average value of anxiety was 2.62, and the need of explanation was 2.52, In addition, 78.2% of the subjects chose guide video as a method of explanation, and satisfaction with UGI was 1.55. In the post education survey, the average value of changed anxiety was 1.45, satisfaction with video explanation was 2.80, helpfulness was 2.79, and intention to watch the video again was 2.80. In the results of T-test between the pre and post education groups, they showed statistically significant in anxiety and satisfaction (p<0.05). Moreover, the findings of this study confirmed that video education and information on UGI provided in advance reduce patients’ anxiety aㅁnd increase their satisfaction with examination.

        The Relationship between Status of Medical Knowledge of Medical Consumers and Satisfaction of Medical Service

        The supply of medical services using high-priced medical equipment is increasing due to the medicalization of medical science. The level of medical knowledge of medical consumers and the ability to select medical institutions are required to provide high quality medical services because of the popularization of medical information. As the attitudes of medical consumers toward medical institutions change, medical institutions also need to change their perception to improve customer satisfaction. The purpose of this study is to analyze the difference of medical service satisfaction according to the medical knowledge about the utility of PET / CT in order to change the consciousness structure of medical service. The results of this study showed that both medical satisfaction and environmental satisfaction were high in those who had medical knowledge about their own illness and the usefulness of PET / CT. Therefore, medical institutions will need a practical medical service countermeasures to provide medical information by noticing the changes in perception of the medical knowledge of medical consumers, rather than providing formal medical services. 의료의 과학화로 인해 고가의료장비를 이용한 의료서비스 공급이 증가하고 있다. 의학 정보의 대중화로 의료소비자들의 의학적 지식수준 및 의료기관 선택능력의 향상으로 높은 수준의 의료서비스가 요구되고 있다. 이처럼 의료소비자들이 의료기관을 대하는 태도가 바뀌면서 의료기관들 역시 고객 만족도 향상을 위한 인식의 변화가 필요하다. 이에 본 연구는 의료서비스의 의식구조 변화를 지향하기 위해 PET/CT의 효용성에 대한 의학적 지식 유무에 따라 의료서비스만족도 차이를 분석하였다. 연구 결과 의료소비자 자신의 질병과 PET/CT의 효용성에 대한 의학적 지식이 있는 사람이 진료만족과 환경만족 모두 높게 나타났다. 따라서, 의료기관에서는 형식적인 의료서비스 제공이 아닌 의료소비자의 의학적 지식에 대한 지각 변화를 파악하여 의학 정보를 제공하는 실질적인 의료서비스 대응책이 필요할 것이다.

        의료의 과학화로 인해 고가의료장비를 이용한 의료서비스 공급이 증가하고 있다. 의학 정보의 대중화로 의료소비자들의 의학적 지식수준 및 의료기관 선택능력의 향상으로 높은 수준의 의료서비스가 요구되고 있다. 이처럼 의료소비자들이 의료기관을 대하는 태도가 바뀌면서 의료기관들 역시 고객 만족도 향상을 위한 인식의 변화가 필요하다. 이에 본 연구는 의료서비스의 의식구조 변화를 지향하기 위해 PET/CT의 효용성에 대한 의학적 지식 유무에 따라 의료서비스만족도 차이를 분석하였다. 연구 결과 의료소비자 자신의 질병과 PET/CT의 효용성에 대한 의학적 지식이 있는 사람이 진료만족과 환경만족 모두 높게 나타났다. 따라서, 의료기관에서는 형식적인 의료서비스 제공이 아닌 의료소비자의 의학적 지식에 대한 지각 변화를 파악하여 의학 정보를 제공하는 실질적인 의료서비스 대응책이 필요할 것이다.         방사선사의 임금 적정성 및 직무 만족도

        This study aims to provide the basic data helpful for the improvement of rights and interests of radiological technologists, by understanding the current working environment and if they get the proper reward through the analysis on the perception of appropriateness of current wage and job satisfaction of radiological technologists. Survey was conducted to evaluate the perception of satisfaction with job and wage system, work intensity, necessity of the radiological technologist limit system, and turnover in accordance. The appropriate starting salary was low in case of women and non-capital area, and when the age, academic background, career, and hospital size were lower. The appropriate wage was low in case of women and non-capital area, and when the academic background and hospital size were lower. The negative perception of the necessity to change the wage system was shown in university hospitals. Regarding the work intensity, in case of women and low career, and when the age, academic background, and hospital size were higher, the work intensity was perceived as high. When the academic degree and hospital size were lower, the positive perception of the radiological technologist limit was shown. The turnover intention was high in case of women, and when the age, academic background, career, and hospital size were lower. In order to increase the job satisfaction of radiological technologists, it would be necessary to the appropriate reward and the environment where they could concentrate on work.

        한국인 임산부의 흉부 후-전 방향 방사선검사 시 적절한 차폐막 높이

        The purpose of this study is to provide the basic data for reducing unnecessary radiation dose to the abdomen and fetus of pregnant women by presenting proper height of shielding protector for efficient abdominal shielding in chest PA examination of Korean pregnant women. The subjects of this study were 288 persons who were eligible for this study among 798 pregnant women who had chest PA examination from January 1, 2015 to December 31, 2016 Retrospective study was performed. Measurements was performed from the apex of the right and left lungs to costophrenic angle of the right and left lungs and to the lowest costophrenic angle among the right and left lungs at the top of the image(this line called Joo s line in this study). The mean of the right and left lung height of pregnant women were 259.09 ㎜ and 263.57 ㎜, respectively. Also, the average height of the Joo s line designed by the researcher for proper abdominal radia-tion protection was 322.15 ㎜. For proper and efficient abdominal radiation protection for pregnant women, it is necessary to adjust the shielding according to the height of the pregnant woman. It is appropriate that the height of the shielding protector should be adjusted so that the upper part of the shield is located at 342.30 ㎜ below from upper part of the detector.

        의료 방사선사용에 따른 납과 텅스텐의 차폐효과 분석

        병원에서 방사선을 차폐하는데 있어 납은 매우 유용하게 사용되고 있다. 하지만 납은 독성을 가지고 있고 대체물질에 대한 연구가 많이 이루어지고 있으며, 대표적으로 텅스텐을 대체물질로 한 연구가 많이 이루어지고 있다. 이에 본 연구에서 납과 텅스텐의 물리적 특성 및 반가층 실험을 진행한 결과 원자번호가 높은 납 원소의 반응단면적이 텅스텐에 비해 높게 나타났으나, 텅스텐의 밀도가 높아 동일한 크기일 경우 텅스텐의 전자밀도가 납에 비해 약 1.7배 높은 것으로 나타났다. MCNPX를 이용한 모의 모사에서도 에너지 따라 다소 차이가 있지만 텅스텐이 납에 비해 약 1.4배 차폐효과가 높은 것으로 나타났으며, 텅스텐이 납에 비해 우수한 차폐효율을 갖고 있는 것으로 확인 되었다. 하지만 경제적 측면을 고려할 때 텅스텐은 희소금속으로 납에 비해 가격이 약 25배 높아 납에 대한 대체물질로는 부적당한 것으로 사료되었다. Lead is a very useful material in shielding radiation in hospitals. But lead is toxic. Therefore, there are many studies on substitutable materials, Typically, there are many studies using tungsten. In this study, we investigated the physical properties of lead and tungsten and the Half value layer. As a result, lead having higher atomic number showed higher cross - sectional area than tungsten. But, at the same size, the electron density of tungsten with a high density is about 1.7 times higher than that of lead. In MCNPX simulation, the shielding effect of tungsten is about 1.4 times higher than that of lead, It was confirmed that tungsten had better shielding efficiency than lead. However, considering the economic aspect, tungsten is a rare metal, which is about 25 times more expensive than lead, which is considered to be inappropriate as an alternative to lead.

        Moire artifact 제거 후 그리드 주파수에 따른 미세병변의 영상분석

        Morphological information such as shape and margin of micro lesion is important information for diagnosis of disease in clinical imaging. In this study, we investigated the morphological changes of the micro lesions by comparing the contrast and area in grid suppressed DR images according to grid frequency. In the profile analysis of the image, the mass showed an average intensity variation of 8.6 ~ 72.4 after suppression, The higher the grid frequency, the more the contrast was increased. However, in the images obtained using 103 lp / inch, which is a grid frequency less than the sampling frequency, the contrast of the mass in the vertical direction decreased after suppression. In the binary image, the area change of the mass was also large. As a result, the shape, size, and margin of the mass changed. In the case of very small calcification, the higher the grid frequency is the larger the change in contrast, so that a clear image can be obtained in the post-suppression image. However, we could confirm that the margin of the lesion was blurred and the lesion was lost in some of the images using the 103 lp / inch grid. The higher the frequency of the grid, The change of the contrast of fiber occurred largely and clear boundary was confirmed. The decrease of the number of pixels was small and morphological change was small. In conclusion, when using a grid frequency that is not suitable for the sample frequency, morphological changes or lesion loss of micro lesions in the post- suppression image may give the possibility of misdiagnosis in diagnosis and differentiation of the image.
